Breast Cancer in Lampang, a Province in Northern Thailand: Analysis of 1993-2012 Incidence Data and Future Trends

  • Background: The recent epidemiologic transition in Thailand, with decreasing incidence of infectious diseases along with increasing rates of chronic conditions, including cancer, is a serious problem for the country. Breast cancer has the highest incidence rates among females throughout Thailand. Lampang is a province in the upper part of Northern Thailand. A study was needed to identify the current burden, and the future trends of breast cancer in upper Northern Thai women. Materials and Methods: Here we used cancer incidence data from the Lampang Cancer Registry to characterize and analyze the local incidence of breast cancer. Joinpoint analysis, age period cohort model and Nordpred package were used to investigate the incidences of breast cancer in the province from 1993 to 2012 and to project future trends from 2013 to 2030. Results: Age-standardized incidence rates (world) of breast cancer in the upper parts of Northern Thailand increased from 16.7 to 26.3 cases per 100,000 female population which is equivalent to an annual percentage change of 2.0-2.8%, according to the method used. Linear drift effects played a role in shaping the increase of incidence. The three projection method suggested that incidence rates would continue to increase in the future with incidence for women aged 50 and above, increasing at a higher rate than for women below the age of 50. Conclusions: The current early detection measures increase detection rates of early disease. Preparation of a budget for treatment facilities and human resources, both in surgical and medical oncology, is essential.

Breast cancer has the highest incidence rates among females, not only in the southern regions, but throughout Thailand. Surat Thani is a province in the upper part of Southern Thailand. A study was needed to identify the current burden, and the future trends of breast cancer. Materials and Methods: Here we used cancer incidence data from the Surat Thani Cancer Registry to characterize the incidences of breast cancer. Joinpoint analysis was used to investigate the incidences in the province from 2004 to 2012 and to project future trends from 2013 to 2030. 신종 바이러스에 대응하는 스마트 고령자지원 시스템의 연구 (A Study on the Smart Elderly Support System in response to the New Virus Disease)

  • 최근 COVID-19와 같은 신종 바이러스 감염증이 확산하여 심각한 공중 보건 문제를 제기하고 있다. 특히 이러한 질병은 고령자에게 치명적으로 작용하여, 생명을 위협하고 심각한 사회적, 경제적 손실을 초래하였다. 이에 많은 산업분야에서 사물 인터넷(IoT) 및 인공 지능(AI)을 응용한 원격진료, 헬스케어, 질병예방 등의 애플리케이션이 소개되어 질병 감지, 모니터링 및 검역 성능을 향상하고 있다. 하지만 기존기술은 갑작스러운 전염병의 출현에 신속하고 통합적으로 적용되지 않기 때문에, 사회 속에 감염병이 대규모 감염 및 전국적 확산되는 것을 차단하지 못하였다. 따라서 본 논문에서는 바이러스 질병 정보 수집기를 통해 지역적 한계가 있는 다양한 감염 정보를 수집하고, AI 브로커를 통해 AI 분석 및 심각도 매칭을 수행하여 감염의 확산을 예측하고자 한다. 최종에는 질병관리본부를 통해 고령자에게 위험경보 발령, 확산 차단 문자 발송 및 감염지역 대피정보를 신속하게 제공한다. 현실적인 고령자 지원시스템은 감염자 발생지역 정보와 고령자의 위치정보를 비교하여 증강현실 기반의 스마트폰 애플리케이션으로 직관적인 위험지역(감염지역) 회피기능을 제공하고 감염지역 방문이 확인되면 자동으로 방역관리 서비스를 제공한다. 향후 제안시스템은 위치기반의 사용자 밀집도를 파악함으로써 갑작스런 인파 집중으로 인한 압사 사고를 사전에 예방하는 방법으로도 활용 가능할 것이다.

치과위생사의 코로나19 감염관련 지식, 감염관리 수행 및 사회심리적 건강 관련 연구 (A study on the relations of COVID-19 infection related knowledge, infection control performance, and psychosocial well-being of dental hygienists)

  • Objectives: This study aimed to verify the relations of COVID-19 infection-related knowledge, dental infection control performance, and psychosocial health of dental hygienists. Methods: From the data was analyzed through the frequency analysis, t-test, one way ANOVA, and pearson's correlation analysis with a total of 198 dental hygienists using SPSS Statistics 21.0. Results: The correct answer rate for knowledge about COVID-19 was higher with a bachelor's degree or higher. Thus, when the level of education was higher, the performance was also high (p<0.01). COVID-19 infection control performance was highly shown in case of high age (4.43) (p<0.05) and the married (4.42) (p<0.01). Infection control performance was high when they had an educational experience of COVID-19 infection control (4.46), and when the infection control guidebooks were equipped in their workplaces (p<0.001). Degree of psychosocial health of research subjects was shown in the order of potential stress (60.6%), high-risk stress (23.7%), and healthy (15.7%). Infection control performance was higher, the psychosocial health was healthy. Conclusions: For the response to COVID-19 infectious disease and the preparation for mutated viruses and new infectious diseases in the future, it would be necessary to establish the concrete measures for establishing the strict infection control system for dental clinics and also enhancing the psychosocial health of dental hygienists.

코로나19(COVID-19) 방역상황에서 공중보건의사의 업무 수행 현황과 지원방안 (Current Status of Work Performance and Support Plan for Public Health Doctors in the COVID-19 Quarantine)

  • 본 연구의 목적은 코로나19 방역상황에 투입된 공중보건의사(PHD)의 업무 수행 현황을 조사하고, 향후 효과적인 국가 감염병 방역을 위한 공중보건의사 지원방안을 제시하는 것이다. 연구결과, 공중보건의사는 주로 검체채취, 문진 및 진료, 치료를 수행했던 것으로 나타났다. 방역에 투입된 공중보건의사의 39%는 음압시설이 없는 곳에서 근무했고, 개인 보호 장비와 복리후생 지원도 열악했던 것으로 나타났다. 또한 높은 감염위험성, 정신적 고통, 의사결정과정에서 배제, 공무원과의 갈등, 업무지침 문제, 사전교육 부족 등을 경험한 것으로 조사되었다. 따라서 향후 효과적인 감염병 관리를 위해서는 공중보건의사에게 적절한 직급 부여, 방역 관련 의사결정 과정 참여, 적절한 보상과 규정 명시, 교육과 정신건강 지원이 필요하다.

Cohort Profile: Korean Tuberculosis and Post-Tuberculosis Cohort Constructed by Linking the Korean National Tuberculosis Surveillance System and National Health Information Database

  • We aimed to review the current data composition of the Korean Tuberculosis and Post-Tuberculosis Cohort, which was constructed by linking the Korean Tuberculosis Surveillance System (KNTSS; established and operated by the Korean Disease Control and Prevention Agency since 2000) and the National Health Information Database (NHID; established by the National Health Insurance Service in 2012). The following data were linked: KNTSS data pertaining to patients diagnosed with tuberculosis between 2011 and 2018, NHID data of patients with a history of tuberculosis and related diseases between 2006 and 2018, and data (obtained from the Statistics Korea database) on causes of death. Data from 300 117 tuberculosis patients (177 206 men and 122 911 women) were linked. The rate of treatment success for new cases was highest in 2015 (86.7%), with a gradual decrease thereafter. The treatment success rate for previously treated cases showed an increasing trend until 2014 (79.0%) and decreased thereafter. In total, 53 906 deaths were confirmed among tuberculosis patients included in the cohort. The Korean Tuberculosis and Post-Tuberculosis Cohort can be used to analyze different measurement variables in an integrated manner depending on the data source. Therefore, these cohort data can be used in future epidemiological studies and research on policy-effect analysis, treatment outcome analysis, and health-related behaviors such as treatment discontinuation.

<사례보고> 코로나바이러스감염증-19 유행과 로컬 거버넌스 - 2020년 대구광역시 유행에 대한 대응을 중심으로 - (<Field Action Report> Local Governance for COVID-19 Response of Daegu Metropolitan City)

  • 이 사례보고의 목적은 1) 대구광역시의 코로나바이러스감염증-19 감염병 위기 대응 사례를 통해 지역사회의 감염병 대응 전략과 성과를 분석하고, 2) 거버넌스 이론과 감염병을 이용한 대응 거버넌스 프레임워크를 활용하여 이번 사례를 해석하는 것이다. 3) 향후 지역사회 감염병 발생에 대비하기 위한 전략적 모델을 제안하고자 하는 것이다. 연구자의 참여적 관찰, 대구광역시 코로나바이러스감염증-19 백서, 대구광역시의사회 코로나바이러스감염증-19 백서, 국내외 거버넌스에 관한 문헌고찰, 행정자료를 통해 대구광역시의 감염병 위기 대응 사례를 분석하였다. 연구자의 참여관찰과 문헌고찰을 통해 1) 대구광역시 감염병 위기 대응을 위한 리더십 및 지휘체계 확립, 2) 범시민대응위원회를 통한 시민참여 및 소통전략, 3) 시민 간 협력 대구시 및 민관의료기관 거버넌스, 4) 대구광역시의료협회, 메디시티대구협의회, 민간 전문가의 참여와 소통을 통한 의사결정 및 위기대응, 5) 증상모니터링 및 환자 중증도 분류 전략 6) 효과적이고 효율적인 의료 대응 역량, 7) 지역 감염병 위기대응 정보시스템 구축 및 활용을 위한 전략 및 시사점을 도출하였다. 이 연구 결과는 시민, 민간 전문가, 지역사회의료기관 등이 참여하는 지역사회의 협력적 거버넌스가 감염병 위기에 효과적으로 대응하기 위한 핵심 요소임을 실증적으로 보여주었다.

Research Trends in Agenda-setting for Climate Change Adaptation Policy in the Public Health Sector in Korea

  • Many studies have been conducted to assess the health effects of climate change in Korea. However, there has been a lack of consideration regarding how the results of these studies can be applied to relevant policies. The current study aims to examine research trends at the agenda-setting stage and to review future ways in which health-related adaptation to climate change can be addressed within national public health policy. A systematic review of previous studies of the health effects of climate change in Korea was conducted. Many studies have evaluated the effect of ambient temperature on health. A large number of studies have examined the effects on deaths and cardio-cerebrovascular diseases, but a limitation of these studies is that it is difficult to apply their findings to climate change adaptation policy in the health sector. Many infectious disease studies were also identified, but these mainly focused on malaria. Regarding climate change-related factors other than ambient temperature, studies of the health effects of these factors (with the exception of air pollution) are limited. In Korea, it can be concluded that studies conducted as part of the agenda-setting stage are insufficient, both because studies on the health effects of climate change have not ventured beyond defining the problem and because health adaptation to climate change has not been set as an important agenda item. In the future, the sharing and development of relevant databases is necessary. In addition, the priority of agenda items should be determined as part of a government initiative.

  • 최근 몇 년 동안, AI 시스템, 블록체인, VR/AR, 3D 프린팅, 로봇 공학, 나노 기술과 같은 기술의 발전은 바로 우리 눈앞에서 건강 관리의 미래를 재편하고 있습니다. 또한, 의료는 소비자의 요구에 초점을 맞춘 예방 중심의 의학으로 패러다임이 전환되었습니다. Covid-19와 같은 전염병의 확산으로 의료 및 치료 시설의 정의가 변경되어 병원의 물리적 환경을 재설계하고 사회적 거리 두기 요구사항을 해결하도록 통신 모델을 조정하고 가상 의료 솔루션을 구현하고 새로운 임상 프로토콜을 수립하기 위한 즉각적인 조치가 필요하게 되었습니다. 전통적으로 의료 시스템의 허브 역할을 해 온 병원은 이러한 환경에 맞서 스스로를 재정립하는 것을 추구하거나 강요당하고 있습니다. 미래의 건강관리는 질병을 치료하는 것뿐만 아니라 건강과 예방에 초점을 맞출 것으로 예상됩니다. 개인화된 진료에서는 장기적인 예방 전략, 원격 모니터링, 조기 진단 및 탐지가 매우 중요합니다. 이러한 현대 기술로 정의되는 스마트 헬스케어에 대한 관심이 높아짐에 따라, 본 연구는 스마트 헬스케어의 정의와 서비스 종류를 조사했습니다. 스마트 병원의 배경과 기술적 측면도 문헌 검토를 통해 탐구했습니다.

COVID-19 전후 단일 한방병원 및 한방 안이비인후피부과 외래 환자들에 대한 비교 분석 (Comparative Analysis of Outpatients Visiting Korean Medicine Hospital and Department of Ophthalmology, Otolaryngology & Dermatology of Korean Medicine Pre- versus Post- COVID-19)

  • Objectives : The objective of this study is to predict how future infectious diseases will affect healthcare utilization patterns within korean medicine hospitals and, specifically, departments of ophthalmology, otolaryngology and dermatology of Korean medicine. Methods : We retrospectively analyzed medical records of patients who visited Kyung Hee University Korean Medicine Hospital during Pre COVID-19(from March 1st, 2018 to Feb 29th, 2019) and Post COVID-19(from March 1st, 2019 to Feb 28th, 2022). The data was analyzed by visit type and diagnosed disease code. Results : 1. After the COVID-19 break-out, the number of first visit patients increased but those of revisit patients, total patients, and revisit/first visit ratio decreased in all departments. 2. In Kyung Hee University Korean Medicine Hospital, the decrease rate of the number of total patients diagnosed with diseases of respiratory system(J00-J99) was the highest, while that for diseases of circulatory system(I00-I99) was the lowest. 3. In the department of Ophthalmology and Otolaryngology of Korean Medicine, the decrease rate of the number of total patients with nose disease was the highest, followed by eye disease and ear disease. 4. In the department of Dermatology of Korean Medicine, the number of total patients with dermatitis remained consistent, while the number of total patients with skin appendage disease showed relatively high decrease rate. Conclusions : Severe or chronic diseases showed relatively low decrease rate of the number of total patients. While mild diseases or diseases with high probability of exposure to COVID-19 infection during treatment showed high decrease rate of the number of total patients.
