A case of cytomegalovirus-negative M$\acute{e}$n$\acute{e}$trier's disease with eosinophilia in a child

  • M$\acute{e}$n$\acute{e}$trier's disease is a rare form of acquired gastropathy characterized by giant rugal folds in the stomach and protein-losing gastropathy. Children with M$\acute{e}$n$\acute{e}$trier's disease tend to follow a benign self-limited course with symptoms typically completely resolving within 2 to 10 weeks in contrast to the chronic course in adults. A 9-year-old girl presented with a history of gradually worsening abdominal distension, increasing body weight, and abdominal pain for 2 weeks. Physical examination on admission indicated periorbital swelling, pitting edema in both the legs, and abdominal distension with mild diffuse tenderness and shifting dullness. Laboratory tests on admission showed hypoalbuminemia, hypoproteinemia, and peripheral eosinophilia. The test result for anti-cytomegalovirus immunoglobulin M was negative. Increased fecal alpha 1 anti-trypsin excretion was observed. Radiological findings showed massive ascites and pleural effusion in both the lungs. On gastroscopy, large gastric folds, erythema, erosion, and exudation were noted in the body and fundus of the stomach. Microscopic findings showed infiltration of eosinophils and neutrophils in the gastric mucosa. Her symptoms improved with conservative treatment from day 7 of hospitalization and resolved completely.

  • 1 . Experimental study Preventive effect of Korea Red Ginseng (KPG) on hypertensive retinal arteriolosis in rabbits was studied. The results as follows: Blood pressure: Hypertensive group (B) was obviously raised up in comparing with that in normal group(A) and in hypertension + KRG group(C). Ocular fundus:Changes in B group including the retinal arteriospasm, crossing arterioveous, exudation and edema. But C group showed lightly. Light microscope: HE stained vascular damage in retina including thickness hyalimisation, execdates and edema Electron microscope: The endothelial cells were arranged irregularly, different shape and showed cytoplasm loose and vacuole. Immunohistochemistry: Ginseng can regulating endothelin-1, angiotension-ll, endothelium grow factor expre,j,iion and secreation in retinal blood clrultion. 2. ClinicAl Study 66 of hypertensive patients (42 men, 23 women,48-68 years old)and 20 normal person (7 men, 13 women,47-68 years old) were administrated(p.0.) by HRG (3g per day for 6 weeks). The results showed that marked effective rate and total effective rate were 53cyo and 60.6alo respectively and no severe side effects were found. The above results suggest that Ginseng have a difinite hypotensive effect and a role of preventing hyperfine sloe arteriosclerosis.

Experimental Proliferative Vitreoretinopathy (증식성 초자체망막증의 실험적 유발)

  • The authors performed experimental study to produce proliferative vitreoretinopathy by intravitreal injection of homologous fibroblasts in pigmented rabbits. Twenty four eyes of 12 rabbits were divided into control and experimental groups. In experimental group, the eyes were pretreated with expanding gas, perfluoropropane for the vitrectomy. In control group, no such procedure was preceded. In both groups, $5.0{\times}10^4$, $1.0{\times}10^5$ and $1.5{\times}10^5$ cells of homologous fibroblasts were injected into the vitreous cavity. All the eyes were observed regularly with indirect ophthalmoscope followed by fundus photography. Those eyes were enucleated in 1, 2, 4 weeks for the light and electron microscopic studies. The development and severity of the proliferative vitreoretinopathy were correlated with the numbers of the injected fibroblast. This was more pronounced in the experimental group.

A Study of Optical System Design for a Retinal Camera (망막 카메라용 광학계 설계)

  • We need a good image of the retina of the human eye in order to inspect or cure it. In this work, an optical system design for a retinal camera is studied and the finite schematic eye model made by Sang Gee Kim and Sung Chan Park is used. The optical system is composed of four lens groups. The rays of the entire object field are collected on the center by the 1st group and the objective is imaged by all the other groups. The image is detected by the CCD array and displayed by a monitor The 1st lens group is employed singlet and other groups are employed triplets. Ray aberrations, spot diagrams, diffraction line spread functions and MTFs are calculated for optical performance assessment. This design may be very useful for the development of a retinal camera with high performance.

  • We report here a case of a gastrointestinal stromal tumor (GIST) in the stomach that was diagnosed by endoscopic ultrasound-guided fine needle aspiration cytology (EUS-FNA). A 67 year old male patient underwent regular check-ups for five years due to the presence of a submucosal tumor that was found in the fundus of the stomach incidentally. EUS-FNA was performed to evaluate the tumor, which had increased in size from 1 cm to 2.8cm. A cytologic smear revealed cohesive sheets or clusters of spindle cells with elongated nuclei. Immunohistochemical staining revealed a strong positive reaction for c-kit and CD34, without any reaction for smooth muscle actin and Ki-67. Therefore, a diagnosis of GIST was made.

  • Objective : This study was carried out to analyze the trends of ophthalmology related articles that have been published in the Journal of Korean Medical Ophthalmology, Otolaryngology, Dermatology(JKOOD). Method : We studied 72 research papers relevant to Ophthalmology that published in JKOOD from 1988 to 2013. All the paper were classified into three categories; original article, review article and case report. We also analyzed the articles according to the sort of diseases. We sub-classified the original articles into three categories descriptive study, analytic study and experimental study. We analyzed the case report according to various types of treatment. Result : 1. The number of searched journals is 72 papers; 21 review articles, 18 original articles, 33 case reports 2. Classification of 18 original articles into three categories; 12 descriptive studies, 0 analytic study, 6 experimental studies. 3. Distribution of diseases in ophthalmology; the percentage of 'strabismus' ranked the highest(25%) in case report. 4. Method of Evaluation in retina disease ; the percentage of fundus photography ranked the highest(50%) Conclusions : It is needed to develop the methods of study in part of eye disease. The studies using modern medical equipment should be increased in ophthalmology of Korean Medicine. It will be helpful to obtain objectivity and increase quality of studies.

Stomach Cancer Surgery after Coronary Artery Bypass Surgery with in situ Right Gastroepiploic Artery Graft (정위 우위대망동맥을 이용하여 관상동맥우회술을 시행한 환자에서의 위암수술)

  • A 59 year-old male diagnosed as unstable angina underwent off-pump coronary artery bypass surgery using in situ left internal mammary and right gastroepiploic artery grafts. During harvesting the right gastroepiploic artery, there was no abnormal finding in intraabdominal organs including stomach and liver. He was discharged at the 3rd postoperative day without complication. In case of using in situ right gastroepiploic artery, we recommend gastrofberscopic study at regular follow-up, The patient underwent the gastrofiberscopic study at postoperative 3rd month and diagnosed as advanced gastric cancer on the posterior wall of gastric fundus. At 5th postoperative month, total gastrectomy without intraoperative injury of the right gastroepiploic artery was performed at the department of general surgery. He was discharged at the 9th postoperative day. Follow-up coronary angiography performed at the 1st postoperative year demonstrated patent grafts including right gastroepiploic artery.

A Case of Belsey Mark IV Fundoplication after Failed Nissen Antireflux Surgery (실패한 Nissen 술을 Belsey Mark IV 위바닥주름술로 교정한 1예)

  • Antireflux surgery has been indicated in gastroesophageal reflux disease (GERD) that does not respond to medical treatments. Although the most commonly performed operation is Nissen fundoplication, Belsey Mark IV fundoplication is indicated for more complicated cases, such as, in cases of a failed Nissen operation or a long lasting hiatal hernia. Here, we report a case of Belsey Mark IV fundoplication for a failed Nissen fundoplication. The infant developed frequent times of aspiration pneumonia after initial Nissen for a hiatal hernia with GERD during the newborn period. At 15 months of age, a $2^{nd}$ Nissen operation was attempted, but fundoplication was not available because of excessive mesenteric adherence to the liver and cardia. Therefore, Belsey Mark IV fundoplication was performed via trans-thoracic approach, which can provide full esophageal mobilization and better visualization of the herniated fundus and the surrounding tissues. Subsequently, she has shown an improved general condition without GERD.

Arterial Stiffness is Associated With Diabetic Retinopathy in Korean Type 2 Diabetic Patients

  • Objectives: We evaluated the association between common carotid artery intima-media thickness (CCA-IMT), brachialankle pulse wave velocity (baPWV), carotid plaque, and peripheral arterial disease (PAD) as indicators of macroangiopathy and diabetic retinopathy as an indicator of microangiopathy in type 2 diabetic patients. Methods: We analyzed 605 type 2 diabetic patients registered at a public health center in Korea. Following overnight fasting, venous blood and urine samples were collected and analyzed. The CCA-IMT, levels of carotid plaque, baPWV, and ankle-brachial index (ABI) of the subjects were assessed. We used non-mydriatic fundus photography to diagnose diabetic retinopathy. Multiple logistic regression analyses were used to evaluate the association between macroangiopathy and diabetic retinopathy. CCA-IMT and baPWV were divided into tertiles: CCA-IMT, 0.39 to 0.65 mm, 0.66 to 0.78 mm, and 0.79 to 1.30 mm; baPWV, 9.9 to 15.8 m/s, 15.9 to 18.9 m/s, and 19.0 to 38.0 m/s. Results: The association between baPWV and diabetic retinopathy remained significant after adjustment, with an increasing odds ratio (OR) in the second tertile (OR, 2.41; 95% confidence interval [CI], 1.27 to 4.55) and the third tertile (OR, 4.63; 95% CI, 2.33 to 9.21). No significant differences were observed in carotid plaque, PAD, and each tertile of CCA-IMT. Conclusions: BaPWV was associated with diabetic retinopathy, while CCA-IMT, carotid plaque, and PAD were not. This study suggests that the association between macroangiopathy and microangiopathy may be attributable to functional processes rather than structural processes within the vascular system.

Treatment Planning Correction Using MRI in the Radiotherapy of Cervical Cancer (자궁 경부암의 방사선 치료계획에서 자기공명 영상을 이용한 조사야 교정)

  • Purpose: To evaluate the role of MRI in the management of cervical cancer treated by conventional four-field whole pelvic irradiation. Method and material: Between 1993-march and 1994-february, 20 patients(4 Stage I B, 3 Stage II A, 13 Stage II B) with invasive cervical cancer were eligible for evaluation of accuracy of conventionally designed lateral treatment field without MRI determination. Results: 5 out of 20 Patients had inadequate margin without MRI. The position of uterine fundus was more important than cervix in correction of field size and the center of treatment field. Conclusion: This Preliminary data show MRI determination of uterine position prior to radiotherapy planning is essential in the case of four-field whole pelvic irradiation technique.
