A Case of Neonatal Tetanus Cured with Neuromuscular Blocking Agent and Ventilatory Support (근이완제 및 인공 환기요법으로 치료한 신생아 파상풍 1례)

  • A neonate born at 38 gestational weeks was admitted due to generalized tonic-clonic seizure and cyanosis. The neonate was born six days previously at home through normal delivery and the umbilical cord was cut using scissors sterilized in boiling water. The neonate weighed 3,180 g at admission. Physical examination revealed cyanosis, opisthotonus, trismus and reactive muscle spasms. Laboratory exam, brain sonogram and EEG showed no significant abnormal findings. Based on her history and physical examination, the neonate was diagnosed with tetanus and put in an incubator isolated in a quiet, dark room. Treatment with tetanus human immunoglobulin along with antibiotics (penicillin G) were started immediately, and mechanical ventilation, administration of neuromuscular blocking agent and muscle relaxant were also started off. The frequency of seizure episode decreased gradually, and on the 32nd hospital day, mechanical ventilatory support was stopped along with extubation two days later. Thereafter, the neonate was in continuous generalized hypertonic state and showed feeding difficulty, but there was gradual improvement. She was dismissed on the 49th hospital day and is currently under OPD follow-up, doing well with no special problems. Respiratory management is critical to neonatal tetanus. We report here a case of tetanus treated with inhibition of self-respiration, neuromuscular blocker and application of ventilator, and present this method as a useful direction for future treatment of neonatal tetanus.

Multi-level Load Shedding Scheme to Increase Spatial Data Stream Query Accuracy (공간 데이터 스트림 질의 정확도 향상을 위한 다단계 부하제한 기법)

  • In spatial data stream management systems, it is needed appropriate load shedding algorithm because real-time input spatial data streams could exceed the limitation of main memory. However previous researches, lack regard for input ratio and spatial utilization rates of spatial data streams, or the characteristics of data source which generates data streams with spatial information efficiently, can lead to decrease the performance and accuracy of spatial data stream query. Therefore, multi-level load shedding scheme for spatial data stream management systems is proposed to increase the spatial query performance and accuracy. This proposed scheme limits overloads in relation to the input rate and the characteristics of data source first, and then, if needed, query data representing low query participation probability based on spatial utilizations are dropped relatively. Our experiments show that the proposed method could decrease load shedding frequency for previous researches by more than 11% despite query results accuracy and query performance are superior at 0.04% and 3%.

  • Though this study examines closely relation of sphere school register special qualities and participation degree, participation motive and mind health that is society based on person and model virtue study finding and the construction enemy about study of effect, woman's. swimming participation and mind satisfaction which exercise gets in company employee's mind health in the morning before attendance positively, there is main purpose. The subject who answered to given questionaire was 298 persons. To analyze collected data, used statistical technique statistical analysis technique anova analysis. Through such method of study and formality, this study deduced following result. First, adult woman's participation among by thing which participation frequency was mind health and difference that keep in mind statistically. Second, adult woman's participation among by thing which participation period was mind health and difference that keep in mind statistically Third, adult woman's participation among by thing which participation strength was mind health and difference that keep in mind statistically.

  • This paper discusses how to control the compensation voltages in dynamic voltage restorers (DVR). On analyzing the power circuit of a DVR system, control limitations and control targets are presented for the voltage compensation in DVRs. Based on the preceded power stage analysis, a novel controller for the compensation voltages of DVRs is proposed by a feed forward control scheme. This paper discusses also the time delay problems in the control system of DVRs. Digitally controlled DVR systems normally have control delay at amount of one sampling time of the control system and a half of the switching period of the DVR inverter. The control delay in digital controllers increases the dimension of the system transfer function one degree higher, which makes the control system more complicate and more unstable. This paper proposes a guide line to design the control gain, appropriate output filter parameters and inverter switching frequency for DVRs with digital controllers. Proposed theory is verified by an experimental DVR system with a full digital controller.

  • The Loose Part Monitoring System(LPMS) has been designed to detect, locate and evaluate detached or loosened parts and foreign objects in the reactor coolant system. In this paper, at first, we presents an application of the back propagation neural network. At the preprocessing step, the moving window average filter is adopted to reject the low frequency background noise components. And then, extracting the acoustic signature such as Starting point of impact signal, Rising time, Half period, and Global time, they are used as the inputs to neural network. Secondly, we applied the neural network algorithm to LPMS in order to estimate the mass of loose parts. We trained the impact test data of YGN3 using the backpropagation method. The input parameter for training is Rising Time, Half Period, Maximum amplitude. The result showed that the neural network would be applied to LPMS. Also, applying the neural network to the Practical false alarm data during startup and impact test signal at nuclear power Plant, the false alarms are reduced effectively. 1.

Effects of Lower Extremity Exercise Program on Muscle Strength and Flexibility in Women with Knee Osteoarthritis (슬 골관절염 여성을 위한 하지 운동프로그램이 근력과 유연성에 미치는 효과)

  • Purpose: The purpose of this study was to determine the effects of a lower extremity exercise program on muscle strength and flexibility in women with knee osteoarthritis. Method: This study was conducted using a non-equivalent control group pretest-posttest design. The participants were 35 women with knee osteoarthritis. The experimental group performed the lower extremity exercise program, which was an adapted BeHaS exercise program developed by Kim(2006). It consisted of approximately 65-130 minutes per one session, two days a week, for twelve weeks. Lower extremity muscle strength was measured by the Seated Leg Press Machine (TY-H2217, made in Korea), back muscle strength by back muscle strength scale (TKK 5402, made in Japan) and flexibility, by Sit and Reach tester (01285, U.S.A). Data were gathered from September 2005 to March 2006. Frequency, $X^2$ test and t-test were used to analyze the data. SPSS Win 11.0 was used to assist analysis. For normal distribution of data, Kolmogorov-Smirnov test was done. Results: There were statistical differences in the mean strength of both legs together, strength in leg with more pain(p=.044; p=.046), sit and standing time (p=.007), and back strength (p=.021). Conclusion: Based on these results, it can be concluded that a lower extremity exercise program can help improve lower extremity muscle strength in women with osteoarthritis.

The Clinical Study of Maxillofacial Bone Fracture (안면골 골절에 관한 임상적 연구)

  • This study is based on 247 patients(348 cases) with facial bone fracture who were admitted to the department of oral and maxillofacial surgery, Kangnam sacred heart Hospital during the period of Jan. 1983 through sep. 1988. The patients with mandible fracture were analysed with the following items such as complications with treatment method undertaken : Bicortical osteosynthesis vs Monocortical osteosynthesis, advantages and Disadvantages. The results obtained are as follows : 1) The most common age groups for the facial bone fracture were 20 years 2) The ratio of Men to women was 5 : 1 3) The most frequent location of facial bone fractures was the Mandible(76.7%), Maxillae(10.6%), Zygoma(9.5%), Nasal bone(3.2%) 4) The ratio of Mandible to Maxillae was 7.2 : 1 5) In mandible fractures, the most frequent fracture site was Symphysis(48.7%) 6) In maxilla fractures, the most frequent case was Le fort III fracture(51.4%), followed by Le fort I fracture(29.7%), Le fort II fracture(18.9%) 7) The main contributing causes of facial bone fractures were Traffic Accident(37.7%), Fight(31.6%), Accident(27.5%) 8) In open reduction methods of mandible fracture, the frequency of postop, malocclusion was 15% in interosseous wiring, 7% in Bicortical osteosynthesis, 3% in Monocortical osteosynthesis. The miniplates show advantages over other forms of fixation, in that they are malleable and easy to insert, they achieve rigid fixation and stability.

A Clinical and Statistical Study on Maxillofacial Fractures. (악안면골(顎顔面骨) 골절환자(骨折患者)의 임상(臨床) 통계적(統計的) 연구(硏究))

  • This is a retrospective study on maxillofacial fractures. This study was based on a series of 442 patients with maxillofacial fractures treated at Dept, of oral and Maxillofacial Surgery, College of Dentistry, Chon Buk National University from Jan, 1984 to Sep. 1988. The results obtained were as follows: 1. The ratio of Male/Female was 4.8 : 1, and 3rd decade (43.9%) was the highest age group in incidence. 2. Monthly incidence was the highest in Oct,(10.6%). 3. The most frequent maxillofacial fracture site was mandible (70.0%), and zygoma & zygomatic arch (13.6%), maxilla(11.7%) and nasal bone (4.7%) were next in order of frequency. 4. Traffic accidents (47.5%), fight(24.8%) were the most common causes of maxillofacial fractures. 5. The most frequent chief complaint was painful swelling(40.7%). 6. In mandibular fractures, the most frequent fracture site was symphyseal area(28.9%) and simple fracture was the most frequent in type of fracture (71.2%). 7. In maxillary fractures, fracture with other facial bones (64.5%) was more frequent than fracture of maxilla only. The most common type of fracture was unilateral fractures(37.1%). 8. In fracture of zygoma complex, zygoma fracture was the most frequent fracture type(40.3%), zygoma and zygomatic arch fx, (30.6%), zygomatic arch fx, (29.1%) were next in order 9. Open reduction was major method of treatment in maxillofacial fractures : Mandible (77.5%), Maxilla (61.3%), Zygoma complex(43.1%). 10. Maxillofacial fractures were most frequently combined with head injury(39.3%), and lower extremities(17.0%), upper extremities(13.6%) were next in order.

The Impact of the Attributes of Fashion Brand Online Communities on High School Students' Commitment to the Community (패션 브랜드 온라인 커뮤니티 특성이 고등학생의 커뮤니티 몰입에 미치는 영향)

  • The purpose of this study was to clarify the factors that characterize the fashion brand online communities that have a high participation rate among high school students, and to determine the impact of those factors on the high school students' commitment to the community, especially in relation to their demographics. The study was implemented through a descriptive survey method using an online questionnaire. The sample consisted of 301 members of online fashion brand communities operated on the Daum portal site. The data were analyzed by frequency, factor analysis, and regression analysis. The results are as follows: (1) Factor analysis revealed that the characteristics of a fashion brand online community are associated with three factors ; compensation/interest, interactivity, and information. (2) Regression analysis showed that the more compensation/interest, opportunity for interaction, and information the community provides, the more likely high school student members are to show a higher level of commitment to the community. (3) There was no difference in the effect of community attribute factors on the high school student members' commitment to the community according to gender. The group of students majoring in human and liberal education was the most influenced by the attributes of fashion brand online communities. The interactivity factor had a greater impact on the group of students majoring in art and physical education than on the other groups. The greater the use of the Internet, the greater the impact of community attributes on commitment. (4) In the process of developing an online community, marketers should fully consider the fact that information factors have the biggest impact on high school students' commitment to the community. Therefore, greater effort should be made to differentiate between community attributes based on high school students' majors and their amount of Internet use in order to motivate commitment to the community and brand loyalty as well.

Geoacoustic Model at the SSDP-105 Long-core Site of the Ulsan Coastal Area, the East Sea (동해 울산 연안해역 SSDP-105 심부코어 지점의 지음향 모델)

  • Geoacoustic model comprises physical and acoustic properties of submarine bottom layers influencing sound transmission through sea water and underwater. This study suggested for the first time that we made a geoacoustic model of long-coring bottom layers at the SSDP-105 drilling site of the Ulsan coastal area, which is located in the southwestern inner shelf of the East Sea. The geoacoustic model of 52 m depth below seafloor with three-layer geoacoustic units was reconstructed in the coastal sedimentary strata at 79 m in water depth. The geoacoustic model was based on the data of a deep-drilled sediment core of SSDP-105 and sparker seismic profiles in the study area. For actual modeling, the geoacoustic property values of the models were compensated to in situ depth values below the sea floor using the Hamilton modeling method. We suggest that the geoacoustic model be used for geoacoustic and underwater acoustic experiments of mid- and low-frequency reflecting on the deep bottom layers in the Ulsan coastal area of the East Sea.
