Polymelia with an extra forelimb at the thoracic regioo in Korean native calf (한우(韓牛) 송아지의 흉부(胸部)에 여분의 전지(前肢)가 부착(附着)된 다지기형(多肢畸形))

  • An abnormal female Korean native calf at five months age with five legs was observed macroscopically and radiographically. External features included two normal forelimbs, and hindlimbs, and an abnormal forelimb which was underdeveloped. A rudimentary forelimb, which was attached to the pectoral region, was observed at the right axillary part. The extra forelimb was scapural and humeral meromelia. It consisted of an underdeveloped forearm (antebrachium), several carpal bones (three bones), large metacarpal bone (Mc. 3 + 4), and two digits with two hoofs. The proximal, middle and distal phalanges, proximal and distal sesamoid bones are normal.

  • Environmental Enrichment (EE) alone is not capable of enhancing the fine digit and the forelimb functions. Therefore, we applied modified constraint-induced movement therapy (mCIMT) under the influence of EE to assess its effect on promoting improved forelimb sensorimotor functions. Focal ischemic brain injury was produced in Sprague-Dawley rats (60 rats, $250{\pm}50$ g) through middle cerebral artery occlusion (MCAO). Before MCAO induction, all rats were trained in modified limb placing tests and reaching tasks for 1 week. Then they were randomly divided into three groups: Group I: application of standard environment (SE) after MCAO induction (n=20), Group II: application of EE after MCAO induction (n=20), Group III: MCAO+EE, mCIMT and task-oriented training that was initiated at 10th day after MCAO induction (n=20). We also applied mCIMT (between 9 AM and 5 PM/daily) which included restraining the forelimb ipsilateral to the lesion using the 'Jones & Schallert' method. We assessed the change of modified limb placing, single pellet reaching test and the immunoreactivity of BDNF by immunohistochemistry (pre, 1st, 5th, 10th and 20th day). Group I showed no improved outcome, whereas group II and III significantly improved on the use of the forelimb and the immunoreactivity. The qualitative analysis of the skilled reaching test, of group III showed the greatest improvement in the fine digit and the forelimb function. These results suggest that EE combined with mCIMT is more functional in promoting enhanced fine digit and forelimb functional movements.

Kinetic gait analysis in a small sized dog with congenital shoulder luxation (소형견의 선천성 어깨관절 탈구에 관한 운동역학적 보행 분석)

  • This study was conducted to find out the compensation strategy through kinetic gait analysis by comparing dog with congenital luxation of the shoulder joint and normal dog. Ground reaction forces were recorded for all limbs while normal poodle dog and poodle dog with shoulder joint luxation was allowed to walk on an instrumented platform. The dogs were evaluated for maximal vertical force (MVF), body load distribution (BLD), and symmetry index (SI). The MVF was increased in the contralateral forelimb of luxated shoulder joint. The SI was also increased in a dog with dislocated shoulder joints in the forelimbs. For BLD, the maximum load distribution increased centrally, but the total load distribution decreased in the ipsilateral forelimb paw. In contrast, total load distribution was increased in the contralateral forelimb paw. During forelimb lameness, changes in weight-bearing load showed compensatory load redistribution. These biomechanical changes may lead to changes in the musculoskeletal system in a dog with luxated shoulder.

  • An abnormal Korean native cow of five years old with an extra leg was observed macroscopically and radiographically. The results were summarized as follows. 1. External features included two normal forelimbs and hindlimbs, and an extra abnormal forelimb which was underdeveloped. The extra forelimb attached to the regions of back on right scapula. 2. The extra forelimb had antebrachial meromelia. It consisted of undeveloped and severely deformed scapula and humerus, fused carpal bones, imperfectly duplicated metacarpal bones, and polydactyly. The polydactyly was consisted of seven rows of digits with seven hooves, and a rudimentary hoof of dewclaw. 3. The ectopic limp was devoid of muscular tissue.

Diagnostic Radiography of Premature Closure of Distal Growth Plate of Ulna in a Dog (개에서 척골의 원위 성장판 조기 폐쇄증에 대한 방사선학적 평가)

  • A 11 month-old Shihtzu was referred to the Veterinary Medical Teaching Hospital, Seoul National University. Clinical signs of this patient were lameness, shortening limb, angular deformity, rotation of foot, subluxaion of elbow joint and restricted range of movement of left forelimb. For the evaluation of the abnormalities of left forelimb, radiographic examination was carried out. Radiographic findings were characteristics of premature closure of distal ulna such as closure of distal ulna growth plate and cranial bowing of radius. With radiographic signs and physical examination, it was diagnosed as premature closure of growth plate of the left forelimb. After osteotomy of the radius and ostectomy of the ulna, radiographic evaluation of limb about angulation of elbow joint was performed every 2-3 weeks for 3 months. In case of premature closure of distal growth plate of ulna, radiography was very useful for diagnositic method of premature closure of distal ulna and monitoring of healing process.

Humeral Hemimelia in a Holstein Calf (유우 송아지의 선천성 상완골 형성 부전)

  • Humeral hemimelia is a rare congenital anomaly characterized by deficiency of the humerus with relatively intact scapular. The holstein calf was healthy expecting the deformity of forelimb humerus. The examination of PCR and serum neutralizing test was carried about BVD, akabane virus, Ibaraki virus and Kasba virus in cattle. The results were all negative. Affected calf showed forelimb deformation. Other abnormalitis was not observed in brain, spinal cord, liver, kidney and other visceral organs. Amelia are very rare limb anomalies. We report on a Holstein calf with forelimb deficiencies. This case appears humeral anomalies with ulnar and radius aplasia in calf. The condition in these calves was considered the result of a recurrence of a genetic mutation affecting a putative hemimelia locus.

Fragmented Medial Coronoid Process in a Korean Jindo (진도개에 발생한 주관절 내측갈고리돌기 단열)

  • A 9-month-old castrated male Korean Jindo was presented for evaluation of a progressive left forelimb lameness of 4 weeks' duration. On physical examination, the dog showed moderate weight bearing lameness on the left forelimb. Firm and mild swelling was palpated, and range of motion was decreased in the left elbow. Signs of pain were elicited by gentle flection and hyperextension of the left elbow. On radiographs, fragmentation of the medial coronoid process with moderate secondary degenerative joint disease was found. The bone fragments could be seen more clearly on stress radiograph of the left elbow joint which was taken while flexing the elbow and inwardly rotating the antebrachium. A medial elbow arthrotomy was performed, and the loose fragments were removed from the coronoid region. Left forelimb lameness improved markedly after surgery.

A Case of New Surgical Correction of Angular Limb Deformities Using One Screw Implant & Periosteal Transection in a Thoroughbred Foal (Thoroughbred 망아지에서 단일나사못 장착 및 골막박리를 이용한 지세교정술)

  • Angular limb deformities (ALD) are common in foals. A 30-days-old Thoroughbred foal was presented for the evaluation of severe ALD of the both forelimbs. On radiographic examination, both distal radiuses were diagnosed as valgus angular limb deformities. But the degree of deviation of right forelimb was so severe that we tried to correct one after the other. We tried new surgical correction method combination of one screw implant on medial aspect for growth retardation and periosteal transection on lateral aspect of the right forelimb. 40 days later, successfully corrected and then removed the screw. After the right forelimb correction, the periosteal transection on left forelimb was performed. We did the inhalation anesthesia using isoflurane. There were no complications such as fibrosis over the screw heads, and overcorrection that produces an opposing deformity identified. These results suggest that combination of one screw implant and periosteal transection technique is able to be a safe and effective method to correct severe ALD in the foal.

Hemangiopericytoma Treated by Surgery with Radiation Therapy in a Dog (개의 혈관외피세포종의 수술적 및 방사선요법 치료례)

  • An 11 kg, 7-year-old male pug dog with lameness and tumor of the 2-3rd interdigital portion of the right forelimb was referred to the Veterinary Medical Center of the Tokyo University. On the clinical examination findings, the tumor size was $3{\times}3 cm but alopecia and necrosis were not found. On the radiological findings, a bone lysis was found in the phalanges of digit II of the right forelimb but pulmonary metastasis was not found. The mass was removed with metacarpal bone. Histopathological examination of the mass revealed highly differentiated hemangiopericytoma. Two weeks after the operation, the dog was irradiated by orthovoltage radiation. The source of irradiation was 300 kV, 4mA, 4Gy and a focal spot to skin distance of 40 cm using 1.0 Cu, 1.0Al filter. The radiation therapy was performed twice a week for 5 weeks. The dog showed no recurrence and no metastasis. It was thought that the surgery and radiation therapy treatment was quite a useful method to treat a canine heman-giopericytoma.

Osteosarcoma of the Proximal Humerus in a Dog (개의 상완골 근위단에 발생한 골육종)

  • A 13-year old male dog with right forelimb lameness, invisible swelling, anorexia and mild depression was referred to Veterinary leaching Hospital, College of Veterinary Medicine, Kyungpook National University. On forelimb radiographic views, periosteal and endosteal proliferation and cortical lysis and extremely aggressive and amorphous periosteal reactions were located along the metaphyseal region of the proximal humerus. Cytologic and histopathologic examinations after amputation showed anaplastic mesenchymal cells that produced osteoid changes, poorly differentiated osteoblast and osteoclast. And also, it included the neoplastic cells with large nucleoli, multiple nucleoli that differ in size and cytoplasm with several clear vacuoles. Based on the physical examination and laboratory, radiolographic, cytologic and histopathologic findings, this case was diagnosed as osteosarcoma of the proximal humerus.
