폐의 경화성 혈관종의 세침흡인 세포학적 소견 - 5예 보고 - (Fine Needle Aspiration Cytology of the Sclerosing Hemangioma of the Lung - A Report of Five Cases -)

  • Sclerosing hemangioma is a rare, benign neoplasm of the lung, usually presented as a solitary pulmonary nodule in an otherwise asymptomatic middle-aged woman. Cytologically it shows papillary, sheet, and cyst-like arrangements representing three main histologic patterns of papillary, solid, and angiomatous ones, respectively. Herein, we report the fine needle aspiration cytology of 5 cases of sclerosing hemangioma of the lung. The most characteristic finding is cyst-like spaces Intimately related to the papillary or solid cell nests. The tumor cells are relatively monotonous round to oval, small to medium in size. They have small amount of eosinophilic cytoplasm. The nuclei are uniform, round and have small but conspicuous nucleoli. The tumor cells in one of the presenting cases are large with abundant cytoplasm and show moderate nuclear pleomorphism. The nuclear chromatin, however, is fine and even without exception, even in the case showing nuclear pleomorphism. Major differential diagnoses based on the cytologic findings are well-differentiated papillary adenocarcinoma, bronchioloalveolar carcinoma, and carcinoid tumor.

유방에 생긴 과립세포종양의 세포 소견 -1예 보고- (Fine Needle Aspiration Cytology of Granular Cell Tumor in Breast -A Case Report-)

  • Granular cell tumor (GCT) of the breast is a rare clinical entity, and is believed to be of schwannian origin and to follow a benign clinical course. A 50-year-old woman presented with a slowly growing mass in the right breast. Fine needle aspiration cytology revealed a cellular smear containing isolated or clustered cells showing round to oval nuclei with abundant oncocytic granular cytoplasm. Nuclei showed a fine granular chromatin pattern and occasional small single nucleoli. Cell boundaries were poorly defined, and naked nuclei were frequently found, Histologically, the tumor showed features of typical GCT, and immunohistochemical staining findings strongly supported the diagnosis. The present study demonstrates that GCT of the breast can mimic malignant lesions of breast both clinically and radiologically. The recognition of its cytologic features and suspicion of this lesion would undoubtedly aid the correct diagnosis of mammary GCT.

갑상샘 세침흡인 세포검사와 조직검사의 진단 일치율 및 정확도 분석 -조직학적으로 확진된 322 예에 대한 분석- (Correlation and Accuracy Between Fine Needle Aspiration Cytology of Thyroid Lesions and Histopathologic Diagnosis -Analysis of 322 Histopathologically Confirmed Cases -)

  • Fine needle aspiration (FNA) cytology is the decisive test in the pre-operative diagnostics of thyroid nodules. Here we share our institutional experience about thyroid aspiration and give suggestions for suspicious cytology results. Three hundred twenty-two cases in 270 patients (mean age 47.4 years, 243 women and 27 men) who underwent thyroidectomy were reviewed. Among the 322 cases, the FNA diagnosis of "positive for malignancy" was 87 cases (27.0%), "suggestive of malignancy" 30 cases (9.3%), "suspicious for malignancy" 61 cases (18.9%), "negative for malignancy" 102 cases (31.7%), and "unsatisfactory smear" 42 cases (13.0%). Eighty seven cases (100%) out of "positive for malignancy", 29 cases (96.7%) out of "suggestive of malignancy", and 39 cases (64.0%) out of "suspicious for malignancy" were papillary carcinoma (148 cases, 95.5%), or follicular carcinoma (2 cases, 1.3%), or metastatic carcinoma (1 case, 0.6%). Seventeen patients who had only negative or unsatisfactory cytology underwent thyroidectomy and nine cases (52.9%) were papillary carcinoma. We suggest that: the cytology diagnosis of "suggestive of malignancy" could be regarded as "positive for malignancy", the cytology diagnosis of "suspicious of malignancy" should be carefully correlated with clinico-radiologic manifestation, and even a negative or unsatisfactory smear should be carefully followed up.

Spectrum of Cytological Findings in Fine Needle Aspiration Cytology of Breast Lumps with Histopathology Correlation: Experience in a Tertiary Care Rural Hospital in India

  • To determine the pattern of disease in patients presenting with breast lumps and to determine the sensitivity and specificity of fine needle aspiration cytology of benign and malignant diseases as a diagnostic tool by correlating with histopathology findings. This retrospective study was carried out in the Department of Pathology, Maharaja Agrasen Medical College, Agroha, from Jan 2008 to April 2012. Fine needle aspiration cytology was performed on 370 cases and out of these 52 cases were received in the Department for histopathological examination. Fibroadenoma was the most common disease encountered, in 88 (24%), with a peak incidence in second and third decade of life. Fibrocystic disease was second, being common in the third and fourth decades of life. Peak incidences of duct ectasia, granulomatous and tubercular mastitis were seen in the third decade. Gynaecomastia showed two peak incidences in second and sixth decades of life. Out of total 370 cases undergoing fine needle aspiration, benign cases were 316 (85.4%), malignant and suspicious were 54 (14.6%) and 10 (2.70%) respectively. Out of total 22 histological confirmed malignant lesions 19 were interpreted as malignant cytologically while two as suspicious and one as benign. All thirty histologically confirmed benign cases were diagnosed as benign cytologically. The sensitivity, specificity, positive and negative predictive values were 98%, 100%, 100% and 96.4% respectively. FNA cytology is highly accurate for diagnosis of breast masses. However, the clinician should correlate FNA cytological results with physical examination and imaging findings to prevent false negative and false positive events and to obtain optimal management of their patients.

  • The purpose of this study was to evaluate the prevalence of thyroid nodules and malignant findings of thyroid nodules in 1,954 patients (654 males and 1,300 females) aged 20 years or older who underwent thyroid ultrasound from January 2018 to December 2018. Examination of the thyroid gland was performed, and fine needle aspiration cytology was performed on the thyroid nodule. As a result, 108 (16.5%) out of 654 males and 368 (28.3%) out of 1,300 females showed higher prevalence than males. The prevalence of single nodules and multiple nodules in gender and age groups was significantly higher for women and for ages (male p=.001, female p=.001). There was a significant difference in males in the nodule size (p=.001) and no significant difference in females (p=.069). Fine - needle aspiration cytology of 476 patients with nodules was diagnosed as malignant in 46 patients (9.6%). Based on pathologic results, 383 benign and 93 malignant groups were analyzed. Ultrasonographic findings were as follows single nodule (p=.000), solid(p=.004), hypoechoic (p=.000), ill-defined peripheral boundary (p=.000), and calcification (p=.000), respectively. In the diagnosis of thyroid nodule, primary ultrasonographic findings through morphological classification of the nodules may reduce indiscriminate fine needle aspiration cytology in benign and malignant nodules.

  • We experienced a case of primary hepatic actinomycosis which was initially diagnosed by means of fine needle aspiration. The patient was a 31-year-old emaciated man with a 2-month history of 10 kg weight loss, right upper quadrant pain and flank pain. The liver was palpable and tender on physical examination. Computerized tomography scan of the liver showed two ill-defined hypodense masses in gallbladder fossa and inferior pole of right lobe. Hepatocellular carcinoma was clinically suspected. A CT-guided fine needle aspiration was peformed. Microscopically, smears showed numerous radiating clusters of filamentous bacteria with many neutrophils and monocytes in necrotic background. The symptoms were improved by incision and drainage and massive administration of penicillin.

  • Nocardia, aerobic members of the order of Actinomycetaceae, produces infections in human lung. Nocardial infection is associated with underlying diseases of immuno-suppression or treatment with corticosteroid. It is difficult to detect Nocardia by sputum examination or histologic sections and it has rarely been diagnosed by fine needle aspiration of the lung. We describe a case of pulmonary nocardiosis in a 72 year-old man, diagnosed by fine needle aspiration, which was confirmed by culture of aspirates. The aspirates showed neutrophil-predominant inflammatory cells with microorganisms demonstrated by Gomori methenamine silver and Gram stain. The organisms had characteristic long blanching filamentous structures. The lesions on chest X-ray were in resolution with antimicrobial therapy.

  • Hepatoblastoma (HB) is a rare embryonic malignant tumor of the liver. Most morphological studies on HB have limited to the histological characteristics and only 3 cases of HB have been described in the cytology literature. We present 2 cases of HB occurring in children aged 1 year and 3 years, respectively. The distinctive cytologic features of fine needle aspiration of HB were clusters of tumor cells showing acinar and trabecular pattern, smaller tumor cells with a high nuclear-cytopalsmic ratio and hyperchromatic nuclei having prominent nucleoli, and the presence of extramedullary hematopoiesis and osteoid material. These features were also found in the cell block and the biopsy specimen, and appeared very useful in the differentiation of HB from hepatocellular carcinoma.

  • A primary squamous cell carcinoma of the thyroid is extremely rare disease. We evaluated this disease by fine needle aspiration cytology in a 43 year-old female. On physical examination, the thyroid was moderately enlarged and palpated with hard consistency. The cytologic aspirate revealed nests of squamous cells, malignant squame and thyroid follicular cells in the necrotic background. We diagnosed this tumor as squamous cell carcinoma with cytologic evidence. Surgically resected thyroid revealed well differentiated squamous cell carcinoma. Immunohistochemical staining for antithyroglobulin proved that this tumor was primary in origin and clinical study confirmed this lesion to be of primary thyroid origin rather than metastasis or direct invasion from contiguous structures.

  • Acinic cell carcinoma is a rare salivary gland tumor of low-grade malignancy. It comprises only about 2.5% of all salivary gland tumors. We recently experienced a case of fine needle aspiration cytology of acinic cell carcinoma of the parotid gland. The characteristic cytopathologic features were 1) cellular aspirate consisting of monomorphic cells in large sheets or singly, 2) formation of acini and/or microcysts, 3) abundant granular cytoplasm with sharp cytoplasmic borders, 4) bland nuclei with micronucleoli, and 5) clear background.
