Effect of a 12-week weight management program on the clinical characteristics and dietary intake of the young obese and the contributing factors to the successful weight loss

  • BACKGROUND/OBJECTIVES: The objectives were to investigate the effect of a 12-wk intervention with behavioral modification on clinical characteristics and dietary intakes of young and otherwise healthy obese and to identify factors for successful weight loss. The goal was to lose 0.5 kg per week by reducing 300-500 kcal/day and by increasing physical activities. SUBJECTS AND METHODS: Forty four obese subjects (BMI > 25) and 19 normal weight subjects (BMI 18.5-23) finished the 12-week intervention. Obese subjects participated in 5 group educations and 6 individual counseling sessions. Normal weight subjects attended 6 individual counseling sessions for evaluations of dietary intake and exercise pattern. Anthropometric and clinical characteristics and 3-day dietary records were evaluated at baseline and week12. RESULTS: Weight and serum triglyceride and free fatty acid concentrations in obese group decreased significantly with intervention. Intakes of energy, fat, and cholesterol decreased significantly in the obese. Active participation, realistic weight loss goal setting, and weight gain after high school graduation not during childhood were identified as key factors for successful weight loss. CONCLUSIONS: The 12-week intervention with behavioral modification resulted in reduced energy and fat intakes and led to significant weight loss and improvements of clinical characteristics in the obese. The finding that those who became obese during childhood lost less weight indicates the importance of 'early' intervention.

Effects of Ruminal Infusion of Garlic Oil on Fermentation Dynamics, Fatty Acid Profile and Abundance of Bacteria Involved in Biohydrogenation in Rumen of Goats

  • This study aimed to investigate the effects of ruminal infusion of garlic oil (GO) on fermentation dynamics, fatty acid (FA) profile, and abundance of bacteria involved in biohydrogenation in the rumen. Six wethers fitted with ruminal fistula were assigned to two groups for cross-over design with a 14-d interval. Each 30-d experimental period consisted of a 27-d adaptation and a 3-d sample collection. Goats were fed a basal diet without (control) or with GO ruminal infusion (0.8 g/d). Ruminal contents collected before (0 h) and at 2, 4, 6, 8, and 10 h after morning feeding were used for fermentation analysis, and 0 h samples were further used for FA determination and DNA extraction. Garlic oil had no influence on dry matter intakes of concentrate and hay. During ruminal fermentation, GO had no effects on total VFA concentration and individual VFA molar proportions, whereas GO increased the concentrations of ammonia nitrogen and microbial crude protein (p<0.05). Compared with control, GO group took a longer time for total VFA concentration and propionate molar proportion to reach their respective maxima after morning feeding. The ratio of acetate to propionate in control reduced sharply after morning feeding, whereas it remained relatively stable in GO group. Fatty acid analysis showed that GO reduced saturated FA proportion (p<0.05), while increasing the proportions of C18, t11-18:1 (TVA), c9,t11-conjugated linoleic acid (c9,t11-CLA), t10,c12-CLA, and polyunsaturated FA (p<0.05). The values of TVA/(c9,t11-CLA+TVA) and C18:0/(TVA+C18:0) were reduced by GO (p<0.05). Real-time PCR showed that GO tended to reduce Butyrivibrio proteoclasticus abundance (p = 0.058), whereas GO had no effect on total abundance of the Butyrivibrio group bacteria. A low correlation was found between B. proteoclasticus abundance and C18:0/(TVA+C18:0) (p = 0.910). The changes of fermentation over time suggested a role of GO in delaying the fermentation process and maintaining a relatively modest change of ruminal environment. The inhibitory effects of GO on the final step of biohydrogenation may be related to its antibacterial activity against B. proteoclasticus and other unknown bacteria involved.

Development and validation of a quantitative food frequency questionnaire to assess nutritional status in Korean adults

  • This study was performed to evaluate the validity of the food frequency questionnaire (FFQ), which is being used at the Samsung Medical Center. In total, 305 (190 males and 115 females) participants consented and completed the 3-day diet records and FFQ. Age, gender and energy-adjusted and de-attenuated correlations ranged from 0.317 (polyunsaturated fatty acid) to 0.748 (carbohydrate) with a median value of 0.550. The weighted kappa value ranged from 0.18 (vitamin A) to 0.57 (carbohydrate) with a median value of 0.36. More than 75% of the subjects were classified into the same or adjacent quartiles. The FFQ had reasonably good validity compared with that of another study. Therefore, our FFQ is considered a proper method to assess nutrient intake in healthy Korean adults.

$\omega-6$ 다중불포화지방을 섭취한 백서에서 비타민 E보충이 인슐린저항성과 산화적 스트레스에 미치는 영향 (The Effect of Vitamin E Supplementation on Insulin Resistance and Oxidative Stress in Sprague Dawley Rats Fed High $\omega-6$ Polyunsaturated Fat Diet)

  • Background : Excessive intakes of $\omega$6 polyunsaturated fatty acid (PUFA) can increase oxidative stress, which may increase insulin resistance and could be the cause of metabolic syndrome X such as diabetes mellitus. One of the ways to reduce oxidative stress is the consumption of antioxidants such as vitamin E. It is controversial that vitamin E intakes may alleviate insulin resistance. The purpose of the study was whether high vitamin E intake may influence whole body glucose disposal rate(GDR), glycogen deposites, triglyceride content, lipid peroxide levels and antioxidant enzyme activities in Sprague Dawley rats fed high $\omega$6 PUFA diest. Methods : Sprague Dawley rats were divided into three groups. The control group consumed chow diet. High and low vitamin E groups consumed 40% PUFA of total energy intakes. One kilogram of diet mixture contained 300IU of $\alpha$-tocopherol in high vitamin E group, while it had 30 IU in low vitamin E group. Diets were given for 8 weeks. After 7 were of diet consumption, indwelling catheters were inserted in carotid artery and jugular vein of all rats so that GDR could be measured in awake and unstressed state. Results : Daily PUFA intakes were lower in the control group than others. Daily vitamin E intake of high vitamin E group was about ten times higher than those of low vitamin E group and the control group(p<0.0001). $\alpha$-tocopherol content in lier was highest in the high vitamin E group. GDR of the control group was 24% higher than others, and vitamin E intakes did not affect GDR. Glycogen deposit of liver in the control group was significantly higher than others, and it was not altered by vitamin E supplementation. Muscle glycogne content showed a similar tendency as liver glycogen in different diet groups. Triglyceride deposit in muscle was not different among groups. Lipid peroxide content of liver in the high vitamin E group was lower than the low of glutathione peroxidase were lowered in low vitamin E group than others, however, those of superoxide dismutase and catalase were not different. Conclusions : High vitamin E intakes can decrease oxidative stress in rats fed high (())-6 PUFA diet, but they cannot alleviate insulin resistance. Thus, increased oxidative stress through high (())-6 PUFA diet may be minimal for influencing insulin resistance.

비만아동의 영양섭취와 혈청 지질농도 및 철분영양지표 (Dietary Intakes and Serum Lipids and Iron Indices in Obese Children)

  • This study was performed to compare nutrient intakes, diet quality and serum indices (TG, Total cholesterol, LDL-cholesterol, HDL-cholesterol, RBC, Hb, Hct, Serum iron, ferritin, MCV) of obese and normal weight children. The subjects were 149 children living in the Seoul and Gyeonggi areas. Each subject was assigned to one of such as normal weight group ($15th{\le}BMI$ percentile < 85th, n = 82) or obese children ($85th{\le}BMI$ percentile, n = 67) according to their percentile of BMI by The Korean Pediatric Society. Data on dietary intakes, body composition and serum indices were obtained. Differences of all the above variables were assessed. Energy, carbohydrate, fat, cholesterol, total fatty acid, SFA, MUFA, PUFA, phosphate, potassium, zinc, vitamin $B_1$ and vitamin $B_6$ intakes of girls in the obese group were higher than those of the normal weight group (p<0.05). The intake of fiber was as insufficient as below 50% of KDRIs in both groups. Nutrient adequacy ratio of calcium, iron, phosphate, zinc and folate in obese boys were lower than those of normal weight boys (p<0.05). Energy intakes of grain and milk and dairy food in the obese girl group ($905.9{\pm}344.5kcal,\;210.9{\pm}166.4kcal$) were higher than those of normal weight girls ($671.2{\pm}360.7kcal,\;184.0{\pm}103.5kcal$) (p<0.05). HDL-cholesterol level of obese boys ($52.7{\pm}6.3mg/dL$) was significantly lower than that of normal weight boys ($65.3{\pm}15.6mg/dL$). Serum iron level of obese boys and girls (boys: $79.4{\pm}32.6mg/dL$, girls: $98.3{\pm}16.2mg/dL$) was significantly lower than those of normal weight children (boys: $104.8{\pm}38.6mg/dL$, girls: $106.7{\pm}28.9mg/dL$) (p<0.05). These results suggest that there should be sensible food selection and more intakes of fruit and vegetable are needed to improve the nutritional status in obese children.

Effect of Lipid Sources with Different Fatty Acid Profiles on Intake, Nutrient Digestion and Ruminal Fermentation of Feedlot Nellore Steers

  • The present study was conducted to determine the effect of lipid sources with different fatty acid profiles on nutrient digestion and ruminal fermentation. Ten rumen and duodenal fistulated Nellore steers (268 body weight${\pm}27kg$) were distributed in a duplicated $5{\times}5$ Latin square. Dietary treatments were as follows: without fat (WF), palm oil (PO), linseed oil (LO), protected fat (PF; Lactoplus), and whole soybeans (WS). The roughage feed was corn silage (600 g/kg on a dry matter [DM] basis) plus concentrate (400 g/kg on a DM basis). The higher intake of DM and organic matter (OM) (p<0.001) was found in animals on the diet with PF and WF (around 4.38 and 4.20 kg/d, respectively). Treatments with PO and LO decreased by around 10% the total digestibility of DM and OM (p<0.05). The addition of LO decreased by around 22.3% the neutral detergent fiber digestibility (p = 0.047) compared with other diets. The higher microbial protein synthesis was found in animals on the diet with LO and WS (33 g N/kg OM apparently digested in the rumen; p = 0.040). The highest C18:0 and linolenic acid intakes occurred in animals fed LO (p<0.001), and the highest intake of oleic (p = 0.002) and C16 acids (p = 0.022) occurred with the diets with LO and PF. Diet with PF decreased biohydrogenation extent (p = 0.05) of C18:1 n9,c, C18:2 n6,c, and total unsaturated fatty acids (UFA; around 20%, 7%, and 13%, respectively). The diet with PF and WF increased the concentration of $NH_3-N$ (p<0.001); however, the diet did not change volatile fatty acids (p>0.05), such as the molar percentage of acetate, propionate, butyrate and the acetate:propionate ratio. Treatments PO, LO and with WS decreased by around 50% the concentration of protozoa (p<0.001). Diets with some type of protection (PF and WS) decreased the effects of lipid on ruminal fermentation and presented similar outflow of benefit UFA as LO.

The Effects of a High-fat or High-sucrose Diet on Serum Lipid Profiles, Hepatic Acyl-CoA Synthetase, Carnitine Palmitoyltransferase-I, and the Acetyl-CoA Carboxylase mRNA Levels in Rats

  • The purpose of this study was to investigate the effects of altering relative intakes of fat and carbohydrates on serum lipid profiles, hepatic acyl-CoA synthetase (ACS), carnitine palmitoyltransferase-I (CPT-I), and the acetyl-CoA carboxlyase (ACC) mRNA level in Sprague-Dawley rats. For four weeks the rats were fed either an AIN-76 diet or one of its modified diets that were supplemented with 20% beef tallow (high-fat diet, HF) and 66.3% sucrose (highsucrose diet, HS). The HS group had significantly higher serum triglyceride and total cholesterol concentrations when compared with the other groups. Serum LDL-cholesterol concentrations in the HS and HF groups were significantly higher when compared to the normal diet (ND) group. Serum HDL-cholesterol levels of the ND and HS groups were significantly higher than those of the HF group. The hepatic total lipid level of the HF group was significantly higher than those of other groups; triglyceride levels of the HS and HF groups were significantly higher than those of the ND group. Hepatic ACS mRNA levels of the HF group were significantly higher than those of the ND group. Hepatic CPT-I mRNA levels were higher in the HF group than other groups. Also, ACC mRNA levels in the liver increased in the HF group. In conclusion, changes in the composition of dietary fat and carbohydrates could affect the hepatic ACS, CPT-I, and ACC mRNA levels. These results facilitate our understanding of the coordinated regulation of the ACS, CPT-I, and ACC mRNA levels and will serve to enhance our understanding of the molecular mechanisms that underlie the regulation of fatty acid metabolism.

한국산 야생식용식물이 당뇨 유발 흰쥐의 혈당과 에너지대사에 미치는 영향 (Effects of Korean Wild Vegetables on Blood Glucose Levels and Energy Metabolites in Streptozotocin Induced Diabetic Rats)

  • The hypoglycemic effects of five Korean wild vegetables, Aralia continentalis (A. con.), Castanea crenata (C. cre.), Xanthium strumarium (K, str.), Alisma canaliculatum (A. can,) and Eupatorium chinense var. simplicifolium for tripartium (E. tri) which have been utilized for the traditional remedies were investigated in this study. Diabetes mellitus was induced in male Sprague-Dawley rats by injections of streptozotocin (STZ) into the tail veins at a dose of 45 mg/kg. Five groups of STZ-induced diabetic rats were fed one of each experimental diet containing 10% of the Korean wild vegetable powder and normal and STZ-control rats were fed the control diet for five weeks. The body weight change, feed efficiency ratio (FER) and organ weights were compared. The plasma levels of glucose, protein, cholesterol, HDL-cholesterol, triglyceride, free fatty acid, and aminotransferase activity were determined. Mineral (Ca, K, Mg, Na, Cu, Fe, Mn and Zn) contents of the Korean wild vegetables were analyzed. The body weight gain was higher in normal, C. cre., A. can. and E. tri. groups than in the diabetic control group. The FER of C. cre., A. can. and E. tri. groups was significantly higher (p.<0.05) than that of diabetic control group. Liver weight was heavier in A. con., X. str. and A. can. groups compared with the diabetic control group. The weights of kidney were lighter in all five Korean wild vegetable groups than in the diabetic control group. After five weeks, the plasma glucose level tends to be decreased in A. con., A. can. and E. tri. groups. Plasma cholesterol level was decreased the Korean wild vegetables except for X. str. group. Plasma HDL- cholesterol level was significantly higher in A. con., A. can. and E. tri. groups compared with the diabetic control group. Plasma triglyceride and free fatty acid levels were significantly higher in X. str. group compared with the diabetic control group. Mineral contents were higher in E. tri. (Ca, K, Na and Fe). The results suggest that the intakes of A. con., A. can. and E. tri. have a hypoglycemic effect in diabetic rats showing the possibility as the valuable food resources for the prevention of diabetic mellitus.

식이 셀레늄 수준과 식이 지방산 조성이 쥐의 지질과 산화 상태와 조직의 지방산 조성에 미치는 영향 (Effect of Dietary Selenium and Fish oil on Lipid Peroxidation and Fatty Acid Profile in the Rat)

  • 식이 셀레늄(Se)의 수준과 식이지방산의 조성이 조직(혈장,동맥,간)의 지질과산화 상태와 조직의 지방산 조성에 미치는 영향에 대해 연구하였다. Sprague Dawley종 수컷쥐들을 식이 Se 수준(0.005mg/kg diet, 0.5mg/kg diet)과 식이 지방산의 주요 급원(옥수수유, 어유)에 따라 4군으로 나누어 8주간 실험하였다. 식이에 함유된 vitamin E의 함량은 117IU정도이다. Se 영양상태의 지표인 glutathione peroxidase 활성도와 Se 수준은 Se이 결핍된 식이를 공급시키면 식이지방 종류에는 상관없이 낮게 나왔다. 어유공급은 간과 동맥의 MDA를 증가시키나 Se을 공급시킨 쥐의 간 MDA수준은 감소되었다. 어유를 공급시키면 혈장 총치질, 동맥 과 간의 인지질에서 20:5(n-3), 22:5(n-3), 22:6(n-3), 20:3(n-6) 수준은 증가하였으며 20:4(n-6)수준은 감소되었다. 비록 Se공급이 n-3 지방산 수준 [22:6(n-3)]을 증가시키나 어유공급 효과에 비하면 그 효과가 적었다. 본 실험결과에 의하면 vitamin E가 현재 권장되는 양보다 충분히 많게 공급되어도 Se이 지질과산화를 방지하는데 중요한 역활을 하고 있으므로 적절한 Se공급이 바람직하다.

영양상담에 따른 심장병환자의 영양소 섭취 및 혈중지질의 변화 (The Effects of Nutrition Counseling on Food Intakes and Bloodlipids in Cardiac Patients)

  • The purpose of this study was to investigate whether nutrition counseling and exercise could be beneficial to patients with acquired cardiac disease (ACD). Twenty-five ACD patients participated in this program, which was based on guidelines for serum lipid management. To measure the effects of the nutrition counseling and exercise education, outpatients with ACD were selected and randomly assigned to three groups which were a control group, a diet only group and a diet and exercise group. Nine diet only group patients and nine diet and exercise group patients received nutrition counseling or nutrition counseling and exercise education every other week. Patients who served as a non counseled control group did not receive any counseling during the same study period. Various markers of disease risk, including lifestyle, anthropometric indices, eating behaviors, and serum lipid levels were measured before and after the program. The program lasted up to 12 weeks, depending on the individuals involved. Database management and statistical analyses were performed using SPSS 7.5 software. As a result, BMI and %IBW showed decreased trends in the diet only and the diet and exercise group. Food habit scores were significantly increased in the diet only group and the frequency of saturated fatty acid, dietary cholesterol, salty food and instant food intake were decreased in the diet only group. Comparisons of the daily nutrient intakes of the groups showed their total calorie, carbohydrate and protein intake had decreased significantly, and also the total fat and dietary cholesterol intake had decreased in the nutrition counseling group. The serum total cholesterol and LDL-cholesterol decreased after 12 weeks in the nutrition counseling group. The diet and exercise group showed less interest in diet control than the diet only group. These results show that a well-planned nutrition counseling program would reduce the risks of ACD and cardiovascular disease and help to care such diseases.
