Objectives: The association between obstructive sleep apnea (OSA) and mortality or serious cardiovascular events over a long period of time is not clearly understood. The aim of this observational study was to estimate the clinical effectiveness of continuous positive airway pressure (CPAP) treatment on an outcome variable combining mortality, acute myocardial infarction (AMI), and cerebrovascular insult (CVI) during a follow-up period of 15.5 years ($186{\pm}58$ months). Methods: The data set consisted of 978 patients with an apnea-hypopnea index (AHI) ${\geq}5.0$. One-third had used CPAP treatment. For the first time, a data-driven causal Bayesian network (DDBN) and a hypothesis-driven causal Bayesian network (HDBN) were used to investigate the effectiveness of CPAP. Results: In the DDBN, coronary heart disease (CHD), congestive heart failure (CHF), and diuretic use were directly associated with the outcome variable. Sleep apnea parameters and CPAP treatment had no direct association with the outcome variable. In the HDBN, CPAP treatment showed an average improvement of 5.3 percentage points in the outcome. The greatest improvement was seen in patients aged ${\leq}55$ years. The effect of CPAP treatment was weaker in older patients (>55 years) and in patients with CHD. In CHF patients, CPAP treatment was associated with an increased risk of mortality, AMI, or CVI. Conclusions: The effectiveness of CPAP is modest in younger patients. Long-term effectiveness is limited in older patients and in patients with heart disease (CHD or CHF).
Purpose: Near drowning refers to immediate survival after asphyxia due to submersion or immersion in water, which is a crucial public safety problem worldwide. Acute lung injury or acute respiratory distress syndrome (ARDS) is a common complication of near drowning. The purpose of this study was to investigate the feasibility and effectiveness of noninvasive nasal positive pressure ventilation (NINPPV). Methods: This retrospective study was conducted at a tertiary emergency department. NINPPV was administered for moderate ARDS caused by submersion or immersion in patients who were older than 18 years, from January 2015 to December 2018. We collected the demographic (age, sex, length of hospital stay, and outcome), laboratory (arterial blood gas, lactate, oxygen saturation, partial pressure of oxygen divided by the fraction of inspired oxygen, complete blood count, blood urea nitrogen, and creatinine), and clinical data (acute lung injury index and ventilator failure) of the patients. A statistical analysis was performed using Statistical Package for the Social Sciences version 20.0 for Windows. Results: NINPPV treatment was provided to 57 patients for near drowning, 45 of whom (78.9%) were successfully treated without complications; in 12 (21.1%), treatment was changed to invasive mechanical ventilation within 48 hours due to ARDS or acute kidney injury. NINPPV treatment was successful in 31 (75.6%) out of 41 sea-water near drowning patients. They were more difficult to treat with NINPPV compared with the fresh-water near drowning patients (p<0.05). Conclusions: NINPPV would be useful and feasible as the initial treatment of moderate ARDS caused by near drowning.

Objective : Minimal data exist regarding non-operative management of suspected pseudarthrosis after pedicle subtraction osteotomy (PSO). This study reports radiographic and clinical outcomes of non-operative management for post-PSO pseudarthrosis at a minimum 5 years post-detection. Methods : Nineteen consecutive patients with implant breakage indicating probable pseudarthrosis after PSO surgery (13 women/six men; mean age at surgery, 58 years) without severe pain and disability were treated with non-operative management (mean follow-up, 5.8 years; range, 5-10 years). Non-operative management included medication, intermittent brace wearing and avoidance of excessive back strain. Radiographic and clinical outcomes analysis was performed. Results : Sagittal vertical axis (SVA), proximal junctional angle, thoracic kyphosis achieved by a PSO were maintained after detection of pseudarthrosis through ultimate follow-up. Lumbar lordosis and PSO angle decreased at final follow-up. There was no significant change in Oswestry Disability Index (ODI) scores and Scoliosis Research Society (SRS) total score, or subscales of pain, self-image, function, satisfaction and mental health between detection of pseudarthrosis and ultimate follow-up. SVA greater than 11 cm showed poorer ODI and SRS total score, as well as the pain, self-image, and function subscales (p<0.05). Conclusion : Non-operative management of implant failure of probable pseudarthrosis after PSO offers acceptable outcomes even at 5 years after detection of implant breakage, provided SVA is maintained. As SVA increased, outcome scores decreased in this patient population.

Composite motor cases fabricated by the filament winding method are structurally weak in the dome when they are required to withstand the internal pressure of the combustion gas. In this study, a finite element analysis is conducted to compare the burst pressure of a composite dome according to the variation of the pressure distribution ratio(PDR). The performance of the composite motor case was compared quantitatively by calculating the stress on the inner and outer dome surfaces and metal boss volume. As a result, the critical point of the failure mode was observed at a PDR between 2.5 and 3.0. A design at a PDR of 2.5-3.5 can reduce the weight of metal boss without fluctuation in the burst pressure of the combustion motor case. Moreover as the design reference value changes according to the dome shape and opening size, further analysis and testing are necessary.
Objective: The usual seminal profile has been customarily used for diagnosing male infertility based on an examination of semen samples. However, sperm DNA fragmentation has also been causally linked to reproductive failure, suggesting that it should be evaluated as part of male infertility assessments. To compare the ability of the five most widely utilized methodologies of measuring DNA fragmentation to predict male infertility and reactive oxygen species by Oxisperm kit assay. Methods: In this case-control study, which received ethical committee approval, the participants were divided into fertile and infertile groups (50 patients in each group). Results: The alkaline comet test showed the best ability to predict male infertility, followed by the terminal deoxynucleotidyl transferase dUTP nick end labelling (TUNEL) assay, the sperm chromatin dispersion (SCD) test, and the sperm chromatin structure assay (SCSA), while the neutral comet test had no predictive power. For our patient population, the projected cut-off point for the DNA fragmentation index was 22.08% using the TUNEL assay, 19.90% using SCSA, 24.74% using the SCD test, 48.47% using the alkaline comet test, and 36.37% using the neutral comet test. Significant correlations were found between the results of the SCD test and those obtained using SCSA and TUNEL (r = 0.70 and r = 0.68, respectively; p< 0.001), and a statistically significant correlation was also found between the results of SCSA and the TUNEL assay (r = 0.77, p< 0.001). Likewise, the results of the alkaline comet test showed significant correlations with those of the SCD, SCSA, and TUNEL tests (r = 0.59, r = 0.57, and r = 0.72, respectively; p< 0.001). Conclusion: The TUNEL assay, SCSA, SCD, and the alkaline comet test were effective for distinguishing between fertile and infertile patients, and the alkaline comet test was the best predictor of male infertility.

Objective: Machine learning is not yet widely used in the medical field. Therefore, this study was conducted to compare the performance of preexisting severity prediction models and machine learning based models (random forest [RF], gradient boosting [GB]) for mortality prediction in pneumonia patients. Methods: We retrospectively collected data from patients who visited the emergency department of a tertiary training hospital in Seoul, Korea from January to March of 2015. The Pneumonia Severity Index (PSI) and Sequential Organ Failure Assessment (SOFA) scores were calculated for both groups and the area under the curve (AUC) for mortality prediction was computed. For the RF and GB models, data were divided into a test set and a validation set by the random split method. The training set was learned in RF and GB models and the AUC was obtained from the validation set. The mean AUC was compared with the other two AUCs. Results: Of the 536 investigated patients, 395 were enrolled and 41 of them died. The AUC values of PSI and SOFA scores were 0.799 (0.737-0.862) and 0.865 (0.811-0.918), respectively. The mean AUC values obtained by the RF and GB models were 0.928 (0.899-0.957) and 0.919 (0.886-0.952), respectively. There were significant differences between preexisting severity prediction models and machine learning based models (P<0.001). Conclusion: Classification through machine learning may help predict the mortality of pneumonia patients visiting the emergency department.
Objective : The treatment of choice for spinal epidural abscess (SEA) generally is urgent surgery in combination with intravenous antibiotic treatment. However, the optimal duration of antibiotic treatment has not been established to date, although 4-8 weeks is generally advised. Moreover, some researchers have reported that bacteremia is a risk factor for failure of antibiotic treatment in SEA. In this study, we investigated the clinical characteristics of SEA accompanied by bacteremia and also determined whether the conventional 4-8 weeks of antibiotic treatment is sufficient. Methods : We retrospectively reviewed the medical records and radiological data of 23 patients with bacterial SEA who underwent open surgery from March 2010 to April 2020. All patients had bacteremia preoperatively and underwent weeks of perioperative antibiotic treatments based on their identified organisms until all symptoms of infection disappeared. All patients underwent microbiological studies of peripheral blood, specimens from SEA and concomitant infections. The mean follow-up duration was 35.2 months, excluding three patients who died. Results : The male : female ratio was 15 : 8, and the mean age was 68.9 years. The SEA most commonly involved the lumbar spinal segment (73.9%), and the mean size was 2.9 vertebral body lengths. Mean time periods of 8.4 days and 16.6 days were required from admission to diagnosis and from admission to surgery, respectively. Concomitant infections more frequently resulted in delayed diagnosis (p=0.032), masking the symptoms of SEA. Methicillin-sensitive Staphylococcus aureus was the most commonly identified pathogen in both blood and surgical specimens. Seventeen patients (73.9%) showed no deficits at the final follow-up. The overall antibiotic treatment duration was a mean of 66.6 days, excluding three patients who died. This duration was longer than the conventionally advised 4-8 weeks (p=0.010), and psoas or paraspinal abscess required prolonged duration of antibiotic treatment (p=0.038). Conclusion : SEA accompanied by bacteremia required a longer duration (>8 weeks) of antibiotic treatment. In addition, the diagnosis was more frequently delayed in patients with concomitant infections. The duration of antibiotic treatment should be extended for SEA with bacteremia, and a high index of suspicion is mandatory for early diagnosis, especially in patients with concomitant infections.
Ball bearing is a device that supports and transmits a load acting on a rotating shaft, and it is a type of rolling bearings that uses the rolling friction of the balls by inserting balls between the inner ring and the outer ring. Grease, which is prepared by mixing a thickener with a base oil, is a lubricant commonly used in bearings and has the advantage of a simple structure and easy handling. Bearings are increasingly being used in high value-added products such as semiconductors, aviation, and robots in the era of the 4th industrial revolution. Accordingly, there is an increasing demand for bearing grease. The selection of grease is an important factor in the bearing design. Therefore, a study must be conducted on the grease life evaluation to select an appropriate grease according to operating conditions such as a high temperature, high rotational speed, and high load. In this study, we evaluate the life of ball-bearing grease according to various operating conditions, namely, temperature, speed, and load changes. For this, we develop and theoretically verify a grease life test machine for ball bearings. We conduct a life test of grease according to various operating conditions of bearings and predict the grease life with a 10% and 50% failure probability using the Weibull analysis. In addition, we analyze the oxide characteristics of the grease over time using the Fourier transform infrared spectroscopy and the deterioration characteristics of the grease using the carbonyl index.
Background: In physical activity or labor, the human body is in a state of high intensity stress, and all parts or physiological functions of the body respond positively to maintain or balance the need for movement. The human body has many physiological changes in the process of movement, and fatigue is the external manifestation of various complex changes inside the human body. Fatigue is also a physiological mechanism of self-protection after the body reaches a certain level of activity, which can prevent the occurrence of life-threatening excessive functional failure. The generation of fatigue is a very complex process, and its mechanism has not been concluded yet. Therefore, it is an important work to search and screen the effective components of natural plants that have anti-fatigue effect and to explore their mechanism. Methods: This was a 8-week, randomized, double-blind, placebo-controlled clinical trial. A total of 110 subjects who passed physical examination were included according to the scheme design, and randomly divided into a test group which was given KRG and a placebo control group. The calculation is carried out according to the standard of sub-high-intensity exercise test. Results: There was no adverse effect on safety index of subjects after taking red ginseng capsule. After KRG treatment, subjective strength grade is significant lower than placebo treatment. Blood lactic acid content is significantly get lower after trial in KRG group, and significant lower than placebo group. Creatine phosphokinase(CK) content is significantly get lower after trial in KRG group, and significant lower than placebo group. Conclusion: According to the criterion in the test scheme, the result shows that KRG is helpful on relieving physical fatigue.
The use of structural control devices to minimize structural response to seismic/dynamic excitations has attracted increased attention in recent years. The use of magnetorheological (MR) dampers as a control device have captured the attention of researchers in this field due to its flexibility, adaptability, easy control, and low power requirement compared to other control devices. However, little attention has been paid to the effect of configuration and number of dampers installed in a structure on responses reduction. This study assesses the control of a five-story structure using one and two MR dampers at different stories to determine the optimal damper positions and configurations based on performance indices. This paper also addresses the fail-safe current value to be applied to the MR damper at each floor in the event of feedback or control failure. The model is mathematically simulated in SIMULINK/MATLAB environment. Linear control strategies for current at 0 A, 0.5 A, 1 A, 1.5 A, 2 A, and 2.5 A are implemented for MR dampers, and the response of the structure to these control strategies for different configurations of dampers is compared with the uncontrolled structure. Based on the performance indices, it was concluded that the dampers should be positioned starting from the ground floor, then the 2nd floor followed by 1st and rest of the floors sequentially. The failsafe value of current for MR dampers located in lower floors (G+1) should be kept at a higher value compared to dampers at top floors for effective passive control of multi-story structures.
