TLR-1, TLR-2, and TLR-6 MYD88-dependent signaling pathway: A potential factor in the interaction of high-DNA fragmentation human sperm with fallopian tube epithelial cells

  • Objective: The DNA integrity of spermatozoa that attach to fallopian tube (FT) cells is higher than spermatozoa that do not attach. FT epithelial cells can distinguish normal and abnormal sperm chromatin. This study investigated the effects of sperm with a high-DNA fragmentation index (DFI) from men with unexplained repeated implantation failure (RIF) on the Toll-like receptor (TLR) signaling pathway in human FT cells in vitro. Methods: Ten men with a RIF history and high-DFI and 10 healthy donors with low-DFI comprised the high-DFI (>30%) and control (<30%) groups, respectively. After fresh semen preparation, sperm were co-cultured with a human FT epithelial cell line (OE-E6/E7) for 24 hours. RNA was extracted from the cell line and the human innate and adaptive immune responses were tested using an RT2 profiler polymerase chain reaction (PCR) array. Results: The PCR array data showed significantly higher TLR-1, TLR-2, TLR-3, TLR-6, interleukin 1α (IL-1α), IL-1β, IL-6, IL-12, interferon α (IFN-α), IFN-β, tumor necrosis factor α (TNF-α), CXCL8, GM-CSF, G-CSF, CD14, ELK1, IRAK1, IRAK2, IRAK4, IRF1, IRF3, LY96, MAP2K3, MAP2K4, MAP3K7, MAP4K4, MAPK8, MAPK8IP3, MYD88, NFKB1, NFKB2, REL, TIRAP, and TRAF6 expression in the high-DFI group than in the control group. These factors are all involved in the TLR-MyD88 signaling pathway. Conclusion: The MyD88-dependent pathway through TLR-1, TLR-2, and TLR-6 activation may be one of the main inflammatory pathways activated by high-DFI sperm from men with RIF. Following activation of this pathway, epithelial cells produce inflammatory cytokines, resulting in neutrophil infiltration, activation, phagocytosis, neutrophil extracellular trap formation, and apoptosis.

Double-Chambered Right Ventricle in an Old Standard Poodle Dog

  • A 12-year-old Standard Poodle presented with intermittent weakness and occasional dyspnea at the Veterinary Medicine Teaching Hospital of Kangwon National University. A grade of 4 out of 6 systolic murmur with an irregular tachycardic rhythm was auscultated on both sides of the chest. Systolic blood pressure was 140 mmHg. Panting was noticed in the hospital, but there was no crackle sound. Blood analysis revealed mild increases in liver panel levels (alanine aminotransferase 149 [reference interval, 19-70] U/L; and alkaline phosphatase, 185 [reference interval, 15-127] U/L) and severe increases in cardiac biomarker levels (n-terminal pro-brain natriuretic peptide, 4169 [reference interval, 50-900] pmol/L; and cardiac troponin I, 0.22 [reference interval, 0.03-0.12] ng/mL). On electrocardiography, irregularly irregular supraventricular tachycardic rhythm with an f-wave and no distinct p-wave was observed. Generalized cardiomegaly with an enlarged right atrium and left ventricle was confirmed on thoracic radiography. Moreover, hepatomegaly and an enlarged caudal vena cava were observed. Echocardiographic evaluation revealed a fibromuscular diaphragm in the right ventricle. Because of the obstructive lesion in the right ventricle, the right atrium and ventricle were enlarged (right atrial area index, 38.82 cm2/m2 [reference interval, 4.2-10.2 cm2/m2]; right ventricle end-diastolic area index, 14.152 cm2/m2 [reference interval, 4.9-10.92 cm2/m2]). Accordingly, the patient was diagnosed with double-chambered right ventricle (DCRV). Pimobendan, furosemide, enalapril, diltiazem, and S-adenosylmethionine (SAMe) were prescribed, and all symptoms were relieved. DCRV is a right-sided congenital heart defect resembling pulmonic valve stenosis. If symptoms are not severe, medical therapy can be facilitated without surgery or the balloon dilation.

Identification of an effective and safe bolus dose and lockout time for patient-controlled sedation (PCS) using dexmedetomidine in dental treatments: a randomized clinical trial

  • Background: This study investigated a safe and effective bolus dose and lockout time for patient-controlled sedation (PCS) with dexmedetomidine for dental treatments. The depth of sedation, vital signs, and patient satisfaction were investigated to demonstrate safety. Methods: Thirty patients requiring dental scaling were enrolled and randomly divided into three groups based on bolus doses and lockout times: group 1 (low dose group, bolus dose 0.05 ㎍/kg, 1-minute lockout time), group 2 (middle dose group, 0.1 ㎍/kg, 1-minute), and group 3 (high dose group, 0.2 ㎍/kg, 3-minute) (n = 10 each). ECG, pulse, oxygen saturation, blood pressure, end-tidal CO2, respiratory rate, and bispectral index scores (BIS) were measured and recorded. The study was conducted in two stages: the first involved sedation without dental treatment and the second included sedation with dental scaling. Patients were instructed to press the drug demand button every 10 s, and the process of falling asleep and waking up was repeated 1-5 times. In the second stage, during dental scaling, patients were instructed to press the drug demand button. Loss of responsiveness (LOR) was defined as failure to respond to auditory stimuli six times, determining sleep onset. Patient and dentist satisfaction were assessed before and after experimentation. Results: Thirty patients (22 males) participated in the study. Scaling was performed in 29 patients after excluding one who experienced dizziness during the first stage. The average number of drug administrations until first LOR was significantly lower in group 3 (2.8 times) than groups 1 and 2 (8.0 and 6.5 times, respectively). The time taken to reach the LOR showed no difference between groups. During the second stage, the average time required to reach the LOR during scaling was 583.4 seconds. The effect site concentrations (Ce) was significantly lower in group 1 than groups 2 and 3. In the participant survey on PCS, 8/10 in group 3 reported partial memory loss, whereas 17/20 in groups 1 and 2 recalled the procedure fully or partially. Conclusion: PCS with dexmedetomidine can provide a rapid onset of sedation, safe vital sign management, and minimal side effects, thus facilitating smooth dental sedation.

전문가치면세정술과 세균막관리교육의 임플란트 주위염 감소 효과 (The Effects of Professional Tooth Cleaning and Plaque Control Instruction on Reduction of Peri-implantitis)

  • 전문가치면세정술과 세균막관리교육이 임플란트 주위염 감소에 미치는 효과를 분석하기 위하여 2009년 12월 9일부터 2011년 1월 31일까지 상 하악 대구치부위에 임플란트 식립 후 보철치료까지 완료한 80명의 부분 무치악 환자와 이들에게 식립된 193개의 임플란트를 대상으로 실험군과 대조군을 추출하였다. 실험군은 Watanabe method로 전문가치면세정술을 실시한 후 같은 방법으로 잇솔질교육을 하였고, 대조군에게는 엔진을 이용한 치면연마와 회전법으로 잇솔질 교육을 실시하였으며, 시작일 이후 3, 6, 9개월 재내원하여 반복시행하였고, 치면세균막 검사, 치은염 검사, 임플란트 변연골소실량 검사를 실시하여 다음과 같은 결론을 얻었다. 1. 전문가치면세정술과 세균막관리교육을 적용한 실험군은 대조군에 비해 6개월 후와 9개월 후에서 치면세균막지수와 치은염지수가 유의하게 낮았고(p<0.05), 변연골소실량에서는 실험군(0.15 mm)이 대조군(0.24 mm)보다 낮아(p=0.155) 차이가 있을 수 있었으며, 전문가치면세정술과 세균막관리교육은 치면세균막지수와 치은염지수에 영향력 있는 요인으로 분석되었다. 2. 임플란트 특성에서 치면세균막지수는 상악이 하악보다 3, 6, 9개월 후에서 유의한 수준으로 낮았고(p<0.05), 기능부하 12개월 이하 군이 13개월 이상 군보다 6개월 후에서 낮았으며(p<0.05), 변연골소실량은 시작일과 9개월 후에서 이회법 시술방식이 일회법 시술방식보다 낮았고, 기능부하 12개월 이하 군이 13개월 이상 군보다 낮게 나타났다(p<0.05). 이상의 결과를 종합해 볼 때 Watanabe method를 이용한 전문가치면세정술과 세균막관리교육은 임플란트 주위염을 예방하고 발생된 임플란트 주위염을 개선하는데 효과적인 방법이라고 할 수 있다. 따라서 임플란트 주위염으로 인한 실패를 감소시킬 수 있는 방법으로 전문가치면세정술을 주기적으로 실시하고, 환자 개인별 적합한 잇솔질 방법을 지속적으로 교육하는 임플란트 계속관리제도 도입에 대한 고려가 필요할 것으로 사료된다.

폭발 손상을 입은 강섬유 및 FRP 시트 보강 철근콘크리트 보의 국부손상 및 잔류성능 평가 (Evaluation of Local Damages and Residual Performance of Blast Damaged RC Beams Strengthened with Steel Fiber and FRP Sheet)

  • 본 연구에서는 폭발에 의한 국부손상이 철근콘크리트 보의 잔류 휨강도 및 연성에 미치는 영향을 알아보기 위하여 $160{\times}290{\times}2200mm$의 철근콘크리트 보에 대하여 근접 폭발 시험과 정하중 휨시험을 수행하였다. 또한 이 시험을 통해 강섬유 및 FRP 시트로 보강한 철근콘크리트 보의 폭발저항성능을 평가하였다. 장약량 1kg, 이격거리 0.1m의 폭발 시험을 수행하였으며, 시험 후 시험체의 crater, spall 지름과 무게손실(weight loss)을 측정하여 시험체의 국부손상을 평가하였다. 또한 폭발하중을 받지 않은 시험체와 폭발하중을 받아 국부손상이 발생한 시험체의 정하중 휨시험을 수행하여 휨강도 및 연성지수를 측정하였다. 시험결과, 보통콘크리트 시험체(NC)는 비교적 큰 crater와 spall이 발생하였으며 국부손상에 의한 무게 손실이 23.5kg 발생하였다. 반면 강섬유보강콘크리트 시험체(SFRC)와 FRP 시트 보강 시험체(NC-F, NC-FS)는 NC에 비해 crater 크기와 무게손실이 감소하며 향상된 폭발저항성능을 나타내었다. 또한 폭발하중에 의해 국부손상이 발생한 시험체들은 휨강도와 연성지수가 감소하였다. 특히 NC는 잔류 휨강도가 기존 강도에 비해 크게 감소하였고 압축철근의 좌굴현상이 나타나며 취성적으로 파괴되었다. 강섬유와 FRP 시트로 보강한 시험체의 경우 잔류 휨강도 및 연성지수가 NC에 비해 증가하였다. 이를 통해 폭발이 발생할 경우, 충분한 이격거리를 확보하지 못하면 장약량이 적더라도 구조부재에 심각한 국부손상을 발생 시킬 수 있다는 것을 알 수 있었으며, 강섬유와 FRP 시트에 의한 보강 방법이 폭발저항성능을 향상시킨다는 것을 확인할 수 있었다.

선천성 방광요관역류 환아의 형제자매에서 방광요관역류와 신반흔의 유병률 (Incidence of Vesicoureteral Reflux and Renal Scar in Asymptomatic Siblings of Patients with Primary Vesicoureteral Reflux)

  • 목적 : 방광요관역류는 역류성 신병증의 주된 원인으로 작용하여 소아기 고혈압과 신부전을 유발하는 주요 원인으로 기여한다. 방광요관역류는 가족적 발생경향이 있어, 고위험군인 환자의 형제자매를 대상으로한 역류 선별검사의 시행이 국외에서는 보편적이나 국내에서는 아직 연구보고가 적어 그 시행이 제한되고 있다. 본 연구에서는 방광요관역류 환아의 형제자매에서 역류와 신손상의 유병률을 조사하여 형제자매 선별검사의 의의를 살피고자 하였다. 또한 지표환자의 임상경과를 분석하여 형제자매의 유병률에 영향을 미치는 요인이 있는지를 살펴보았다. 방법 : 1994년 6월부터 2001년 5월까지 아주대학교 병원 소아과에서 일차성 방광요관역류로 진단받은 지표환아 37명과 그들의 형제자매 50명을 대상으로 임상양상을 조사하고 배뇨 중 방광 요도조영검사, $^{99m}Tc-DMSA$ 신주사 검사, 요화학 및 요배양 검사를 시행하였다. 결과 : 지표환자 37명은 평균 $3.35{\pm}1.13$등급의 역류상태를 나타냈고 이들 중 59.5%에서는 신주사 검사상 신피질 결손을 보였다. 지표환자의 형제자매 50명 중 8명(16%)에서 방광요관역류가 있었고, 이들 중 7명(87.5%)에서는 신피질 결손을 보였으며 2명은 비가역적인 신반흔이 있었다. 역류가 없이 신피질 결손이 있는 경우도 1례 있었다. 형제자매의 방광요관역류 존재를 예측할만한 지표환자의 임상적 요인은 없었다. 결론 : 선별검사로 발견된 방광요관역류 환아의 형제자매에서 방광요관역류와 신피질손상의 유병률은 각각 16.0%, 16.0%로 형제자매가 '신손상의 고위험군'에 속함을 알 수 있었다. 따라서 방광요관역류 환아를 진료할 때, 그들의 형제자매에 대한 선별검사를 추천 시행하는 것이 가능한 신손상을 예방할 수 있는 효과적인 방법이라 판단된다. 또한 형제자매 선별검사를 할 때, 과거 시점의 역류로 인하여 형성된 신손상의 존재도 밝혀내기 위하여, 배뇨 중 방광요도조영 검사뿐만이 아니라 신주사 동위원소 검사도 포함시켜야 함을 주장하는 바이다.

30대 성인남녀의 성별에 따른 가속도맥파 차이에 관한 연구 (Gender-Related Differences in the Second Derivative of Photoplethysmogram Waveforms in the Fourth Decade)

  • Objective: There exist gender differences in pulsatile contour waveform. Women have a greater age-related increase in left ventricular mass than do men and more likely to experience symptomatic heart failure after infarction. SDPTG (the second derivative of photoplethysmogram waveform) is a noninvasive method for evaluating the pulse wave and is correlated with age and other risk factors for atherosclerosis. We studied the effect of gender on SDPTG and made clear why the gender differences appear. Methods: To study the effects of effect factors, including height and blood pressure, on SDPTG in the fourth decade, data on height, weight, PTD (pulse transit distance), blood pressure, serum lipid levels, and SDPTG were collected in 115 laboratory healthy men and women. SDPTG is derived from double-differential processing of fingertip photoplethysmography and consists of a, b, c, and d waves in systole and an e wave in diastole; SDPTG aging index (AI) was calculated as (b-c-d-e)/a. Results: There were significant gender-related differences of SDPTG AI, height, and blood pressure. Age, height, and mean blood pressure were respectively and significantly correlated with SDPTG AI. SDPTG is dependent upon age, height, and blood pressure. Restricting analysis to SDPTG AI, age, height, and mean blood pressure, yielded that there were gender-related differences in SDPTG AI (P<0.05) which were derived from those of height (F<0.001, df=l, P=0.994). Conclusions: These new data may help to explain previous findings about age-related differences in pulsatile contour waveforms and why gender differences of SDPTG appear. The results of this study suggest that SDPTG AI, used for evaluation of biological vascular aging, should be calibrated by height as well as age and blood pressure.

철도 사면에서 확률론적 기법을 이용한 사면안정성 매개변수 비교연구 (A Comparative and Parametric Study of Slope Stability Using a Probability-based Method in Railway Slope)

  • 본 논문에서는 고속철도 표준사면에 대해 안전율 평가와 확률론적 방법에 의한 신뢰성 해석을 수행하였다. 원지반과 흙 쌓기재료의 지반정수에 따른 안전율 변화경향과 신뢰도 지수 및 파괴확률의 분포경향을 확인하였다. 철도표준 성토사면에 대한 사면안정 해석결과 점착력과 내부마찰각은 단위중량에 비하여 안정성에 상대적으로 큰 영향을 미치는 것으로 나타났다. 또한 국내 철도 표준성토단면에 대한 신뢰성 해석결과 미공병단 기준에 의거하여 대체적으로 "평균이하(Below average)"의 수준이며, 건기 시 성토높이 12.0m에서는 "나쁨(Poor)"으로 나타났다.

Quasi-static test of the precast-concrete pile foundation for railway bridge construction

  • Precast concrete elements in accelerated bridge construction (ABC) extends from superstructure to substructure, precast pile foundation has proven a benefit for regions with fragile ecological environment and adverse geological condition. There is still a lack of knowledge of the seismic behavior and performance of the precast pile foundation. In this study, a 1/8 scaled model of precast pile foundation with elevated cap is fabricated for quasi-static test. The failure mechanism and responses of the precast pile-soil interaction system are analyzed. It is shown that damage occurs primarily in precast pile-soil interaction system and the bridge pier keeps elastic state because of its relatively large cross-section designed for railways. The vulnerable part of the precast pile with elevated cap is located at the embedded section, but no plastic hinge forms along the pile depth under cyclic loading. Hysteretic curves show no significant strength degradation but obvious stiffness degradation throughout the loading process. The energy dissipation capacity of the precast pile-soil interaction system is discussed by using index of the equivalent viscous damping ratio. It can be found that the energy dissipation capacity decreases with the increase of loading displacement due to the unyielding pile reinforcements and potential pile uplift. It is expected to promote the use of precast pile foundation in accelerated bridge construction (ABC) of railways designed in seismic regions.

Transit Time Flowmetry and Vein Size Are Predictive of Arteriovenous Fistula Maturation

  • Background: This study aimed to assess the effect of vessel size and flow characteristics on the maturation of autogenous radiocephalic arteriovenous fistulae (RCAVFs). Methods: We retrospectively reviewed records of patients undergoing RCAVF creation at a single medical center from January 2013 to December 2019. Operative variables were compared between patients whose fistulae matured and those whose fistulae failed to mature. Results: Overall, 152 patients (33 of whom were women) with a mean age of 62.6±13.6 years underwent RCAVF creation; functional maturation was achieved in 123. No statistically significant differences were observed between patients in whom maturation was or was not achieved in terms of the following variables: female sex (20.3% vs. 25.0%), radial artery size (2.5 vs. 2.4 mm), and pulsatility index (0.69 vs. 0.62). Low intraoperative transit time flowmetry (TTF; 150.4 vs. 98.1 mL/min) and small vein size (2.4 vs. 2.0 mm) were associated with failure of maturation. The best cutoff diameter for RCAVF TTF and cephalic vein size were 105 mL/min and 2.45 mm, respectively. Conclusion: In patients who undergo RCAVF creation, vein diameter on preoperative ultrasonography and intraoperative TTF are predictors of functional maturation. We identified an intraoperative TTF cutoff value that can be used for intraoperative decision-making.
