FMEA에서 고장 심각도의 탐지시간에 따른 위험성 평가 (Risk Evaluation in FMEA when the Failure Severity Depends on the Detection Time)

  • The FMEA is a widely used technique to pre-evaluate and avoid risks due to potential failures for developing an improved design. The conventional FMEA does not consider the possible time gap between occurrence and detection of failure cause. When a failure cause is detected and corrected before the failure itself occurs, there will be no other effect except the correction cost. But, if its cause is detected after the failure actually occurs, its effects will become more severe depending on the duration of the uncorrected failure. Taking this situation into account, a risk metric is developed as an alternative to the RPN of the conventional FMEA. The severity of a failure effect is first modeled as linear and quadratic severity functions of undetected failure time duration. Assuming exponential probability distribution for occurrence and detection time of failures and causes, the expected severity is derived for each failure cause. A new risk metric REM is defined as the product of a failure cause occurrence rate and the expected severity of its corresponding failure. A numerical example and some discussions are provided for illustration.

  • This paper try to find root-cause of failure in a connector used in transmitting signals for throttle body control in automotives by analyzing possible failure causes and performing experiments to simulate the cable failure in field. The connector comprises fins, wires, and case moldings. The failure is due to degradation of initial clamping force required fixing fins and wires in the connector. Expansion and compression of the case molding material surrounding fins would cause the degradation. Investigations of strict initial claming force and control of thermal expansion property of the molding are required to prevent the failure.

  • Aircraft maintenance failure doesn't only threaten flight safety but also causes significant economic losses such as flight delays, flight cancellations, air turn backs and diversion and so on. Therefore, the efforts should be made to detect the contributing factors that cause such maintenance failure and eliminate them. In this study, by analyzing factors contributing to system failure using the Maintenance Error Decision Aid (MEDA) model based on stochastic relations between the contribution factors, system failure and events, we will draw a improvement plan to aircraft maintenance defects.

와이블 지연시간 모형 하에서의 FMEA를 위한 고장원인의 위험평가 (Risk Evaluation of Failure Cause for FMEA under a Weibull Time Delay Model)

  • This paper suggests a weibull time delay model to evaluate failure risks in FMEA(failure modes and effects analysis). Assuming three types of loss functions for delayed time in failure cause detection, the risk of each failure cause is evaluated as its occurring frequency and expected loss. Since the closed form solution of the risk metric cannot be obtained, a statistical computer software R program is used for numerical calculation. When the occurrence and detection times have a common shape parameter, though, some simple results of mathematical derivation are also available. As an enormous quantity of field data becomes available under recent progress of data acquisition system, the proposed risk metric will provide a more practical and reasonable tool for evaluating the risks of failure causes in FMEA.

  • Brittle failure mechanism has been well known as growth of initial micro-damage, that causes macro crack and failure in the end. Several precise criteria are suggested recently, based on experiments values in a whole load range. Among them, Mohr-Coulomb's criterion is used widely these days, but it has a big error compared with the real failure behavior since it does not show reciprocal actions of stresses. In this study, a new brittle failure criterion is proposed, which includes the effects of brittle damage evolution by taking a brittle damage parameter specifically. Comparisons between the proposed model and the previous ones are also given.

코팅지 박리파손에 대한 근본원인분석 (Root Cause Analysis on Delamination Failure between Coating Film and Paper)

  • In the calendar and the advertising catalog, the surface is usually coated by coating polypropylene film. The delamination failure of coating film depends on surface roughness and quality of the substrate paper. In this paper, the mechanisms of delamination failure between the coating film and the paper is investigated by using the root cause analysis as one of techniques of reliability evaluation. The papers used in failure analysis are three kind products made by two domestic and one foreign companies. It found that the main causes of delamination failure between the coating film and the paper were the creation of microvoids caused by shape of filler and their growth caused by contraction of paper.

  • FMEA(Failure Mode and Effect Analysis) is a failure analysis method for the system to identify the potential failure modes, and their effects and causes to reduce or mitigate the critical effects of the system. FMEA for railway was introduced with reliability of railway system, and this was used for identifying and analysing the possible hazards qualitatively to meet the requirements in early stage of business. In general, the overall failure data of system could be managed from design stage by FMEA, and also the countermeasures to prevent many predicted failures could be established by identification of failure modes and assement of failure effects by FMEA. Using these advantages of FMEA, the effectiveness of reliability improvement could be expected if FMEA is applied continuously in operation stage. It is essential that railway vehicles are maintained with high level of safety and reliability not to happen any failures in operation. This paper is proposed the proper FMEA for maintenance of railway vehicles compared with existing FMEA.

  • The inside and outside of the country utilities considered that they focused energy and economic aspect for short-term in new environment change of restructure. but it need service reliability preservation and improvement countermeasure, that is no to use existent estimation method for reliability preservation. therefore, analyze the equipment failure rate for suitable reliability preservation through equipment failure rate analysis of power distribution r system and evaluated equipment that is composing power distribution system by the failure rate. This paper estimated failure causes and the TVFR(Time Varying Failure Rate) for main equipment that is composing power distribution system using Weibull distribution.

디칸터의 고장분석 및 성능 향상에 관한 연구 (A Study on the Failure Analysis and Performance Improvement of a Decanter)

  • In this study, the failure analysis of a decanter is carried out and the methods of performance improvement are presented. The decanter is a centrifugal separator that is used to separate water and solids from municipal and industrial sludge. Therefore, the decanter should be designed to improve the dewatering of sludge. Besides high performance, the decanter should guarantee its life time under a severe using condition. For theses reasons, the failure analysis and performance improvement of the decanter are studied. It is found from this study that the failure is caused by mass unbalance, wear, clogging or crack. If these failure causes are prevented, the life time as well as the performance is expected to be improved.

Feeder Cable Assembly의 가속수명시험법 개발 (Test methodology of acceleration life test on feeder cable assembly)

  • Feeder cable assembly는 정보통신용 자동차부품이다 기능을 제대로 하지 않으면 자동차의 제어와 안전에 큰 문제가 발생한다. 가속수명시험은 시험시간을 단축할 목적으로 짧은 시간에 잠재적인 고장모드와 고장을 찾기 위하여 기속조건(스트레스, 스트레인, 온도 등)에서 실험을 실시하는 것이다. 고장원인은 부품의 품질과, 고장으로 유도하는 공정, 디자인에 의해 발생하는 결함이다. 열충격은 온도차에 의하여 부품의 열팽창 정도가 다른 것에 기인한다. 열충격시험은 부품이 급격한 온도차를 견디는 능력을 실험하는 것이다. 본 연구에서는 가속시험의 coffin-manson 식을 이용하여 정상조건(최고온도 $80^{\circ}C$, 최저온도 $-40^{\circ}C$)과 가속조건(최고온도 $120^{\circ}C$, 최저온도 $-60^{\circ}C$)을 이용하여 가속계수를 계산하여 2.25 값을 얻었다. 정상조건에서 1,000 사이클 실험이 가속조건에서 444 사이클 실험만하여도 됨을 알 수 있었다. Bx 수명 식을 이용하여 가속조건에서 시료가 5개, B0.04%.10years 조건에서 747 사이클 결과를 얻었다. 정상조건에서 1,000 사이클, 가속조건에서 747 사이클 실험한 feeder cable assembly 시료 5개를 각각 네크워크 분석기를 이용하여 성능시험을 실시하고, 와이블분포의 모수분포값을 비교한 결과 가속이 잘 되었음을 알 수 있었다. 동등한 신뢰도에서 가속실험을 통하여 시험시간을 1/4 정도 단축할 수 있었다.
