Rapidly Increasing Prevalence of Obesity and Their Confident Determinants in Korea

  • Objectives: The 2003 WHO/FAO technical report described that Korea has largely maintained its traditional high-vegetable diet despite major social and economic changes, and had lower than expected levels of obesity prevalence than other industrialized countries. However, the prevalence of obesity in Korea has recently been rapidly increasing. The aim of this study was to elucidate the determinants of this rapid growth of obesity prevalence in Korea and to compare the results of national nutrition surveys between Korea and Japan. Methods: The trends of the National Health and Nutrition Survey in Korea instituted every 3 years and that in Japan conducted every year were compared. The results of obesity prevalence defined as more than 25 of the Body Mass Index, the percentage of habitual exercisers and the results of the nutritional surveys were examined from 1992 to 2005 in Korea and from 1992 to 2004 in Japan. Results: The prevalence of obesity in males has been gradually increasing in both Korea and Japan since 1992. Though until 1995 the prevalence of obesity in the Korean male population was less than that in Japan, after 1998 Korea surpassed Japan and a markedly increasing trend was observed. In females, the increasing trend of obesity was slower than males in both Korea and Japan. However, the prevalence of obesity was much higher in Korea compared with that in Japan. The percentage of exercisers was much lower in Korea than in Japan. Although, the definition of an exerciser varies with the survey year in Korea and is different from that in Japan, almost 70% of the population was not regularly engaging in moderate or hard intensity exercise in Korea. From 1995, the total energy intake was increased by 9.8% in Korea but it was decreased by 6.9% in Japan. Presently, the energy intake per capita per day in Korea exceeded that in Japan. Remarkable increases in the intake of meat and poultry, vegetable oils and fats, and milk and dairy products were observed in Korea from 1995 to 2005. On the other hand, these values decreased during the same period in Japan. Conclusion: The prevalence of obesity in Korea is increasing and has surpassed that of Japan. The current trends could be attributed to the low prevalence of habitual exercisers, and an increase in energy intake and the proportion of energy intake from fat.

전기노인 여성의 삶의 질 중 기운에 따른 건강행태와 영양상태 비교: 2019년, 2021년 국민건강영양조사 자료를 이용하여 (Comparison of the health behavior and nutrition status of young-old women according to the vitality in their quality of life: based on the 2019, 2021 Korea National Health and Nutrition Examination Survey)

  • 본 연구는 국민건강영양조사 2019년, 2021년 자료를 활용하여 전기노인 여성의 기운에 따른 건강행태와 영양상태를 비교하고자 하였다. 65-74세 전기노인 여성을 대상으로 하였으며 (n = 1,113) 기운 정도에 따라 네 그룹 (항상 기운 있음, 자주 기운 있음, 가끔 기운 있음, 전혀 기운 없음)으로 나누어 비교하였다. 사회경제적 요인에서 교육수준이 높을수록, 가구소득이 많을수록, 식생활 형편이 좋을수록, 경제활동을 하고 있는 경우에 기운이 높게 나타났다. 만성질환 유병률을 비교했을 때, 관절염, 당뇨병, 골다공증 유병의 경우 기운이 낮게 나타났다. 건강행태를 비교했을 때, 주관적 건강인지가 좋을수록, 유산소 신체활동을 실천할수록 기운이 높게 나타났다. 흡연과 음주에서는 유의적인 차이가 없었다. 정신건강 요인에서 6-8시간 정상 수면 시간일수록, 스트레스 정도가 낮을수록, 우울감이 적을수록 기운이 높았다. 식품 섭취를 비교한 결과 식품 섭취량과 감자·전분류, 버섯류, 과일류, 육류, 우유류, 동물성 유지류, 음료류 섭취량이 많을수록 기운이 높았다. 영양소 섭취를 비교한 결과 단백질, 지방, 포화지방산, 단일불포화지방산, 다가불포화지방산, n-6계 지방산, 식이섬유, 당, 인, 칼륨, 마그네슘, 철, 아연, 리보플라빈 섭취량이 많을수록 기운이 높았다. 본 연구를 통해 전기노인 여성의 기운 넘치는 생활을 위해서는 사회경제적인 안정, 관절염, 당뇨, 골다공증 등의 만성질환 예방, 운동, 충분한 수면, 정신건강, 고른 영양섭취가 필요함을 확인하였다. 본 연구 결과가 향후 전기노인 여성의 기운을 향상시키기 위한 기초자료로 활용될 수 있을 것이다.

조리 전공 대학생의 생활습관과 자신의 건강상태 인식이 체지방률에 미치는 영향 (Effects of the Life Style and Self-Recognition of Health Conditions on the of Body Fat % in Hotel Culinary College Students)

  • This study was to investigate the effects of the body fat $\%$ on life styles and self-recognition of health conditions in hotel culinary college students. This survey was conducted using the questionnaires for 144 students (110 males, 34 female) in Hongseung-located college. More exercise time and less frequency of alcoholic beverage they intake, lowered their body fat $\%$. Males did stronger exercise and had lower body fat $\%$ than females. The stronger exercise they did, the lower body fat $\%$ they had, and were more satisfied with their present weight. Since they had more alcoholic beverage, they smoked more. Less smoking, more abdominal obesity they were. The less satisfaction with their body weights, the higher body fat $\%$ they had. The obese less were content with their weights. More recognition of obese shape, higher body fat $\%$ they had and abdominal obesity they were. Female had higher body fat $\%$ and were more abdominal obesity, did weak action, less smoke and weak exercise than male. Higher body fat $\%$ they had, more abdominal obesity they were. The more part time jobs they had, they did not smoke or were not satisfied with their body weights. Female students in culinary arts division had higher body fat $\%$ and were dissatisfied with their body weights and did less exercise or subnormal exercises. 서울지역 대학생의 주관적 건강상태에 따른 체형인식, 건강관련 생활습관 및 식습관 비교 (Comparisons of Body Image Perception, Health Related Lifestyle and Dietary Behavior Based on the Self-Rated Health of University Students in Seoul)

  • This study was conducted to examine the differences in perceived body weight and image and various lifestyles based on the self-rated health of university students when gender was adjusted. Five hundred fifty-five participants were asked their perceived health condition, and 58, 289, 160 and 48 students answered themselves as "very healthy", "healthy", "normal", and "unhealthy", respectively. As compared to the other 3 groups, "unhealthy" group showed higher proportions in dissatisfaction of body weight and negative perception of body image (P < 0.01). As health related lifestyles, "very healthy" group reported longer sleeping time than "unhealthy" group (P < 0.05), and had a higher proportion of people with regular exercise. Among the dietary behaviors, the frequencies of followings significantly different among the groups: "Regularity of meal time" (P < 0.01), "Eat protein foods more than twice a day" (P < 0.001), "Eat vegetables" (P < 0.01), "Eat fruit and fruit juice" (P < 0.01), "Eat vegetable oil added foods" (P < 0.01), "Eat seaweed" (P < 0.01), "Eat breakfast" (P < 0.01), "Modulation in animal fat and high in cholesterol intake" (P < 0.01). Particularly, higher proportion of subjects answered "very healthy" had higher frequencies (6-7 times/week) of these dietary behaviors. Overall results suggest that healthy lifestyle including adequate sleeping time, regular exercise, and good dietary behaviors might be potential factors affecting positive perception of health. In addition, positive perception of body weight and image were related with positive perception of health.

대학생의 식행동과 건강 상태 및 스트레스에 관한 연구 (Dietary Behavior, Health Status, and Perceived Stress of University Students)

  • This study was performed to investigate the dietary behaviors, perceived stress, and health-related factors along with their correlations in University students. The number of subjects was 150(64 males and 86 females), and the results of the study were as follows: Stress was felt by over 80% of the male and female students. The male students were more stressed than the females for economic value, gender difference, professor and drinking-related stress factors, and there was a significant difference between the genders. The female students had more clinical symptoms from stress than the male students in terms of dizziness, indigestion and constipation(p<0.01, p<0.001). The amount of food intake under stress was significantly different by gender. The female students preferred sweet tasting items, whereas the male students preferred hot tasting item when under stress. Also the intake of candy and chocolate when under stress in the male and female students was $2.00{\pm}0.74$ and $2.41{\pm}0.75$, respectively. The female students consumed candy and chocolate more than the male(p<0.01). Furthermore, 62.5% of the male students and 30.2% of the female students exercised to get rid of stress: the use of ball games as exercise in male and female students was $1.59{\pm}0.83$, $1.01{\pm}0.11$, respectively. Many students drank alcohol and smoked while under stress. In particular, the male students smoked more than normal as compared to the females, and there was a significant gender difference(p<0.001). The clinical symptoms positively correlated with the life stress level were degree of nervousness, sweaty, achy neck and shoulders, sore back, dizziness, eye fatigue, headache and indigestion. The amount of food intake under stress was positively correlated to the clinical symptom of headache. These results indicate the effects of life stress on dietary behaviors, food choice, and health status. Stress not only changed dietary behaviors, but was also related to health status. Therefore, education with regard to nutrition is necessary if college students are to practice food eating habits to correctly manage life stresses.

금연.금주 남자대학생의 골밀도, 영양소 섭취, 혈액 성상 및 식습관 (Bone Density, Nutrient Intake, Blood Composition and Food Habits in Non-Smoking and Non-Alcohol Drinking Male University Students)

  • This study was conducted in order to investigate and compare anthropometric measurements, bone density, nutrient intake, blood composition and food habits between non-smoking, non-alcohol drinking and smoking, alcohol drinking male university students in Seoul, South Korea. The data for food habits and health-related behaviors were obtained by selfadministered questionnaires. The BQIs of the subjects were measured by Quantitative Ultrasound (QUS). The subjects were divided into two groups: NSND (non-smoking and non-alcohol drinking, n=62) group and General (smoking and alcohol drinking, n=160) group. The results were analyzed using the SPSS program and were as follows: The average heights, weights, and BMIs of the two groups were 173.3 cm, 66.5 kg and 22.1 and 173.4 cm, 68.7 kg and 22.9, respectively. There were no differences between the groups regarding height, weight or BMI. SBP and DBP, however, were significantly higher in the general group than in the NSND group (p<0.01). The BQIs, Z-scores and T-scores of the two groups were 99.83, -0.23, and -0.31 and 98.24, -0.27 and -0.39, respectively, producing no significant differences between the two groups. The percentages for normal bone status, osteopenia and osteoporosis were 83.88%, 16.12% and 0.0% and 74.37%, 25.62% and 0.01%, respectively. Mean intakes of animal protein (p<0.05), animal fat (p<0.05), fiber (p<0.05), animal Ca(p<0.05), animal Fe (p<0.001), Zn(p<0.05), vitamin B1 (p<0.05) and niacin (p<0.05) were significantly different between the two groups, and mean serum levels of SGOT (p<0.01), SGPT (p<0.001), ${\gamma}$-GTP (p<0.001), triglycerides (p<0.01), total cholesterol (p<0.05) and hematocrit (p<0.05) were also significantly different between the two groups. Overall, there were no differences in meal regularity, frequency of snacking, reasons for overeating, exercise and defecation between the groups. However, favorite foods (p<0.05) and night-time meals (p<0.05) were significantly different. In conclusion, the health status of the NSND group was superior compared to the general group. Thus, students who smoke and alcohol drink should receive a practical and systematically-organized education regarding the increased health benefits of quitting smoking and alcohol drinking.

운동이 복부형 비만여성의 혈청지질에 미치는 영향 (Effect of Exercise on Serum Lipids in Abdominal Obese Women)

  • 허리 둔부 둘레비가 0.85이상인 여성을 복부 비만으로 측정하고 8주간 계속해서 50분씩 달리기를 시행하였다 5Km의 실외 달리기 트랙을 50분간 강약 조절로 달리게 하였고, 운동을 시행한 후 체성분 변화에 대한 결과를 분석하였다. 본 연구의 결과, 8주간 운동에 의해 변화된 총 cholesterol이 LDL-C과 양의 상관관계를 보였다. (P<0.001), 허리 둔부 둘레비(WHR)는 총 cholesterol과 triglyceride의 변화량과 양의 상관관계를 보였으며(P<0.05) 허리 둔부 둘레비는 혈청 지질의 수준에 영향을 주었다. 또한 운동에 의하여 BMI와 체지방량은 유의적인 감소를 하였고 혈청지질 중 triglyceride는 역시 유의적인 감소를 보였다. 따라서 운동은 복부비만 여성에게 혈청지질 농도를 변화시켜 고지혈증이나 성인병의 유발을 감소시킬 것으로 사료된다. 한편 HDL-C과 LDL-C은 유의적인 변화를 보이지 않았으므로 장기간 운동에 의한 신체의 영향을 지속적으로 연구하는 일이 요구된다고 할 수 있다. 운동전, 혈청지질 간의 상관관계에서 총 cholesterol은 triglyceride, LDL-C과 양의 상관관계를 보였으며 운동 후 변화량을 비교한 상관관계에서도 총 cholesterol과 LDL-C은 양의 상관관계를 보였다. 허리 둔부 둘레비도 총 cholesterol과 triglyceride에 유의적인 상관성을 보여 체지방량 감소에 의하여 체형이 변화될 수 있는 가능성을 시사하였다. 복부비만 여성을 대상으로 실시한 운동은 혈청지질 농도 개선에 전반적인 효과가 있는 것으로 나타났으나 운동이 LDL-C과 HDL-C에 미치는 영향을 구체적으로 연구하기 위하여 좀 더 장기적인 연구가 지속되어야 하며, 앞으로 허리 둔부 둘레비와 심혈관 질환과의 상관성도 깊게 연구되어야 할 필요가 있다고 사료된다.

농.어촌지역 65세 이상 노인들의 주관적 건강평가에 따른 건강관련행동 및 음식섭취빈도 (Health related Behavior and Food Intake Frequency According to Self-rated Health of Older Adults in Agricultural and Fishery Areas)

  • This study attempted to analyze the general and physical characteristics, the status of physical function, the type and number of current diagnosis and self-reported symptoms, healthy life mode and food preference according to self-rated health (SRH) of older adults (135 men and 270 women). It also attempted to assess the factors affecting the SRH standards in agricultural and fishery areas located in southwestern Korea. The subjects considered themselves as being 'good'(57.6%), 'normal'(29.6%) or 'poor'(12.85) SRH, meaning positive self-rating of health and it was found that the correlation between factors such as the gender, current marriage status, monthly wage, the reception of the government's livelihood subsidy and subjective economical status and the SRH were significant. Regardless of the categories of obesity, a large number of the participants rated themselves as 'healthy', but not statistically significant. The subjects who reported poor ADL and IADL capacities, indicators of the status of physical function, were significantly more classified to the 'poor' health category and vice versa (P<0.001). The reported chronic diseases in this study that have lasted more than three months, were lumbago, sciatica, arthritis, high blood pressure and peptic ulcer. The current ratios of smoking, drinking and exercise were 85.3, 39.0 and 18.6%, respectively. The less drinking and exercising there was, the higher the 'good' SRH categories obtained (P<0.05). The respondents who had less chance of eating sour and hot foods estimated their health status as being better.

The Analysis of Factors Causing the High Prevalence of Child Obesity in Jeju Island

  • Purpose: For 3 consecutive years from 2012-2014, we analyzed the causative factors for why the Jeju Island had the highest obesity prevalences of school children among the 15 provinces in Korea. Methods: From our analysis of 28,026 elementary school children with obesity or normal weight in the 15 provinces, we analyzed 12 factors related to eating habits, exercise habits, lifestyle, and mental health. The differences between the obese and normal weight children were researched. Finally, Jeju was compared with Seoul, which has the lowest obesity prevalence in school age children. Statistical analysis was performed using the chi square test of PASW Statistics ver. 18.0. Results: Compared to the normal weight group, the obese group had significantly higher rates of consuming soft drinks (p<0.001), fast food intake (p=0.019), skipping breakfast (p<0.001), insufficient sleep (p<0.001), bullying experiences (p=0.001) and runaway impulses (p=0.012). Compared to Seoul, Jeju Island had significantly higher rates of Ramen intake (3.4% vs. 5.4%, p=0.021) and meat intake (46.0% vs. 52.9%, p=0.003). On the other hand, Jeju Island was significantly lower than was Seoul in their fruit intake (83.4% vs. 67.1%, p<0.001), vegetable intake (71.4% vs. 64.2%, p=0.001), and intense physical activity (63.4% vs. 47.7%, p<0.001). Meanwhile, insufficient sleep (15.4% vs. 9.6%, p<0.001) and runaway impulses (5.6% vs. 3.3%, p=0.027) in children were significantly lower in Jeju Island than in Seoul. Conclusion: The results of the obesity factor analysis of elementary school students in Jeju Island can be used as useful educational material for lowering the obesity prevalence in Jeju community.

Dietary intake based on physical activity level in Korean elementary school students

  • Prevalence of childhood obesity is increasing significantly worldwide due to energy imbalance perhaps stemming from undesirable dietary behavior and physical activity level. The objective of the study was to examine the effects of physical activity level on nutritional status in elementary school students. The subjects were comprised of 287 elementary school students between 4th and 6th grades in Seoul, Korea. The level of physical activity was scored with a modified Godin leisure-time exercise questionnaire and was categorized as active, moderately active, and sedentary. Dietary intakes were obtained using a 24-hour food recall method. An analysis of variance (ANOVA) was conducted to test for global significant differences of nutrient intakes by physical activity level. Boys were more active than girls. Daily intakes of energy in moderately active boys were significantly higher than in the sedentary group, but intakes of calcium and iron in moderately active boys were lower than active boys. For girls, physical activity level did not affect nutrient density at all. Intakes of calcium, vitamin C, and folate for both boys and girls were below 50% of recommended intake. Physical activity did not affect nutrient density and our participants were exposed to nutritional imbalance. Therefore, the results suggest that nutrition education regarding balanced diet and optimum physical activity is required for children's health and growth.
