Endoscopie Removal of Adult Esophageal Foreign Bodies (성인 식도이물의 내시경적 치료)

  • The majority of esophageal Foreign body ingestions occur in the pediatric population. In adults, true foreign object ingestion occurs more commonly among those with psychiatric disorders, mental impairment. The management of esophageal foreign bodies is influenced by the age, clinical condition of ingested material, anatomic location and technical abilities of the endoscopist. Recently the therapeutic endoscopy is becoming wider and more rational in application. We evaluated the role of endoscopy for removal of esophageal foreign bodies during the period of 4 years from January 2000 to December 2003 at the Department of Otolaryngology and Gastroenterology, Seonam University Hospital. The results were as follow, 1) The age ranged from 21 to 74 years old (mean 50.5), most frequent age group was between 61-70 years old and male to female ratio was 1:1.4. 2) Fish bone was the most frequent foreign body in the esophagus ($47.1\%$), food material ($23.5\%$) and meats ($17.6\%$) were next frequent foreign bodies. The most frequent site of lodgement was the first ($78.4\%$), second ($17.6\%$) and third narrowing ($3.9\%$) in order. 3) The most common symptom was foreign body sensation (28.6%). the next common symptoms were chest discomfort($23.8\%$) and dysphagia($19\%$). 4) In duration of lodgement, 49cases ($96.1\%$) were lodged for less than one day. 5) The foreign bodies of esophagus were removed successfully by flexible endoscope with basket, snare, forceps, overtube and endoscopic variceal ligation cap. There were only 3 cases of minimal complications, esophageal mucosal tearing. In conclusion, endoscopic esophageal foreign body removal is useful and safe with minimal or no complications.

Esophageal manometry in the patients with foreign body sensation on the pharyngo-esopgageal region (인두 및 상부식도부 이물감 환자의 식도내)

  • Although it is suspected that the foreign body sensation on the pharyngoesophageal region is caused by motility disturbance of upper esophageal sphincter, its pathophysiology is not yet clear. Esophageal manometry has become an important diagnostic tool in the evaluation of esophageal motor disorders such as dysfunction of upper esophageal sphincter. Intraluminal esophageal pressures were measured by perfusion manometry in fifteen patients with foreign body sensation on the pharyngoesophageal region and in twenty six controls. In upper esophageal sphincter, mean value of resting pressure of the patients by rapid pull-through technique was 45,9\ulcorner 15.6mmHg and 80.9\ulcorner9.7mmHg in the controls. The difference between the two groups was statistically significant. The distance from nostril to sphincter, length of sphincter, and resting pressure by station pull-through technique were not significantly different. The amplitude of esophageal peristalsis in the patients was reduced significantly at the level of the upper, mid and lower esophagus. The wave duration of the patients was reduced significantly at the level the upper and mid esophagus. The speed showed no difference between two groups. Length and resting pressure of lower esophageal sphincter revealed almost same values in two groups.

Endoscopic Removal of Esophageal Foreign Body in a Moluccan Cockatoo (Cacatua moluccensis) (몰루칸 앵무새에서의 내시경을 이용한 식도 이물 제거 일례)

  • A thirty-month-old male Moluccan Cockatoo (Cacatua moluccansis) with mild anorexia was referred. Through physical examination, fireign body was palpated at the crop region. Radiopaque, lineal foreign body was visualized on the lateral radiographs of the thoracic esophageal region. The patient was definitively diagnosed esophageal foreign body which is ingested feeding tube. The foreign body removal was undertaken using a flexible endoscope and a grasping forcep without any other complications. This case report demonstrated that successful esophageal foreign body removal with endoscopy in birds.

  • Rupture of the esophagus is an uncommon condition that can be a formidable challenge to treat. Unless early treatment is applied, life-threatening complications, such as mediastinitis, sepsis, occurs and continue to be associated with a mortality rate of more than 20%. Definitive repair of esophageal perforation is considered the preferred treatment in the past. In the present study, conservative treatment acquired a good results by help of the development of antibiotics and nutritional supportive methods. Iatrogenic causes constituted most of the injuries, followed by external trauma, spontaneous, ingested foreign bodies, and malignancy. The incidence of perforation following ingestion of foreign bodies is very rare, so recommendations regarding treatment remain controversial. We analyzed the course of 31 patients with esophageal perforation by foreign bodies, concentrating on the diagnostic & treatment delay and methods of treatment, complications.

Esophageal Foreign Bodies with Periesophageal Abscess (식도주위농양을 병발한 식도이물 2례)

  • Foreign bodies in air and food passages are not uncommon problems in the otolaryngological fields and its etiological factors are closely related to the social environment and mode of life. It may complicate of esophageal perforation, periesphagitis, periesophageal abscess, mediastinitis, pneumothorax, pyothorax and lung abscess which may lead to the problems of life and death. The majority of esophageal foreign bodies which lodge in the esophagus can be removed endoscopically, but the following types of foreign bodies may require removal by the external route: 1. an impacted foreign body, 2. a foreign body producing periesophagitis after unsuccessful attempts at removal through the esophagoscope, and 3. a periesophageal abscess with a foreign body lodging in the abscess itself. Many interesting cases and statistical analysis of esophageal foreign bodies were reported by many authors, but only a few complicated cases were reported. Recently, we experienced 2 cases of esophageal foreign bodies which penetrate the cervical and thoracic esophageal wall and formed periesophageal abscess in 12 and 40 years-old males who swallowed of wire accidentally. The foreign bodies are successfully removed by the external routes through the lateral neck and chest. The postoperative courses were uneventful.

Esophageal foreign body with esophageal perforation (식도천공을 초래한 식도이물)

  • The authors experienced an unusual case of esophageal foreign body with esophageal perforation. A 8-month-old boy swallowed a long (about 20 cm) thick (about 3mm in diameter) wire, which had a hooked end and the another straight. The hooked end was located at near the first esophageal narrowing with swelling on the left lateral neck and the another end was out of his mouth. As failure of complete removal of the foreign body with esophagoscopy, the hooked end was cut and removed with the lateral incision of the neck and the remainder was removed was removed with esophagoscopy. After removal of the foreign body, nothing was given by mouth and the patient feed through nasogastric tube for about 4 weeks and the perforated esophagus was healed completely without any other complication.

Clinical Analysis of Upper Aerodigestive Tract Foreign Body (기도 및 식도 이물에 대한 임상적 고찰)

  • Background and Objectives: Foreign bodies of upper aerodigestive tract are common problem for primary care physicians. Delayed diagnosis or failure of removal might cause fatal problemsand complications. Therefore proper diagnosis and management is imperative. In this study, we described clinical features of upper aerodigestive tract foreign body, and analyzed efficacy of different management modality. Materials and Methods: 250 cases of foreign bodies in the esophagus and trachea, between Jan. 1998 through Jan. 2009 has been retrospectively analyzed. A total of 24 cases and 226 cases had been found each as airway foreign bodies and esophageal foreign bodies. The clinical features are described and treatment outcomes, prognosis, and rate of complications of each management modality have been compared. Results: In airway foreign bodies, ventilating bronchoscopy yielded better results, 19 success out of 19 trials than fiberoptic bronchoscopy, 3 success out of 5 trials. Hospitalization days after removal of foreign body didn't show difference between two treatment modalities, although patients who had ventilating bronchoscopy had gone through general anesthesia. And there was no complication after removal of foreign body. In esophageal foreign bodies, rigid esophagoscope yielded better results, 99% of successful removal rate, compared to the EGD, only 78% of successful removal rate. There was no difference of hospitalization days between two modalities. And complication rate was even low in patients who had done rigid esophagoscopic foreign body removal. Conclusion: In upper aerodigestivetract foreign body. Rapid diagnosis and successful foreign body removal is important. Removal by rigid scope(ventilating bronchoscope, rigid esophagoscope) revealed less failure in both airway and esophageal foreign bodies.

  • Esophageal foreign body is not uncommon problem among the esophageal disease and it is cured by removal of foreign body under the esophagoscopy in the most case. But it can cause esophageal perforation, periesophageal abscess, mediastinitis, pneumothorax, pyothorax, lung abscess and subcutaneous emphysema, and then may threat the life if early diagnosis and prompt management is not carried out. Esophageal perforation can be developed by sharp pieces of metal, bone or long term lodgement of foreign bodies in the esophagus. The authors have experienced the patient with periesophageal abscess after drawing out the sharp fish bone, and achived the good result by drainage via cervical mediastinotomy with continuous irrigation.

A Case of Esophageal Foreign Body Complicated by Pneumothorax (기흉을 초래한 식도이물)

  • Foreign body in the esophagus is not uncommom in the otolaryngological field, but esophageal perforation followed by pneumothrax due to esophageal foreign body is very rare. Authors recently experienced such a case developed in 1 year old male baby. This baby had been treated at local clinic for 2 weeks prior to admission under the impression of U.R.I.. Thereafter foreign body in the esophagus (fine wire pin) with left pneumothorax was detected by chest X-ray and the body was transfered to our hospital. Closed chest tube was inserted on left and under the general anesthesia, foreign body was removed by esophagoscopy. He was discharged on 16th postoperative day uneventfully.

  • Children tend to ingest foreign bodies. The majority present in children between the ages of 6 months and 3 years. We experienced 2 cases of unusual gastrointestinal complications caused by ingested foreign bodies. First case was a 10-month-old male with intestinal perforation due to two pieces of ingested magnetic heads. Second case was a 7-month-old girl with esophageal stricture due to an ingested particle of plastic toy.
