후인두 농양을 초래한 식도이물 1례 (A Case of Retropharyngeal Abscess Caused by Esophagal Foreign Body)

  • Esophageal foreign body are not uncommon problems in the otolaryngologic field. Esophageal foreign body may cause severe complications such as esophageal ulceration, esophageal perforation, periesophagitis, tracheo-esophageal fistula, mediastinitis, pneumothorax pyothorax according to the kinds, shape, size, duration of lodgement of foreign body. The majority of esophageal foreign U which lodge in the esophagus can be removed endoscopically, but the following type of foreign body may require removal by external route. 1. an impacted foreign body 2. a foreign body producing esophagitis after unsuccessful attempts at removal through the esopahgoscope 3. a periesophageal abscess with a foreign body lodging in the abscess itself. Recently, we experienced a case of esophageal foreign body (fish bone) which penetrate the cervical esophageal wall and formed retropharyngeal abscess in 54-year old female. The foreign body are successfully removed and abscess was drained by external route through the lat neck.

Risk factors predicting development of complications in 72 dogs with esophageal foreign bodies

  • The aim of this study was to determine predictive risk factors implicated in complications in dogs with esophageal foreign bodies. Medical records of 72 dogs diagnosed with esophageal foreign bodies by endoscopy were reviewed retrospectively. Factors analyzed included age; breed; gender; body weight, location, dimension, and type of foreign body; and duration of impaction. To identify risk factors associated with complications after foreign body ingestion, categorical variables were analyzed using the chisquare or Fisher's exact tests and multivariate analysis, as appropriate. Complications secondary to esophageal foreign body ingestion included megaesophagus, esophagitis, perforation, laceration, diverticulum, and pleuritis. Univariate analysis revealed that the location and duration of impaction after foreign body ingestion were associated with an increased risk of esophageal laceration and perforation. Multivariate analysis showed that age, duration of impaction, and foreign body dimension were significant independent risk factors associated with the development of complications in dogs with esophageal foreign bodies. In conclusion, these results showed that longer duration of impaction and larger foreign body dimensions may increase the risks of esophageal laceration, perforation, and plueritis in dogs.

식도 이물 모델에서 이물 탐색을 위한 삼차원 재구성법의 활용 (Detection of Foreign Body in Esophageal Foreign Body Model Using Three Dimensional Reconstruction Technique)

  • Objective This study was conducted to gather basic information of 3D CT in detecting and gaining information of esophageal foreign body (FB) models. Materials and Methods The chest model was made using PVC bottle, rubber balloon and plaster. Fish bone, Persimmon stone were used to mimic foreign bodies of esophageal model. The foreign body models were inserted into the balloon removing air from it and the balloon was sealed. The esophageal FB model was inserted into the chest model. The remaining space in the chest model was filled with fish paste and water to simulate soft tissue around esophagus. CT of chest model was reconstructed three-dimensionally by Rapidia software to make images of foreign body models. The axial CT, MPR image and VOI image were compared with real foreign body materials as to shape, size, location and orientation. Results Esophageal FB models were easily made. CT data gave good 3D images and showed realistic foreign body materials. Conclusion The results indicate the usefulness of 3D CT technique to help in diagnosis of esophageal foreign body models.

단추형 수은전지에 의한 식도이물 1례 (A Case of Esophageal Foreign Body by Mercury Button Battery)

  • The risk of foreign body by small button-disc battery in children has been increasing because the batteries are widely used in electronic device and plaything and easily contact with children. Foreign body of mercury type button-disc battery, when lodged in the esophagus, leak a caustic solution which causes a rapid necrosis of esophageal mucosa. So it should be removed immediately when the radiologic diagnosis is made. We experienced a case of esophageal foreign body by mercuric disc battery in a 14-month-old male. This case will be discussed with literature review.

Fogarty 카테터를 이용한 식도 이물(바둑알) 제거 2예 (Two Cases of esophageal foreign body removal using Fogarty catheter)

  • Unintentional foreign body ingestion is common in children, and coins are the most common foreign body ingested. Foreign body remaining in the esophagus may be associated with mucosal ulceration or esophageal obstruction and can potentially lead to significant morbidity and even death. Removal of esophageal foreign body is therefore generally recommended. Several methods are utilized including esophagoscopy in operating room, flexible endoscopy in out patient setting, fluoroscopic Foley catheter technique, and advancement using bougienage. We report two cases of esophageal foreign body(paduk stone) which were hard to be removed by usual esophagoscopic removal. Successful removal was accomplished with the aid of Fogarty catheter under the general anesthesia.

  • 이물에 의한 식도천공은 매우 희유하며 대부분은 날카로운 금속성이물과 골편에 의하며 드물게는 이물의 장기잔류에 의하여 초래할 수 있다. 저자등은 1976년 1월부터 1977년 12월까지 충남대학교 의료대학 부속병원 이비인후과에 내원한 식도이물 48례중 식도천공을 일으킨 5례 즉 구부러진 철사 2례, 안전핀 l례, 낚시바늘 1례, 골편 1례를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

  • Esophageal foreign bodies are common problems in the part of otolaryngology department, and may cause severe complications such as esophageal ulceration, esophageal perforation, periesophagitis, tracheoesophageal fisula, pneumothorax and pyothorax. Therefore, early diagnosis and intervention is needed to reduce morbidity and motality. But, calcification of the laryngeal cartilages may masquerade as foreign body in some patients with a history of foreign body ingestion. Recently, We experienced a case of calcification of thyroid cartilage which was misunderstood as an esophageal foreign body and report this case with a review of literatures.

강직형식도경술을 이용하여 치료한 이물에 대한 임상적 고찰 (Treatments of esophageal foreign body treated using Rigid esophagoscopy)

  • OBJECTIVE : To study the management (diagnostic and therapeutic) of esophageal foreign bodies with rigid esophagoscopy. MATERIAL AND METHODS : All 100 patients admitted to the Dankook University Hospital for ingestion of foreign bodies between May 1994 and July 1999. The outcome for each patient was determined by examining hospital records of demographic information, identification of the foreign body and the removal procedure used. RESULTS : Rigid esophagoscopy was performed for suspected foreign bodies in 100 cases an impacted. The most frequent location was the upper third of the esophagus (68%). The most common type of foreign body was fish bones in adult(61%) and coins in children(70%). In 12 patients. flexible endoscopy had failed previously to remove the foreign body and 4 cases were migrated esophageal metal stent in esophageal stenosis. CONCLUSIONS : The rigid esophagoscopy is appropriate techniques for managing esophageal foreign bodies.

개에서 식도질환의 진단영상학적 평가 (Diagnostic Imaging of Esophageal Diseases in Dogs)

  • Fourteen dogs referred to veterinary Medical Teaching Hospital, Seoul National University were diagnosed as esophageal foreign body (9 cases), megaesophagus (4 cases) and esophageal stricture (1 case). Patients showed a variety of clinical signs including regurgitation, vomiting, anorexia, hypersalivation, and retching. Survey radiographic examination included the entire esophagus, including the caudal pharynx and cranial abdomen. contrast radiographs were done to identify lesions or to characterize abnormal radiographic findings on survey films. In case static contrast studies were not sufficient were not sufficient to differentiate the diseases, dynamic fluoroscopic studies were performed. In thoracic megaesophagus, when gas filled, it provided several hallmark findings such as visualization of paired longus colli muscle and tracheal stripe sign. When gas-distended, the caudal thoracic esophagus was seen as a pair of thin, soft-tissue stripes that converged into a point overlying the diaphragm and cranial abdomen. All cases of megaesophagus could be solely identified by survey radiographs. In esophageal foreign body, 6 cases out of 9 patients had the history of having foreign body and others not. Most of esophageal foreign body could be diagnosed on survey radiographs and one case with radiolucent foreign body was confirmed by esophagram. It appeared as radiopaque material along the path of esophagus and the radiopacity was determined by its nature. Obstruction caused by foreign body eventually led to dilation of the esophageal lumen cranial to the site in 3 cases. In esophageal stricture, there was no remarkable findings on survey radiograph of the thorax. However, esophagography with barium sulfate showed the narrowing of the esophagus near hiatus. On fluoroscopy, swallowed barium was stagnated cranial to the site despite the esophageal peristalsis.

  • Esophageal foreign body is common condition for visit emergency room, and most of them were successfully removed with endoscopy. However, in case of esophageal foreign body impaction, it can be difficult to localize the foreign body. We report a 29-year old man with esophageal foreign body impaction. We successfully localized and removed the esophageal submucosal foreign body using rigid endoscopy under C-arm guidance, avoiding more invasive procedure. The patient was followed up without any complication.
