선천성 식도 폐쇄및 기관식도루 수술치험 3례 (Surgical Repair of Esophageal Atresia with Tracheoesophageal Fistula - Report of Three Cases -)

  • Esophageal atresia with tracheoesophageal fistula may occur as separate, but usually occur in combination. First described by Thomas Gibson in 1696, esophageal atresia was not successfully treated until 1939 when the first two survivors of staged correction were described by Ladd and Leven. In 1941, Haight and Towsley performed the first successful one-stage primary repair. We report three cases of esophageal atresia with tracheoesophageal fistula of which were treated with one-stage surgical repair method. The operation was performed tra-nspleurally through right 4th intercostal space. The fistula in the trachea was closed with interrupted 5-O prolene sutures and esophagoesophageal anastomosis was performed with 3-O prolene single layer sutures in all cases. All patients tolerated the operative procedures and recovered uneventfully. On follow-up study, anastomotic stricture was developed in one patients, so esophageal dilatation was performed for it with the Griintzig balloon catheter and the result was satisfactory. The other patients were well-being without any complication.

  • The repair of esophageal atresia with a long gap continues to pose difficulties for the surgeon. There is a general agreement that the child's own esophagus is the best, however, primary repair is not always possible. Foker JE et al. (1997) developed a technique of esophageal lengthening using external traction sutures. We successfully treated one patient with a 4.5cm gap esophageal atresia (4 vertebral spaces) using the external traction suture technique.

  • Transhiatal gastric transposition was performed in two case of long gap esophageal atresia without tracheoesophageal fistula. The patients were a 12 months old female and an 18 months old male. Stamm type gastrostomies were performed at other hospitals in both cases. The stomach was mobilized preserving the right gastric artery, the right gastroepiploic artery and spleen. A portion of the proximal and the distal esophageal segment were excised by transcervical and transhiatal route, respectively. The mobilized stomach was pulled up to the neck through the esophageal hiatus and posterior mediastinum. The esophagogastrostomy, the only one anastomosis of this procedure, was performed in the neck. There was no clinical evidence of anastomotic leakage, stricture, regurgitation, difficulty of gastric emptying, hoarseness or respiratory problem. Transhiatal gastric transposition seems to be a safe and easy alternative surgical procedure for esophageal replacement in long gap esophageal atresia.

  • The history of esophageal replacement in infants or children is the history of development of various kinds of alternative conduits such as stomach, colon, and small bowel. The gastric tube has been the most widely used conduit. From January 1988 to May 2003, 23 esophageal replacements with gastric tube were performed at the Department of Pediatric Surgery, Seoul National University Childrens Hospital. Statistical analysis was performed using Windows SPSS11.0 Pearson exact test. There were Gross type A(n=10), type B(n=1), type C(n=11), type D(n=1). Ten patients who had long gap esophageal atresia (type A-8, type B-1, type C-1) and 13 patients (type A 2, type C-10, type D-1) who had stenosis, leakage, recurred tracheoesophageal fistula, and esophagocutaneous fistula after previous corrective operations, had esophageal replacement with gastric tube. Mean follow-up periods were 4 year 2 months (7 months-15 year 1 month). There were postoperative complications including GERD in 16 (69.6 %), leakages in 7 (30.4 %), diverticulum at anastomosis in 2 (8.7 %), anastomosis site stenosis in 4 (17.3 %), and distal stenosis of the gastric tube in 1 (4.3 %). There was no statistical significance between operation types and postoperative leakage and gastroesophageal reflux. In conclusion, esophageal replacement with gastric tube may be a useful surgical option in esophageal atresia with long gap and esophageal atresia complicated by previous corrective operation.

  • The onset of hypertrophic pyloric stenosis in the postoperative course of esophageal atresia with tracheoesophageal fistula is rarely reported. The diagnosis could be delayed due to its mimicking symptoms of other postoperative complications including gastroesophageal reflux or anastomotic stricture. We present an infant who had surgery for esophageal atresia with tracheoesophageal fistula. He had never fed since birth. The infant presented with an increased amount of orogastric tube drainage and consistently distended gastric air on simple abdominal X-ray. Abdominal ultrasonography showed hypertrophic thick pyloric muscle. The diagnosis of pyloric stenosis was confirmed d is rarely reported. The diagnosis could be delayed due to its mimicking symptoms of other postoperative complications including gastroesophageal reflux or anastomotic stricture. We present an infant who had surgery for esophageal atresia with tracheoesophageal fistula. He had never fed. The infant presented with uring surgery, After pyloromyotomy, the patient's condition improved.

선천성 식도 폐쇄 수술 후 시행한 재 수술의 성적 (Result of Secondary Surgery after Primary Surgery for Esophageal Atresia Anomalies)

  • We reviewed the records of 25 patients who were re-operated upon after primary repair of esophageal atresia with or without fistula at the Department of Pediatric Surgery, Seoul National University Children's Hospital, from January 1997 to March 2007. Types of the esophageal atresia anomalies were Gross type A in 5 patients, C in 18, and E in 2. The indications for re-operation were anastomosis stricture (n = 14), tracheo-bronchial remnant (n = 4), persistent anastomosis leakage (n = 3), recurrent tracheo-esophageal fistula (n = 2) and esophageal web (n = 2). The interval between primary and secondary surgery was from 48 days to 26 years 5 months (mean: 2 years and 4 months). Four patients required a third operation. The interval between the second and third operation was between 1 year 1 month and 3 year 10 month (mean: 2 years 5 months). Mean follow up period after last operation was 35 months (1 years-8 years 6 months). The secondary surgery was end-to-end esophageal anastomosis in 15, esophagoplasty in 5, gastric tube replacement in 5. After secondary operation, 6 patients had anastomosis stricture (4 patients were relieved of the symptoms by balloon dilatation, 2 patients underwent tertiary operation). Five patients had leakage (sealed on conservative management in all). Two patients had recurrent tracheo-esophagel fistula (1 patient received chemical cauterization and 1 patient underwent tertiary operation). Currently, only one patient has feeding problems. There were no mortalities. Secondary esophageal surgery after primary surgery for esophageal atresia was effective and safe, should be positively considered when complications do not respond to nonoperative therapy.

선천성 식도폐쇄 및 기관식도루 -1예 보고 (Congenital esophageal atresia with tracheoesophageal fistula -Report of one case-)

  • Esophageal atresia and tracheoesophageal fistula may occur as separate entities but usually occur in combination. Recently we were experienced a case of esophageal atresia with distal tracheoesophageal fistula in infant patient who presented the symptoms of projectile vomiting and dyspnea. The diagnosis was made by the esophagography and the Haight`s operation was performed transpleurally through 4th intercostal space after gastrostomy. Operative patient tolerated all the operative procedures well in spite of postoperative respiratory complication and recovered uneventfully, permitted feeding on 10th postoperative day. On follow up study after 5th months, Patient reveals good health without other problem.

성인에서 발생된 선천성 식도기관지루 수술 치험: 5례 보고 (Congenital bronchoesophageal fistula without esophageal atresia in adults-report of 5 cases-)

  • Congenital bronchoesophageal fistula without esophageal atresia is very rare and often has an insidious clinical course that occasionally persists into adult life. Here are presented five cases of congenital bronchoesophageal fistula without esophageal atresia in adults treated successfully in the Seoul National University Hospital. The patients included two women and three men in the range of 16 and 45 years old. [mean age: 32 years old] Three of five cases could be diagnosed preoperatively by esophagogram and bronchogram but two of them could only be found in operative field. Cineesophagogram is recommended, on review of the literature, to be the most rewarding diagnostic procedure.

  • Esophageal atresia and Tracheoesophageal fistula may occur as separate entities but usually occur in combination. First described by Durston in 1970, esophageal atresia was not successfully treated until 1939 when the first two survivors of staged correction were described by Ladd and Leven. In 1941, Haight and Towsley performed the first successful primary repair. Authors report four cases of esophageal atresia of which two cases were treated surgically in success with Haight`s method. The type of four cases were all the same as upper blind pouch and lower tracheoesphageal fistula. Two of them were associated with verterbral defect, imperforate anus and/or rib fusion. Two cases died within seven days due to parent`s refusal for operative therapy, others were treated surgically with Haight`s method. Operative patients tolerated all the operative procedure and recovered uneventfully, permitted feeding on 7th postoperative day. On follow up study, one patient revealed intermittent regurgitation and corrected with bougienation another with good health without complication.
