A Comparison of Adult and Pediatric Methicillin-Resistant Staphylococcus aureus Isolates Collected from Patients at a University Hospital in Korea

  • In this study, we compared the phenotypic and genotypic characteristics of 138 MRSA isolates obtained from adult and pediatric patients (adult, 50; children, 88). The resistance rates against gentamicin, clindamycin, and ciprofloxacin were much higher in the adult MRSA isolates than in the pediatric MRSA isolates. The ermC gene, which is responsible for inducible clindamycin resistance, was detected in 52(59.1%) of the 88 pediatric MRSA isolates but in only 5(10.0%) of the 50 adult MRSA isolates. MRSA isolates of clonal type ST5 with an integration of SCCmec type II/II variants was the most predominant clone among the adult isolates, while clonal type ST72 with an integration of SCCmec IV/IVA was the most predominant clone among the pediatric MRSA isolates. Staphylococcal enterotoxin A and toxic shock syndrome toxin-1 were prevalent among the adult MRSA isolates but not among the pediatric MRSA isolates. The results of this study demonstrated remarkable differences between adult and pediatric MRSA isolates in terms of their antimicrobial susceptibility profiles, SCCmec type, multilocus sequence type, staphylococcal toxin genes, and erythromycin resistance genes.

  • Actinomycetes are Gram-positive soil bacteria and one of the most important industrial microorganisms due to superior biosynthetic capabilities of many valuable secondary metabolites as well as production of various valuable bioconversion enzymes. Among them are cytochrome P450 hydroxylase (CYP), which are hemoproteins encoded by a super family of genes, are universally distributed in most of the organisms from all biological kingdoms. Actinomycetes are a rich source of soluble CYP enzymes, which play critical roles in the bioactivation and detoxification of a wide variety of metabolite biosynthesis and xenobiotic transformation. Cyclosporin A (CyA), one of the most commonly-prescribed immunosuppressive drugs, was previously reported to be hydroxylated at the position of 4th N-methyl leucine by a rare actinomycetes called Sebekia benihana, leading to display different biological activity spectrum such as loss of immunosuppressive activities yet retaining hair growth-stimulating side effect. In order to improve this regio-selective CyA hydroxylation in S. benihana, previously-identified several secondary metabolite up-regulatory genes from Streptomyces coelicolor and S. avermitilis were heterologously overexpressed in S. benihana using an $ermE^*$ promoter-containing Streptomyces integrative expression vector. Among tested, SCO4967 encoding a conserved hypothetical protein significantly stimulated region-specific CyA hydroxylation in S. benihana, implying that some common regulatory systems functioning in both biosynthesis and bioconversion of secondary metabolite might be present in different actinomycetes species.

Identification and Characterization of a Pantothenate Kinase (PanK-sp) from Streptomyces peucetius ATCC 27952

  • Pantothenate kinase (PanK) catalyzes the first step in the biosynthesis of the essential and ubiquitous cofactor coenzyme A (CoA) in all organisms. Here, we report the identification, cloning, and characterization of panK-sp from Streptomyces peucetius ATCC 27952. The gene encoded a protein of 332 amino acids with a calculated molecular mass of 36.8 kDa and high homology with PanK from S. avermitilis and S. coelicolor A3(2). To elucidate the putative function of PanK-sp, it was cloned into pET32a(+) to construct pPKSP32, and the PanK-sp was then expressed in E. coli BL21(DE3) as a His-tag fusion protein and purified by immobilized metal affinity chromatography. The enzyme assay of PanK-sp was carried out as a coupling assay. The gradual decrease in NADH concentration with time clearly indicated the phosphorylating activity of PanK-sp. Furthermore, the ca. 1.4-fold increase of DXR and the ca. 1.5-fold increase of actinorhodin by in vivo overexpression of panK-sp, constructed in pIBR25 under the control of a strong $ermE^*$ promoter, established its positive role in secondary metabolite production from S. peucetius and S. coelicolor, respectively.

  • Der Computer kann ein nutzliches, herausforderndes und kreatives Werkzeug und Hilfsmittel beim Sprachenlernen darstellen. Bei der Anwendung auf die Situation des Grammatiklernens sind die analytischen und interaktiven $F\"{a}higkeiten$ des Computers mit gutem Erfolg eingesetzt worden. Eine der attraktivsten Eigenschaften des Computers ist nun seine $F\"{a}higkeit$, mit dem Schiller zu interagieren. Gute Computerprogramme $erm\"{o}glichen$ eine ausgedehnte Interaktion zwischen Computer und $Sch\"{u}ler$. Die Computerprogramme zum Sprachenlemen befassen sich mit Grammtik. Hier habe ich ein Lemprogramm des Intelligent Tutoring Systems $f\"{u}r$ die Grammatik Perfekt programmiert. Das Intelligent Tutoring System ist ein Computerlehrer. Wenn dem $Sch\'{u}ler$ Fehler unterlaufen, gibt ein guter Lehrer nicht nur die richtige Antwort, sondern hilft dem Schiller auch, seinen Fehler zu erkennen, ihn zu analysieren und zur richtigen Antwort zu finden. Das ist eine Funktion des Intelligent Tutoring Systems. Die Programme geben die Fehlermeldung richtig oder falsch. Dem Schuler werden Aufgaben vorgelegt, die sich aus mehreren Schritten und Prozessen zusammensetzen, wobei der Computer als Werkzeug bei der Suche nach einer $L\"{o}sung$ dient In guten $Probleml\"{o}sungsprogrammen$ verfolgt der Computer den $L\"{o}sungsansatz$ des $Sch\"{u}lers$ und analysiert seine Fehler. Wenn der Schuler bei der $\"{U}bung$ die richtige Antwort $ausw\"{a}hlt$, lauft das nachste Lernprogrammn des Computers. Die $\"{U}bungstyp$ kann auf dem Computer angereichert werden, indem $pr\"{a}zise$ und hilfreiche Kommentare $f\"{u}r$ jede $Auswahlm\"{o}glichkeit$ geboten werden.

  • Sprachverstehen ist ein interaktiver kognitiver Prozess. Der Verlauf des Sprachverstehens ist durch die kontinuierliche Interaktion von Bottom-up- und Top­down-Prozessen gekennzeichnet. Die eingehende Information wird durch die interaktive Nutzung verschiedener sprachlicher und aussersprachlicher Wissenssysteme verarbeitet. Herangewgen wird z. B. syntaktisches Wissen, lexikalisches und semantisches Wissen, Weltwissen und pragmatisches Wissen. Das hier vorgestellte Modell stellt den Begriff des lexikalischen Kopfes als $f\"{u}r$ die Verarbeitung von $S\"{a}tzen$ zentrales Konzept heraus. $Ambiguit\"{a}ten$ werden unter $Ber\"{u}cksichtigung$ der in ihnen $repr\"{a}sentierten\;Information\;aufzul\"{o}sen$ versucht Im Unterschied zu Prinzipien wie 'minimal attachment' rekurriert 'head attachment' nicht auf eine dem Satz zugrundeliegende Struktur, sondem allein auf lexikalische Information von $K\"{o}pfen$ und deren Reihenfolge an der $Oberfl\"{a}che$ eines Satzes. Head attachment ist daher $unabh\"{a}ngig$ von Annahmen $\"{u}ber$ syntaktische Strukturen. $W\"{a}hrend$ der 'tiefere Sinn' von Prinzipien wie 'minimal attachment' darin liegen soll, eine $m\"{o}glichst$ geringe Arbeitsspeicherbelastung durch Aufbau $m\"{o}glichst$ wenig komplexer Strukturen zu $gew\"{a}hrleisten$, erreicht 'head attachment' eine Entlastung durch die $Erm\"{o}glichung\;einer\;fr\"{u}hen$ semantischen Integration, da es Anbindungen in Abh\"{a}ngigkeit$ von dem Vorhandensein von $K\"{o}pfen\;vorschl\"{a}gt$.   • Recently, IMO STW Committee has released a model course of Bridge Resource Management. The Leadership and Teamwork among non-technical skills related to human factor is included at STCW 2010 Manila Amendments. Until now, the BRM education in Korea has been independently operated by each related educational institution, i.e., Shipping Company, University due to absence of model course. In this research the focus is on the simulation module among items for the BRM program. During the simulation from the BRM training, the trainee's reaction and popular functions on simulator have been investigated and analyzed to develop a simulation module for effective BRM education.

Trends in Integrated Electronic Resource Management in Academic Libraries (대학도서관의 전자자원 통합관리 유형 연구)

  • The explosion in the number of electronic resources has necessitated that academic libraries rethink the way they manage the life cycle of their electronic resources within the structure and processes of their library. With this momentum, led by the Digital Library Federation's Electronic Resources Management Initiative, specifications and tools for electronic resources has been devised. This study surveys the terrain of electronic resource management as viewed through the current development of ERM Systems and their adaptations into major academic libraries in the United States. For this, we tried to report how such tools are implemented with various additional modules. The potential of the module to integrate the management of its electronic resources into the new process has been suggested, as well.

First Report on Multidrug-Resistant Methicillin-Resistant Staphylococcus aureus Isolates in Children Admitted to Tertiary Hospitals in Vietnam

  • The extensive distribution of multidrug-resistant (MDR) methicillin-resistant Staphylococcus aureus (MRSA) poses a threat to healthcare worldwide. This study aimed to investigate the MDR and molecular patterns of MRSA isolates in children admitted to the two biggest tertiary care pediatric hospitals in northern and southern Vietnam. A total of 168 MRSA strains were collected to determine antibiotic susceptibility by minimum inhibitory concentration tests. Antibiotic-resistant genes, pulsed-field gel electrophoresis, staphylococcal cassette chromosome mec (SCCmec) typing, and multilocus sequence typing were used for the molecular characterization of MRSA. Among the total strains, the MDR rate (51.8%) was significantly higher in the northern hospital than in the southern hospital (73% vs. 39%, p < 0.0001). The MDR-MRSA with the highest rates were "ciprofloxacin-erythromycin-gentamicintetracyclines" (35.6%), followed by "erythromycin-tetracycline-chloramphenicol" (24.1%), and "ciprofloxacin-erythromycin-gentamicin" (19.5%), showing an accumulative total of 79.3%. The most susceptible antibiotics were rifampicin (100%) and vancomycin (100%), followed by doxycycline (94.0%), meropenem (78.0%), and cefotaxime (75.0%). The SCCmecII strains showed greater resistance to gentamicin, ciprofloxacin, tetracycline, meropenem and cephalosporins compared with the other strains. The SCCmecII strains exhibited the highest rate in the tested genes (aacA/aphD: 55.2%, ermA/B/C: 89.7%, and tetK/M: 82.8%). ST5-SCCmecII was the predominant clone in the northern hospital, whereas SCCmecIVa was more pronounced in the southern hospital. In conclusion, our results raised concerns about the predominant MDR-MRSA strains in the pediatric hospitals in Vietnam. The north-south difference in the antibiotic resistance patterns and genetic structure of MRSA suggests different MRSA origins and various uses of antimicrobial agents between the two regions.

An Fingerprint Authentication Model of ERM System using Private Key Escrow Management Server (개인키 위탁관리 서버를 이용한 전자의무기록 지문인증 모델)

  • Medical information is an important personal information for patients, and it must be protected. In particular, when medical personnel approach electronic medical records, authentication for enhanced security is essential. However, the existing public certificate-based certification model did not reflect the security characteristics of the electronic medical record(EMR) due to problems such as personal key management and authority delegation. In this study, we propose a fingerprint recognition-based authentication model with enhanced security to solve problems in the approach of the existing electronic medical record system. The proposed authentication model is an EMR system based on fingerprint recognition using PEMS (Private-key Escrow Management Server), which is applied with the private key commission protocol and the private key withdrawal protocol, enabling the problem of personal key management and authority delegation to be resolved at source. The performance experiment of the proposed certification model confirmed that the performance time was improved compared to the existing public certificate-based authentication, and the user's convenience was increased by recognizing fingerprints by replacing the electronic signature password.

Occurrence of a Hybrid Between Taenia saginata and Taenia asiatica Tapeworms in Cambodia

  • Human infection with Taenia asiatica or a hybrid between Taenia saginata and T. asiatica has not been reported in Cambodia. We detected for the first time a hybrid form between T. saginata and T. asiatica in Preah Vihear Province, Cambodia. An adult tapeworm specimen, i.e., 75 cm long strobila without scolex, was expelled from a 27-year-old man after praziquantel medication and purging. It was morphologically indistinguishable between T. saginata and T. asiatica. Several proglottids were molecularly analyzed to confirm the tapeworm species. The mitochondrial gene encoding cytochrome c oxidase subunit 1 (cox1) and nuclear genes encoding elongation factor-1α (ef1) and ezrin-radixin-moesin (ERM)-like protein (elp) were sequenced, and a single-allele analysis was performed to confirm the haploid genotype. The results revealed that our sample showed a discrepancy between the mitochondrial and 2 nuclear genes. It possessed homozygous sequences typical of T. saginata at cox1 and ef1 loci. However, it was heterozygous at the elp locus, with 1 allele in T. asiatica (elpA) and 1 in T. saginata (elpC), which indicates that it is a hybrid between T. saginata and T. asiatica. The present results confirmed the presence of a hybrid between T. saginata and T. asiatica in Cambodia and strongly suggest the existence of also 'pure' T. asiatica in Cambodia.
