A Pathological Survey on Liver Lesion of Slaughtered Native Cattle with Special Reference to Fascioliasis (한우(韓牛)의 도축검사례(屠畜檢査例)에 의한 간병변조사(肝病變調査) : 특히 간질증병변(肝蛭症病變)에 관하여)

  • The livers of 300 cases of Korean native cattle which were collected at slaughted house in Pusan were examined by gross and histopathological means to study patterns of pathological changes of the organ. 1. The most dominated lesion of the liver was those of fascioliasis showing incidence rate as 907 per cent of inspected cattle. 2. On age distribution of the fascioliasis lesion in the liver, higher incidence was seen in animals that were from 6 to 9 years of age. 3. Significant changes of the hepaticparenchyma caused by immature flukes included thrombosis, migrational tracks such as hemorrhagic foci with massive eosinophilia and hepatic necrosis. The changes of chronically infected livers of cattle were recognized as proliferation of periductal eosinophilic granuloma of various degrees in each portal triad. And this easential cholangiohepatitis might develop into multiple focal cirrhosis or diffuse perilobular cirrhosis. 4. Most cases with gross lesion of swelled bile ducts on visceral surface of the liver showed histological evidences of fascioliasis lesion throughout hepatic lobes.

Kimura Disease Misdiagnosed as Chronic Submandibular Sialadenitis : A Case Report (만성 악하선염으로 오인된 Kimura씨 병 1례)

  • Kimura disease is an uncommon chronic inflammatory disorder of unknown etiology. Clinically, patients present nontender subcutaneous swelling in the head and neck region. Peripheral eosinophilia, an elevated serum IgE are also present. The clinical course of Kimura disease is often progressive, and the main problem with treatment is disease recurrence. Treatment options in the recurrent cases range from observation to surgical excision, steroid therapy, and radiotherapy. We report a case of recurrent Kimura disease, initially thought to be chronic submandibular sialadenitis, along with the appropriate review.

  • Kimura disease (KD) is an immune-mediated chronic inflammatory disease of unknown etiology. KD has many complications associated with hypereosinophilia, including various forms of allergic reactions and eosinophilic lung disease. Additionally, hypereosinophilia is associated with hypercoagulability, which may lead to thromboembolic events. A 36-year-old man with KD presented with acute limb ischemia and coronary artery occlusion. He underwent thrombectomy, partial endarterectomy of both popliteal arteries, and coronary artery stent insertion. KD is a systemic disease that affects many organs and presents with thromboembolism and vasculitis. In a patient with KD, physicians should evaluate the vascular system, including the coronary arteries.

  • Eosinophilic pleural effusion (EPE) is defined as a pleural effusion that contains at least 10% eosinophils. EPE occurs due to a variety of causes such as blood or air in the pleural space, infection, malignancy, or an autoimmune disease. Undifferentiated connective tissue disease (UCTD) associated with eosinophilic pleural effusion is a rare condition generally characterized by the presence of the signs and symptoms but not fulfilling the existing classification criteria. We report a case involving a 67-year-old man with UCTD and EPE, who has been successfully treated with a single intrapleural corticosteroid injection.

Clinical Effects of Chuongsangboha-tang in Asthmatic Patients (청상보하탕의 기관지 천식 환자에 대한 임상적 효과)

  • Background: Nowadays asthma is considered to be an inflammatory disease characterized by airway hyperresponsiveness and pulmonary eosinophilia. Production of cytokines by bronchial epithelial cells may contribution to the local accumulation of inflammatory cells in patients with bronchial asthma. In many recent studies molecular biological methods have been used to investigate the role of cytokines in pathogenesis and new therapeutic targets of asthma. Objectives: We aimed to identify the clinical effects of Chuongsangboha-tang and the effects of Chuongsangboha-tang on serum cytokines in asthmatic patients. Materials and Methods: The subjects consisted of 36 patients with asthma who had been treated with Chuongsangboha-tang for four weeks. Chuongsangboha-tang is an herbal decoction which has been used of the traditional therapeutic agent of asthma. PFT, Quality of Life Questionnaire for Adult Korean Asthmatics (QLQAKA), blood eosinophil, serum IgE, Serum IL-4, IL-5, IFN- were checked before and 4 weeks after treatments. Results: Treatment with Chuongsangboha-tang for four weeks resulted in significant increase in FEV1.0%, PEFR%, QLQAKA. The blood eosinophil, serum IgE, IL-4 and IL-5 in asthmatic patients increased significantly compared to the normal control group, while the serum IFN-decreased significantly. Conclusions: This study shows that Chuongsangboha-tang has effects on improvement of pulmonary function and quality of life in asthmatic patients. Obviously, further research concerning this is still necessary.

Eosinophilic gastroenteritis in an 18-year-old male with prolonged nephrotic syndrome

  • Eosinophilic gastroenteritis is a rare disease characterized by prominent eosinophilic tissue infiltration of the gastrointestinal tract. Here, we report a case of eosinophilic gastroenteritis in an 18-year-old patient with prolonged nephrotic syndrome who presented with abdominal pain and peripheral hypereosinophilia. During the previous 2 years, he had visited local Emergency Department several times because of epigastric pain and nausea. He had been treated with steroid-dependent nephrotic syndrome since 3 years of age. Tests ruled out allergic and parasitic disease etiologies. Gastroduodenoscopy with biopsy revealed marked eosinophilic infiltration in the duodenum. Renal biopsy findings indicated minimal change disease spectrum without eosinophilic infiltration. The oral deflazacort dosage was increased, and the patient was discharged after abdominal pain resolved. To our knowledge, this is the first report of eosinophilic gastroenteritis in a patient with minimal change disease.

The Effects of Sochongryong-tang on Serum IL-4, IL-5, and $IFN-{\gamma}$ in Asthmatic Patients (소청용탕이 기관지천식 환자의 혈청 IL-4, IL-5, $IFN-{\gamma}$변화에 미치는 영향)

Sochongryong-tang is an herbal decoction which has traditionally been used as a therapeutic agent for asthma. Results : The serum IL-4 in asthmatic patients was increased significantly compared to the serum IL-4 in the normal control group. However, the serum IL-5, $IFN-{\gamma}$ in asthmatic patients showed no significant difference from the serum IL-5, $IFN-{\gamma}$ in the normal control group. The patients were treated with Sochongryong-tang for two weeks with no significant difference in the serum IL-4, IL-5, and $IFN-{\gamma}$. Conclusion : This study shows that the serum IL-4 may be a new therapeutic target of asthma. Further long-term studies must be made in a larger number of asthmatic patients.

Familial Case of Visceral Larval Migrans of Toxocara Canis after Ingestion of Raw Chicken Liver (닭 간 생식 후 가족에서 집단으로 발생한 개회충에 의한 내장 유충 이행증 1예)

  • We report a familial case of visceral larva migrans of Toxocara canis after eating raw chicken liver. A 9-year-old female ate raw chicken liver with her father and older brother and was admitted to the hospital with periumbilical pain, a mild fever, and headache. The total peripheral eosinophil count was 9,884/$mm^3$ and the total lgE concentration was 2,317 IU/dL. Chest and abdominal computed tomography (CT) scans demonstrated multiple, poorly-defined, small, nodular lesions scattered in the liver and lung parenchyma. Toxocara ELISA and Western blot tests were positive in the patient, and her father and brother. A liver biopsy revealed extensive eosinophilic infiltrations in the portal and lobular areas. She took albendazole for 5 days and was discharged in good condition. These results suggest that clinicians should consider foodborne toxocariasis in patients with multiple, small nodules in the liver and lung parenchyma with eosinophilia and a history of raw meat ingestion.

Postpartum Changes in the Uterus of Goats (산양에 있어서 분만후 자궁의 변화)

  • This study was attempted to investigate the processes of regression of the corpus luteum and uterus after parturition in 2∼3 multiparous Korean native goats. The concentrations of LH, prolactin and progesterone in blood plasma of native goats were measured at 5 day intervals from 10 days prepartum to 35 days postpartum. The pregnancy corpus luteum from goats at Days(D) 1, 10, 20 and 30 days of postpartum were examined by light microscopy. Changes in the uterus fo goats were studies by macroscopic and light microscopic observations during the postpartum period. Mean concentrations of plasma LH were low after parturition and the levels of plasma LH were similar during late gestation and throughout the postpartum period. Mean plasma concentrations of prolactin were 0.30 0.06 and 0.38 0.13ng/ml at Day 5 and Day 10 prepartum, respectively, but PRL levels remained slightly high for 5 weeks after kidding. Mean levels of progesterone in plasma were 0.33 0.05ng/ml on Day 1 postpartum(P<0.01). Through light microscopic survey, pregnancy corpora lutea were quite degeneration by day 10 pospartum. Microscopic changes of CL regression consisted of cytoplasmic eosinophilia and vacuolation, and pyknosis and karyorrhexis of the nucleus of luteal cells. Vascular changes were distended at the periphery ofthe CL. From macroscopic measurements of the uterus, the uteri were returned to their initial non-pregnant stage within a period of 21 dyas after parturition. Following partuition the intercaruncular epithelium was reparied by 20 days. The uterine epithelium was partially recovered in the carucle by 30 days postpartum.

A Case Report of Chronic Eosinophilic Pneumonia (만성 호산구성 폐렴 1예)

  • Chronic eosinophilic pneumonia is a very rare disorder of unknown etiology characterized by striking systemic and pulmonary manifestations such as fever, weight loss, dyspnea, blood eosinophilia, and fluffy peripheral opacities on chest radiograph. A number of these patients developed asthma before or with the onset of illness. The roentgenographic lesion rapidly resoluted with corticosteroid and recurrence was occasionally occured in the same location. Histopathologic features of chronic eosinophilic pneumonia include dense aggregates of eosinophils, histiocytes, and multinucleated giant cells within alveolar spaces, interstitium, and bronchioles associated with scattered lymphocytes and plasma cells. We report a case of chronic eosinophilic pneumonia diagnosed by clinical, radiographic, and histologic findings with review of the literature.
