간흡충에 대한 항체검출을 위한 Thin Layer Immounoassay (TIA) (Thin Layer Immnunoassay (TIA) for circulating antibody detection in clonorchiasis)

  • 간츱충증 환자로부터 혈액을 채취하여 그 혈청에서 간흡충에 대한 항체를 Thin Layer Immunoassay(TIA)에 의하여 검출하였다. TIA는 Elwing et al.(1976)이 기술한 방법에 의하여 시행하였고, 항원은 이등(1981)의 방법으로 제조된 간흡충 성충 생리식염수 추출물을 사용하였으며 그 단백질 함량은 2.87mg/ml이었다. 간흡충중 환자 60명의 혈청에서의 항체가는 건강대조군 및 이질아메바 감염자에서의 항체가와 비교할 때 그 차이가 유의하였으나, 폐흡충증 환자와의 차이는 유의하지 않았다. 간흡충중 환자의 성별이나 연령과 TIA에 의한 항체가 사이에 상관 관계가 없었다. 환자의 분변내 EPG, 말초혈액내 호산구의 수와도 상관이 없었다. TIA에 의한 항체가와 간기능 검사 결과와의 관계를 보면 alkaline phosphatase, SGOT 및 SGPT 측정치와 모두 상관이 없음을 알 수 있었다.

Mucosal immunity against parasitic gastrointestinal nematodes

  • The last two decades witnessed significant advances in the efforts of immune-parasitologists to elucidate the nature and role of the host mucosal defence mechanisms against intestinal nematode parasites. Aided by recent advances in basic immunology and biotechnology with the concomitant development of well defined laboratory models of infection, immunoparasitologists have more precisely analyzed and defined the different immune effector mechanisms during the infection; resulting in great improvement in our current knowledge and understanding of protective immunity against gastrointestinal (GI) nematode parasites. Much of this current understanding comes from experimental studies in laboratory rodents, which have been used as models of livestock and human GI nematode infections. These rodent studies, which have concentrated on Heligmosomoides polygyrus, Nippostrongylus brasiliensis, Strongyloides ratti/5. venezuelensis. Trichinella spiralis and trichuris muris infections in mice and rats, have helped in defining the types of T cell responses that regulate effector mechanisms and the effector mechanisms responsible for worm expulsion. In addition, these studies bear indications that traditionally accepted mechanisms of resistance such as eosinophilia and IgE responses may not play as important roles in protection as were previously conceived. In this review, we shall, from these rodent studies, attempt an overview of the mucosal and other effector responses against intestinal nematode parasites beginning with the indices of immune protection as a model of the protective immune responses that may occur in animals and man.

한우사육단지(韓牛飼育團地)에 발생(發生)한 우폐충증(牛肺蟲症)에 대하여 (An Outbreak of Lnugworm Disease in a Flock of Korean Native Cattle)

  • In a pasture located at Kyong Puk area, a group of 200 Korean native casttle showed severe coughing, dyspnea and emaciation, of which 30 cattle died from the respiratory disease. To the alived cattle, clinical, pathologicoparasitological observations and treatment with terramycin, Mintic and tetramisole (Bonasol) were carried out and the following results were obtained. 1) Paroxymal coughighings, increased respiratory rate and heart rate were observed in almost all cases. But, larval output in feces was recognized in 32 percent of the cases. 2) Morderate eosinophilia was observed in a half of the cases. 3) At autopsy, lungs were enlarged, and areas of dark consolidations were scattered in all lobes. In the bronchi, numerous adult lungworms were found. 4) The adult worms detected from the bronchi were morphologically identified as Dictyocaulus viviparus. 5) The clinical signs and larval output were disappeared after the administration of Mintic or Bonasol. 6) From these results, it was concluded that the respiratory disease of the cattle was caused by the infestation of lungworm, Dictyocaulus viviparus.

Baculovirus-based Vaccine Displaying Respiratory Syncytial Virus Glycoprotein Induces Protective Immunity against RSV Infection without Vaccine-Enhanced Disease

  • Background: Respiratory syncytial virus (RSV) is a major cause of severe lower respiratory tract diseases in infancy and early childhood. Despite its importance as a pathogen, there is no licensed vaccine against RSV yet. The attachment glycoprotein (G) of RSV is a potentially important target for protective antiviral immune responses. Recombinant baculovirus has been recently emerged as a new vaccine vector, since it has intrinsic immunostimulatory properties and good bio-safety profile. Methods: We have constructed a recombinant baculovirus-based RSV vaccine, Bac-RSV/G, displaying G glycoprotein, and evaluated immunogenicity and protective efficacy by intranasal immunization of BALB/c mice with Bac-RSV/G. Results: Bac-RSV/G efficiently provides protective immunity against RSV challenge. Strong serum IgG and mucosal IgA responses were induced by intranasal immunization with Bac-RSV/G. In addition to humoral immunity, G-specific Th17- as well as Th1-type T-cell responses were detected in the lungs of Bac-RSV/G-immune mice upon RSV challenge. Neither lung eosinophilia nor vaccine-induced weight loss was observed upon Bac-RSV/G immunization and subsequent RSV infection. Conclusion: Our data demonstrate that intranasal administration of baculovirus-based Bac-RSV/G vaccine is efficient for the induction of protection against RSV and represents a promising prophylactic vaccination regimen.

The first human case of Trichinella spiralis infection in Korea

  • Three cases of human infection by Trichinella spiralis were first confirmed by detecting encysted larvae in the biopsied muscle in December 1997, in Korea. The patients were one 35-and two 39-year-old males residing in Kochang-gun, Kyongsangnam-do. They had a common past history of eating raw liver, spleen, blood and muscle of a badger, Meles meles melanogenvs, and complained of high fever, facial and periorbital edema, and myalgia. Hematologic and biochemical examinations revealed leukocytosis and eosinophilia, and highly elevated levels of GOT, GPT, LDH and CPK. In the gastrocnemius muscle of a patient, roundly coiled nematode larvae were detected. The larvae measured 0.775-1.050 (av 0.908) mm in length, and 0.026-0.042 (av. 0.035) mm in maximum width. The specific IgG antibody levels in three patients' sera were significantly higher when compared with those of normal controls. The patients were treated with flubendazole and albendazole for 15-30 days, and discharged at 13-34 days post-admission. From the above findings, it was confirmed that T. spiralis is present in Korea, and the badger plays a role of as the natural host.

Eosinophilic Pleuritis due to Sparganum: A Case Report

  • Sparganosis is a rare parasitic disease caused by migrating plerocercoid tapeworm larva of the genus Spirometra. Infection in humans is mainly caused by the ingestion of raw or inadequately cooked flesh of infected frogs, snakes, and chickens. Here, we report a rare case of a 45-year-old man who was admitted to our hospital with left lower chest pain. The chest radiograph and computed tomography (CT) scan revealed localized pleural effusion in the left lower lobe; further, peripheral blood eosinophilia and eosinophilic pleural effusion were present. Percutaneous catheter drainage was performed, which revealed long worm-shaped material that was identified as a sparganum by DNA sequencing. The patient showed clinical improvement after drainage of the sparganum. This study demonstrates the importance of considering parasitic diseases in the differential diagnosis of eosinophilic pleural effusion.

  • Idiopathic acute eosinophilic pneumonia (IAEP), characterized by acute febrile respiratory failure associated with diffuse radiographic infiltrates and pulmonary eosinophilia, is rarely reported in children. Diagnosis is based on an association of characteristic features including acute respiratory failure with fever, bilateral infiltrates on the chest X-ray, severe hypoxemia and bronchoalveolar lavage fluid >25% eosinophils or a predominant eosinophilic infiltrate in lung biopsies in the absence of any identifiable etiology. We present a 14-month-old girl who was admitted to our pediatric intensive care unit because of acute respiratory distress. She had a fever, dry cough, and progressive dyspnea for 1 day. Chest X-ray showed multifocal consolidations, increased interstitial markings, parenchymal emphysema and pneumothorax. IAEP was confirmed by marked pulmonary infiltrates of eosinophils in the lung biopsy specimen. Most known causes of acute eosinophilic pneumonia, such as exposure to causative drugs, toxins, second-hand smoking and infections were excluded. Her symptoms were resolved quickly after corticosteroid therapy.

  • 저자들은 스테로이드 의존형 신증후군 환자에서 진단 8년 만에 두경부 종물이 발생, 혈액 검사상 IgE 증가와 호산구 증가, 경부 컴퓨터 단층 촬영으로 기무라병을 진단하였고, 스테로이드로 치료하였으며, cyclosporine을 투여한 이후 장기간 두경부 종물과 신증후군의 재발을 억제할 수 있었기에 문헌고찰과 함께 보고하는 바이다.

  • Nowadays Asthma is considered to be an inflamatory disease characterized by airway hyperresponsiveness and pulmonary eosinophilia. Hominis Placenta is the dried placenta of a healthy women. It has correspondence to the meridians of lung and kindey. Hominis Placenta acupuncture therapy has effect on invigoration of vital energy nourishing blood and tonifying the essence. It can be applied to the disease as Asthma, pulmonary tuberculosis, chronic hepatitis, liver cirrhosis, degenerative change and cerebrovascular disease. We treated two patients of Asthma with CVA by Homins Placenta Aqua-Acupuncture. The effect of Homins Placenta Aqua-Acupuncture was assessed by analyzing the pulmonary function test(PFT) and Quality of Life Questionnaire for adult Korean Asthmatics(QLQAKA) in patients before and after treatment. Total score was increased. The patients are satisfied our treatment. But further research concerning this is still necessary.

A Case of Immunoglobulin A Nephropathy in a Patient with Kimura's Disease

  • Kimura's disease is an angiolymphoid-proliferative disorder that manifests with benign subcutaneous swelling predominantly in the head and the neck. Kidney involvement, including proteinuria, occurs in 12-16% of patients with the disease, and 60-78% of such cases is nephrotic syndrome. Reported etiologies of nephrotic syndrome in Kimura's disease include membranous glomerulonephritis, mesangial proliferative glomerulonephritis, minimal-change disease, focal segmental glomerulosclerosis, diffuse proliferative glomerulonephritis and immunoglobulin A (IgA) nephropathy. There have been only two case reports of IgA nephropathy in Kimura's disease, in 1998. In this report, we present a third case of IgA nephropathy associated with Kimura's disease.
