Through Knee Amputation: Technique Modifications and Surgical Outcomes

  • Background Knee disarticulations (KD) are most commonly employed following trauma or tumor resection but represent less than 2% of all lower extremity amputations performed in the United States annually. KDs provide enhanced proprioception, a long lever arm, preservation of adductor muscle insertion, decreased metabolic cost of ambulation, and an end weight-bearing stump. The role for KDs in the setting of arterial insufficiency or overwhelming infection is less clear. The purpose of this study is to describe technique modifications and report surgical outcomes following KDs at a high-volume Limb Salvage Center. Methods A retrospective study of medical records for all patients who underwent a through-knee amputation performed by the senior author (C.E.A.) between 2004 and 2012 was completed. Medical records were reviewed to collect demographic, operative, and postoperative information for each of the patients identified. Results Between 2004 and 2012, 46 through-knee amputations for 41 patients were performed. The mean patient age was 68 and indications for surgery included infection (56%), arterial thrombosis (35%), and trauma (9%). Postoperative complications included superficial cellulitis (13%), soft tissue infection (4%), and flap ischemia (4%) necessitating one case of surgical debridement (4%) and four transfemoral amputations (9%). 9 (22%) patients went on to ambulate. Postoperative ambulation was greatest in the traumatic cohort and for patients less than 50 years of age, P<0.05. Alternatively, diabetes mellitus and infection reduced the likelihood of postoperative ambulation, P<0.01. Conclusions Knee disarticulations are a safe and effective alternative to other lower extremity amputations when clinically feasible. For patient unlikely to ambulate, a through-knee amputation maximizes ease of transfers, promotes mobility by providing a counterbalance, and eliminates the potential for knee flexion contracture with subsequent skin breakdown.

산업계 배출수에서 포름알데히드의 배출허용기준 설정방안 고찰 (A Study on the Determination of Formaldehyde Effluent Limitation in the Industrial Wastewater)

  • This study looked at how to establish effluent limitation standards for formaldehyde, a toxic chemical widely used in industries. To this end, we reviewed Water Quality Based Effluent Limitation (WQBEL), Technology Based Effluent Limitation (TBEL), and water quality criteria for protection of human health and aquatic organism. Based on the results, we estimated formaldehyde effluent limitation standards appropriate to control water quality of industrial wastewater in Korea. However, this study has limits due to the lack of some data necessary in estimating formaldehyde effluent limitation. For example, although water quality criteria based on non-carcinogenic properties of formaldehyde were calculated, those based on carcinogenic properties were not be able to estimate because of the absence of applicable cancer potency factor q1. Without applicable factor, we calculated water quality standards for formaldehyde based on water quality criteria of advanced countries including the United States, while with no water quality standard we referred to applicable drinking water quality standards of other countries. For eco-toxicity based on water quality criteria, proper figures could not be calculated since there have been few reliable data.

탄천 이용자의 인식조사를 통한 생태계서비스의 트레이드오프 관계 고찰 (A Study of Ecosystem Services Trade-off based on user Perception in Tancheon)

  • Since the Millennium Ecosystem Assessment Report was published in 2005, the conflict between development and the preservation of the ecological environment has turned to paying attention to value assessments in rational decision-making. In recent years, the concept of ecosystem services has been recognized as a viable one for making significant decisions. However, a trade-off between increase and decrease occurs between the ecosystem services sub-categories (cultural service, regulating service, supporting service, and provisioning service), for which research centering on target sites is needed. To this end, the present study aims to investigate the trade-off relationship between service categories to search for reasonable decision-making strategies. As the research method, a survey was conducted using the translated version of SoIVES(Social Value for Ecosystem Services) 3.0 questionnaire of the United States Geological Survey. The research findings demonstrate the economic value, based on the derived monetary value of each service category, the economic value of the target site was compared among landscape aesthetic value 8,050,000 won, recreation value 6,750,000 won, biodiversity value 4,610,000 won, healing value 3,970,000 won, life-sustaining value 2,090,000 won, and productive value 220,000 won. And then the primarily recognized value criteria of the Tancheon ecosystem services illustrate landscape aesthetic and recreation value. Besides, this study illustrates visualized trade-off relationships based on user perception, and the derived relations illustrate the trade-off relationship between the cultural service, regulating service, supporting service, and provisioning service, as well as relationships between the components of the sub-categories.

문화적 차이에 따른 침묵의 나선 효과 검증 (A Cross-Cultural Study of the Spiral of Silence Theory with Individualism-Collectivism and Uncertainty-Avoidance)

  • 본 연구는 침묵의 나선 이론의 핵심 구성요소인 소외에 대한 두려움과 공개 발언 의지가 문화적 가치에 따라 어떠한 영향을 받는지를 분석한 글이다. 그동안의 문화 간 침묵의 나선 이론 검증은 대부분 '개인주의-집단주의' 성향의 차이가 침묵의 나선 현상의 차이를 초래했다는 결론으로 이어졌다. 본 연구는 침묵의 나선 이론에 대한 문화적 가치의 영향을 살펴보기 위해 '개인주의-집단주의' 뿐만 아니라 '불확실성의 회피' 성향도 중요 변수로 검증하였다. 이를 위해 인도와 한국, 미국, 스페인 등 4개국에서 온라인 설문을 진행한 뒤 얻은 780명의 응답을 분석하였다. 그 결과, 개인주의는 소외에 대한 두려움을 낮추고, 집단주의 및 불확실성 회피성향은 소외에 대한 두려움을 높이는 것으로 조사됐다. 또 개인주의와 불확실성 회피성향은 공개발언 의지를 강화시키는 것으로, 소외에 대한 두려움은 공개발언 의지를 약화시키는 것으로 나타났다. 소외에 대한 두려움은 개인주의와 집단주의의 공개발언 의지에 대한 영향력을 매개하는 것으로 확인됐다.

불법조업 중국어선에 대한 해양경찰의 단속에 관한 고찰 (A Study on the Law Enforcement of Korea Coast Guard against the Illegal Chinese Fishing Vessels)

  • 우리 배타적 경제수역에서는 불법조업과 폭력행위 등을 반복하는 중국어선을 단속하기 위한 우리 정부의 계속적인 노력에도 불구하고 배타적 경제수역에서의 불법조업행위는 멈추지 않고 오히려 단속 해양경찰의 피해가 속출하고 있다. 한편, 범정부적인 노력에도 불구하고 해양경찰의 불법외국조업선에 대한 단속 과정에서 여러 문제점이 노출되고 있다. 해양에서 외국선박에 대한 단속은 한편으로는 정당한 사법절차의 진행으로 당연한 국가의 권한이지만, 이러한 집행이 적절하지 못하게 이루어질 경우 국제적 분쟁이 발생할 가능성도 존재한다. 따라서 본 논문은 관련 자료, 전문가의 견해 및 최근 사건 등을 바탕으로 우리나라 관할수역에서 발생하는 불법조업 중국어선에 대한 검문 검색이나 사법절차의 진행 등 단속과정 진행 중 제기되는 문제점을 분석하고 이에 대한 해결방안을 제시하고자 한다.

Ginseng Leaf Extract Prevents High Fat Diet-Induced Hyperglycemia and Hyperlipidemia through AMPK Activation

  • This study evaluated the protective effects of ginseng leaf extract (GLE) against high fat-diet-induced hyperglycemia and hyperlipidemia, and explored the potential mechanism underlying these effects in C57BL/6J mice. The mice were randomly divided into four groups: normal control, high fat diet control (HFD), GLE-treated at 250 mg/kg, and GLE-treated at 500 mg/kg. To induce hyperglycemic and hyperlipidemic states, mice were fed a high fat diet for 6 weeks and then administered GLE once daily for 8 weeks. At the end of the treatment, we examined the effects of GLE on plasma glucose, lipid levels, and the expression of genes related to lipogenesis, lipolysis, and gluconeogenesis. Both GLE groups lowered levels of plasma glucose, insulin, triglycerides, total cholesterol, and non-esterified fatty acids when compared to those in HFD group. Histological analysis revealed significantly fewer lipid droplets in the livers of GLE-treated mice compared with HFD mice. To elucidate the mechanism, Western blots and RT-PCR were performed using liver tissue. Compared with HFD mice, GLE-treated mice showed higher levels of phosphorylation of AMP-activated protein kinase (AMPK) and its substrate, acetyl-CoA carboxylase, but no differences in the expression of lipogenic genes such as sterol regulatory element-binding protein 1a, fatty acid synthase, sterol-CoA desaturase 1 and glycerol-3-phosphate acyltransferase. However, the expression levels of lipolysis and fatty acid uptake genes such as peroxisome proliferator-activated receptor-$\alpha$ and CD36 were increased. In addition, phosphoenolpyruvate carboxykinase gene expression was decreased. These results suggest that GLE ameliorates hyperglycemia and hyperlipidemia by inhibiting gluconeogenesis and stimulating lipolysis, respectively, via AMPK activation.

흰가루병균(病菌) Microsphaera속(屬)에서 동양(東洋) 3종(種)의 무성세대(無性世代) 형태(形態) (Anamorphic Morphology in Three Oriental Species of Microsphaera)

  • Microsphaera속(屬) 흰가루병균(病菌) 중에서 아시아의 극동지방(極東地方)에서만 채집(採集)되는 M. akebiae, M. picrasmae, 그리고 M. schizandrae 등 3종(種)을 각각 으름, 소태나무 그리고 오미자에서 채집(採集)하여 이들이 갖는 무성세대(無性世代)의 형태적(形態的) 분류특성(分類特性)을 검경(檢鏡)하였다. 그 결과, 지금까지 잘못 알려진 형태적(形態的) 특성(特性)을 수정기재(修正記載)하였으며, 보고(報告)되지 않았던 몇 가지 분류특성(分類特性)을 추가기재(追加記載)하였다. 이들 3종(種)에서 관찰된 많은 분류특성(分類特性)들은 Microsphaera속(屬)에 공통적(共通的)으로 나타나는 것이었다. 즉, 부착기(附着器)는 파상(波狀) 내지 굴곡상(屈曲狀)으로서 쌍(雙) 또는 단독(單獨)으로 존재(存在)하였고, 분생자경(分生子梗)은 모세포(母細胞) 당(當) 1-2개(個)였고, 분생포자(分生胞子)를 단생(單生)하였으며, 기부격막(基部隔膜)은 균사(園絲)와의 분기점(分技點)에 있었다 . 분생포자(分生胞子)는 피브로신체(體)를 함유(含有)하지 않았으며, 발아관(發芽管)은 포자(胞子)의 끝에 위치(位置)하였다. 그러나 이들 3종(種)은 모두 분생자경(分生子梗)의 기부세포(基部細胞)가 굴곡상(屈曲狀)으로서 대부분의 Microsphaera속(屬) 흰가루병균(病菌)과 달랐으며, 각 종(種)은 분생자경(分生子梗)의 크기와 분생포자(分生胞子)의 크기 및 형태(形態) 등으로 서로 유별(有別)할 수 있었다. 따라서 이들 흰가루병균을 무성세대(無性世代)에서도 동정(同定)할 수 있게 되었다.

강릉지역의 전통마직물에 관한 연구 (A study on the Traditional Hemp-Textile in Kang-Reung Probvince Area)

  • In KOREA, the history of weaving is so long even in prinitive, there are the traces of fabric emmision using Spindle already in the Neolithic age. And coming up to the period of Three States, becoming active of cultivation of fibers, it can be known the variety of kinds and the production of fine products by improvement of new method of weaving using weaving machine. In the period of shilla state, there is a record of making the fine fabrics with 28 bracts. But wearing of silks fabrics was limitted for only the nobility and common people were wearing native thick hemp clothes. Also in the period of Korea state, they were wearing the hemp clothes by cultivation of hemp. The good quality products were worn by the King or the nobility and women in KOREA made their best with whole efforts to pay to the authorities with woven products of hemp clothes, so in the 14th year of King Chung- Ryul, King prohibited the presentation of fine hemp clothes by his order. By the end of KOREA state, before the production, common people was mainly wearing the hemp clothes. Coming up to the Kingdom of Chosun the sericulture was promoted by establishment of the sericulture encouraging low. Therefore the working hours of women were highly increased. The products of Song-do, Chin-ju for cotton clothes, those of Han-san-the same now as in old times-for ramie clothes, those of Han-Kyung province and An-dong for hemp fabrics were estimated as the best qualities. And the hemp clothes of Kang-won province is not so fine but is very useful and famous for mourning clothes and summer clothes for the farmer. It is true that our history of weaving was begun with hemp as a continous and precious friend of common people during all the historical periods-even though for a while it went backward because of chinese silks.

병원 인력관리에 관한 관계법규 고찰 (Related Regulations of Hospital Personnel Management)

  • As a hospital deals with people in their particular states, namely patients, computerization and automation in organization systems are very limited. Even though state-of-the-art medical systems such as the OCS, the HIS, the EMR, and the FACS are facilitating the computerization and informatization processes, they are for convenience and effectiveness. Ultimately, however, we should depend on specialists including doctors, nurses, pharmacists, and medical engineers. Therefore, a hospital is a representative labor-intensive body. Like other similar organizations, hospitals require a lot of manpower. But they are quite different in that hospital people hold variety and complexity in their qualifications and licenses. In personnel management, a hospital is twice controlled owing to the special characteristics that human life is at stake. First, the quota of medical manpower should be obeyed lest the quality of medical services should be lowered, and their roles and interrelations are even regulated. Second, in spite of the peculiarity of hospitals, the duties of obligatory employment and social insurances should not be neglected like other companies. In order that each hospital can preserve the proper level of medical services, securing the appropriate level of medical personnel has to be regulated. However, as the personnel cost is one of the important indices of hospital management, too much regulation in manpower supply can lead to poor hospital management and, in the end, the drop of the quality of medical services. In sum, as far as hospital personnel is concerned, some autonomy ought to be given to each hospital so that it can control the quality of hospital services. In addition to this minimum regulation of personnel, certain incentive and reward systems like the graded nursing system need to be prepared.

SIMULATION AND ANALYSIS OF AN AUTOMOBILE PRODUCTION FACILITY

  • 벤츠(현 다임-클라이슬러)자동차회사는 새로운 공장을 미국 앨라배마 주 Vance 지역에 건설하고 이곳 공장으로부터 스포츠-레저용의 4륜 구동 M-class 자동차를 생산하고 있다. 이 벤츠공장의 생산라인은 차체형성공정, 도장공정, 그리고 엔진조립 공정으로 나누어져 있고 이들 생산라인사이에는 한 라인에서의 고장(down)이 다른 라인에 파급되지 않도록 완충라인(Buffer)이 설치되어 있다. 본 연구의 목적은 일정한 비율의 고장률을 각 생산라인에 적용시켰을 때 주어진 생산목표(270대)를 달성할 수 있는지, 만약 달성할 수 없다면 가장 문제가 되는 생산라인은 어디인지, 또 생산라인 사이에 존재하는 완충라인이 어느 정도의 완충역할을 하는지, 최적의 생산능력을 유지하기 위한 완충라인의 크기는 어느 정도인지 등, 각종 생산라인의 운영지침(operation policy)들을 시뮬레이션 기법을 이용하여 탐색한 후 병목생산시설(bottleneck)을 찾아내는 것이다. 본 연구를 위한 시뮬레이션 도구로는 ARENA를 이용했고 20일 간의 시뮬레이션 결과를 종합하여 분석하였다. 또 신뢰구간 95% 범위에서 각 생산라인의 최대 생산능력도 측정하여 실제 생산대수와의 차이를 규명할 수 있는 근거를 제공하였다.
