제주도 119구조·구급대의 활성화 및 전문화 방안 (A Study on the Activation·Specification of 119 Rescue & Care in JeJu)

  • Since 1992, conventional fire fighting businesses have been converted into a metropolitan autonomous fire fighting system to be ready for a variety of disasters. However, the corresponding investment has been overlapped due to the non-integration of businesses to prevent any potential disasters, and a series of collaborative systems have been not functioning so well. In the meantime, our fire fighting sector has been trying to set up its own clean and faithful position by abolishing any inconvenient system or outsourcing private sectors, and expanded its work scope from conventional fire fighting even to rescue and emergency works. While focusing on handling disaster, the fire fighting sector has been widely trusted and reliable throughtout our nation. Moreover, our fire fighting sector has secured nation wide mobile organizations, technical personnel by field, special equipments and independent communication network. In addition, the fire fighting sector has knowledges, expertise and capabilities required for managing disasters, while in charge of almost every disaster management works including fire, explosion, collapse, disaster and flood. It becomes an organization for comprehensive disaster management under an absolute national trust, which is based on the system for prevention, preparation and countermeasure against a variety of disasters. Thus, our fire fighting sector must make many efforts and try to modernize conventional education and training. The ways to facilitate rescue and emergency works may include the nurture of technical fire fighting personnel along with modernized equipments, the reinforcement of rescue and emergency education, the facilitation of operating civil defense corps, the facilitation of operating volunteer fire fighting corps, the better arrangement of 119 briefing room for public healthcare in provincial offices, the sterilization of rescue instruments and equipments the better repair education for emergency rescue member, the establishment of regional emergency assistant system and the expansion of fire fighting personnel and equipments. In terms of reinforcing the functions and services of rescue 119 and emergency corps, we must review the following considerations: Building up security system along with operational expansion, building up a system for emergency medical treatment, building up a comprehensive information management system for rescue and emergency, constructing a provincial safety museum and so forth. For the ways to better the works of rescue 119 we can review the following considerations : Improving the education for fire fighting training corps under Jeju Provincial Fire and Disaster Management Department, providing rescue members with more opportunities for clinical practices, enhancing the morale of rescue members, installing a comprehensive briefing room for emergency rescue members, building up medical networks along with reasonable policies for information service, operating the consulting system for rescue 119 and so on. If these requirements are met, it is expected that the fire fighting departments in Jeju province can cope with every accident and disaster a little more rapidly and quickly in compliance with local needs, so that they can keep their own position as a public fire fighting organization which may be trusted by the public.

스마트 응급의료 시스템 구현 (Implementation of the Smart Emergency Medical System)

  • 최근 구급차에 탑승하는 응급 구조사의 인원이 턱없이 부족하여 소방관 한 사람만이 구급차에 타고 환자를 이송하는 일이 종종 문제시 되곤 한다. 응급환자가 발생 시 신속하고 정확한 조치가 무엇보다도 필요하며 특히, 환자에게 적합한 의료기기가 있는 전문적인 병원으로 이송해야한다. 본 논문은 스마트 폰을 이용하여 병원 전 단계의 응급의료 시스템을 구현한다. 구현된 시스템은 환자 이송 중에 환자의 심박동을 실시간적으로 모니터링이나, 과거 환자의 병력 정보 등을 파악할 수 있으며, 환자가 필요로 하는 응급 병원 정보도 제공하여 병원 전 단계에서 응급 환자에게 신속하고 효율적인 처치가 가능한 환경을 제공한다.

응급의료센터로 전원된 환자의 진료의뢰서 표준화 및 충실도에 관한 연구 (A study on standardization & completion of transfer consultation record for patients transferred to emergency medical center)

  • The purpose of this research which was conducted by surveying the transfer consultation records from 360 medical institutions such as general hospitals, hospitals, clinics to the Emergency Medical Center at E University Hospital for six months(Jan. 1, 2000 - Jun. 30, 2000) are to standardize & complete transfer consultation record of hospitals at the 1st & 2nd referral level and to give patients transferred emergency medical center medical information services on a better quality. The conclusions and suggestions from this study were summarized as follows; (1) Examing the distribution of the referral medical consultation(transfer) sheet type, surgery part local clinic sheet types were 34.4%, medical part local clinic sheet types were 26.7%, undifferentiated local clinic sheet types were 23.9% and hospital level sheet types were 15.0%. (2) The items of the transfer consultation records had been standardized more than 75% in the order of patient's name, date, doctor's name, diagnosis, patient's status, impressions. (3) That the degree of recording completion on these items is in the order of patient's name, date, diagnosis, impressions was revealed. (4) Because the standardization and the degree of recording completion are very low in the patient's gender, age, address, electronic recording system was needed for more perfect input of initial patient informations. (5) This standardizing & complete recording on examination and medication will prevent re-examination and abuse of medication for patients transferred emergency medical center. (6) EMT Transfer System should be fixed in all medical institute for the standardizing & complete recording on care period and departure time will give many emergency patients the proper treatments at the proper time. (7) It was revealed that developing new standardized transfer consultation record & using electronic recording system are needed. (8) The complete recording & Fast Track System were needed for higher rate of bed operation at emergency medical center and more hospital profit.

An Application Design for Emergency Medical System: A Software Engineering Approach

  • The proposed application design for emergency medical system includes the functions that help securing the window of opportunity for the patients in an emergency situation and efficiently link rescue agency, emergency medical treatment center and patients together. First, the treatment time can be saved at the emergency room by understanding the patient's condition by entering his/her health information such as anamnesis, family history, social history and blood type together with an identification number. Also, if the siren function which signals whereabouts of patients to the rescue crew is used, it will be much easier to find the location of the patient in need. The design allows rescue crew to call the patient to check whether he/she is conscious or not. Just by receiving the call, he/she will be deemed conscious.

환자 의료정보 접근을 위한 상황인식 기반의 인증서비스 모델 개발 (Development of Authentication Service Model Based Context-Awareness for Accessing Patient's Medical Information)

  • 최근 유비쿼터스 기반의 의료 및 헬스케어 환경이 구축됨에 따라 다양한 센서들로부터 상황정보를 얻을 수 있는 의료정보시스템이 증가되고 있다. 이러한 상황인식 기반의 의료정보시스템 환경에서는 상황정보를 이용하여 환자상황을 정상 또는 응급으로 판별이 가능하며, 또한 의료진은 스마트기기의 애플리케이션을 통해 ID와 Password를 이용한 간단한 사용자인증이후 환자정보에 쉽게 접근이 가능하다. 그러나 이러한 인증서비스 및 환자정보접근서비스는 의료진 중심의 시스템이며, 유비쿼터스 기반의 의료정보시스템 환경을 충분히 고려하지 못한다. 이에 본 논문에서는 의료정보에 접근하는 사용자에 대해 기존 의료진 중심의 인증과는 달리 상황정보 중심의 인증서비스를 제공하기 위한 상황인식 기반의 인증서비스 모델을 제시하고 구현하였다. 본 논문에서 제안한 상황인식 기반의 인증서비스 모델은 센서들을 통해 환자상황을 인식하여 환자상황에 의하여 의료진의 인증방식 및 권한부여를 다르게 진행하는 서비스이다. 다양한 상황정보 측정환경을 구성하기 위해 웨어러블 및 생체데이터측정모듈, 카메라센서 등을 이용하여 구현하였다. 환자상황이 응급상황일 경우, 의료정보서버는 담당의료진의 스마트기기에 응급메시지를 전송하고, 응급메시지를 받은 담당의료진은 환자정보에 접근하기 위해 스마트기기의 애플리케이션을 이용하여 인증을 시도한다. 모든 인증이 완료되면 의료진은 상위등급 의료정보 접근권한을 부여받아 정상상황에서 볼 수 없었던 환자 의료정보까지 확인이 가능하도록 하였다. 제안한 상황인식 기반의 인증서비스 모델을 통하여 유비쿼터스 의료정보시스템 환경을 충분히 고려할 뿐만 아니라, 환자중심의 시스템적인 보안 및 접근투명성을 강화하였다.

MBAN(Medical Body Area Network)에서의 온톨로지 기반 상황인지 시스템 개발에 관한 연구 (A Study on the Ontology-Based Context Aware System for MBAN)

  • The u-Healthcare system, a new paradigm, provides healthcare and medical service anytime, anywhere in daily life using wired and wireless networks. It only doesn't reach u-Hospital at home, to manage efficient personal health in fitness space, it is essential to feedback process through measuring and analyzing a personal vital signs. MBAN(Medical Body Area Network) is a core of this technology. MBAN, a new paradigm of the u-Healthcare system, can provide healthcare and medical service anytime, anywhere on real time in daily life using u-sensor networks. In this paper, an ontology-based context-awareness in MBAN proposed system development methodology. Accordingly, ontology-based context awareness system on MBAN to Elderly/severe patients/aged/, with measured respiratory rate/temperature/pulse and vital signs having small variables through u-sensor network in real-time, discovered abnormal signs and emergency situations which may happen to people at sleep or activity, alarmed and connected with members of a family or medical emergency alarm(Emergency Call) and 119 system to avoid sudden accidents for early detection. Therefore, We have proposed that accuracy of biological signal sensing and the confidence of ontology should be inspected.

효율적인 응급의료 정보전달매체로서의 119구급활동일지 분석 (An Analysis of the 119 EMS System using the Standardized Record on the Efficient Emergency Medical Information Delivery Media)

  • 이 연구를 위해 2009년 1월1일부터 2009년 2월 8일 까지 일개 종합병원에 119구급대로 내원한 모든 환자의 구급활동일지 255부를 조사하였다. 구급활동일지의 전체 기재율 62.1%, 가장 높은 항목은 환자 인수자에 대한 기록으로 100.0%, 가장 낮은 항목은 의사지도에 관한 항목으로 0.4%로 나타나 기대치에 미치지 못한 것으로 조사되었다. 효율적인 응급의료 정보전달매체로서의 119구급활동일지의 기재율을 높이기 위해서는 전문 인력의 확충과 의료진의 적극적인 관심 및 피드백, 구급활동일지의 항목 배열의 규칙성, 기록의 중요성에 대한 지속적인 교육이 필요할 것으로 사료된다.

국가재난의료매뉴얼의 개발 (Development of the Disaster Medical Manual in Korea)

  • 세월호 참사 등 각종 재난에 있어 재난의료대응체계의 문제점이 노출되었는데, 이 중 현장응급의료지원팀의 현장대응 불가, 현장에서의 소방, 의료진의 협업 곤란, 현장의료지원 및 부상자의 수용, 진료를 위한 체계적이고 세부적인 매뉴얼 미비가 중요한 사항이었다. 따라서 2014년 5월부터 중앙응급의료센터 내 재난응급의료상황실 설치 운용에 따른 기존의 비공식적으로 흩어져 있던 재난의료대응 매뉴얼 수집, 수정 및 이에 대한 교육이 필요한 상황이 되었으며, 재난응급의료상황실을 중심으로 한 재난 초기 의료대응체계를 확립할 필요성이 대두되었다. 중앙응급의료센터의 재난의료대응 개선 방안은 신속대응반 중심의 빠른 출동 체계 수립이나, 실제 적용할 체계가 미흡하여, 2014년 2월 응급의료법 개정 시행규칙에 따라 국가와 지자체에 재난대응 매뉴얼을 개발하고 기본안을 마련하기로 하여, 본 연구진은 학술적, 실무적으로 적절한 국가재난의료 매뉴얼을 국내 최초로 개발하였으며, 이를 훈련 및 시뮬레이션을 통하여 적용하였고 그 과정을 정리 분석하였다.

제주 지역 중증 응급 질환의 병원 전 이송 취약 지역에 대한 격자 기반 지리 공간 분석 (Grid-based geospatial analysis of areas vulnerable to prehospital transportation of emergency patients in Jeju)

  • During emergencies, the time from symptom onset to definitive treatment determines the final outcome. Therefore, the emergency medical service (EMS) system in Korea, aims to transfer patients requiring emergency care to appropriate medical facilities within 30 minutes. This is in an attempt to improve the chances of survival and reduce sequelae. We attempted to locate areas vulnerable to prehospital transportation and identify hot spots with high demand for emergency medical helicopters in Jeju, by using a grid-based geospatial analysis. This retrospective cross-sectional observational study employed EMS data of 119 ambulance run sheets spanning from January 1, 2010 to September 30, 2018 in Jeju. The location data of emergency patients was superimposed on the spatial analysis frame using the geographic information system (GIS). Subsequently, the locations of long-distance transfer and delayed transfers to the hospital were analyzed, to identify hot spots where the demand for helicopter emergency services would be high. Of the total analysis targets, 42.2% (20,288 people) took more than 30 minutes from reporting to 119 dispatchers to hospital transfer. As the transfer time interval increased, the patient occurrence time increased in the city of Jeju, increased in Seogwipo, and the ratio of patients/guardians to select a transfer hospital rose with significant differences. This study identified the characteristics related to time delays in prehospital transfer of emergency patients in Jeju, and the areas vulnerable to prehospital emergency care were derived and visualized through spatial analysis using the GIS.

COVID-19 기간 119구급차를 통해 일개 응급의료센터에 내원한 환자 중증도의 변화 (Change in the severity of patients visiting emergency medical centers through the 119 ambulance during the COVID-19 pandemic)

  • Purpose: Despite the fact that emergency medical centers (ED) are used by emergency patients, more than 50% of non-emergency patients have been reported to be admitted in EDs, of which more than 10% of them used the 119 ambulance. Therefore, this study investigated whether there was a change in the severity of patients visiting EDs during the first year of the COVID-19 pandemic. Methods: This was a retrospective study of patients who visited the ED through the 119 ambulance in 2019 (13,735) and 2020 (11,015). Patient data were analyzed using the medical information system. Results: There was an increase in non-emergency patients at the emergency departments (KTAS levels 4-5) in 2020 with a rate of 58.0% (6,393), as compared to the 2019 data with a rate of 44.9% (6,169). However, the 2020 median length of stay in the emergency department was 188 minutes (IQR: 0-5,909minutes) (p=.000), which was decreased as compared to the 2019 median length of stay of 231 minutes (IQR: 5-6,211minutes) (p=.003). Conclusion: Providing emergency ambulance guidelines for selecting proper hospitals and educating patients to refrain from using the ED for non-emergency patients should be taken into consideration to prevent overcrowding and construct a more effective emergency medical system (EMS).
