The Effects of Abdominal Relaxed Breathing Training on Stress response and Blood Pressure for Elderly Hypertensionve Patient (고혈압 노인에서 복식호흡 이완훈련이 혈압 및 스트레스 반응에 미치는 영향)

  • In order to improve self-management of elderly hypertensives in this study, the researcher examined the effect of blood pressure regulation by an abdominal relaxed breathing training which regulates physical response to stress and lessens the activity of the sympathetic nerve. Method: A quasi-experimental pre-test and post-test design was used. Thirty elderly hypertensive patient ; Sixteen in the abdominal breathing group and fourteen in the control group participated in this study. The abdominal relaxed breathing training consisted of eight sessions twice a week for four weeks. There was no intervention for the control group. Result: 1. There was a significant decrease in systolic and diastolic blood pressure between the experimental group and control group over three different times, and interaction by groups and over time. 2. There was no significant difference in the level of total stress response, physical stress response, behavioral-cognitive stress response between the experimental group and control group over three different times, and interaction by groups and over time. Conclusion: Findings indicate that this study will contribute to develop nursing strategies for the regulation of blood pressure for the elderly, which is easy for the elderly to learn as a nonpharmacologic approach.

Protein Electrophoresis Fraction in Serum of the Rural Elderly Patient

  • We analyzed total protein (TP), albumin (AL), aspartic aminotransferase (AST), alanine aminotransferase (ALT), urea nitrogen (UN), creatinine (CRE) and serum protein electrophoretic fractions in sera of rural elderly patients to evaluate the health status in rural elderly patients. We observed that the frequencies of patients showed lower level of total protein, albumin, and both total protein and albumin than them of reference range were 20.3%, 22.8% and 19.0%, respectively. The rates of patients showed higher level of AST, ALT, both of AST and ALT, UN, creatinine and both of UN and creatinine than them of reference range were 33.8%, 40.0%, 30.0%, 17.7%, 15.2% and 7.9%, respectively. Comparison of protein fractions of each patient to reference range showed that 77.2% of patients showed normal in all of patterns. Few of patients showed abnormal pattern in albumin (13.6%), ${\alpha}1-globulin (0.0%),\;{\alpha}_2-g10bulin(1.1%),\;{\beta}-globulin(1.9%)\;and\;{\gamma}-globulin(6.2%)$. These data suggest that many of rural elderly patients may suffer from heart, liver and kidney diseases. The serum protein fractions are not typical criterion to evaluate the disease, but production of proteins in rural elderly patients may be affected by liver disease and kidney disease.

A Study on the Quality of Life in Pneumonconiosis Elderly Patient (진폐 노인환자의 삶의 질에 관한 연구)

  • Purpose: The purpose of this study was to find out the quality of life and to identify the related characteristics for nursing care in pneumconiosis elderly patients. Methods: The subjects for this study were 206 elderly patients who received pneumoconiosis treatment in the two pneumoconiosis specialized hospitals located in Gangwondo, Korea. The instrument used for this study was Quality of Life Index Pulmonary version III. The data was collected from February 17 to March 10, 2003, and analyzed by t-test, ANOVA, Duncan test using SPSS. Results: The results of this study were as follows : 1. The highest frequency in chief complaints was dyspnea, 91 subjects(44.2%), and complications was tuberculosis, 88 subjects (42.7%). 2. The total mean score of the level of quality of life was $14.49{\pm}2.18$. 3. The score of quality of life showed higher in good family relationship group than the other after diagnosed pneumoconisis(F=5.486, P=0.001). 4. The comparison of quality of life was significant according to oxygen use(t=2.674, P=0.008), bronchodilators use(t=2.678, P=0.008), and prospect of future health status concerning pneumoconiosis(F=2.960, P=0.021). Conclusions: In conclusion, adequate nursing intervention as effective management of respiratory symptoms and improvement of family support will be needed to improve the quality of life in pneumoconiosis elderly patients.

A Study of Depression and Quality of Life in Family Care Givers of the Stroke Elderly Patient (뇌혈관질환 노인을 돌보는 가족원의 우울과 삶의 질에 영향을 미치는 요인에 관한 연구)

  • This study was designed and undertaken to identify the related factors of family caregivers' depression & quality of life with stroke elderly patient. The data was collected from August 16th to September 5th. 1999. The subjects in this study were 70 caregivers and 70 patients with stroke who were hospitalized in 2 oriental medicine hospitals and 3 hospitals located in Junla-buk do. The data was analized using percentage. means. t-test. ANOVA and pearson's correlation coefficients, step-wise multiple regression done with the SAS program. The results of this study are as follows; 1. The score for family caregivers' depression was 45.2 when total score was 80. The family caregivers who got more than 50 scores belongs to highly depression group amount to $29\%$. 2. The score for family caregivers' quality of life was 37.04 when total score was 56. 3. In the significant relationship between family caregivers' depression and general characteristics of the family caregivers ; age. sex, income. In the significant relationship between family caregivers' quality of life and general characteristics of family caregivers: age, education, income. In the significant relationship between family caregivers' depression and quality of life and general characteristics of stroke elderly patient ; sex. 4. The depression degree showed significant differences in the variables of family caregiver's physical health(r=-0.307, p=0.011), stress(r=0.463. p=0.011). social support (r=-0.241. p=0.046) and elderly stroke patient's ADL(r=-0.313, p=0.009). The quality of life degree showed significant differences in the variables of family caregivers' depression(r=-0.564, p=0.001), stress(r=-0.322, p=0.008), social support (r=0.353. p=0.003). 5. The most important variable affecting family caregiver's depression was caregivers' physical symptom which accounted for $32.0\%$ of the total variance in which multiple regression analysis. Total variance affecting the family caregivers' depression was $49\%$. The most important variable affecting family caregivers' quality of life was caregivers' depression which accounted for $48\%$ of the total variance in which multiple regression analysis. Total variance affecting the family caregivers' quality of life was $61\%$.

A Case Report of Oriental Medical Treatments Combined with Exercise Therapy on the Frail Elderly Patient with Gait Disturbance and Cognitive Decline (노쇠 환자의 보행장애 및 인지 기능 저하에 대한 운동요법을 병행한 한방치료 증례보고)

  • The purpose of this study is to report the effect of oriental medical treatments with exercise therapy on the frail elderly patient with gait disturbance and cognitive decline. The frail elderly patient with gait disturbance and cognitive decline was treated by oriental medical treatments combined with exercise therapy during 3 months. The improvement of clinical symptom was evaluated by Manual Muscle Test (MMT), Passive Range Of Motion (PROM), Mini Mental State Examination-Korea (MMSE-K). After 3 months, all of both lower extremity Manual Muscle Test (MMT) improved to Grade 4. Both knee extension Passive Range Of Motion (PROM) and Mini Mental State Examination-Korea (MMSE-K) also increased from -30/-30 to 0/-5, from 7 to 25 respectively. It could be suggested that oriental medical treatments with exercise therapy were effective on the frail elderly patient with gait disturbance and cognitive decline. And further research is encouraged to confirm the effectiveness of this treatment with a larger number of patients.

The Effect of Preoperative Education about Patient Controlled Analgesia on Postoperative Pain Control of Elderly Receiving Total Knee Arthroplasty (수술 전 통증자가조절기 교육이 슬관절치환술 노인의 수술 후 통증에 미치는 효과)

  • Purpose: The purpose of this study was to investigate the effect of preoperative education about patient controlled analgesia (PCA) on postoperative pain control for elderly after total knee arthroplasty. Methods: The study applied a quasi-experimental design. To prevent communication between experimental group and control groups, data from control group were collected before provision of preoperative education for the experimental group. A total of 50 elderly patients who underwent total knee arthroplasty and older than 65 years old participated in this study. The preoperative education about PCA was provided for the experimental group before surgery. The preoperative education program consisted of fifteen minute education about pain control, and PCA use, as well as demonstration of PCA use. Results: The experimental group had higher knowledge score about pain and PCA use, and more positive attitudes toward pain and use of analgesics after surgery than the control group. There was no significant difference in use of additional analgesics after surgery between the two groups. The experimental group had significantly lower pain score at 8, 24 and 36 hours after surgery than the control group. The experimental group had higher level of satisfaction about PCA use than the control group. Conclusion: The preoperative education about PCA, customized for elderly patients could be an effective nursing intervention for postoperative pain control after total knee arthroplasty.

A Study on the Burdens and Depressive Reactions on Families who Cared for Patients Suffering from Senile Dementia (치매노인을 돌보는 가족의 부담감과 우울반응에 관한 연구)

  • The purpose of this study was to investigate the relationship between the burden on families who live with an elderly person suffering from senile dementia, and the degree of their depression. There were 400 participants in this study, staying in the Seoul and Kyonggi areas from August 1, 1997 to February 28, 1998. Among the group, 100 participants took care of their patient at home, and another 300 participants left 100 patient at a day-care center, 100 sanatorium for senile dementia(asylum for helpless elderly people), 100 an infirmary for elderly people. Eventually 242 subjects out of the 400 were selected for the data analysis. The Zarit (1980) tool was employed to measure the degree of burden and Zung's(1965) “Self-Rating Depression Scale” was employed for the data analysis. The data was analyzed, and the percentage, t-test, ANOVA and Person's Correlation Coefficient were calculated. The results are as follows : 1. The average degree of burden that care-giving families felt was 49.13, which is somewhat high. 2. The average degree of depression that care -giving families felt was 51.95, which is relatively high. 3. The degree of burden was directly affected by the relation with the patient(F=2.48, P<.05), and the socio-economic status of the family(F=5.17, P<.05). Its also affected by the patient's educational status(F=2.17, P<.05). 4. The degree of depression of the family was significantly dependent on sex(t=-2.05, P<.05), age (F=2.99, P<.05), the relationship with the patient(F=3.65, P<.01), socio-economic status (F=7.74, P<.001), occupation(t=2.82, P<.01), health status(F=4.42, P<.01), and the place of residence(F=4.30, P<.01), The patient characteristics was significantly dependent on his/her educational status(F=3.85, P<.01), the period of suffering from senile dementia(F=2.47, P<.05), and smoking habit(F=6.17, P<.001). 5. The relationship between the degree of burden and that of depression reads r=0.43, which is statistically positive correlation in the high significant level. Upon analyzing the entire summation, most care-givers for elderly patients suffering from senile dementia lack time in caring for themselves. They also experience chronic fatigue and mental discomfort caused by the isolation from society, curtailment of certain activities, a sense of responsibility for their patients, and limits of their endurance in taking care of their patients over time. In conclusion, this study emphasizes the necessity for the following propositions : 1. In order to measure the degree of burden that Korean care-giving families undergo, a new tool must be developed on the basis of Korean culture. 2. An educational program based on the demands that care-giving families undergo must be developed, and its clinical effect also has to be examined.

A Study on the Status of Utilization of Health Care Institution by Geriatric Patients -Focus on the Utilization of Physical Therapy- (노인환자 보건의료기관 이용실태 분석)

  • This study was conducted to provide basic materials required to enforce and develop welfare policies, as well as the health system, for the aged, by surveying the status of health care utilized by the daily increasing old population and the importance of receiving physical therapy. Data that need in this research was gathered from over ages of 65, during the period from Jan 4, 1996 to Jan 31, using the inquiries previously made by geriatric researchers and through literatures investigator by this writer. The data were analyzed by $X^2$, Z-test, Likert scale. The findings were as follows : 1) General characteristics of subjects. People in the age group between 65 and 69 were 55.6% and the highest number, while male were 37% and female 63%. Analysis of income group disclosed 60.6% whose monthly income, including the pocket money given by children, was less than 200,000 won. 91.1% of the elderly people surveyed owned houses; only 36.4% live with spouses; while 15.6% live alone. 2) Characteristics with respect to utilization of health care institution. 56% of the total medical institutions used by the elderly people were clinics and the rates of chronic disease and musculoskeletal disease were 73.2%. 3) Characteristics with respect to approach of health care institution. 45.1% of the respondent stated it took 20 minutes to arrive at hospital, and bus accounted for 48.6% of all transportation means used to go to hospital. 4) Degree of cognition with respect to the rights of geriatric patients. (1) There is no financial support from the government for geriatric patients(71.4%). (2) Government financial support is needed for geriatric patients(95.3%). (3) Have never been regionally surveyed or called upon for interviews with respect to treatment desire and problems relating to geriatric patients(87.2%). (4) Health and medical policies for geriatric patients must be established rapidly(98.4). (5) Expansion and construction of specialized medical facilities for geriatric patients such as elderly hospital and medical center are needed(90.2%). (6) Government's welfare policies for the elderly people is insufficient(82.0%) 5) Degree of cognition on importance of physical therapy with respect to geriatric patient. (1) Physical therapy is considered most effective in treating geriatric patients(82.9%). (2) Physical therapists specializing in only elderly people must be need of separately(76.2%). (3) It is desirable for medical specialists to visit geriatric patients at home to provide physical therapy(82.9%). (4) Hospitals specializing in physical therapy for geriatric patient are required(85.6%). Based on the result for this research, the following suggestions are presented to facilitate the utilization of health care institution for the welfare of geriatric patients. Medical facilities such as elderly hospital and geriatric patient's medical center specializing in elderly people must be constructed as early as possible; and home-visiting physical therapist system must be important to treat chronic geriatric patients; our government must establish policies to provide the old ages with means for the health care and curing chronic diseases, and carry out the plans of reasonable distribution and effective untilization of medical resources.

Arthroscopic Decompression of an Inferior Paralabral Cyst of the Shoulder in an Elderly Patient: A Case Report

  • The widespread use of diagnostic radiography, especially using magnetic resonance imaging, has helped to increase the diagnosis of paralabral cysts in patients with chronic shoulder pain. These paralabral cysts are frequent in the anterior, the superior, and the posterior compartment of the shoulder joint but are rare in the inferior compartment. Paralabral cysts in the shoulder appear particularly in men in their third and fourth decades but rarely in elderly patients. We report a case of an inferior paralabral cyst in an elderly patient whom we treated through arthroscopic decompression.

Unilateral Isthmus Resection for Elderly Foraminal Stenosis

  • We present an elderly patient with unilateral foraminal stenosis treated by isthmus resection. An 83-year-old female could not walk due to severe leg pain along right L5 sensory dermatome. Despite the laminotomy for spinal stenosis on the right side at the L4-5 level, her leg pain did not improve. Careful review of computed tomography scans and coronal source images of magnetic resonance myelography revealed foraminal stenosis on the right side at the L5 vertebra. Because of medical problem, she underwent isthmus resection on the right side at the L5 level instead of total facetectomy and fusion. After surgery, her leg pain was markedly improved. Isthmus resection showed successful result for this medically compromised elderly patient with unilateral foraminal stenosis.
