Mandibular Advancement Devices for Treating Snoring and Obstructive Sleep Apnea

  • Many therapeutic modalities including continuous positive airway pressure, surgery, and oral appliances are used to treat patients with sleep-disordered breathing. However, there are no definitive treatment modalities for individual patients due to various causes of sleep-disordered breathing. Clinicians should have select best options for individual patients and it is quite challenging process. Oral appliances attracted clinical attention for its convenience and safety. Several designs of oral appliances are introduces such as soft palate lifter, tongue retaining device, and various appliances which aimed to mandibular advancement. Among these oral appliances, mandibular advancement devices (MADs) are considered the most excellent based on their effectiveness and patient tolerance. Although MADs are not guarantee dramatic outcome and less consistent than continuous positive airway pressure, they offer several advantages over continuous positive airway pressure and surgical methods, including non-invasiveness, silence, portability, and tolerability, simplicity. Therefore, general dental practitioner who had passed sleep dental curriculum or coursework can treat the patients with sleep problems. This article reviews the history, clinical indications, suggested mechanism of actions, various positive effects and several side effects, factors predicting a favorable outcome, determining amounts of mandibular advancement, compliance and long-term efficacy of MADs use.

  • There have been the current controversy over effectiveness of the pressurized smoke control systems, which are installed within escape shaft to provide 'smoke-free' access for escaping peoples. Therefore, many effective measures were given in this paper by means of the production of a design guide for various types of escape routes used in domestic buildings. The solutions were established on basis of both an investigating current door closing device application in existing facilities and global standardization for pressurized smoke control system, especially in British Standard.   • In this paper, DSPOCS(Digital Switchgear-Panel Operation and Control Solution) is designed, which is the intelligent inference based operation and control solution to obtain the safety and reliability of electric power supply in substation based on IED. DSPOCS is designed as a scheduled monitoring and control task and a real-time alarm inference task, and is interlinked with BRES(Bus Reconfiguration Expert System) in the required case. The intelligent alarm inference task consists of the alarm knowledge generation part and the real-time pattern matching part. The alarm knowledge generation part generates automatically alarm knowledge from DB saves it in alarm knowledge base. On the other hand, the pattern matching part inferences the real-time event by comparing the real-time event information furnished from IEDs of substation with the patterns of the saved alarm knowledge base.; Especially, alarm knowledge base includes the knowledge patterns related with fault alarm, the overload alarm and the diagnosis alarm. In order to design the database independently in substation structure, busbar is represented as a connectivity node which makes the more generalized graph theory possible. Finally, DSPOCS is implemented in MS Visual $C^{++}$, MFC, the effectiveness and accuracy of the design is verified by simulation study to the typical distribution substation.

  • The purpose of this research is to develop a safe and convenient wearable technology wear for bikers. For this, we studied the current usage of two-wheeled vehicles and have also researched the rate of accidents and its causes. We then used them along with previous studies in terms of visual perception as factors to decide the crucial elements of the riders' apparel. Case studies and the break down for the established prototypes for bikers were practiced as well. Based on this process, a survey was conducted to find out the needs of the bikers in the areas of both apparel and technology and then proceeded to produce the appropriate design and device modules. In the apparel sector, the result of the survey indicated that it was considerable that any digital devices were not shown to sustain a natural visible look. It also was essential that the materials were durable and made for safety and easy movement. In the digital function sector, it was significant that a motion input interface which will be embedded into the wear was needed to avoid any dangerous situations. This would ensure the safety of not only the rider but the surrounding riders as well. Lastly, protecting the rider's skin from any harmful elements was regarded necessary as well. Based on these requirements, a new prototype was created and will be tested if the requirements stated above are all met and will be evaluated according to the effectiveness of its functions.

  • In this paper, a developed device for detecting target's location and avoiding collision is proposed. Velocity and acceleration model of target are derived to estimate target's information, i.e. position, velocity and acceleration considering process and measurement noise. Kalman filtering method applied to the estimation process and its results was confirmed by simulation. The distance measurements system using laser sensor for moving target system is also developed to confirm the effectiveness of the proposed scheme. Experiments to get information of moving target with velocity and acceleration model was executed. The data with filtering and without filtering was compared by experiments. Discontinuous measured data was changed to smooth and continuous data by Kalman filtering. It is confirmed that desired data was obtained by applying proposed scheme. UI for measuring and monitoring the target data is developed and visual and auditory alarm function is attached on the system Finally, position estimation system of moving target with good performance is achieved by low price equipments.

  • Major causes of industrial accidents include falls and gas leak. The existing safety helmet and smart device combination products are focused on convenience, so the functions to prevent such accidents are insufficient. We developed a smart helmet focusing on fall accident detection and gas leak detection. We also developed management system to manage workers efficiently. Its core function is to detect dangerous conditions of employees, to communicate with managers and to confirm the situations of workers. The effectiveness of the combustible gas measurement capability was verified through experiments. However, since a significant amount of power consumption is founded due to continuous operation of the board and the sensor, countermeasures such as replacing with a large capacity battery are required.

  • Recently, huge life losses occurred in the elderly long-term medical care fires due to lack of escape safety. As part of the measures to enhance the effectiveness of fire escape safety, while they prolong the available safe egress time (ASET) of non fire compartments, a measure to shorten fire-fighter's arrival time by fire alarm notifying device should be implemented in these facilities. The four categories from the aspects of fire prevention/protection engineering were provided with the necessary component technologies for carrying out these helper-guided evacuations. Fire prevention engineered technology was presented by two provisions; one for ensuring small compartment sections by installing the fire rated wall between bed rooms and another for ensuring the fire retardant or/and non-flammable performance of finishing materials. Also fire protection engineered technology was presented by two items; one for imposing cooling effects by sprinklers and another for providing automatic fire alarm notifying functions to fire stations. In order to improve the escape safety of these facilities in Korea, alternative revisions may presented by considering insufficient provisions in the architectural/fire law provisions by analyzing the provisions of Japanese and domestic laws in detail.

Comparison between Gel Pad Cooling Device and Water Blanket during Target Temperature Management in Cardiac Arrest Patients

  • Background: Target temperature management (TTM) improves neurological outcomes for comatose survivors of out-of-hospital cardiac arrest. We compared the efficacy and safety of a gel pad cooling device (GP) and a water blanket (WB) during TTM. Methods: We performed a retrospective analysis in a single hospital, wherein we measured the time to target temperature ($<34^{\circ}C$) after initiation of cooling to evaluate the effectiveness of the cooling method. The temperature farthest from $33^{\circ}C$ was selected every hour during maintenance. Generalized estimation equation analysis was used to compare the absolute temperature differences from $33^{\circ}C$ during the maintenance period. If the selected temperature was not between $32^{\circ}C$ and $34^{\circ}C$, the hour was considered a deviation from the target. We compared the deviation rates during hypothermia maintenance to evaluate the safety of the different methods. Results: A GP was used for 23 patients among of 53 patients, and a WB was used for the remaining. There was no difference in baseline temperature at the start of cooling between the two patient groups (GP, $35.7^{\circ}C$ vs. WB, $35.6^{\circ}C$; P=0.741). The time to target temperature (134.2 minutes vs. 233.4 minutes, P=0.056) was shorter in the GP patient group. Deviation from maintenance temperature (2.0% vs. 23.7%, P<0.001) occurred significantly more frequently in the WB group. The mean absolute temperature difference from $33^{\circ}C$ during the maintenance period was $0.19^{\circ}C$ (95% confidence interval [CI], $0.17^{\circ}C$ to $0.21^{\circ}C$) in the GP group and $0.76^{\circ}C$ (95% CI, $0.71^{\circ}C$ to $0.80^{\circ}C$) in the WB group. GP significantly decreased this difference by $0.59^{\circ}C$ (95% CI, $0.44^{\circ}C$ to $0.75^{\circ}C$; P<0.001). Conclusions: The GP was superior to the WB for strict temperature control during TTM.

Study on the Analysis of Differential Pressure of the Access Door for a Smoke Control Zone and the Effectiveness of the Measurement Criteria of its Opening Force (제연구역 출입문의 차압 및 개방력 측정기준의 실효성 분석에 관한 연구)

  • The purpose of this study is to analyze the problem in measuring the differential pressure between the fire area and the neighboring smoke control zone as well as the opening force of a fire door and to present the actual values measured by an objective method. NFSC 501A specifies that the force necessary to open an access door when operating a smoke control system shall be less than 110 N. When the smoke control system does not operate in the space where it is installed, the door opening force can be measured by the test method in KS F 2805. However, when the smoke control system operates, additional opening force is required to overcome the force generated by the differential pressure between the fire area and smoke control room. Therefore, it can be seen that the method proposed by the standard has insufficient reliability. The analog measuring device and digital measuring device showed that the opening forces, $F_a$ and $F_d$, of the fully closed door before the smoke control system were 27.8 N and 27.4 N, respectively. When the door remained open by $5^{\circ}$, the opening forces, $F_a$ and $F_d$, were 33 N and 33.6 N, respectively. When the smoke control system operated and the door was fully closed, the door opening forces, $F_a$ and $F_d$, were 77.6 N and 76.0 N, respectively. Therefore, since the door opening forces are different from the criteria presented by KS F 2805, it is required to review the criteria appropriately.

Usability of CPR Training System based on Extended Reality (확장현실 기반의 심폐소생술 교육 시스템의 사용성 평가)

  • Recently, the importance of CPR training for the layperson has been emphasized to improve the survival rate of out-of-hospital cardiac arrest patients. An accurate and realistic training strategy is required for the CPR training effect for laypersons. In this study, we develop an extended reality (XR) based CPR training system and evaluate its usability. The XR based CPR training system consisted of three applications. First, a 3D heart anatomy image registered to the manikin is transmitted to the smart glasses to guide the chest compression point. The second application provides visual and auditory information about the CPR process through smart glasses. At the same time, the smartwatch sends a vibration notification to guide the compression rate. The 'Add-on-kit' is a device that detects the depth and speed of chest compression via sensors installed on the manikin and sends immediate feedback to the smartphone. One hundred laypersons who participated in this study agreed that the XR based CPR training system has realism and effectiveness. XR based registration technology will contribute to improving the efficiency of CPR training by enhancing realism, immersion, and self-directed learning.
