A Case Report on Functional Dyspepsia Treated with Korean Medicine in a Patient with Panic Disorder and Lower Back Pain (공황장애와 요통을 동반한 기능성 소화불량 환자에 대한 한방치료 1례)

  • Objectives: This study reports on the efficacy of Korean medical treatments for a functional dyspepsia patient with panic disorder and lower back pain. Methods: A 51-year-old male patient with functional dyspepsia was treated with herbal medicines and acupuncture for 17 days. The treatment effect was evaluated by measurements drawn from the Numerical Rating Scale, range of motion, Oswestry Disability Index, European Quality of Life 5 Dimensions Scale, and Gastrointestinal Symptom Score. Results: Following the treatment, the patient showed a decrease on the Numerical Rating Scale and Oswestry Disability Index and in Gastrointestinal Symptom Score, as well as an improvement in range of motion and European Quality of Life 5 Dimensions score. Conclusions: Korean medical treatments appear to be effective in reducing functional dyspepsia. Further clinical research on patients with functional dyspepsia is needed.

Effectiveness of Thread-embedding Therapy Combined with Korean Medicine Treatment on Acute phase of Bell's Palsy (벨 마비 급성기에 대한 매선요법 병행치료의 임상적 효과)

  • Objectives : This study was to investigate the effectiveness of thread-embedding therapy for the treatment of acute phase of Bell's palsy. Methods : Thread-embedding therapy was performed once a week on 29 patients within 1 weeks after the onset. We investigated the general characters, Yanagihara's score, House-Brackmann scale and FDI were used to evaluate the effectiveness of thread-embedding therapy. Results : Facial muscle recovery began at 13.37th day, that was about 0.36 day earlier than the previous patients who were treated without thread-embedding therapy in 2017 at the same hospital, but there was no statistical significance. In the change of each scale before and after treatment, Yanagihara's score, House-Brackmann scale, physical function and social function of FDI were changed significantly. Compared with the patients treated without thread-embedding therapy in 2017, Yanagihara's score was a little higher and House-Brackmann scale was a little lower. Conclusions : These findings suggested that thread-embedding therapy on acute phase of Bell's palsy was a little more effective than the conventional therapy without thread-embedding therapy.

Two Case Reports of Patients Diagnosed with Avascular Necrosis of the Femoral Head Treated with Korean Medicine Treatment Including Chuna Manual Therapy, Pharmacopuncture and Acupuncture (추나 요법 및 약침, 침치료를 비롯한 한방치료로 증상이 호전된 대퇴골두 무혈성 괴사 환자 2예 보고)

  • Objectives The purpose of this study is to report the effects of Korean medicine treatments including Chuna manual therapy, pharmacopuncture and acupuncture for two patients who have pain in hip joint diagnosed with avascular necrosis of the femoral head. Methods The treatment effect was evaluated with a numeric rating scale (NRS), range of motion (ROM), physical examination (Patrick test), and score of Harris hip score (HHS). Results After treatment, symptoms of the two patients such as hip joint pain and gait dysfunction were improved. The NRS decreased from 8 (right [Rt.]), 7 (left [Lt.]) to 4 (Rt.), 3 (Lt.) in case 1 and from 9 to 2 in case 2. Also the patients showed improvement in ROM and score of HHS increased from 55.5 to 88.5 in case 1 and from 63.5 to 85.5 in case 2. In addition, positive Patrick test improved to negative. Conclusions Korean medicine treatments including Chuna manual therapy, pharmacopuncture and acupuncture can be effective in relieving pain and recovering the fucnction of hip joint associated with avascular necrosis of the femoral head, even when surgery is recommended. Although further study such as controlled studies is needed.

A Case Report on Peripheral Facial Nerve Palsy and Sleep Disorders After Microvascular Decompression (미세혈관 감압술 후 발생한 말초성 안면신경 마비와 수면장애에 대한 한방치험 1례)

  • Objectives: This study reports on the efficacy of Korean medicine treatments for peripheral facial nerve palsy and sleep disorders that occur after microvascular decompression. Methods: A 57-year-old female patient with right facial palsy was treated with herbal medicines and acupuncture for 36 days. The treatment effect was evaluated using the House Brackmann Grading System (HBGS), Yanagihara's Unweighed Grading System (Yanagihara's score), and the Korean Modified Leeds Sleep Evaluation Questionnaire (KMLSEQ). Results: Following treatment, the patient showed a decrease in HBGS and an improvement in Yanagihara's score and KMLSEQ score. Conclusions: Korean medicine treatments appeared to be effective in reducing facial nerve palsy. Further clinical research on patients with facial nerve palsy is needed.

The Effect of Graston Technique and Chuna manual therapy combined with Korean Medical Treatment for fibromyalgia: A Case Report

  • Objectives : The purpose of this study was to report the clinical effects of the Graston Technique and Chuna manual therapy, combined with Korean Medical Treatment for fibromyalgia. Methods : We treated a patient diagnosed with fibromyalgia. We used acupuncture, the Graston Technique, Chuna manual therapy, pharmacopuncture, herbal medicine, moxibustion and physical therapy. Outcomes were evaluated using the American College of Rheumatology Preliminary Diagnostic Criteria (ACR), the Fibromyalgia Impact Questionnaire (FIQ), and the Numeric Rating Scale (NRS). Results : The widespread pain index (WPI) scale score of the ACR decreased from 12 to 9, and the symptom severity scale (SS) score of the ACR decreased from 8 to 6. The FIQ score decreased from 63.69 to 50.15. On the NRS, lower back pain & lower limb pain decreased from 6 to 2; neck pain from 6 to 3; muscle tenderness & morning stiffness from 6 to 4; fatigue from 6 to 3; urticaria from 6 to 2. Conclusion : This case study suggests that the Graston Technique and Chuna manual therapy combined with Korean Medical Treatment may be effective treatments for fibromyalgia. However, further studies are needed.

Safe and time-saving treatment method for acute cerebellar infarction: Navigation-guided burr-hole aspiration - 6-years single center experience

  • Objective: While patients with medically intractable acute cerebellar infarction typically undergo suboccipital craniectomy and removal of the infarcted tissue, this procedure is associated with long operating times and postoperative complications. This study aimed to investigate the effectiveness of minimally invasive navigation-guided burr hole aspiration surgery for the treatment of acute cerebellar infarction. Methods: Between January 2015 and December 2021, 14 patients with acute cerebellar infarction, who underwent navigation-guided burr hole aspiration surgery, were enrolled in this study. Results: The preoperative mean Glasgow Coma Scale (GCS) score was 12.7, and the postoperative mean GCS score was 14.3. The mean infarction volume was 34.3 cc at admission and 23.5 cc immediately following surgery. Seven days after surgery, the mean infarction volume was 15.6 cc. There were no surgery-related complications during the 6-month follow-up period and no evidence of clinical deterioration. The mean operation time from skin incision to catheter insertion was 28 min, with approximately an additional 13 min for extra-ventricular drainage. The mean Glasgow Outcome Scale score after 6 months was 4.8. Conclusions: Navigation-guided burr hole aspiration surgery is less time-consuming and invasive than conventional craniectomy, and is a safe and effective treatment option for acute cerebellar infarction in selected cases, with no surgery-related complication.

One Year Follow up for Severe Adult Atopic Dermatitis of 15 Patients After Sasang Constitutional Therapy (중증 성인형 아토피 피부염을 사상처방으로 치료하여 호전된 환자 15례에 대한 1년간의 추적 관찰)

  • Background : Atopic Dermatitis is a chronic and recurrent skin disease. it is characterized as itch-scratch cycle and topical or systemic use of corticosteroids is required in western medicine. Though its effects on major symptoms are impressive, these treatments are prone to several side effects and tend to recur after months or years. So potential treatments have been actively studied in Estern medicine, especially Sasang constitutional therapy, which has fewer side effects. Objectives : The purpose of this study was to examine the effect of Sasang constitutional therapy on treating severe adult Atopic Dermatitis and preventing its recurrent symptoms. Methods : fifteen patients who had severe adult Atopic Dermatitis attended treatment of Sasang constitutional therapy and were followed up for one year. An oriental internal specialist first isolated Sasang constitution and a distinctive body type. Grading of Atopic dermatitis was measured by the guidelines offered by Rajka G. and Langeland T. Results : After treatment with Sasang constitutional therapy, the patients symptoms were improved and the mean Rajka and Langeland score reduced significantly from $7.73{\pm}0.79\;to\;3.4{\pm}10.91$. During the follow-up period of one year, the mean Rajka and Langeland score was $2.72{\pm}2.06$ which was lower than last treatment. Conclusions : The results of this study support Sasang constitutional therapy as an effective therapy for severe adult Atopic Dermatitis. Follow up data suggest that this Eastern medical approach is of greater clinical value than the western medical approach in the long term.

A Case of Chronic Glossitis Improved by Korean Medical Treatments (한방 치료로 호전된 만성 설염 치험 1례)

  • Objectives : The purpose of this study is to report the effectiveness of Korean medical treatments on chronic glossitis using Yanggyeok-sangagam based on tutakbeop(透托法), and Ikgibohyeol-tanggagam based on botakbeop(補托法) and bobeop(補法). Methods : We treated a 83-year-old female patient who had glossitis by Korean medicine and acupuncture treatment. These treatments were performed from 2020.8.4. to 2021.3.4. We have confirmed the improvement of white plaque like a pseudomembranous pattern lesion and pain. We recorded images that include change of symptoms. Results : After the treatments were done, the scores of numeric rating scale(NRS), oral assessment guide(OAG) and oral health impact profile-14(OHIP-14) were decreased. The NRS score was decreased from 7 to 1. The OAG score was decreased from 20 to 9. The OHIP-14 score was decreased from 34 to 5. Conclusions : According to the result, Korean medical treatments such as Korean medicine and acupuncture treatment, followed by the cure rules- tutakbeop(透托法), botakbeop(補托法) and bobeop(補法) can be effective for the treatment of chronic glossitis.

A Study of Prognostic Factors of Conservative Treatment in Plantar Fasciitis (족저 근막염 환자의 보존적 치료시 예후 인자 연구)

  • Purpose: We analyzed to find out prognostic factors in the conservative treatment for the plantar fasciitis. Materials and Methods: The data were collected from 145 patients, 159 feet (M:F = 51:108) with conservative treatment and analyzed for possible prognostic factors : sex, age of onset, the duration of symptom before treatment, pain score before treatment, the duration for symptom remission, medication period, calcaneal pitch angle, and presence of calcaneal spur. Results: The duration of symptom before conservative treatment is affected to the prognosis, and the borderline of the effective period was about 6 months. Conclusion: With the conservative treatment of the plantar fasciitis, we found that (1) the duration of symptom before the conservative treatment was a prognostic factor, and (2) if the period before the conservative treatment was more than 6 months, the other treatment option such as surgery should be considered for this chronic group.

A Case Report of Functional Dyspepsia Treated with Beewha-eum (비화음으로 호전된 기능성 소화불량 환자 치험 1례)

  • Objective: The purpose of this case study is to report the effectiveness of the Korean medicine Beewha-eum as a treatment for functional dyspepsia (FD). Methods: A patient diagnosed with a spleen-stomach deficiency-cold pattern in Korean medicine was treated with herbal medicines, including Beewha-eum. Symptom severity was assessed using the degree of self-reported dyspepsia (%), self-reported energy level, amount of food intake (%), total parenteral nutrition rate (cc/hour), body weight (kg), Nepean dyspepsia index (NDI-K), functional dyspepsia-related quality of life questionnaire (FD-QoL), and short form 36-item health survey (SF-36) score. Results: After treatment, the symptom severity of self-reported dyspepsia decreased from 100% to 10 to 15%, and the self-reported energy level increased from 20 to 80. The amount of food intake increased from 5% to 100%, and the total parenteral nutrition rate started from 60 cc/hour end in week 5. Body weight increased from 53.1 kg to 62 kg. The NDI-K score decreased from 105 to 31. The FD-QoL score increased from 19 to 68. The SF-36 score also increased from 102 to 116. Conclusion: The results suggest that the Korean medicine Beewha-eum could be an effective option for treating FD.
