Antidiabetic effect and mechanisms of SPH-2 in db/db mice

  • SPH-2 is a herbal medicine composing oriental prescription. We have studied the antidiabetic effect and mechanism of SPH-2 in insulin-resistant diabetic db/db mice. Mice were grouped and treated for 3 weeks as follows: control group was administrated with tap water orally: treated group was administrated with SPH-2 orally at dose of 500 mg/kg. SPH-2 lowered plasma glucose level by 43% as compared to the diabetic control. Total cholesterol. triglyceride and free fatty acid were all reduced in SPH-2 treated group. (omitted)

목단피((牧丹皮).도인(桃仁) 배합(配合)이 항염증(抗炎症) 작용(作用)에 미치는 영향 (Effect of Water Extract of Peonia Suffruticosa and Prunus Percica on Anti-inflammation)

  • Paeonia Suffruticosa and Prunus Persica have been used as oriental medicine for removal of fever, alleviation of pain, an anti-phlogistic effect and removal of extravasated blood. However, it has been never shown the effects of these herbal medicines on anti-inflammatory processes. This experiment was performed to show how these herbs could act as anti-inflammatory medicines at cellular level. Anti-inflammation effects of water extracts from Paeonia Suffruticosa and Prunus Persica as well as their mixture have been investigated, and the results were follows; 1) each extract slightly suppressed the expression and production of inflammatory mediators and enzymes such as NO, iNOS, IL-$1{\beta}$, and TNF-$\alpha$ in lipopolysaccharid(LPS)-stimulated RAW264.7 cells and mouse primary peritoneal macrophages in a dose-dependent manner. These suppressive effects, however, were synergistically increased by their mixture. 2) Each extract of Paeonia Suffruticosa and Prunus Persica insignificantly suppressed the activation and activity of NF-${\kappa}B$ in LPS-stimulated RAW264.7 cells, which controls the expression of inflammatory mediators such as NO, iNOS, IL-$1{\beta}$, and TNF-$\alpha$. However, extract mixture of Paeonia Suffruticosa and Prunus Persica suppressed effectively the activation and activity of NF-${\kappa}B$. 3) Each of Paeonia Suffruticosa and Prunus Persica induced translocation of NF-${\kappa}B$ to the nucleus from the cytosol and DNA-binding activity of nuclear NF-${\kappa}B$ in LPS-activated RAW264.7 cells. The extract mixture of Paeonia Suffruticosa and Prunus Persica showed more significant suppression of the NF-${\kappa}B$ translocation and its DNA-binding activity, as compared to those of the each extract. These results suggest that the extract mixture of Paeonia Suffruticosa and Prunus Persica may affect different control mechanisms for NF-${\kappa}B$ activation and the expression and production of NF-${\kappa}B$-dependent inflammatory mediators, indicating that this extract mixture may be useful for treatment of inflammatory diseases.

Dosimetric comparison of coplanar and non-coplanar volumetric-modulated arc therapy in head and neck cancer treated with radiotherapy

  • Purpose: To evaluate the dosimetric variations in patients of head and neck cancer treated with definitive or adjuvant radiotherapy using optimized non-coplanar (ncVMAT) beams with coplanar (cVMAT) beams using volumetric arc therapy. Materials and Methods: Twenty-two patients of head and neck cancer that had received radiotherapy using VMAT in our department were retrospectively analyzed. Each of the patients was planned using coplanar and non-coplanar orientations using an optimized couch angle and fluences. We analyzed the Conformity Index (CIRTOG), Dose Homogeneity Index (DHI), Heterogeneity Index (HIRTOG), low dose volume, target and organs-at-risk coverage in both the plans without changing planning optimization parameters. Results: The prescription dose ranged from 60 Gy to 70 Gy. Using ncVMAT, CIRTOG, DHI and HIRTOG, and tumor coverage (ID95%) had improved, low dose spillage volume in the body V5Gy was increased and V10Gy was reduced. Integral dose and intensity-modulated radiation therapy factor had increased in ncVMAT. In the case of non-coplanar beam arrangements, maximum dose (Dmax) of right and left humeral head were reduced significantly whereas apex of the right and left lung mean dose were increased. Conclusion: The use of ncVMAT produced better target coverage and sparing of the shoulder and soft tissue of the neck as well as the critical organ compared with the cVMAT in patients of head and neck malignancy.

강녕탕(康寧湯)의 LPS 질내 주입 생쥐에 미치는 항염증(抗炎症) 효과 (Anti-inflammatory Effects of Gangyeong-Tang (康寧湯) on Mice injected LPS in Vagina)

  • Objective: We made the genital organs inflammatory mice model by vaginal injection of Lipopolysaccharide(LPS), and we intended to make study about anti-inflammatory effects of Gangyeong-Tang, among oral and rectal medication and Herbal-Acupuncture treatment. Method: The female ICR(20-30g) mice were used, the temperature was controlled within $22{\pm}0.5^{\circ}C$ and water and food was not limited. The environment was manipulated to simulate 12 hours of day and 12 hours of night. After LPS injection into vagina, we confirmed on-set of inflammation. 2 days before LPS injection, we started to medicate Gangyeong-Tang in Oral and Rectal and Aqua-acupucture treatment. After 3days from LPS injection, we mesured the White Blood Cell(WBC), Interrleukin-6(IL-6), Tumor Necrosis Factor-${\alpha}$(TNF-${\alpha}$) in blood which was collected from the Retro-orbital Plexus. Results: 1. We made the genital organs inflammatory mice model by vaginal injection of LPS successfully. 2. The number of WBC was decreased significantly as we medicated Gangyeong- Tang in Oral 1g/kg dose, 3g/kg dose and Rectal 1g/kg dose, rectal 3g/kg dose. 3. The concentration of IL-6 was decreased significantly as we medicated Gangyeong-Tang in all group of the Oral, Rectal, Herbal-Acupuncture treatment. 4. The concentration of TNF-${\alpha}$ was decreased significantly as we medicated Gangyeong-Tang in Oral 3g/kg dose and Rectal 1g/kg dose, Rectal 3g/kg dose group. Herbal-Acupuncture treatment group datas showed reductive tendency. Conclusion: As a result of this experiment, we made the genital organs inflammatory mice model by vaginal injection of LPS successfully and demonstrated anti-inflammatory effect of Gangyeong-Tang.

근린약국에서 산제로 조제된 아테놀올정의 안정성 (Stability of Atenolol Tablet After Dispensing to Powder form at Community Pharmacies)

  • Prescription filling in powder form is performed in community pharmacy practice to adjust dose for children and patients who cannot swallow whole tablet. However, there are few reports regarding the stability of the active ingredient and possible microbial growth after the medication is dispensed to powder form. This study examined the stability of atenolol, an antihypertensive agent, and microbial growth in the unit dose pouches dispensed at twenty-one community pharmacies located in Taegu area. Randomly chosen first unit dose pouch contained 77.4% of the prescribed dose of the drug and there were only four community pharmacies that dispensed the drug within 10% deviation from the dose prescribed by physician. Surprisingly, there were three community pharmacies that dispensed the drug with greater than 40% deviation, which may pose a major concern regarding the efficacy and safety of the drug prescribed for the treatment of hypertension. Atenolol content during a month did not indicate significant change, showing 5.4%, 4.3%, and 3.3% of decrease in 50%, 80%, and 90% relative humidity conditions, respectively. Microbiological examination during a month showed less than 0.5 microorganism in high power field (hpf) in all the relative humidity conditions tested. Based on this study, pharmacy practice in community pharmacy needs to be rigorously regulated to ensure that the dose of the prescribed drug is properly incorporated into the unit dose pouch dispensed as powder form.

Dosimetric Effects of Air Pocket during Magnetic Resonance-Guided Adaptive Radiation Therapy for Pancreatic Cancer

  • Purpose: Online magnetic resonance-guided adaptive radiotherapy (MRgART), an emerging technique, is used to address the change in anatomical structures, such as treatment target region, during the treatment period. However, the electron density map used for dose calculation differs from that for daily treatment, owing to the variation in organ location and, notably, air pockets. In this study, we evaluate the dosimetric effect of electron density override on air pockets during online ART for pancreatic cancer cases. Methods: Five pancreatic cancer patients, who were treated with MRgART at the Seoul National University Hospital, were enrolled in the study. Intensity modulated radiation therapy plans were generated for each patient with 60Co beams on a ViewrayTM system, with a 45 Gy prescription dose for stereotactic body radiation therapy. During the treatment, the electron density map was modified based on the daily MR image. We recalculated the dose distribution on the plan, and the dosimetric parameters were obtained from the dose volume histograms of the planning target volume (PTV) and organs at risk. Results: The average dose difference in the PTV was 0.86Gy, and the observed difference at the maximum dose was up to 2.07 Gy. The variation in air pockets during treatment resulted in an under- or overdose in the PTV. Conclusions: We recommend the re-contouring of the air pockets to deliver an accurate radiation dose to the target in MRgART, even though it is a time-consuming method.

전신방사선조사(Total Body Irradiation) 시 Beam Spoiler와 환자 간의 거리에 관한 고찰 (Analysis of Surface Dose Refer to Distance between Beam Spoiler and Patient in Total Body Irradiation)

  • 목 적: 골수 또는 주변 줄기세포 이식을 목적으로 시행하는 전신방사선 조사시, 표면선량의 증가를 위하여 사용되어지는 산란판(beam spoiler)과 환자간의 거리에 따른 표면선량 변화를 측정하여 적정거리를 찾고자 한다. 대상 및 방법: 본 연구를 위해 6 MV X선을 사용하였으며, 조직등가고체팬텀($30{\times}30{\times}30cm^3$)과 평형평판형 전리함 및 전위계를 사용하였다. 선원에서 기준이 되는 팬텀 중심까지의 거리를 400 cm로 위치시키고 조사야 $40{\times}40cm^2$에서 얻은 측정치를 기준값으로 하였다. 또한 평형평판형 전리함을 고체팬텀의 선원 쪽 표면과 선원 반대쪽 표면에 각각 위치시켜 입사지점 선량과 출구지점 선량을 측정하였다. 산란판과 환자와의 적정 거리를 연구하기 위하여 조직 등가 고체 팬텀과 산란판 간의 거리를 $1{\sim}200cm$ 까지 변화시켜 가며 표면 선량을 측정하였다. 이때 조직등가 고체 팬텀은 고정시키고 산란판을 선원방향으로 일정한 간격으로 변화시켜 입사지점 선량과 출구지점 선량을 측정하였으며 이 값을 기측정된 팬텀 중심부 선량으로 나누어 백분율로 분석하였다. 각각의 경우에 대해 MU는 300씩 조사하였다. 결 과: 기준이 되는 체내 중심 선량을 처방 선량으로 간주하였고 산란판의 유무에 관계없이 체내 중심 선량과 출구 선량은 각각 10.7 cGy, 6.7 cGy로 측정되었다. 조직 등가 고체 팬텀과 산란판 사이의 간격이 $50{\sim}60cm$일 때 입사지점 선량 값은 $14.58{\sim}14.92cGy$로 측정되어 기준점 대비 $99.4{\sim}101%$ 범위를 보임에 따라 표면 선량값이 처방선량에 가장 근접한 값으로 측정되었다. 결 론: 본 실험 결과 전신방사선조사시, 조직등가 고체팬텀과 산란판 사이의 간격이 $50{\sim}60cm$일 때 표면선량을 처방선량에 가장 근접하게 조사할 수 있다는 것을 확인 할 수 있었다. 산란판과 환자와의 거리를 $50{\sim}60cm$에 위치시켜 표면선량의 over dose나 under dose의 발생 방지를 위한 주의가 반드시 필요하겠다.

병원 및 의원급 일일사용량 기준 항생제 사용량에 영향을 미치는 요인 (Analysis of factors affecting antibiotic use at hospitals and clinics based on the defined daily dose)

  • Inappropriate antibiotic use significantly contributes to antibiotic resistance, resulting in reduced antibiotic efficacy and an increased burden of disease. The objective of this study was to investigate the characteristics of prescribers whose antibiotics use was high and to explore factors affecting the use of antibiotics by medical institutions. This study analyzed the National Health Insurance claims data from 2015. Antibiotic prescription data were analyzed in terms of the number of defined daily doses per 1,000 patients per day, according to the World Health Organization anatomical-therapeutic-chemical classification and methodologies for measuring the defined daily dose. We investigated the characteristics of prescribers and medical institutions with high antibiotic use. Multivariate regression analyses were performed on the basis of characteristics of the medical institution (number of patients, type of medical institution [hospital or clinic], age of the physician, etc.). The number of patients and number of beds were found to be significant factors affecting antibiotic use in hospitals, and the number of patients, region, and medical department were significant factors affecting antibiotic use at the level of medical institutions. These findings are expected to help policy-makers to better target future interventions to promote prudent antibiotic prescription.

외래 호흡기계 질환에서 항생제 사용에 대한 후향적 평가방안 (Retrospective Drug Utilization Review of Antibiotics for Respiratory Tract Infection(RTI) in Ambulatory Outpatient Care)

  • As respiratory tract infections (RTI) account for about 60% of all antibiotic prescriptions in outpatient care setting, there are significant concerns about emerging resistance that are largely due to the excessive or inappropriate use of antibacterial agents for viral respiratory infections. This study was aimed to develop retrospective drug utilization review (DUR) program of antibiotics for RTIs using Delphi methods. Retrospective DUR criteria of antibiotics for RTIs were identified based on clinical practice guidelines and opinion of experts. Expert panel members were clinical doctors and pharmacists and Delphi method was applied by survey on 16 members of panels. The claim data from Korean Health Insurance Review & Assessment (HIRA) were used to examine trends in outpatient antibiotic prescription between Janunary to December of 2008. As results, Quality index for RTI was assessed for the claim type, antibiotics use of quantity, duration, number and cost. Antibiotic prescription rate for RTIs, Defined Daily Dose (DDD), and duration of antibiotics use were more recognized as significant quality index by experts' opinion. Use of first line agents suggested by guidelines was low and duration of antibiotics use was shorter compared to the recommendations. Antibiotics were over prescribed for RITs. However, dose and duration of antibiotics were under-used.

Kami-bang-pung-tong-sung-san is Involved in Protecting Neuronal Cells from Cytotoxic Insults

  • KBPTS is the fortified prescription of Bang-pung-tong-sung-san (BPTS) by adding Spatholobi Clulis and Salviae Miltiorrzae Radix. BPTS prescription has been used in Qriental medicine for the treatments of vascular diseases including hypertension, stroke, and arteriosclerosis, and nervous system diseases. Yet, the overall mechanism underlying its activity at the cellular levels remains unknown. To investigate the protective role of KBPTS on brain functions, noxious stimulations were applied to neurons in vitro and in vivo. KBPTS pretreatment in cultured cortical neurons of albino ICR mice rescued death caused by AMPA, NMDA, and kainate as well as by buthionine sulfoximine (BSO) and ferrous chloride (Fe/sup 2+/) treatments. Furthermore, KBPTS promoted animal's recovery from coma induced by a sublethal dose of KCN and improved survival by a lethal dose of KCN. To examine its physiological effects on the nervous system, we induced ischemia in the Sprague-Dawley rat's brain by middle cerebral artery (MCA) occlusion. Neurological examination showed that KBPTS reduced the time which is required for the animal after MCA occlusion to respond in terms of forelimb and hindlimb movement$. Histological examination revealed that KBPTS reduced ischemic area and edema rate and also protected neurons in the cerebral cortex and hippocampus from ischemic damage. Thus, the present data suggest that KBPTS may play an important role in protecting neuronal cells from external noxious stimulations.
