조선(朝鮮) 의서(醫書)의 중풍(中風) 침구법(鍼灸法) 비교 (Comparative Study of Stroke Treated by Acupuncture in the Medical Documents Of the Chosun-Korea Dynasty)

  • Objectives : The objective of this article is to compare acupuncture treatment of stroke in the medical documents of the Chosun-Korea dynasty. Through such inquisition, the development process of acupuncture of stroke treatment could be found. Methods : We used Cross-comparison analysis of the contents of five medical books, HyangYakZipSungBang (HYZSB), UiRimChwarYo(URCY), DongEuiBoGam(DEBG), ChimGuGyungHumBang(CGGHB) and Saam-Acupuncture(SA) of the Chosun-Korea dynasty. Results & Conclusion : 1. Distinction between Stroke of Viscera and Stroke of Bowel was important. Firts two books divided stroke into three type. But last three books separated into Stroke of Viscera and Stroke of Bowel. 2. They emphasized Symptoms of Five Visceral Disease. They devised Symptoms of Five Visceral Disease for Stroke of Viscera treatment. This method appears very rarely in clinical medicine. 3. They used common important acupoint for treatment of stroke. Most of these acupoints exist at the limbs. 4. They chose acupoints on the opposite side against the symptom.

열공성 뇌경색으로 한방병원에 입원한 환자의 자립생활에 영향을 미치는 인자에 대한 연구 (Factors Affecting Independent Living of Lacunar Infarction Patients)

  • This study sought a clinical analysis of 43 lacunar infarction patients treated by oriental medical therapies. The patients were divided into two groups (independent living group and dependent living group) according to K-MBI independence level. We had analyzed correlation of many factors like past history, several symptoms of lacunar infarction and ect between independent living group and dependent living group. The male to the female ratio of patients diagnosed lacunar infarction was 1:1.39, female(58.1%) was more dominant than male. The most common age group was 8th decade. The most common past history was Heart disease and without heart disease patients are treated well. Without hemiparesis, cognitive disorder, dysphagia patients are treated well. Patients arrived hopital within 72 hours after onset had treated well In regard to K-MBI of lacunar infarction patients, 65.1% improved and no one got worse. After treatment, 74.4% patients were able to independent living. The correlation between herbal medication and improvement was not founded.

Intracranial abscess from facial cellulitis

  • Cellulitis, one of most common diseases of everyday life, is often overlooked for its significance. Although cellulitis does not cause or lead to serious problems usually, its possibility to cause lifethreatening problem should be known. In present case, a patient who had received acupuncture treatment a week earlier presented to the clinic with symptoms of facial cellulitis. The disease resolved within few weeks under empirical antibiotic treatment but recurred after 3 months. Under close history review of the patient, we found out that the patient had received craniectomy 20 years ago. The patient had blunt headache with no other neurological symptoms that could suspect cranial infection, but considering the risk originating from the patient's surgical history, brain computed tomography (CT) was taken. CT images revealed abscess formation in the subgaleal and epidural spaces. Craniotomy with abscess evacuation was done promptly. With additional antibiotic treatment postoperatively, the disease resolved, and the 1-month postoperative followup brain CT showed no signs of abscess formation.

조사(燥邪)와 조병(燥病)의 인식에 대한 고찰 - 역대 주요 논쟁을 중심으로 - (A Study on the Recognition of Dryness pathogen and Dryness disease - Focusing on the main argument in history -)

  • Objectives : Dryness pathogen, which is one of six pathogenic factors, causes dryness diseases. Currently, the theory on dryness disease is composed of external dryness and internal dryness. External dryness, in turn, is composed of cool dryness and warm dryness. However, these categorizations and their symptoms bear ambiguity for many reasons. Therefore, this paper aims to review various texts in order to study the special features of dryness pathogen and dryness disease. Methods : Texts that deal with dryness pathogen and dryness disease were studied. Most texts are comprised of dissertations and historical medical texts, therefore, CNKI and The Sikuquanshu's databases, and Traditional Chinese Medical(TCM) book webdatabases were utilized. Materials are listed in chronological order, and their main points regarding dryness pathogen and dryness disease are compared. Results & Conclusions : It is difficult to accept the assertion that dryness pathogen does not lead to external dryness. Dryness does not have the elements of chill and fever in itself. Dryness's elements of chill and fever are determined in the ways they combine with each individual element. Moreover, the symptoms of chill and fever on dryness disease are subject to the host's body type. External dryness and internal dryness cannot be discussed within an identical premise. Whereas the dryness in external dryness signifies the cause of a disease, the dryness in internal dryness is the consequence of a disease. In other words, internal dryness revolves around cause of disease and external dryness revolves around the mechanic of disease. It's difficult to determine whether these diseases are caused by dryness or wetness in Autumn. There is an understanding which integrates these together through the Yunqi theory, but it is imperfect.

Modeling of Human Genetic Diseases Via Cellular, Reprogramming

  • The generation of induced pluripotent stem cells (iPSCs) derived from patients' somatic cells provides a new paradigm for studying human genetic diseases. Human iPSCs which have similar properties of human embryonic stem cells (hESCs) provide a powerful platform to recapitulate the disease-specific cell types by using various differentiation techniques. This promising technology has being realized the possibility to explore pathophysiology of many human genetic diseases at the molecular and cellular levels. Furthermore, disease-specific human iPSCs can also be used for patient-based drug screening and new drug discovery at the stage of the pre-clinical test in vitro. In this review, we summarized the concept and history of cellular reprogramming or iPSC generation and highlight recent progresses for disease modeling using patient-specific iPSCs.

해외유입 기생충 감염질환 (Imported Parasitic Diseases)

  • Recently, peoples of travelling to endemic area of parasitit diseases are rapidly increased and the imported parasitic diseases by tourists have become a public health problem. Here author describess briefly about the imported parasitic in Korea. The 15 kinds of parasitic diseases, I.e., malaria, babesiosis, cutaneous leishmaniasis, visceral leishmaniasis, ancylostomiasis, cutaneous larva migrans, angiostrongylosis, gnathostomiasis,loiasis, heterophyiasis, urinary schistosomiasis, hydatis disease, pentastomiasis, cutaneous myiasis and syngamosis were imported during last thirty years. The most prevalent imported parasitic disease was malaria. Indigenous vivax malaria has been eradicated since 1970s. However imported malaria(1970~1985) was reported 107 cases of patient with a history of travel abroad. Futhermore a case of reemerging vivax malaria was patient were occurred in 2000.These parasitic disease are sometimes overlooked or misdiagnosed. There is a need to concern about travel medicine and imported parasitic diseases.

만성폐쇄성폐질환환자 1례에 대한 임상적 고찰 (The clinical study of chronic obstructive pulmonary disease)

  • We report the treatment case of patient for chronic obstructive pulmonary disease. The patient was 65 year-old woman with the history of smoking and asthma. She had complained of dyspnea, hyperpnea, cough and wheezing sound. We treated her with general management, bronchodilator and five kinds of herbal medicine(Samjahwadamjeon etc). Among them, the major herbal medicine is Samjahwadamjeon. Having been treated for 35 days, the patient improved in clinical symptoms and pulmonary function in PFT(Pulmonary function test).

  • Bilateral striopallidodentate calcinosis, popularly referred to as Fahr's disease, is a disorder radiologically characterized by bilateral calcifications of the basal ganglia, thalami, dentate nuclei of the cerebellum, and the white matter of the cerebral hemisphere without serum calcium-phosphorus metabolism and related endocrinologic abnormalities. Intracranial calcifications are easily visible as high-density on CT. On magnetic resonance images, the calcifications exhibit different signal intensities. The differences in signal intensity are thought to be related to the stage of the disease, differences in calcium metabolism, and the volume of the calcium deposit. Based on literature review, I report the case of a 63 year man with bilateral symmetrical calcification in the basal ganglia, dentate nuclei of the cerebellum, and the white matter of the cerebral hemisphere who present a 5 year history of progressive dysarthria associated with left thalamic infarction.

  • Coronary artery disease has historically been a contraindication to lung transplantation. We report a successful combined bilateral lung transplantation and off-pump coronary artery bypass in a 62-year-old man. The patient had a progressive decline in lung function due to idiopathic pulmonary fibrosis and a history of severe occlusive coronary artery disease.

베체트병에서 발생한 재발성 손발톱박리증 (Recurrent onycholysis in a patient with Behcet's disease)

  • Onycholysis is defined as a distal or distal lateral separation of the nail plate from the underlying or lateral supporting structures including nail bed, hyponychium, and lateral nail fold. Trauma, infection, psoriasis, thyrotoxicosis, and numerous drugs are common causes of onycholysis. However, there are few specific data on nail findings in Behcet's disease (BD). In this paper, we report on a 60-year-old man with BD, with no past history except BD, who developed recurrent onycholysis. The symptoms of onycholysis are considered to be recurrent depending on the activity of BD. The nail lesion showed improvement after classic treatment of BD and topical steroid ointment.
