Background: Low back pain secondary to discopathy is a common pain disorder. Multiple minimally invasive therapeutic modalities have been proposed; however, to date no study has compared percutaneous laser disc decompression (PLDD) with intradiscal injection of radiopaque gelified ethanol (DiscoGel®). We are introducing the first study on patient-reported outcomes of DiscoGel® vs. PLDD for radiculopathy. Methods: Seventy-two patients were randomly selected from either a previous strategy of PLDD or DiscoGel®, which had been performed in our center during 2016-2017. Participants were asked about their numeric rating scale (NRS) scores, Oswestry disability index (ODI) scores, and progression to secondary treatment. Results: The mean NRS scores in the total cohort before intervention was 8.0, and was reduced to 4.3 in the DiscoGel® group and 4.2 in the PLDD group after 12 months, which was statistically significant. The mean ODI score before intervention was 81.25% which was reduced to 41.14% in the DiscoGel® group and 52.86% in the PLDD group after 12 months, which was statistically significant. Between-group comparison of NRS scores after two follow-ups were not statistically different (P = 0.62) but the ODI score in DiscoGel® was statistically lower (P = 0.001). Six cases (16.67%) from each group reported undergoing surgery after the follow-up period which was not statistically different. Conclusions: Both techniques were equivalent in pain reduction but DiscoGel® had a greater effect on decreasing disability after 12 months, although the rate of progression to secondary treatments and/or surgery was almost equal in the two groups.
The object of this article is to examine the effect of the quota-levy system on employment outcome of people with disabilities in Korea. Merging the data from disability employment report of 16,246 businesses in 2011 and the macroeconomic indicators such as regional economic condition, the author analyzes the effect of quota-levy system on employment outcome of persons with disabilities by using Hierarchical Linear Model(HLM). The finding is that imposing the levy on businesses affects employment outcome of people with disabilities but regional economic condition does not. The rate of employees with disabilities of the levied business is 0.7%p higher than that of the other business. The result of analysis implies that employment outcome of people with disabilities is influenced by the quota-levy system rather than regional economic condition.
Purpose: The purpose of this study was to identify factors that affect the intention of HPV vaccination in 12-year-old female adolescents and their mothers. Method: A descriptive study was conducted with 64 HPV-unvaccinated female adolescents and their mothers in W city, Republic of Korea. Data collection was conducted from October to November in 2019. The data were analyzed using SPSS WIN 24.0 Program. Descriptive statistics, paired t-test, independent t-test, χ2-test and logistic regression analysis were conducted. Result: According to the results, 57.8% and 62.5% of female adolescents and mothers had the intentions of HPV vaccination, respectively. In the logistic regression analysis for the factors affecting female adolescents' and mothers' HPV vaccination intentions, there was no significant factor in female adolescents. In the case of mothers, the significant factors were cervical cancer knowledge and perceived disability. Conclusion: In order to increase the HPV vaccination rate, educational programs are needed to provide knowledge of cervical cancer. A variety of intervention strategies should be developed to decrease the perceived disability of HPV vaccination health beliefs.

Purpose: As the interest and demand for esthetics grow up with medical development, the number of patients who want to correct dentofacial deformities by orthognathic surgery has increased. And the success of orthognathic surgery is dependent upon satisfaction of patients in great measure. Therefore the purpose of this study is to examine functional disability, sensory disturbance and esthetic satisfaction of the patients after orthognathic surgery. Material and methods: Among 89 patients who orthognathic surgery was performed at Seoul National University Bundang Hospital during the period from July 2003 to July 2008, 46 patients were considered in the study. Patients were surveyed with questionnaires and visual analogue scale(VAS) which author has designed. The analysis and comparison were performed according to gender, follow up span, and type of surgery based on answer of questionnaires. Results and conclusions: Total satisfaction rate was 76%. And there was statistical significant difference between subjective evaluation of facial esthetics before the surgery and that of after surgery(p<0.05). Differences in occurrence of functional disability, sensory disturbance between groups were not observed. In conclusion, the operator should understand the postoperative self-satisfaction of the patients and complications. If we can communicate with the patients before operation. patients' satisfaction rate will increase after operation.
Objectives: This study was intended to see the relationship between psychiatric consequences and injury severity following traffic accidents. Methods: We surveyed the 134 patients who were hospitalized from 1994 to 2003 at Chosun University Hospital for psychiatic disability evaluation following traffic accident. We reevaluate demographic factors from admission note. Psychiatric symptoms from mental status exam in medical records. Psychological tests(MMPI, BAI, BDI, K-WAIS) were done. Then we calculate the injury severity score and McBride's rate of disability due to diagnosis from emergency care hospital records. Their relationships were evaluated by statistical methods which were t-test and Pearson correlation analysis using SPSS-10. Results: When physical injury was not severe, suicidal attempt was more frequent and depression, hysteria, psychasthenia, psychopathic deviation subscales were high in MMPI. But when physical injury was severe, they have diffficulty in concentration, impaired orientation, and changed in IQ score. There was no relationship between physical injury severity and faking bad scales(F, Ds-r). Conclusion: We must not assume when physical injury was not severe, the severe sympomts are just faking for their benefit.

Objective : This multi-center clinical study was designed to determine the long-term results of patients who received a one-level posterior lumbar interbody fusion with expandable cage ($Tyche^{(R)}$ cage) for degenerative spinal diseases during the same period in each hospital. Methods : Fifty-seven patients with low back pain who had a one-level posterior lumbar interbody fusion using a newly designed expandable cage were enrolled in this study at five centers from June 2003 to December 2004 and followed up for 24 months. Pain improvement was checked with a Visual Analogue Scale (VAS) and their disability was evaluated with the Oswestry Disability Index. Radiographs were obtained before and after surgery. At the final follow-up, dynamic stability, quality of bone fusion, interveretebral disc height, and lumbar lordosis were assessed. In some cases, a lumbar computed tomography scan was also obtained. Results : The mean VAS score of back pain was improved from 6.44 points preoperatively to 0.44 at the final visit and the score of sciatica was reduced from 4.84 to 0.26. Also, the Oswestry Disability Index was improved from 32.62 points preoperatively to 18.25 at the final visit. The fusion rate was 92.5%. Intervertebral disc height, recorded as $9.94{\pm}2.69\;mm$ before surgery was increased to $12.23{\pm}3.31\;mm$ at postoperative 1 month and was stabilized at $11.43{\pm}2.23\;mm$ on final visit. The segmental angle of lordosis was changed significantly from $3.54{\pm}3.70^{\circ}$ before surgery to $6.37{\pm}3.97^{\circ}$ by 24 months postoperative, and total lumbar lordosis was $20.37{\pm}11.30^{\circ}$ preoperatively and $24.71{\pm}11.70^{\circ}$ at 24 months postoperative. Conclusion : There have been no special complications regarding the expandable cage during the follow-up period and the results of this study demonstrates a high fusion rate and clinical success.
Objectives: This study estimated the burden of disease especially caused by psychiatric disorders in Korea by using DALY, a composite indicator that was recently developed by the Global Burden of Disease study group. Methods: First, 11 of the major psychiatric disorders in Korea were selected based on the ICD-10. Second, the burden of disease due to premature death was estimated by using YLLs (years of life lost due to premature death). Third, for the calculation of the YLD (years lived with disability), the following parameters were estimated in the formula: the incidence rate, the prevalence rate and the disability weight of each psychiatric disorder. Last, we estimated the DALY of the psychiatric disorders by adding the YLLs and YLDs. Results: The burden of psychiatric disorder per 100,000 people was attributed mainly to unipolar major depression (1,278 person-years), schizophrenia (638 person-years) and alcohol use disorder (287 person-years). For males, schizophrenia (596 person-years) and alcohol use disorder (491 person-years) caused the highest burden. For females, unipolar major depression (1,749 person-years) and schizophrenia (680 person-years) cause the highest burden. As analyzed by gender and age group, alcohol use disorder causes a higher burden than schizophrenia in men aged 40 years and older. For females, unipolar major depression causes the highest burden in all age groups. Conclusions: We found that each of the psychiatric disorders that cause the highest burden is different according to gender and age group. This study's results can provide a rational basis to plan a national health policy regarding the burden of disease caused by psychiatric disorders.

The purpose of this study is to suggest the potential domestic and international market conditions vehicles for people with disabilities through a review of the literature and internet resources, and to propose a road map developing South Korean vehicles for people with disabilities taking into consideration the current state of domestic and international technology and the usage of the vehicles for people with disabilities depending on the type of disability. The results of the study are as follows: First, vehicles for people with disabilities safety standards of South Korea do not meet the safety standards of developed countries and the technology of development related vehicles for people with disabilities is not as advanced as that of developed countries. Vehicles for people with disabilities installation methods, specifications, features and safety standards should be designed to be more similar to those of Japan and the United States. Second, vehicle modifications for people with disabilities will become more common as measures directed at promoting the welfare of people with disabilities, such as travel and transportation measures, employment opportunities, education, and funding for vehicle modifications, are implemented along with laws, such as The Americans with Disabilities Act, The British Disability Discrimination Act, and Japan's barrier-free transport law, that protect people with disabilities generally. Third, through a comparative analysis of domestic and international market conditions and demands of people with disabilities in vehicle modifications, important things to pay attention by purchasing a modified vehicle for people with disabilities are the employment rate for people with disabilities and monthly income. To enable this, government agencies and industry bodies should work together through the revision of the current Disability Welfare Act or the enactment of the special law for guiding principles of Disability Welfare Act. Through the future changes in vehicles for people with disabilities-related system or society of South Korean, the annual demand of the vehicles is expected to be more than 10,000.
