The Review on the Study of Oriental Obesity Pattern Identification: Focused on Korean Research Papers (비만(肥滿) 변증 연구에 대한 고찰(국내 연구 중심으로))

  • Objectives The aim of this study is to analyse research trends about oriental obesity pattern identification in Korea. Methods We searched the papers with key words of 'obesity' and 'Pattern identification', 'Syndrom differentiation' in Korean database (Korean traditional knowledge portal, KISS, NDSL, DBPIA, KMBASE, Journal of Korean Medicine Rehabilitation, Journal of Korean Medicine for Obesity Research). We classified the papers by year and content. Results We reviewed 28 searched papers. Papers were published between 1992 and 2012. More than half of the total papers were published since 2008. There are 5 studies that focus on development and improvement of oriental obesity pattern identification questionnaire. 9 studies are research about using oriental obesity pattern identification questionnaire. 7 studies are research about Type of oriental obesity pattern identification. 4 studies are literature review of oriental obesity pattern identification. Other studies related to oriental obesity pattern identification are three. Conclusions To improve application and objectification about oriental obesity pattern identification, more clinical and oriental obesity pattern identification questionnaire studies are needed.

Studies on the Standard Measure of Compound Patterns of Eight Principles for Rapid Pattern Differentiation against Epidemic Contagious Diseases (전염성 감염병에 대한 신속변증 시행을 위한 팔강복합증형 표준안 연구)

  • In order to secure practising rapid pattern(證, zheng) differentiation against acute infectious diseases like corona virus disease-19(COVID-19) showing rapid variation and contagion, a simplified classification of stages centering on the exterior-interior pattern identification with 2 step-subdivision by cold, heat, deficiency, excess pattern and pathogens is proposed. Pattern differentiation by compound patterns of 8 principles is made for the non-severe stage of general cold and the early mild stage of epidemic disease. Compound pattern's names of 8 principles about external infectious diseases are composed of three stages, that is disease site-characters-etiology. Based on early stage symptoms of fever or chilling etc., exterior, interior and half exterior and half interior patterns are determined first, and then cold, heat, deficiency, excess patterns of exterior and interior pattern respectively are determined, and then more concrete differentiation on pathogens of wind, dryness, dampness and dearth of qi, blood, yin, yang accompanied with constitutional and personal illness factors. Summarizing above descriptions, 4 patterns of exterior cold, exterior heat, exterior deficiency, exterior excess and their secondary compound patterns of exterior cold deficiency and exterior cold excess and so on are classified together with treatment method and available decoction for a standard measure of eight principle pattern differentiation.

  • Objective : Syndrome differentiation(辨證) has clinical importance in East Asian Traditional Medicine. There are several ways for Syndrome differentiation. However, Eight Principle Pattern Identification(八綱辨證) is the base of other Syndrome differentiations. Of the physicians focused on Eight Principle Patterns(八綱), I have researched for Eight Principle Pattern Identification concerning with Zhang Jie-Bin(張介賓), Cheng Guo-Peng(程國彭), and Jiang Han-Tun(江涵暾) in Ming(明) and Qing(淸.) Method : Applying to Eight Principle Pattern Identification, I have researched comparatively for 3 kinds for. First, Zhang Jie-Bin's Jingyuequanshu(景岳全書) that involves Yinyangpian(陰陽篇), Liubianbian(六變辨), Biaozhengpian(表證篇), Lizhengpian(裏證篇), Xushipian(虛實篇), and Hanrepian(寒熱篇), secondly, Cheng Guo-Peng's Yixuexinwu(醫學心悟) Hanrexushibiaoliyinyangbian(寒熱虛實表裏陰陽辨), at lastly Jiang Han-Tun's Bihuayijing(筆花醫鏡) Biaolixushihanreyinyangbian(表裏虛實寒熱辨). Results : All of sick cases can be explained totally by Eight Principle Patterns. Of Eight Principle Patterns, Yin(陰) and Yang(陽) include last Six Principle Patterns(六綱 : 表裏, 寒熱, 虛實). Six Principle Patterns can be divided normally by 6 pulses(六脈 : 浮沈 遲數 虛實). In all of pain cases, feeling comfortable(可按) or discomfortable(拒按) to palpation can be important foundation for distinguishing Xu(虛) from Shi(實). Physical constitution(體質) for Hanre Xushi(寒熱 虛實) and tongue-diagnosis(舌診) for Biaoli Hanre(表裏 寒熱) are used effectively. Related with tongue-diagnosis, tongue-coating(舌苔) for Biaoli and tongue status(舌質) for Hanre are used effectively. Symptoms should be divided following this sequence, Biaoli ${\rightarrow}$ Hanre ${\rightarrow}$ Xushi and lastly should be summarized of Yinyang. Conclusion : Diagnosing with Eight Principle Patterns, digestive function, urine, and feces should be checked at first. In addition, the pulse, tongue, physical constitution, and good or bad from palpation(觸診), these should be checked and give a result. And then the result can be an important evidence of syndrome differentiation. As a result, it would be the best to diagnosis that discriminating the sequence as Biaoli Hanre Xushi and summarizing with Yinyang.

The Difference of Syndrome Differentiation Patterns between Premenopausal and Climacteric Obese Korean Women (폐경전 및 갱년기 과체중 한국 성인 여성의 변증 지표 차이에 대한 연구)

  • Objectives The aim of the study was to investigate the difference between pattern identification of premenopausal(n=39) and climacteric(n=40) korean obese and overweight women using Syndrome Differentiation Questionaire. Methods 39 premenopausal obese women(BMI${\geq}25kg/m^2$) and 40 climacteric overweight and obese women(BMI${\geq}23kg/m^2$) were recruited from October 2007 to March 2008 in Seoul, Korea. Subjects who had other disease were rejected. Basic anthropometry and body composition were measured. Every subjects were given and filled out the Syndrome Differentiation Questionaire, and we analyzed that using Fisher's exact test. Results 1. Premenopausal women showed high frequency of food accumulation pattern(43.6%), but in climacteric women, liver qi depression pattern was frequent(57.5%, p=0.021). 2. In weighted Syndrome Differentiation Questionaire score, Premenopausal women showed high frequency of food accumulation pattern(43.6%), but in climacteric women, liver qi depression pattern was frequent(47.5%, p=0.004). 3. There were no correlation between anthropometry and scores of the each patterns. Conclusions In this study, we can find out that the dietary factors play major roles in obesity of premenopausal women and emotional factors in obese climacteric women in the view of oriental pattern identification diagnosis. But it seemed that there lacked of consideration that reflected the degree of obesity in this Syndrome Differentiation Questionaire.

(longitude obesity 153 person, altitude obesity 90 person) against it analyzes. In order to analyze the reliability of the items which diagnose each Pattern-Identification it used Cronbach alpha coefficient and escape it did the alpha of each item. Alpha value of each Pattern-Identification than appears more highly the item which it will be able to consider an elimination in the item which decreases a reliability. In that phlegm-retention syndrome is bigger alpha coefficient 0.784 than 'meal quantity is few'(0.787) a possibility of seeing in the item which decreases a reliability to the place where it diagnoses it puts in phlegm-retention syndrome.

Study of the Indicators of Dampness-Phlegm Pattern Identification Based on Tentative Korean Standard Differentiation of the Symptoms and Signs for Stroke (한국형 중풍 변증 표준시안의 습담 변증 지표에 대한 연구)

  • Objectives : This study was done to investigate clinical frequency and correlation among the indicators of dampness-phlegm pattern identification settled by tentative Korean Standard Differentiation of the Symptoms and Signs for Stroke. Methods : The subjects were 147 hospitalized patients with stroke, and a list of registry was made for each of them. Among the five types of pattern identification, fire-heat, dampness-phlegm, blood-stasis and deficiency of Qi and of Yin, those that have shown a high frequency in dampness-phlegm type were categorized as the dampness-phlegm pattern group. Frequency of dampness-phlegm indicators was compared with those from the non-dampness-phlegm pattern group. Correlations among dampness-phlegm indicators were also studied. Results : 1. Dampness-phlegm pattern group included 26 patients out of 147. 2. Among the indicators of dampness-phlegm pattern. those, in order of highest frequency, were 'tiredness or sluggishness', 'white Coated tongue' and 'sputum'. 3. In comparing dampness-phlegm pattern group with non-dampness-phlegml group, the indicators such as 'lightheadedness', 'nigrescence', 'sputum', 'dermatic dysaesthesia' showed significantly high frequency. 4. Among the indicators, 'sputum' and 'tiredness or sluggishness', 'sputum' and 'yellow coated tongue', and 'white coated tongue' and 'yellow coated tongue' showed significant correlation. 5. In investigation of the correlation of scale in symptoms, various results such as positive correlation and negative correlation were obtained. Conclusion : In this study, more sensitive indicators of dampness-phlegm pattern identification were found. Based on these results, it is suggested that a more practical Korean Standard Differentiation of the Symptoms and Signs of Stroke would be established through continuous clinical studies by giving weight on each specific type of pattern identification.

Study of the Herbal Medicine for Stroke by Korean Standization of Pattern Identification for Stroke (한국형 중풍변증표준안-Ⅱ의 변증별 사용 처방에 대한 연구)

  • This study was performed to investigate using of herbal medicine based on pattern identification of symptoms, such as Qi deficiency pattern, Yin deficiency pattern, Phlegm -dampness pattern, Static blood pattern, Fire-heat pattern. These patterns settled by Korean Standard Differentiation of the Symptoms and Signs for Stroke in 2005. This study was done with 177 patients in Daejeon University Oriental Medical Hospital in the period of November 2006 to July 2007. Among the five types of pattern identification, Phlegm -dampness pattern showed significantly high frequency especially in 3 weeks after stroke. The sort of herbal medicine was 43 and Dodamhwalhyoel-tang(26.61%), Sunhwanki 1 hobang(14.52%), Banhabaekchulcheonma-tang(4.84%), Ansinchongnoi-tang(4.84%), Chongryuldodam-tang(4.03%) were most frequently used. Especially Dodamhwalhyoel-tang showed significantly high frequency in 2 weeks after stroke. Based on these results, it is suggested that more practical Korean Standard Differentiation of the Symptoms and Signs for Stroke would be established through continuous clinical studies by giving weight on relationship between herbal medicine and pattern identification.

Definition, Role and Method of Yinyang Pattern Differentiation (음양변증(陰陽辨證)의 정의와 기능 및 판별방법 연구)

  • In order to ensure the fact that eight principle pattern differentiation is used clinically as a basic guideline for Korean medicine practice, the definition, role and method of yin-yang pattern differentiation with its case report were explored at first. Yinyang Pattern Differentiation is a method of discriminating human tendencies or morbidity based on the yin and yang characteristics expressed in living bodies. And yin and yang are the two contrasting characteristics and aspects of the interaction when certain physical conditions that have a lasting effect on the human physiological metabolic function are correlated with the morbidity. Specific methods of yinyang pattern differentiation can be divided into several types of yin and yang indicators. First, time and space factors like day and night, hot and cold seasons, above and below, topographical districts. Second, colors and pulse and their/or relative clearness and muddiness, hardness and softness, moving and resting. Third, diagnose yin and yang patterns through distinguishing the true and false of a fever and cold in an emergency phase such as increase of brain pressure and shock state. Fourth, general characteristics of the propensity and constitution of a subject such as body type, speech, behavior, and physiological metabolism. And for clinical use, these were summarized again as a symptom indicators of physical signs and color, pulse, tongue and questionnaire indicators of propensity, body type, and space-time characteristics. Conclusively, it was confirmed that yinyang pattern differentiation has its own diagnostic significance which is distinct from exterior-interior, cold-heat and deficiency-excess pattern differentiation.

Literature Review of Clinical Studies for the Relationship between Ultrasonographic Examination and Syndrome Differentiation Classification in Chinese Medicine (초음파영상검사와 한의변증분류와의 관계와 관련된 중의학 임상연구에 대한 문헌고찰)

  • This study was to investigate the relationship between ultrasonographic examination and pattern identification classification on cinical studies in chinese medicine. We searched clinical studies related correlation between ultrasonographic examination and pattern identification classification in chinese medicine, that published from 2013 to 2016 in China National Knowledge Infrastructure (CNKI) databases by keywords, 'ultrasound(超?)', 'chinese medicine(中?)', 'syndrome differentiation (辨?)'. Seventeen studies were found. There were 7 studies of gynecological diseases including polycystic ovary syndrome and uterine myoma, 5 studies of fatty liver, 3 studies of arthritis, and 1 studie of thyroid nodule and lymphadenopathy respectively. As a result, ii is thought that there was a certain degree of correlation between the change of the ultrasonographic image and the pathological types according to traditional chinese medicine (TCM) syndrome differentiation and ultrasonographic examination could be used as secondary means for the TCM syndrome differentiation classification. In conclusion, by using ultrasonograph device in a medicinal way of TCM and traditional korean medicine (TKM), it is thought that more detailed and accurate diagnosis and treatment are possible and the evidence for reasonableness of syndrome differentiation in TCM and TKM its validity can be secured.

Study of the Indicators of Each Pattern Identification Based on Korean Standard Differentiation of the Symptoms and Signs for Stroke (한국형 중풍 변증 표준안-Ⅱ의 변증별 변증지표의 분포 및 타당도에 관한 연구)

  • The purpose of this study is to investigate clinical frequency and priority of five pattern identification settled by Korean Standard Differentiation of the Symptoms and Sign for Stroke. The present study was done over 177 hospitalized patient with stroke in the Daejeon University Oriental Medical Hospital in the period of November 2006 to July 2007. Stroke patients had been interviewed by residents and specialists who studied standard operation procedures in Fundamental study for Standardization and Objectification of Differentiation and Pattern Identification of Syndrome of Oriental Medicine for Stroke. In Heat-transformation pattern group, men showed significantly high frequency and in Deficiency of Gi pattern group, women showed significantly high frequency. In Heat-transformation pattern group, the indicators such as 'aversion to heat during fever', 'flushed face', 'full and rapid pulse', 'dryness of the eyes, 'yellow coating of the tongue', 'feverishness of the limbs', 'dryness of the eyes' showed significantly high frequency. In Dampphlegm syndrome group, the indicators such as 'white coating of the tongue', 'yellowish face', 'thick coating of the tongue', 'wheezing in the throat with sputum', 'swollen tongue', 'slippery pulse' showed significantly high frequency. In Deficiency of Gi pattern group, the indicators such as 'pale tongue', 'lassitude', 'pale face', 'weakness pulse' showed significantly high frequency. In Deficiency of Eum group, the indicators such as 'short and rapid pulse', 'mirror-like tongue' showed significantly high frequency. For more sensitive Korean Standard Differentiation of the Symptoms and Signs for Stroke, Large scale study is to be done, giving weight on the important indicators.
