A study on Palpation of the back-shu points (배유혈(背兪穴) 안진(按診)에 관(關)한 고찰(考察))

  • The diagnosis in Oriental medicine is done by inspection, auscultation and olfaction, interrogation, four diagnostics of pulse feeling and palpation, and various system of identification like identification according to Qi(vital energy), Xue and body fluids, identification according to fair principles, identification according to principles of Wei, Qi, Ying and Xue, identification according to Sanjiao(the triple heater), identification according to four type physical constitution. Sometimes, symptoms and diagnosis techniques according to symptoms is selectively applied for the diagnosis. Among them the pulse feeling and palpation diagnosis technique using the sense of finger and palm of the hand is divided into feeling of pulse and palpation and pressing maneuver. Pressing maneuver is a diagnosis technique pressing and rubbing the affected part in order to attain data of identification including inside and outside condition of the body with regard to the nature, condition and relative seriousness of disease. There are palpation of the skin, palpation the hand and foot, palpation the chest and the abdomen, palpation shu points in pressing maneuver. The diagnosis of the Back Shu points is a technique to examine the change of disease condition from pressure ache, spontaneous ache, tension, relaxation, solidification revealed through channels and collaterals. I investigates starting disease and an attack of disease of twelve pulse and pulse condition through the study relative to the substance and technique of pressing maneuver, and adjusts diagnosis techniques of a region for acupuncture and matters to be attended. The conclusions are as follows. 1. The Shu or stream points in which pathogenic factors go are important to medical treatment of dormant diseases like bowels disease, cold symptom complex and insufficiency symptom complex. 2. Disease classified by system is diagnosed by the condition of process part like pro-trusion, cave-in, tension, relaxation, pressure ache through palpating the Shu or stream points, that is pressing upward or downward left and right sides of the backbone process by hands. 3. In real clinic pressing maneuver of one's back side is very important to patient's diagnosis treatment. Thus, pressing maneuver of one's back side have to be done without omission. 4. Diagnosis must be accomplished through the perception about the diversity of diagnosis technique of bowels disease, the exact knowledge about pressing maneuver of one's back side for enlargement of treatment range and rising of treatment rate, and pressing maneuver of the Shu or the stream points.

  • Objects : To evaluate clinical efficiency of The A-Shi point for the herniated lumbar intervertebral disc. Methods : The patients who had a diagnosis of herniated lumbar intervertebral disc by lumber CT and MRI, and were observed from the twentieth March 2000 to the tenth Novemsber 2000, were divided into two classes ; the "A" group was 15 cases practiced with Acupuncture treatment used of A-Shi point and the twelve-Meridian, the "B"group 15cases only Acupuncture treatment used of twelve-Meridian, Results : the most type of low back pain was, the Gall bladder channel of Foot-souyang 15 cases (50%) and the Gall-Bladder Meridian Muscle was 45 cases the most in the existence part of A-Shi point On the result of treatment due to clinical symptoms, the "A" group was 57.9% as excellent and "B" group 19.8%. On the measurement of Lumber flexion, the "A" group proceed more excellent result than the "B group at the whole grade. Conclusion : These results suggest that The A-shi point was effective treatment of herniated intervertebral disc.

3 Case Study That Classified and Performed Treatment on Patients with Taeyangin constitution (태양인 환자의 임상적 치료 3례)

  • In clinic, it's very difficult to diagnose and treat a Taeyangin patient accurately. Taeyangin are rarer than other constitutions in number and there are few references in documents. Therefore, it will be helpful to understand Taeyangin if we accumulate their clinical cases. To that end, I introduce a case study concerning 3 persons. Among patients who have visited the Oriental Hospital of Dongguk University, these 3 persons were classified as Taeyangin and then their symptoms were reduced by Taeyangin treatment. Through reporting the case studies concerning these 3 persons - a male patient aged 14 complaining of feeling oppressed and dyspnea, a female patient aged 39 complaining of dyspepsia, feeling heavy in the head, and fatigue, and a male patient aged 23 complaining of acid regurgitation and nausea, I want to contribute to diagnosis and treatment in Taeyangin.

Analysis of Clinical Research Trends on Traditional Chinese Medicine Treatment for Depression Syndrome Similar to Hwabyung: Focusing on CNKI (화병(火病) 유사 병증의 한의학적 치료에 대한 임상연구 동향분석 - CNKI를 중심으로)

  • Objectives: To analyze whether a concept similar to Korean 'Hwabyung' exists in China. We investigated the status of clinical studies conducted in China for relevant diseases and trends of the traditional Chinese medicinal (TCM) treatment adopted in clinical studies. Methods: To explore the concept of a condition similar to Korean Hwabyung in China, we searched for the existence of concept and pattern that were parallel or similar to those of Hwabyung in TCM text-books and diagnostic guidelines. We searched and analyzed clinical studies of TCM treatment for depression syndrome similar to Hwabyung from CNKI using terms 'depression' and 'qi stagnation transforming into fire'. Using extracted data, characteristics of clinical research, herbal medicine, and acupuncture treatment used in the clinical research and their effects were systematically reviewed. Results: Symptoms of 'qi stagnation transforming into fire' were most similar to those of Hwabyung. Nine articles were selected from a total 258 articles. Most of them used DSM-IV or CCMD-3 for depression diagnosis. They applied 'diagnostic and efficacy guidelines for TCM diseases and syndromes' for pattern diagnosis of 'qi stagnation transforming into fire'. Danzhixiaoyao-san and Jiaweixiaoyao-san were found to be effective when they were used alone or in combination with antidepressants. Acupuncture treatment also showed remarkable effect on LR3, HT7, LI4, PC6, GV20 when it was used alone or in combination with antidepressants. However, careful interpretation is required because a small number of studies are included.Conclusions: 'Qi stagnation transforming into fire' seemed to have symptoms similar to Hwabyung. However, further research is needed to determine its diseases and pattern types compared to Hwabyung. According to included studies of 'depression with Qi stagnation transforming into fire', herbal medicine and acupuncture treatment tended to be effective in relieving depressive symptoms. However, more discussion is required for future application of herbal medicine and acupuncture for treating Hwabyung.

The Effect of Korean Medical Combination Treatment on 72 Cases of Herniated Intervertebral Lumbar Disc Patients: An Observational Study (요추 추간판탈출증 입원환자 72례에 대한 한의학적 복합치료 효과의 관찰 연구)

  • Objectives : This study was designed to assess the general distribution and clinical effectiveness of Korean medical treatment on lumbar disc herniation. Methods : This is an observational study. 72 patients admitted to Daejeon Jaseng Hospital of Korean Medicine with a diagnosis of herniated intervertebral disc(HIVD) by lumbar-CT of lumbar-MRI were observed from July, 2014 to April, 2015. They were analyzed according to sex, age, the period of disease, causal factors, symptoms on admission, admission day, disc herniation type and treatment efficacy. All patients received a combination of treatments during hospitalization, including acupuncture, Chuna, herbal medicines and physical therapy. A zero to ten numerating rating scale(NRS) assessing pain, Oswestry disability index(ODI) and EuroQol-5 dimension(EQ-5D) was used before and after treatments. Results : Average admission duration was $28.00{\pm}12.85$ days in lumbar disc patients. For lumbar patients, lower back pain NRS decreased from $5.89{\pm}2.00$ to $3.42{\pm}1.87$(p<0.001) and radiating pain from $5.96{\pm}2.12$ to $3.38{\pm}1.83$(p<0.001). ODI decreased from $46.69{\pm}19.25$ to $35.69{\pm}16.67$(p<0.001), and EQ-5D index increased from $0.63{\pm}0.26$ to $0.71{\pm}0.20$(p<0.05) after treatment in lumbar disc patients. Conclusions : Korean medical combination treatment might be effective in reducing pain and improving quality of life for patients with lumbar disc herniation. This study further confirmed the efficacy of Korean medical treatment on HIVD.

Two Cases of Improved Dizziness and Ataxia due to Pontine Stroke in Bojungikki-tang-gamibang (보중익기탕가미방(補中益氣湯加味方) 투여를 통한 뇌교병변 현훈과 보행실조 환자 치험 2례)

  • Bojungikki-tang-gamibang is a widely used herbal prescription in traditional medicine in Korea. The aim of this study is to investigate the effectiveness of Bojungikki-tang-gamibang for patients who have pontine stroke. They have problems with type of Qi deficiency(氣虛) in the rehabilitation stage. The type of Qi deficiency caused the dizziness and ataxia for the patients. In this study there are two types of patients. The first case is a patient with dizziness and ataxia because of potine hemorrhage. The second case is a patient who has the same symptoms as the first one but caused by pontine infraction. We treated the two patients with Bojungikki-tang-gamibang. The Improvement of symptom was evaluated by Korean vestibular disorders activities of daily living scale(K-VADL). As a result the degree of dizziness decreased noticeably while other symptoms improved as well. This study suggests that Bojungikki-tang-gamibang may be an effective treatment for patients who suffer stroke with type of Qi deficiency(氣虛) in the rehabilitations stage. However, more cases and methods of diagnosis are required to prove that the oriental medicine, Bojungikki-tang-gamibang can apply to patients universally.

Clinical Study on Effect of Scolopendrid Aquacupuncture Classified by the Type of Lumbar Disc Herniation (요추간판(腰椎間板) 탈출형태별(脫出形態別) 오공약침(蜈蚣藥鍼)의 효과(效果)에 관한 임상적(臨床的) 연구(硏究))

  • Objective : This study was designed to find out the effect of scolopendrid aquacupuncture classified by the type of HIVD(Herniation of Inter-Vertebral Disc) in lumbar spine. Methods : The 50 patients who had a diagnosis of HIVD by lumbar-CT or lumbar-MRI and admitted to Gwangju oriental medical hospital in wonkwang university from June 2003 to March 2004 were observed. The symptom of inpatients is low back pain with or without sciatica. We treated 50 patients by scolopendrid aquacupuncture besides the general consevative treatment of oriental medicine. Results and Conclusion : The scolopendrid aquacupuncture treatment led to improvement in the pain and symptom of HIVD as determined by all efficacy measures. After scolopendrid aquacupuncture treatment, there was improvement in VAS, oswestry disability index, ROM and SLRT. The improvement index of scolopendrid aquacupuncture treatment classified by the type of HIVD showed that the effects of scolopendrid aquacupuncture had correlation with the type of HIVD. The more the herniation of intervertebral disc decreased, the more the effect of scolopendrid aquacupuncture increased. This results suggest that scolopendrid aquacupuncture is good method for treatment of HIVD but we have to consider the clinical correlation with the degree of herniation.

  • The subject of Abdominal syndrome in the field of Jin Kui Yao Lue takes a quarter of the whole research quantity, and has greatly contributed to the development of diagnoses due to the well-growth of syndrome differentiation through 'the differentiation of symptoms and signs based on prescriptions'-centered abdominal syndrome. Since then, while the diagnostic has been developed mainly in the field of pulse feeling and the inspection of the tongue, application of the abdominal diagnostic has not been continuously improved because of special historical and social environments. Recently, since interest in the differentiation of symptoms and signs based on abdominal syndrome has been raised by emphasis of Oriental diagnostic methods and. medical treatment which have led to the study of Abdominal Syndrome in Jin Kui Yao Lue. The following is the results of the study. 1. Jin Kui Yao Lue abdominal syndrome is categorized into all abdominal symptoms Epigastric symptoms, Hypochondrium symptoms, and Lower abdomen symptoms. 2. Subjective symptoms and Objective symptoms have been found in Abdominal syndrome, and Subjective symptoms have been more often than Objective symptoms. Both of the symptoms have been found more to co-exist in abdominal syndromes. 3. More cases of fullness of abdomen symptoms in All abdominal symptoms, a smaller number of cases in Taiyang disease, Yangming disease, disease involving all three yang, Tayin disease and Jueyin disease have been found, but there have not been found in Shaoyin disease. 4. More cases of Epigastric fullness and rigidity in Epigastric symptoms, Epigastric throbs in Palpitation symptoms, and sense of fullness-in-chest in Abdominal syndrome of chest and hypochondrium have been recognized. 5. Any regularity caused by abdominal symptoms has not been identified. 6. Diagnosis of the abdomen caused by abdominal symptoms has been identified in Epigastric fullness, Epigastric pain, Epigastric procrastination, Epigastric throb, fullness of abdomen and distension of lower abdomen.

A Study on the Characteristics of Patients Treated for Dizziness with Jeoreongchajeonja-tang: A Retrospective Analysis of 63 Cases (저령차전자탕을 이용해 현훈을 치료한 환자의 특징 고찰 : 63례의 후향적 분석)

  • Purpose: The purpose of this study was to conduct a clinical analysis of patients treated for dizziness with Jeoreongchajeonja-tang, evaluate the effect of Jeoreongchajeonja-tang administration, and present indications of Jeoreongchajeonja-tang in dizziness. Methods: A retrospective study was conducted on 63 patients who were admitted to the Oriental Hospital of Se-Myung University from August 2012 to June 2019, complaining of dizziness, so who took Jeoreongchajeonja-tang. Results: 1) As a result of the analysis of the general characteristics of patients who took Jeoreongchajeonja-tang with dizziness, women were 2.7 times more than men. The age distribution was: patients in their 70s, 36.5%; in their 50s, 20.6%; in their 60s, 15.8%; and in their 80s, 12.6%. Patients had a history of hypertension (46.0%), diabetes (22.2%), stroke (17.4%), lung disease (12.6%), and cancer (11.1%). According to the diagnosis, 48 patients (76.1%) were assigned to the peripheral dizziness group, 7 patients (11.1%) to the central dizziness group, and 8 patients (12.6%) to other dizziness groups. 2) Compared with the peripheral dizziness group, the central dizziness group was older, their history of hypertension 8.4 times greater, their history of stroke 7.8 times greater, and the duration of herbal medicine administration was longer. 3) The vertigo grade was significantly decreased after taking Jeoreongchajeonja-tang (p=0.000). 4) As age increased by one year, odds ratio of improvement in the vertigo grade decreased by 0.924 times after taking Jeoreongchajeonja-tang (p=0.010). 5) Sex, disease duration, past history, diagnosis, the duration of herbal medicine administration, whether or not Yanggyuksanhwa-tang was combined with the Jeoreongchajeonja-tang and whether or not Western medicine was used did not affect improvement in the vertigo grade after taking Jeoreongchajeonja-tang. Conclusions: In patients with dizziness, Jeoreongchajeonja-tang can be used as a treatment without side effects.

A Narrative Review of Carpal Tunnel Syndrome in Traditional Chinese Medicine: Using China National Knowledge Infrastructure Database (수근관 증후군의 최근 6년간의 연구 동향 분석: China National Knowledge Ingrastructure (CNKI) 보고 연구를 중심으로)

  • Objective The purpose of this study was to investigate clinical studies between 2015 and 2020 on the traditional Chinese medicine treatment for carpal tunnel syndrome (CTS) using the China National Knowledge Infrastructure (CNKI) database. Methods We searched clinical studies on the Chinese medicine treatment published in traditional Chinese medical journals on CTS through the CNKI search. Fifteen published studies between 2015 and 2020 were chosen and examined for the year of publication, sample size, study design type, diagnosis and evaluation of diverse criteria, treatment period, purpose of the study, and classification based on the treatment technique. Results Out of 73 studies, 15 had practiced various Chinese medical treatments, such as acupuncture, moxibustion, manipulation, herbal medicine application therapy, and external herbal treatment. Conclusions Various studies have investigated the Chinese medicine treatment for CTS in China. To envelope the treatment of CTS in Korean medicine, steady and valuable therapies for high-level clinical research are required. In the future, this study could be helpful in the development of the treatment of CTS with Korean medicine.
