A Case of Segmental Vitiligo Occurring Where Herpes Zoster Occurred Previously as an Isotopic Response (제목을입력해주세요대상포진 발생 부위에 발생한 분절형 백반증 1예: Isotopic 반응)

  • Wolf's isotopic response is defined as the occurrence of a new skin disorder at the site of another unrelated skin disease that has already healed. In most cases of isotopic response, the initial dermatosis is herpes infection, and the most frequent second dermatoses are granulomatous reactions. Various interpretations of this phenomenon have been attempted. However, the exact mechanism has not been identified yet. Herein, we report a case in which the secondary disease was segmental vitiligo that appeared over the same dermatomes of herpes zoster. A 71-year-old woman presented with well-defined, depigmented patches on the left chest and back. She had been diagnosed with herpes zoster on the same dermatomes and treated with an antiviral agent 3 years ago. Histological examination showed decreased basal melanin pigments and melanocytes. Consequently, the patient was diagnosed with segmental vitiligo based on the clinical and histological findings.

Study on the Usage of Acne Patches in Patients with Acne (여드름 환자에서 여드름 패치 사용에 관한 연구)

  • Background: Patients with acne are at risk of using various over-the-counter drugs (OTCs). In addition, several OTCs are introduced in pharmacies in Korea. However, only few drugs are supported by well-established data. Among the treatments, acne patches are easy to purchase and use. Thus, they are widely used by patients. Objective: This study aimed to investigate the patient's perception and usage of acne patches. Methods: A questionnaire survey was conducted on 80 patients with acne who visited the outpatient department of the National Medical Center for 2 months. Results: A total of 42 (52.5%) patients answered that they had used acne patches, and 38 (47.5%) patients stated that they had never used patches. Moreover, 27 (64.29%) patients responded that they used the patches on inflammatory lesions. Moreover, several patients were using acne patches after pricking themselves. There was only one response to confirm the ingredient when purchasing acne patch. Moreover, 14 (33.33%) participants responded that they noted side effects after using acne patches. Overall, the satisfaction score for the use of acne patches was 5.05. Conclusion: Patients often use acne patches because they can be easily purchased and used. However, they do not know how to properly choose and use the product that fits their skin. Therefore, doctors must educate patients on how to properly select and use the products.

The Role of Bloodletting and Cupping in Severe Acute Urticaria and Angioedema as Skin Emergencies in Persian Medicine

  • Objectives: Some dermatological diseases can be life-threatening. Urticaria and angioedema are common reasons for patients to seek treatment at an emergency department. Severe, generalized urticaria and angioedema can endanger patients' lives by involving the airways and causing anaphylactic shock. The humor-based Persian Medicine (PM) concepts of Shara and Mashara, referring to two kinds of skin lesions, have similarities to urticaria and angioedema, respectively. This article aims to provide scientific evidence regarding the application of PM as an early intervention strategy in the emergency management of urticaria and angioedema. Methods: This was a narrative review of PM studies identified by searching medical databases using search terms related to these diseases, as well as risk-associated keywords such as "fatal", "death", "life-threatening", "emergency", "cupping", and "bloodletting". Data were then compared, interpreted, and analyzed. Results: PM scholars consider the human body as a unified whole and believe in an inner power (Nature) which stems from the body. When the presence of excessive hot substances cause an imbalance of bodily humors, Nature directs their heated vapors sharply toward the skin, thus causing Shara and Mashara. If there is a high risk of inflammation spreading to vital organs under severe conditions, urgent manual interventions are crucial. Conclusion: In serious conditions of urticaria and angioedema, Fasd or bloodletting and Hijama can be effective in speeding up the control of lesions and reducing morbidity and mortality. Consequently, the development of integrated Persian and conventional medicines may provide new therapeutic pathways for skin emergencies.

Effects of Green Gram Extract on Skin Lesion and Histopathological Changes in Mice with Contact Dermatitis (녹두 추출물이 생쥐에 유발된 접촉피부염의 피부 증상과 조직병리학적 소견에 미치는 영향)

  • Objectives : Green gram (mung bean) has a cold nature and has been known to detoxify against various side effects that belong to hot in nature. In particular, since it has the effect of detoxifying fever and detoxification to treat swelling, it was also used externally to treat febrile dermatological diseases such as erysipelas and rubella. This study was designed to determine whether green gram exhibits anti-inflammatory effects on contact dermatitis in mice. Methods : We investigated the effects of green gram extract (70% ethanol extract) on skin lesion, skin thickness and weights, melanin and erythema index and spleen body weight ratio in mice with contact dermatitis induced by repeated application of 1-Fluoro-2,4-dinitrobenzene. Results : Topical application of green gram extract ameliorates skin lesions of contact dermatitis such as scale and roughness induces by 1-Fluoro-2,4-dinitrobenzene. green gram extract also suppressed enlargement of skin thicknesses and weights significantly. In addition, green gram extract treatment also lowered erythema index significantly compared to those in the control group. In the histopathological observation, green gram extract prevented epidermal hyperplasia and hyperkeratosis in inflamed tissues. Finally, green gram extract did not affect changes in body weights and the spleen body weight ratio, unlike dexamethasone, which significantly prevented body weight gain and lowered the spleen body weight ratio. Conclusions : These results imply that green gram, which is known to have a detoxifying effect in Korean medicine, can be used in the treatment of contact dermatitis.

Two Treatment Cases of Combined Korean Medicine for Patients with Dermatological Diseases Exacerbated by COVID-19 (COVID-19로 악화된 피부과 질환 환자의 한의 치료 2례에 대한 증례보고)

  • Objectives : The purpose of this study is to report the clinical effect of herbal medicine decoction on COVID-19 symptoms remaining after administering Western medicine and worsening eczema during the COVID-19 prevalence period in patients with chronic eczema. Methods : The two chronic eczema patients suffering from COVID-19 symptoms and worsening eczema received a series of herbal medicine decoction for treatment. The effectiveness of treatment was evaluated by VAS, NRS, EASI, PASI, DLQI, photography and subjective evaluation of the patient's improvement of symptoms. Results : After treatment, all symptoms of COVID-19 disappered and aggravated eczema lesions were remarkably improved in both cases. Compared to its peak, the first case shows that PASI improved by 88% and DLQI by 44%. According to photo and symptom changes, the size of the affected area of facial acne and lip eczema was decreased. The second case shows that EASI improved by 43% and DLQI by 65%. According to photo and symptom changes, redness, rash were stopped and pruritus was decreased. Conclusion : This study suggests that herbal medicine could induce effectiveness on both COVID-19 symptoms as well as chronic eczema.

  • It has been known that the color of skin reflects the blood flow within. In lips, the capillaries close to the skin surface are numerous; hence lips are redder than the rest of face. However, dermatological research on lips is not as advanced as research on facial or body skin, and little was known about the relationship between relatively dull-colored lips and skin blood flow. The physiological differences between colorful and dull-colored lips were studied by a two-dimensional laser Doppler blood flow analyzer, a spectrometer for the measurement of the degree of oxygen saturation, and a confocal microscope for observing inside lips non-invasively. Dull-colored lips and the corner of lips (dull-colored compared to the center) showed relatively poor blood flow and lower oxygenated hemoglobin. It was found that colorful lips (generally the young) had a blood flow that tended to run straight in parallel with the skin surface. This unique blood-capillary structure can express clear red blood. Those with dull-colored lips had lost this unique structure. Their blood ran perpendicularly from the deep of the skin and down back again into the deep part as like the blood circulation patterns of facial skin. Therefore, the lips of the latter group had fewer blood capillaries near skin surface in the lips than that of the colorful-lips group. A lipstick containing a-glucosyl-hesperidin, which is derived from certain citrus fruits and can enhance blood circulation, was applied for evaluating its effects. Blood flow was increased 30 min after the application. After two weeks of daily application, the lips' condition became noticeably less dull. These findings suggest that the decrease of blood flow in dull-colored lips is caused by the loss of the unique capillary structure and the use of the lipstick to increase blood flow can give a vivid color to lips.

  • Drug eruption is a relatively commin dermatological disorder and may cause significant problems. The treatment is to stop using a drug that may cuase a drug eruption. In some cases, steroids and antihistamines can be used supportively. According to some reports, Onchungeum(溫淸飮) has steroid-like effects and antihistaminic effects. We administered Onchungeum(溫淸飮) a patient with severe itching because of a drug eruption. After administration of Onchungeum(溫淸飮), itching decreased to much degrees. It can be suggested that Onchungeum(溫淸飮) can be used to treat symptoms of allergic skin disease like a drug eruption.

Cutaneous Xanthoma in a Dog

  • A 2-year-old, 12.5 kg, castrated male, mixed-breed dog was presented with a 1-year history of pruritus and progressive alopecia. On physical examination, no remarkable findings were detected including body condition score (5/9). A dermatological examination of the dog revealed generalized erythema, papules or plaques, especially on the face, auricle, dorsum, and shoulder. A fine-needle aspiration of the dorsum and face lesions revealed various numbers of macrophages with foamy cytoplasm and multinucleated giant cells. A bacterial culture test showed the growth of Staphylococcus pseudintermedius. A complete blood cell count was unremarkable and biochemical abnormalities included hyperglobulinemia (4.8 g/dL, reference interval 2.5-4.5 g/dL), mild hypertriglyceridemia (277 mg/dL, reference interval 10-100 mg/dL) and mild hypercholesterolemia (383 mg/dL, reference interval 110-320 mg/dL). Additional diagnostic tests were performed to identify the underlying cause of hyperlipidemia. Canine pancreatic lipase immunoreactivity (<50 ng/mL, reference interval 0-200 ng/mL) and total T4 (1.4 ㎍/dL, reference interval 1.1-5.6 ㎍/dL) were within the reference intervals. For a definitive diagnosis, skin biopsy specimen was collected from the papular lesions on the dorsum by using a 4 mm biopsy punch. A histopathological examination revealed numerous large macrophages with abundant foamy cytoplasm in the dermis. The foamy macrophages were located diffusely between the collagen fibers. Extracellular amorphous lipid deposits were also presented in the dermal tissue. A definitive diagnosis of cutaneous xanthoma was made based on clinical signs and cytological and histopathological results.

Adjunctive Therapy of Pimecrolimus for Treatment of Facial Discoid Lupus Erythematosus in a Dog

  • A two-year-old, spayed female, 22.5 kg Pungsan was referred with chronic crusts and erosion on the nose. A referring veterinarian prescribed an anti-inflammatory dose (0.5-1 mg/kg/day) of oral glucocorticoids for 5 months, but skin lesions showed no meaningful improvement. A dermatological evaluation revealed a crust, depigmentation, erosion, and erythematous lesion over the nasal planum with a loss of the normal cobblestone texture. Also, firm and multifocal plaques over the thigh, groin, axilla, and dorsum were detected. A cytology examination on the nose, thigh, groin, axilla, and dorsum revealed moderate neutrophilic inflammation and bacterial infection. Abdominal radiography and ultrasonography revealed subcutaneous calcified materials along the thigh, groin, axilla, and dorsum. Calcinosis cutis was suspected because of the adverse effect of previous prolonged corticosteroid therapy. A histopathology examination of the nose lesion revealed moderate to severe degenerative or apoptotic changes of the basal layer and lymphoplasmacytic interface dermatitis. Facial discoid lupus erythematosus (FDLE) was diagnosed based on the history and the clinical, cytological, and histopathological results. Minocycline (7 mg/kg PO q 12 h) and niacinamide (500 mg/dog PO q 12 h) were prescribed as initial treatment. Glucocorticoids were not administered due to the presence of calcinosis cutis induced by previous corticosteroid treatment. After 6 weeks of treatment, the clinical signs on the nose were mildly improved. At this time, topical 1% pimecrolimus cream (twice daily) was initiated, while minocycline and niacinamide were continued at the same dose. The nasal planum markedly improved after 6 weeks of additional treatment, hence minocycline and niacinamide were prescribed for an additional 2 weeks and stopped, and the patient was continued solely on topical pimecrolimus. The dog's skin lesion has been maintained in clinical remission with topical 1% pimecrolimus twice daily for more than 5 months.

Comparison of Acute Symptoms Between a Alpine Agricultural Workers and General Workers in Gangwon-do (강원도 일부지역 고랭지 농업 농민과 일반 농업 농민의 농약중독 증상 비교)

  • Objectives: This study was performed to evaluate the pesticide exposure status and acute pesticide poisoning symtoms among agricultural workers at Gangwon-do province alpine area. Alpine area was defined as the area higher than 400m. Methods: We analyzed 257 interviwed questionnaire about pesticide exposure, acute pesticide poisoning symptoms and other variables. Results: The result was shown that agricultural workers at alpine area used more pesticide than general agricultural workers for annual usage days(35.9 days vs 14.4 days), daily usage hours(6.7 hrs vs 2.8hrs, p<0.05). But there was no difference between general and alpine agricultural worker's herbicide exposure. Moreover, the alpine agricultural worker's acute pesticide poisoning symtom score was higher than general agricultural worker's. These difference was also found at result of regression analysis, under control the age, sex, monthly income. Most frequently suffered symptom was headache and dermatological problems. The symptom prevalence of dermatological problem, headache, general weakness, eye irritation, nausea were higher among alpine agricultural workers than general agricultural workers. Conclusions: As a result, agricultural workers at alpine area were more exposed to pesticide and suffered from pesticide poisoning symptoms. To prevent the symptoms and disease from pesticide exposure among agricultural workers at alpine area, more research and political effort will be needed.
