항문주위 연쇄구균성 피부염(perianal streptococcal dermatitis, PSD)은 종종 다른 소아 항문주위 질환과 혼돈되어 잘못된 치료로 질병을 악화시키므로 빠르고 정확한 진단이 중요하다. 조기 진단과 치료는 환아의 불편과 부모의 스트레스를 경감시키며, 항생제 투여를 빨리 할수록 증상이 극적으로 빨리 호전된다. 저자들은 드물지는 않지만 현실적으로 잘 알려져 있지 않은 항문주위 연쇄구균성 피부염 1례를 경험하였기에 보고하는 바이다.

Exfoliative Dermatitis is a name of a group of symptoms which is characterized by red rash, itching and scale in almost whole body. The patients are suffered from hypoalbuminemia, general weakness and fever sign in addition to characteristic skin lesions. So in the treatment of this disease, adequate symptomatic treatment is important with adequate topical skin management. We experienced a 69 year old Korean male patient diagnosed as Exfoliative Dermatitis based on clinical symptoms and a medical certificate. We concluded that the symptoms of this patient are Sun-symptoms(癬症) and gave him Bangpungtongsung-san(剝脫性皮膚炎) and Geunsun-bang(乾癬方). After taking Oriental Medicine(the hebal medicine of internal use), clinical improvement was noticed. Based on this experience, internal use of Oriental Medicine have a good effect in Exfoliative Dermatitis.

We concluded that the symptoms of this patient are Sun-symptoms(癬症) and gave him Sunpunghwangi-san(宣風換肌散) and Chungdae-san(靑黛散). After taking herbal medicine, clinical improvement was noticed. Based on this experience, both internal and external use of oriental medicine have a good effect on exfoliative dermatitis.

Objective: This study aimed to report the improvement in a patient with allergic contact dermatitis treated with Sihogyeji-tang based on the Shanghanlun disease pattern identification diagnostic system (DPIDS). Methods: According to the Shanghanlun DPIDS, the patient with allergic contact dermatitis was diagnosed with greater yang disease chest bind (Shanghanlun provision 146), and was administered Sihogyeji-tang for 90 days. The results were evaluated using the Dermatological Life Quality Index (DLQI) and a visual analog scale (VAS). Results: After taking Sihogyeji-tang for 90 days, the DLQI decreased from 28 points to 8 points and the VAS decreased from 10 points to 3 points. Conclusions: This case shows the potential of treatment through herbal medicine for allergic contact dermatitis using the Shanghanlun DPIDS.

Objectives The purpose of this case study is to report that a Soeumin patient diagnosed as adult atopic dermatitis was treated with Sipyimigwanjung-tang and then his symptoms improved. Methods We diagnosed the patient as Soeumin Greater Yin Pattern based on his nature & emotion, physical characteristics, symptoms. So we treated him with Sipyimigwanjung-tang and used Visual Analogue Scale(VAS), SCORAD(Scoring of Atopic Dermatitis) index and pictures of the affected part for the assessment. Results & Conclusions After treatment, his SCORAD index results significantly decreased from 76.3 to 41.8, VAS of itching decreased from 10 to 4 and the patient's subjective symptom and general condition considerably improved. This case study describe the effectiveness of Sipyimigwanjung-tang on Soeumin adult atopic dermatitis symptoms.

Objective : This study reports the effect of Gamchosasim-tang on atopic dermatitis. Methods : According to shanghanlun six meridian patterns diagnostic system, We diagnosed Taeyang-byung kyeolhyung. The symptoms of atopic dermatitis were worsen when the patient had bad sleeping habits by stress(心煩不得安). So the patient treated with Gamchosasim-tang for 3 months. The severity of atopic dermatitis was evaluated by SCORAD index, EASI and pictures. Results : After the treatment, SCORAD index and EASI decreased $75.9{\rightarrow}27.9$, $19{\rightarrow}5.7$ each. And sleeping habits also improved. Conclusions : Gamchosasim-tang improved the symptoms of atopic dermatitis on this study. Although Gamchosasim-tang is not an usual treatment for atopic dermatitis, we could prescribe the medicine due to shanghanlun six meridian patterns diagnostic system. And Further research would be needed the relation between bad sleeping habits cased by stress(心煩不得安) and Gamchosasim-tang.

Atopic allergy implies a familial tendency to manifest alone or in combination such as asthma, rhinitis, urticaria and atopic dermatitis. The patient with atopic dermatitis shows a variety of humoral and cell-mediated immune dysfunction, including an elevation of serum IgE level, multiple positive immediate skin tests to a variety of antigen. A 30 year old female patient suffering from an atopic dermatitis on face, has been administered a stellate ganglion block(SGB) with a 1% mepivacaine 5cc. She complained of bronchial hyperresponsivness such as cough, soreness, and inspiration difficulty for 5 days following the treatment, so we have reduced 1% mepivacaine dose from 5cc to 3cc. She had no complain of bronchial hyperresponsivenss and the lesion of atopic dermatitis was improved. From our result we conclude that SGB appears to be a good choice for the treatment of the atopic dermatitis and only 1% mepivacaine 3cc is possible in complete SGB.

Objectives This study investigated the effect of camping style Hospitalization program for atopic dermatitis children patients. Methods 6 patients were admitted for camping style hospitalization program. The program includes acupuncture, herbal medicine, examination, education, psychotherapy, dressing, exercise. The patients were analyzed by Severity Scoring Atopic Dermatitis (SCORAD) index and measured skin pH value (pH). Also, patients evaluated themselves by writing an atopic dermatitis diary which consists of diet, emotion, severity of pruritus, and sleep loss. Checked questionnaires which consists of atopic dermatitis, self treatment methods and each program satisfaction. Results After 7 days of hospitalization program, the patient's SCORAD index scores were decreased and most of the patient's symptoms were improved. Conclusions The hospitalization program could be helpful for atopic dermatitis children patients.

Objective : This case report aims to show the effect of Osuyu-tang on an atopic dermatitis. The patient was diagnosed with Yangmyung-byung based on Shanghanlun provisions. Methods : According to Diagnostic system based on Shanghanlun provisions, the patient was diagnosed with Yanamyung-byung and has been unusual dietary habits. So she took Osuyu-tang decoction for 2 weeks. We evaluated the results of treatment by SCORAD Index and before and after treatment photos. Results : After taking Osuyu-tang decoction for 2weeks, the patient's SCORAD INDEX and symptoms was decreased from 49.5(severe) to 3.5(mile). Also, the patient's chronic diagestive disorder improved. Conclusions : This case report showed an effectiveness of using Osuyu-tang according to Diagnostic system based on Shanghanlun provisions on atopic dermatitis.

1. Objectives: The purpose of this case study is to evaluate the effects of Sasang constitutional therapy and treatment of an adult atopic dermatitis patient, who was diagnosed Soyangin, Interior febrile disease induced from the stomach affected by heat(少陽人 胃受熱裏熱病). 2. Methods: The adult Atopic Dermatitis patient was diagnosed Soyangin, Interior febrile disease induced from the stomach affected by heat(少陽人 胃受熱裏熱病) which was based on his Nature & Emotion(性情), physical characteristics, symptoms. He was medicated with Prescriptions of Soyangin. 3. Results and Conclusions: The clinical evaluation was made by SCORAD index, Total eosinophil count and Total IgE. After treatment, his SCORAD results significantly decreased from 88.4 to 21.9, Total eosinophil count decreased from 1320 to 120 and Total IgE also decreased from 1406.6 to 897.3. And the patient's subjective symptom and gerneral condition considerably improved after treatment. This case study describe the effectiveness on adult Atopic Aermatitis symptom by using Prescriptions of Soyangin.
