Comparative evaluation of the effectiveness of a mobile app (Little Lovely Dentist) and the tell-show-do technique in the management of dental anxiety and fear: a randomized controlled trial

  • Background: Behavior guidance is a technique used to subdue inappropriate behavior by establishing communication that meets the needs of a child. This study aimed to measure the effectiveness of a mobile app (Little Lovely Dentist) compared to the tell-show-do (TSD) technique in managing anxious children during their first dental visit. Methods: Fifty children (30 boys and 20 girls) without any past dental experience, aged from 7 to 11 years, were randomly allocated into either the dental app group or the TSD group. The pre- and post-operative anxiety of children who underwent prophylactic cleaning was assessed both physiologically and subjectively using a heart rate measurement and the RMS pictorial scale, respectively. Results: The intragroup comparison of heart rate and RMS scores for children allocated to the dental app group was statistically significant (P value ≤ 0.001). However, a significant reduction only occurred in the RMS scores, but not the heart rate measurements, in the TSD group. Conversely, there was an increase in heart rates in the TSD group. Conclusion: Educating the child prior to a dental procedure using a smartphone application such as Little Lovely Dentist can significantly alleviate the anticipatory anxiety and engage children in dental treatment during their first visit.

  • Purpose: Infection with human immunodeficiency virus (HIV) remains a major global threat, and although the prevalence is comparatively still very low, the number of HIV-positive Koreans is increasing. However, there are no official guidelines as to how to treat people living with HIV/acquired immunodeficiency syndrome (AIDS) (PLWHA) or how to screen for potentially infectious people. This study assessed the level of knowledge and attitudes of dentists in Korea toward PLWHA, and their attitudes to screening patients for HIV infections. Methods: A cross-sectional prospective survey targeting dentists working in Korea was conducted using a self-administered questionnaire. Results: A satisfactory level of knowledge about HIV/AIDS and a relatively positive attitude toward PLWHA was found. Most of the respondents preferred rapid HIV testing using oral fluid as a specimen. The general attitude of dentists toward HIV/AIDS is sufficiently positive to enable provision of the best treatment to the patients in need. Conclusions: Most of the dentists require HIV testing in dental clinics. In spite of their needs, there are several obstacles. It is hoped that financial considerations and official legal requirements related to HIV testing strategies will be considered.

  • Langerhans Cell Histiocytosis(Idiopathic Histiocytosis, Histiocytosis-X) is most often found in children and young adults, and cell proliferation with specific phenotype shows ultrastructural and immunohistochemical similarities with Langerhans Cells that normally exist in epithelium and mucosa. This disease occurs as single or multiple lesions in skull, ribs, vertebrae, mandible and long bones, and when it involves mandible, clinical sign and symptoms such as bone swelling and pain are noticed. When it involves alveolar bone, severe tooth mobility as well as gigival inflammation, proliferation, and ulceration are commonly found, and so it is not easy to differentiate it from general inflammatory diseases. Any local lesion at the tooth apex on the x-ray view needs to be differentiated from inflammatory disease, and multiple lesions from multiple ostoeoma and chronic multifocal osteomyelitis. This case is LCH in 51-year-old male patient ; this is a rare case, for the patient belongs to an age group with very low incidence rate of the disease. although three-timed biopsy tests and longterm observation at two university hospitals, it was misdiagnosed as multifocal osteomyelitis.

EVALUATION OF APICAL SEAL ACCORDING TO TYPES OF SPREADER AND COMPATIBILITY OF ACCESSORY CONE USED IN LATERAL CONDENSATION METHOD WITHOUT SEALER (Spreader의 종류와 Accessory cone의 접합성에 따른 근단폐쇄성에 관한 실험적 연구)

  • The purpose of this study was to evaluate the sealing ability according to types of spreader and compatibility of accessory cone used in lateral condensation method. 120 plastic blocks with canal preformed were instrumented with K-,H-files and Gates-Gllidden bur. Shaped plastic blocks were divided into six experimental groups according to spreader and accessory cone used in lateral condensation. Then they were obturated by lateral condensation method without -sealer. Six experimental groups were as follows: Group 1 : Filling with #30 spreader & #25 gutta-percha cone Group 2 : Filling with #30 spreader & Fine accessory cone Group 3 : Filling with #3 spreader & #25 gutta-percha cone Group 4 : Filling with #3 spreader & Fine accessory cone Group 5 : Filling with #20,#30 spreader & #25 gutta-percha cone Group 6 : Filling with #2, #3 spreader & Fine accessory cone All the blocks were stored in 100% humidor at room temperature for 2 days. Each block was placed in centrifuged for 20 minutes at 3,000 rpm. Apical leakage was mesured from the apical foramen to the most coronal level of- dye leakage in millimeter under a stereoscope. The data were analysed by ANOVA. The obtained results were as follows; 1. In groups using two spreaders(Group 5,6), the linear leakage was less than one spreader using groups(Group 1-4). 2. Tn groups using two spreaders(Group 5,6), there was no significant difference in linear leakage between standardized -spreader group and non standardized group (p>0.01). 3. When one spreader was used(Group 1-4), standardized-spreader groups showed less linear leakage than nonstandardized spreader groups(P<0.01). 4. In case of using same spreader(Groupl,2 & Group 3,4), there was no significant difference in linear leakage according to accessory cone type. 5. It needs to use one more spreaders to increase apical sealability.

  • Recording of the research procedure and results is the compulsory virtue to scientists. This ensures the objectivity and integrity of research and has become a cornerstone for a junior scholar. Furthermore, we can save time and efforts by avoiding duplication study. Recently on its patent application, the value of laboratory notebook is going to increase. However, there is a limitation to record a voluminous work in a traditional writing laboratory notebook. So, it could be an alternative proposal to convert that into an electronic work or develop an electronic laboratory notebook. Our journal has continuously pursued globalization. Laboratory notebook could be one of the key factors for that goal. In the present study, the authors reviewed the needs for laboratory notebook and tried developing a proto-type electronic laboratory notebook.

A scientometric, bibliometric, and thematic map analysis of hydraulic calcium silicate root canal sealers

  • Objectives: This scientometric and bibliometric analysis explored scientific publications related to hydraulic calcium silicate-based (HCSB) sealers used in endodontology, aiming to describe basic bibliometric indicators and analyze current research trends. Materials and Methods: A comprehensive search was conducted in Web of Science and Scopus using specific HCSB sealer and general endodontic-related terms. Basic research parameters were collected, including publication year, authorship, countries, institutions, journals, level of evidence, study design and topic of interest, title terms, author keywords, citation counts, and density. Results: In total, 498 articles published in 136 journals were retrieved for the period 2008-2023. Brazil was the leading country, and the universities of Bologna in Italy and Sao Paolo in Brazil were represented equally as leading institutions. The most frequently occurring keywords were "calcium silicate," "root canal sealer MTA-Fillapex," and "biocompatibility," while title terms such as "calcium," "sealers," "root," "canal," "silicate based," and "endodontic" occurred most often. According to the thematic map analysis, "solubility" appeared as a basic theme of concentrated research interest, and "single-cone technique" was identified as an emerging, inadequately developed theme. The co-occurrence analysis revealed 4 major clusters centered on sealers' biological and physicochemical properties, obturation techniques, retreatability, and adhesion. Conclusions: This analysis presents bibliographic features and outlines changing trends in HCSB sealer research. The research output is dominated by basic science articles scrutinizing the biological and specific physicochemical properties of commonly used HCSB sealers. Future research needs to be guided by studies with a high level of evidence that utilize innovative, sophisticated technologies.

  • Class III malocclusion or anterior crossbite is commonly seen in Asian. This problem is easily recognized by dentists and parents. During the primary dentition period, anterior crossbite with functional shift and deep overbite could develop to skeletal protrusive mandible. So, early and proper diagnosis of anterior crossbite which needs prompt treatment is important. These cases showed the early management of crossbite with functional shift in primary dentition using intraoral removable appliance resulting in improvement of intermaxillary relationship. And I analyzed the positional change and the dimensional change during treatment with lateral cephalometric x-ray analysis. Our patients showed vertical dimensional change of lower anterior facial height and clockwise rotation which results crossbite correction in 1 year of treatment period.

Fracture Strength and Translucency of CAD/CAM Zirconia Crown for Primary Anterior Tooth (CAD/CAM으로 제작한 유전치 지르코니아 전장관의 두께에 따른 파절강도와 반투명도 비교)

  • The purpose of this study is to evaluate the validity of primary anterior zirconia crown made with Computer Aided Design/Computer Aided Manufacturing (CAD/CAM) technology by analyzing fracture strength and translucency parameter. Zirconia crown was designed with CAD software, using 3D scanned data of #61 tooth model. Crown fabrication was performed with CAM machine using zirconia block. Zirconia crowns were divided into 3 groups according to thickness(0.3, 0.5, and 0.7 mm), and fracture strength was compared with 1.0 mm thickness of resin strip crown. The compressive force was applied with universal testing machine at 30° along the incisal edge at increments of 1 mm/min. For translucency evaluation, 0.3, 0.5, and 0.7 mm thickness of zirconia specimens were fabricated and translucency was measured with spectrophotometer. Among zirconia groups, there was a significant increase in fracture strength as thickness increased (p < 0.05). The fracture strength of zirconia crown was significantly higher than resin strip crown in all groups (p < 0.05). Translucency parameter was highest in 0.3 mm group, and significantly decreased as thickness increased to 0.5 and 0.7 mm (p < 0.05). Thin primary anterior zirconia crown can be designed and fabricated according to individual needs by using CAD/CAM. Restoration with thin crown would reduce the amount of tooth reduction, risk of pulp exposure, and make more esthetic restoration possible.

Green Teeth Associated Hyperbilirubinemia in Primary Dentition (유치열에서의 고빌리루빈혈증과 연관된 초록색 변색)

  • There are many reasons for tooth discoloration. An increase in the bilirubin level may cause tooth discolorations. Such cases are rare, but most involve tooth discoloration with a greenish hue. The purpose of this case report is to describe green discoloration of the primary dentition in the presence of neonatal hyperbilirubinemia. 2 boys aged 16 and 22-months presented with chief complaints of erupting teeth of abnormal color. Their primary teeth exhibited a greenish discoloration along enamel hypoplasia. Both patients were born prematurely with a low birth weight and had been diagnosed with neonatal hyperbilirubinemia. Systematic diseases can affect the hard tissue of teeth during their formation and result in changes in tooth color. Periodic follow-ups are required for establishing a normal dental condition and meeting the esthetic needs of patients. A pediatric dentist may be the first person to observe patients with discoloration in their primary dentition. In such cases the dentist can deduce the systematic disease responsible for this discoloration.

  • General anesthesia (GA) for dental care in handicapped patients is necessary to facilitate the provision of safe, efficient, and effective quality treatment. The aims of this study were to determine the anesthetic characteristics of handicapped patients in need of dental treatment in these day care units, and to establish for plan to provide better services. 325 patients who had outpatient general anesthesia from January 2005 to March 2014 were assessed for this study. Patients' distribution and treatment patterns were examined. The proportion of male patients (202, 62.2%) was higher than female patients (123, 37.8%) and the largest group of patients were 5 to 10 years old (85, 26.2%). The reasons for general anesthesia included mental and physical disabilities (207, 63.7%), behavior management (84, 25.8%), parent needs (14, 4.3%), and so on. Restorative treatment was the most common procedure with the average of 4.2 teeth treated per one patient and 43 (13.2%) patients underwent general anesthesia for dental treatment more than once. To expand and improve access to the dental care of the disabled, improvement of the health care system, enhancement of their training for dental care by professionals, and enlarging caregivers'understanding of the importance of oral care in the early stages are required.
