Subjective Awareness and the Quality of Life Related to Oral Health in Industrial Workers (산업장 근로자의 주관적 인식과 구강건강관련 삶의 질)

  • The purpose of this study was to examine the oral symptoms, self-rated systemic health state and oral health status of industrial workers in a bid to provide some information on oral health education geared toward industrial workers. The subjects in this study were 294 workers who got a medical checkup in Korea Industrial Health Association in North Gyeongsang Province. Those who felt less inconveniences in the oral cavity led a better quality of life related to oral health in every area except physical handicaps (p<.001, p<.01). The men's OHIP-14 was higher than the women's, and the younger workers led a better quality of life related to oral health in the area of social separation(p<.01), and the married ones did in the area of physical handicaps, mental disorder and social separation(p<.05). And the workers whose household income was larger lived a better quality of life related to oral health in the areas of Psychological discomfort, physical handicaps, mental disorder, social disorder and social separation(p<.05). Those who found themselves to be generally in good health(p<.001, p<.05) and whose subjective oral health state was led a better quality of life in every area(p<.001, p<.05). The findings of the study showed that there were differences in the quality of life according to awareness of oral symptoms, general health status and oral health state.

GROWTH AND DEVELOPMENT OF ARCH FORM (치열궁의 성장 변화)

  • Study on growth change of dental arch is considered to both an important data in orthodontic diagonsis and treatment planning as well as analysis of treatment results , also, arch form is important in anthropology and dentistry, even more so in prosthodontics and orthodontics. In the field of orthodontics, studies on the functional aspect of upper and lower teeth and maintenance of stability of dentition and occlusion were carried out from the early days. Some of the early studies include explanation of growth change in dental arch from measuring directly fom human stroll, and afterwards, cephalometrics x-rays were introduced; accordingly, studies using cephalometric measurement and linear measurements of study models were often performed. By this method, arch width, arch depth and perimeters were measured, and growth change or dental arch was studied. The subject ror this study were sn children(boys and girls or ages from 3 yens to 12 years from Kang-won district and Seoul, who has no history of orthodontic treatment and who show healthy status and normal growth and development. Cephalometric x-ray, panoramic x-ray, and study model were taken for each subject consecutively for 2 years, and the subjects are still followed up. 400 pairs of study models from the past two years were used in this study; mesio-distal diameater of each tooth, intercanine width, intermolar width, canine depth, molar depth and arch perimeters were measured. Afterwards, mean value and each standard deviation of each age group and each gender were obtained, and representation graph were drawn. The following conclusion were obtained. 1. Intercanine width showed gradual increase until the age of 10-years and after that, showed no increase. 2. Intermolar width in upper arch showed gradual increase : intermolar width in lower arch showed no significant chang, and after the age of 9-years, showed increase. 3. Cainine arch depth showed relatively rapid increase after the age of 6-years, and this pattern was more obvious in lower arch. 4. Molar arch depth increased gradually in both archs and it decrease after the age of 10-years : this phenomenon was more prominent in the lower arch. 5. Arch perimeter showed gradual inerease and convert to plateau at the age of 10-years, after that, it decreased. this pattern was more prominent in lower arch.

An Association between Vitamin Intakes and Established Periodontitis in Korean Adult Population (한국 성인에서 비타민 섭취와 치주염 유병간의 관련성)

  • The aim of this study was to examine whether there is an association between vitamin intakes and established periodontitis in Korean adult population. The 6,245 subjects aged over 19 years old, who participated in health survey, oral examination and nutrition survey were selected for this study from the database of the Fourth Korean National Health and Nutrition Examination Survey. Established periodontitis was defined as ${\geq}$ code 3 in community periodontal index. Vitamin intake was assessed with the food-frequency questionnaire. In analysis, participants were classified by quintile of vitamin intakes. We also considered covariates as socio-demographic characteristics, health-related behaviors including physical activities, systemic diseases and oral health-related behaviors. Multiple logistic regression was performed to assess the crude and adjusted associations. All analyses considered a complex sampling design using SAS 9.2. In crude analysis, less intake of vitamin A, retinol and vitamin B2 significantly increased the risk of periodontitis (vitamin A, odds ratio [OR] Q1=1.00, Q2=0.73, Q3=0.80, Q4=0.77, Q5=0.78; retinol, OR: Q1=1.00, Q2=0.86, Q3=0.73, Q4=0.62, Q5=0.55; vitamin B2, OR: Q1=1.00, Q2=0.70, Q3=0.63, Q4=0.67, Q5=0.68). However, after adjusting for socio-demographics, general and oral health status and behaviors, only vitamin B2 was significantly associated with established periodontitis (OR: Q1=1.00, Q2=0.72, Q3=0.73, Q4=0.76, Q5=0.84). An adequate vitamin B2 intake was significantly associated with a decreased risk of periodontitis. This finding shows that nutrient intake is slightly correlated with periodontitis in Korean adult population. Further studies are needed to understand this association between nutrients intake and periodontitis in more details.

DMFT Index, Periodontal Index and Oral Hygiene Status in Diabetic Patients (당뇨병 환자에서 우식경험영구치지수, 치주질환지수 및 구강위생 상태)

  • Background: This study examined the oral health condition of diabetic patients and evaluated the oral health behavior through a questionnaires survey. In addition, this study provide primary informations for developing a dental health program for diabetic patients. Materials and Methods: There were fifty patients with diabetes mellitus or glucose tolerance (controlled by a community health center at Dalsung-Gun in Daegu City and fifty nondiabetic age, gender and location matched control subjects. Data were collected by a direct oral examination and by questionnaires from 100 subjects in July, 2004. Results: The DMFT (Decay, Missing, Filling Teeth) mean values in diabetics were significantly higher than that in non-diabetics (p<0.05). The decay teeth index and Filling teeth index was similar in the two groups, but the mean Missing teeth index was significantly higher in the diabetics than in non-diabetics. The PI (Periodontal Index) mean values were significantly higher in the diabetics than in the non-diabetics (p<0.01). There was a significant difference between the diabetics and non diabetics aged in their 50s and 60s, but there was no difference with those aged in their 70s. The oral hygiene status was similar in the two groups. The average number of remaining teeth was significantly lower in the diabetics ($16.1{\pm}10.35$) than in the non-diabetics ($20.04{\pm}8.70$) (p<0.05). Conclusion: In the diabetic patients, the M component of the DMFT index was found to be higher than the controls. Diabetics lose their teeth mainly due to periodontal disease, which is supported by the increased PI mean values.

Halitosis and Related Factors among Rural Residents (농촌지역 주민들의 구취실태와 유발요인)

  • This study was conducted through an interview process in which questionnaires were administered to 293 people. The questionnaires related to the behaviors of oral hygiene care, and disease history related to halitosis, and status of halitosis, halitosis measurement, oral examination, and caries activity tests such as the snyder test, Salivary flow rate test, and Salivary buffering capacity test. Our sample was taken from 293 rural residents within the period from 4th to 21st of January 2006. This was done in order to provide basic data to prepare both policies of halitosis prevention and a device to efficiently measure halitosis status and investigate the factors related therein. The major findings of this study results are as follows: 1. As for frequency of tooth brushing, twice a day occupied the greatest portion at 46.1 % Women exceeded men in frequency of tooth brushing. Tongue brushing everyday produced a 25.6 % result among subjects and The use of auxiliary oral hygiene devices occupied 9.2 %. 2. As for degree of usual self-awareness of halitosis: 62.5 %. This result also demonstrate that the severest time of self-awareness in regards to halitosis is wake up time in the morning. The time period produced the highest portion of 72.7 % in times of self-awareness. In terms of the area in which halitosis was observed, gum resulted in 23.0 %. As for types of halitosis, fetid smell was the most frequent at 37.2 %. 3. As for the result of halitosis measurement, values of OG less than 50 ppm occupied 54.3 % and $50{\sim}100ppm$ occupied 41.6 %. As for $NH_3$ values, $20{\sim}60ppm$ showed the highest value range of 52.6 %. 4. As for OG per disease history related to halitosis, values of OG were significantly high in the ranges of $50{\sim}100ppm$ within family history groups of food impaction by dental caries, diabetes mellitus and halitosis. As for values of $NH_3$, there showed a significant difference in respiratory system disease groups. 5 Value range of OG per ordinary halitosis self-awareness degree: values ranging less than 50 ppm were recorded at 55.9 % from the group realizing not aware of smell. 57.5 % from groups only realizing sometimes, while values range of $50{\sim}100ppm$ were recorded at 52.0 % from groups always aware of smell. 63.6 % from groups always strongly aware of smell. Meanwhile as for the values ranges of $NH_3$, $20{\sim}60ppm$. they occupied high portions for all groups of exams. 6. Values of OG per oral examination: the more pulp-exposed teeth and food impaction and the higher the tongue plaque index, values of OG increased within the range of $50{\sim}100ppm$. As for values of $NH_3$, the more prosthetic teeth and the higher the tongue plaque index, this value increased significantly, and the values increased up to no less than 60 ppm for groups of mandibular partial denture. 7. Within the realm of caries activity test: as for the Snyder test, high activity was highest by 43.0 % wherewith the higher the activity of acidogenic bacteria the higher the OG values. As for the salivary flow rate test, the number of cases below 8.0 ml showed the highest tendency by 62.5 %. The larger the salivary flow rate the more decreased OG values distribution. As for the salivary buffering capacity test, $6{\sim}10$ drops of 0.1N lactic acid showed the overwhelming trend by 58.7 % whereby the higher the salivary buffering capacity the greater distribution occupancy ratio of OG values below 50 ppm which is scentless to on ordinary person. 8. As for the correlation between oral environment and halitosis, OG showed the positive correlation with pulp exposed teeth, filled teeth, present teeth, tongue plaque index, and food impaction, while the negative correlation with salivary flow rate and prosthetic teeth. $NH_3$ showed a positive correlation with prosthetic teeth and frequency of tooth brushing, while decayed teeth was negative correlation. 9. As for the multiple regression analysis result, there have been selected female, pulp exposed teeth, prosthetic teeth, food impaction, salivary flow rate, tongue plaque index and severe activities in the Snyder test as factors affecting OG wherein explanatory power on it was 45.1 %. There have been selected females, pulp exposed teeth, tongue plaque index, and prosthetic teeth as factors affecting on $NH_3$ wherein explanatory power on it was 6.6 %. With the aforementioned results in mind, the status of halitosis among rural residents is considered to bare a close relation with oral environments and other factors related to halitosis such as the Snyder test from caries activity test, and salivary flow rate test. For the prevention of halitosis of residents in rural areas, we have to focus on correct tooth brushing methods and tongue brushing, with using auxiliary oral hygiene devices to remove fur of tongue plaque and food impaction. Also, when the cause and ingredients of halitosis are diverse and complex, in order to analyze exactly the factors of individual halitosis development, we need continuous and systematic study in order to provide rural residents with programs of oral hygiene education and encourage the use of dental hygienists in public health centers.

Construction of a reference stature growth curve using spline function and prediction of final stature in Korean (스플라인 함수를 이용한 한국인 키 기준 성장 곡선 구성과 최종 키 예측 연구)

  • Objective: Evaluation of individual growth is important in orthodontics. The aim of this study was to develop a convenient software that can evaluate current growth status and predict further growth. Methods: Stature data of 2 to 20 year-old Koreans (4893 boys and 4987 girls) were extracted from a nationwide data. Age-sex-specific continuous functions describing percentile growth curves were constructed using natural cubic spline function (NCSF). Then, final stature prediction algorithm was developed and its validity was tested using longitudinal series of stature measurements on randomly selected 200 samples. Various accuracy measurements and analyses of errors between observed and predicted stature using NCSF growth curves were performed. Results: NCSF growth curves were shown to be excellent models in describing reference percentile stature growth curie over age. The prediction accuracy compared favorably with previous prediction models, even more accurate. The current prediction models gave more accurate results in girls than boys. Although the prediction accuracy was high, the error pattern of the validation data showed that in most cases, there were a lot of residuals with the same sign, suggestive of autocorrelation among them. Conclusion: More sophisticated growth prediction algorithm is warranted to enhance a more appropriate goodness of model fit for individual growth.

Differences in the Direct Medical Expenditures among Physical Activity Levels of the Community Residents (지역주민의 신체활동 수준에 따른 직접 의료비 지출의 차이)

  • The purpose of this study was to examine the differences in the direct medical expenditure of community residents according to their physical activity level. The quota sampling method was used to select samples from a youth community center, senior center, community cultural center, and community recreational sports center in G city in Gyeongbuk-do. Of the 773 questionnaires distributed, 716 were completed. The questionnaires consisted of the demographic characteristics, regular exercise and physical activity levels, health status and medical costs. While the beneficial effect of regular exercise and physical activity on dental costs was not (significant), its effect on the costs of visiting hospital services was (significant). The mean differences in the total monthly direct medical expenditures on outpatient services and medicine for those with very high, high, moderate, low, and very low levels of physical activity were 7,500, 26,299, 47,517, 9,314, and 9,9978 won, respectively. The result of this study supported the findings of previous studies that regular exercise and physical activity. are not associated with the reduction of medical expenditure.

Cervico-facial Infection Due to Dental Origin: A Retrospective Clinical Study (치성 원인에 의한 경안면 감염에 대한 후향적 연구)

  • Purpose: The objective of this retrospective study is to evaluate the factors affecting the spread of odontogenic infection. Furthermore, this study was performed to apply to future treatments. Methods: A total of 65 patients, who had received treatment for odontogenic infections from 2010 to 2012 for 3 years, were enrolled in this study. The causes of infection, presence of systemic disease, and complications, durations of treatment, treatment methods, and inflammation levels were compared with the data. Results: Patients over 70 years with systemic disease required immediate drainage, systemic antibiotic therapy and hospitalization. We can determine the direction of the early diagnosis and treatment through blood tests (white blood cells, neutrophil, C-reactive protein [CRP]) and computed tomography. Patients over 70 years with systemic disease had the highest percentage. In addition, these patients showed high levels of inflammation index, such as CRP average of 24.8 and needed for a long-term treatment period and a wide range of surgical incision & drainage several times. Systemic diseases, particularly diabetes mellitus and hypertension, accelerate the spread of infection and had a negative effect that delays healing. Eventually, five of the 65 patients showed serious systemic complications. Conclusion: When evaluating cervico-facial infected patients due to odontogenic infection, the most important thing is deciding the appropriate diagnosis and degree of disease. Considering the patient's systemic status and age, we need to decide the treatment plan. Especially, those patients over 70 years with systemic disease should be treated with rapid surgical approach, and the use of a wide range of antibiotics and intensive care. If proper treatment principle does not apply, severe life-threatening complications will result, such as necrotizing fascitis, acute airway obstruction, mediastinitis, and others.

Relationship between Adults' Smoking Realities and Periodontal Disease - 2009 Korea National Health and Nutrition Examination Survey Data - (성인의 흡연과 치주질환의 관련성 - 2009년 국민건강영양조사 자료 -)

  • This study was attempted in order to grasp the influence of smoking upon periodontal disease by analyzing relationship between the smoking realities and the periodontal disease risk in adults aged over 19 in our country. In the findings, the mean in CPI of the whole subjects was 1.92. Among 6989 whole subjects, the group whose CPI is 0-2 included 4707 people(67.3%). The group whose CPI is 3-4 included 2282 people(32.7%). As for the group whose CPI is more than 3 when confounder wasn't corrected, the risk on incidence of periodontal disease in the group with smoking in the past increased by 1.78 times(95% CI:1.56-2.02), compared to the group with no smoking. In the group with the current smoking, it increased by 1.75 times(95% CI:1.55-1.82). Even with correcting gender, age, educational level, and diabetes appearance, the risk on incidence of periodontal disease was higher by 1.28 times(95% CI:1.08-1.53) in the group with smoking in the past and by 1.82 times(95% CI:1.54-2.16) in the group with the current smoking than the group without smoking at all. Accordingly, smoking was indicated to be related to periodontal disease. Thus, the positive publicity needs to allow patients themselves to establish firm consciousness. In addition, dental hygienists need to perform systematic non-smoking educational guidance within doctor's office. Thus, it is thought to necessarily contribute to improving periodontal health.

Knowledge and Satisfaction Status of Tooth Bleaching (치아미백에 대한 지식과 만족도 현황)

  • The purpose of this study was to investigate the knowledge and satisfaction of tooth bleaching. From June 1st, 2017, 328 patients who visited a dental clinic in Ulsan Metropolitan City for two weeks were asked to do a self - filling questionnaire. As a result of the study, the tooth bleaching satisfaction according to tooth bleaching knowledge was not statistically significant (p<0.05), and the highest(8-10) degree of satisfaction was 3.86 when the knowledge of tooth bleaching was high, and the lowest(0-3) was 3.00. The tooth bleaching satisfaction according to general characteristics was significantly different in age and gender. The score of women were 3.74 and higher than men's 3.11 The tooth bleaching experience and tooth bleaching satisfaction were statistically significant (p <0.05) and professional tooth bleaching (4.41) was higher than OCT bleaching (2.64). Also, when the tooth bleaching was done by the invitation of the hospital, it was higher than the case of experiencing tooth bleaching through media advertising. Therefore, the satisfaction of tooth bleaching is required not only for general characteristics but also for tooth tooth bleaching, and it is influenced by tooth bleaching method and tooth bleaching motivation. These factors may be useful to establish a treatment plan to improve the satisfaction of tooth bleaching.
