치과의원과 대학치과병원 교정환자의 통제소재 특성 (Comparing locus of control in orthodontic patients of a university dental hospital with those of a private dental clinic.)

  • 환자가 의료기관을 선택할 때 작용하는 요인은 매우 다양하며 이에 대한 많은 연구들이 있었으나 대부분 외부적인 요인에 대한 연구가 많았고 의료기관 선택 시 환자의 정신-심리학적인 면에 대한 정보 부족한 실정이다. 이에 본 연구에서는 비교적 선택진료(elective treatment)의 측면이 많은 교정치료를 받기 위해 치과의원과 대학치과병원에 내원한 환자들이 의료기관을 선택하는 여러 요인들 중 정신-심리학적 요인을 알아보기 위해 인간의 행동을 예측하는데 유용하다고 알려진 통제소재(Locus of control)검사를 이용하였다. 교정치료를 목적으로 서울대학교 치과병원에 내원한 환자 934명 및 치과의원에 내원한 환자 1466명을 대상으로 내외통제소재검사를 시행한 결과 대학치과병원 교정환자들이 치과의원 교정환자에 비하여 외적통제 경향이 높았으며 이러한 결과에 비추어 대학치과병원 교정환자들이 치과의원 교정환자들에 비해 의료진의 권위에 의존하는 정신-심리학적 경향이 존재하는 것으로 나타났다.

일부 고등학생의 치과치료공포감 및 치과의사불신감 관련 요인 분석 (The Related Factors of Fear of Dental Treatment and Distrust for Dentists in High School Students)

  • The purpose of this study was to investigate the factors related to high school students' fear of dental treatment and distrust for dentists, analyze the relation between these factors and dental health services utilization, and then apply basis data for providing individualized dental health service. The period of the data collection was from November 1st to November 22th 2013, and the subjects were 300 of the first-year high school students in Daugu, and the final analysis data were total 280. using frequency, T-test, correlation and multiple regression analysis, the results of this study were as follows. The higher the distrust for dentist(${\beta}=0.322$) and the burden of dental visit(${\beta}=0.161$), fear of Dental treatment was increased. The larger the fear of Dental treatment and Past experienced pain level, distrust for dentist was increased. therefore, In order to decrease patients' distrust for dentists, good communication between patients and doctors, and efficient treat plans were needed. As a result, patients should have positive attitude on treat. Through early detection and early treatment of oral disease, effective dental health care services with decreasing the fear of dental care system should be provided.

Prognosis after treatment with multiple dental implants under general anesthesia and sedation in a cerebral palsy patient with mental retardation: A case report

  • Cerebral palsy is a non-progressive disorder resulting from central nervous system damage caused by multiple factors. Almost all cerebral palsy patients have a movement disorder that makes dental treatment difficult. Oral hygiene management is difficult and the risks for periodontitis, dental caries and loss of multiple teeth are high. Placement of dental implants for multiple missing teeth in cerebral palsy patients needs multiple rounds of general anesthesia, and the prognosis is poor despite the expense. Therefore, making the decision to perform multiple dental implant treatments on cerebral palsy patients is difficult. A 33-year-old female patient with cerebral palsy and mental retardation was scheduled for multiple implant treatments. She underwent computed tomography (CT) under sedation and the operation of nine dental implants under general anesthesia. Implant-supported fixed prosthesis treatment was completed. During follow-up, she had the anterior incisors extracted and underwent the surgery of 3 additional dental implants, completing the prosthetic treatment. Although oral parafunctions existed due to cerebral palsy, no implant failure was observed 9 years after the first implant surgery.

치과위생사가 근무지에서 경험하는 무례함과 이직의도의 관련성 (The relationship between dental hygienist's exposure to incivility at workplace and their turnover intention)

  • Objectives: This research is to examine the relationship between dental hygienists' exposure to incivility from doctors, bosses, colleagues, patients, their family and visitors, and their turnover intention. Methods: The research was conducted using Nursing Incivility Scale (NIS) and a survey. The survey was conducted from September 10 to October 20, 2015. 240 dental hygienists were conveniently sampled from Forty dental clinics and offices in Seoul, Incheon, Gyeongi-do, and Chungcheog-do. Results: The dental hygienists' exposure to incivility and turnover intention showed correlation (p<0.01), and its sub-factors, - the incivility from colleagues, doctors, patients and visitors - are also correlated with their turnover intention (p<0.01). The strongest factor affecting dental hygienists' turnover intention was incivility from doctors (${\beta}=0.279$, p<0.001), followed by colleagues (${\beta}=0.168$, p=0.029), patients and visitors (${\beta}=0.148$, p=0.026). The modified explanation ability of this model is 24.9%. Conclusions: Dental hygienists' exposure to incivility in workplace is highly correlated to their turnover intention. A program to educate dental hygienists positive mental attitude, communication skill to deal with incivility, and stress reduction method is therefore required in order to reduce their turnover intention.

치위생(학)과 학생의 치매에 대한 지식 및 태도와 교육요구도 (Knowledge, attitudes, and the educational needs on dementia for the students of dental hygiene department)

  • Objectives: The purpose of this study was to contribute to develope dental health intervention standard curriculum for dementia patients by identifying the extent of knowledge, attitudes and educational needs toward dementia in the dental hygiene department students. Methods: The study was conducted in the students of the dental hygiene department in colleges and universities, nationwide (from the first year students to the 3rd or 4th year students) for approximately 2 months from March to April, 2018. Among them, 545 students were selected as the subjects for the final analysis. Results: 91.0% of the subjects were not currently receiving education on dementia. The research on the factors to impact the dementia education needs of the subjects showed that the dementia education necessity (p<0.001) and the dementia attitude (p<0.001) had statistically significant effects on the educational needs on dementia. Conclusions: It is considered that the standard education curriculum for the dental health intervention for dementia patients in the department of dental hygiene should be developed and disseminated. This will provide a basis for the dental hygienists to be equipped with the relevant expertise in the intervention in the dental health of dementia patients in the future.

치과위생사의 노인환자 치과치료 질 관리를 위한 케어, 감염 관리 및 교육에 관한 연구 (A study on care, infection management and education for quality management of dental care of elderly patients by dental hygienists)

  • This study investigated factors affecting care for elderly patients, management of systemic disease, infection management, improvement of medical care quality, and educational requirements, and provide basic data for quality care for elderly patients with systemic diseases. An online survey was conducted among dental hygienists working at dental institutions and data of 172 individuals were analyzed using the SPSS 21.0 program. Descriptive statistics was performed for general characteristics. For factor analysis by general characteristics, t-test and ANOVA were performed. For differences between groups Scheffe verification was performed. A correlation analysis between factors was performed using Pearson's correlation analysis, and a factor analysis affecting the improvement of healthcare quality was performed using multiple linear regression analysis. Care for elderly patients was 52.33/75, management of systemic disease was 12.65/17, and infection management was 96.92/130. Improvement of medical quality (institutional effort) was 22.87/35, and improvement of medical quality (personal effort) was 22.72/35. The most common educational needs were "necessary". Analysis revealed a positive correlation between factors.. Factors affecting improvement of medical quality (personal effort) were age, final academic background, place of work, and the presence or absence of annual medical examinations. The findings of this study indicate that there is a significant correlation between factors. For the qualitative care of elderly patients, education programs related to elderly characteristics, systemic diseases, and infection management should be actively conducted by dental hygienists, and interest and research on geriatric dental hygiene is needed, centering on associations and educational institutions.

A retrospective clinical investigation for the effectiveness of closed reduction on nasal bone fracture

  • Background: The nasal bone is the most protruding bony structure of the facial bones. Nasal bone fracture is the most common facial bone fracture. The high rate of incidence of nasal bone fracture emphasizes the need for systematical investigation of epidemiology, surgical techniques, and complications after surgery. The objective of this study is to investigate the current trends in the treatment of nasal bone fractures and the effectiveness of closed reduction depending on the severity of the nasal bone fracture. Patients and methods: A total of 179 patients with a nasal bone fracture from 2009 to 2017 were enrolled. Their clinical examination, patient's records, and radiographic images of nasal bone fractures were evaluated. Results: Patients ranged from children to elderly. There were 156 (87.2%) males and 23 (12.8%) females. Traffic accident (36.9%) was the most common cause of nasal fracture. Orbit fracture (44 patients, 24.6%) was the most common fracture associated with a nasal bone fracture. Complications after surgery included postoperative deformity in 20 (11.2%) patients, nasal obstruction in 11 (6.1%) patients, and olfactory disturbances in 2 (1.1%) patients and patients with more severe nasal bone fractures had higher rates of these complications. Conclusion: Closed reduction could be performed successfully within 2 weeks after injury.

The role of oral physicians in predicting the risk of obstructive sleep apnea: A case-control study

  • Purpose: Obstructive sleep apnea (OSA) is a common medical disorder with serious complications if untreated. Dentists play a vital role in the early diagnosis of this condition, thereby improving patients' prognoses. The purpose of this study was to identify patients with a high risk of OSA using simple cephalometric measurements in patients receiving routine dental care. Materials and Methods: The present study was conducted on 206 patients divided into a high-risk group and a control group after answering the Berlin questionnaire. Cephalometric analysis of a digital cephalogram was performed to measure the upper airway diameter (UAD) and mandibular-to-hyoid bone distance (MP-H) by 2 observers at 2 different times. Results: Among 206 patients, 93 (45%) were included in the high-risk group and 113 (55%) were in the control group. No significant difference was present between the groups with regard to gender, and the patients ranged in age from 18 to 65 years. The UAD measurements in the high-risk group were significantly lower than in the control group, and the MP-H measurements were significantly higher in the high-risk group than in the control group. The UAD was lower in middle-aged patients in both groups. Conclusion: Our study found that the UAD was lower in individuals with a high risk of OSA. Also, we found that middle-aged individuals of both genders were more likely to develop OSA. Dentists play a vital role in diagnosing patients at a high risk for OSA via thorough clinical examinations, risk factor analyses, and simple cephalometric analyses.

Associations between body dysmorphic disorder (BDD) with the dental health component of the index of orthodontic treatment need (IOTN-DHC) and other BDD risk factors in orthodontic patients: A preliminary study

  • Objective: Body dysmorphic disorder (BDD) is a form of obsessive-compulsive disorder that may be negatively associated with the self-image. It might be associated with orthodontic treatment demand and outcome, and therefore is important. Thus, this study was conducted. Methods: The Yale-Brown Obsessive-Compulsive Scale modified for Body Dysmorphic Disorder (BDD-YBOCS) questionnaire was used in 699 orthodontic patients above 12 years of age (222 males, 477 females), at seven clinics in two cities (2020-2021). BDD diagnosis and severity were calculated based on the first 3 items and all 12 items of the questionnaire. The dental health component of the index of orthodontic treatment need (IOTN-DHC) was assessed by orthodontists. Multivariable and bivariable statistical analyses were performed on ordinal and dichotomized BDD diagnoses to assess potentially associated factors (IOTN-DHC, age, sex, marital status, education level, and previous orthodontic consultation) (α = 0.05). Results: IOTN-DHC scores 1-5 were seen in 13.0%, 39.9%, 29.8%, 12.4%, and 4.9% of patients. Age/sex/marital status/education were not associated with IOTN-DHC (p > 0.05). Based on 3-item questionnaire, 17.02% of patients had BDD (14.02% mild). Based on 12-item questionnaire, 2.86% had BDD. BDD was more prevalent or severer in females, married patients, patients with a previous history of orthodontic consultation, and patients with milder IOTN-DHCs (p < 0.05). Conclusions: IOTNDHC was negatively/slightly associated with BDD in orthodontic patients. Being female and married may increase BDD risk.

연세대학교 치과대학 부속병원에 내원한 신환에 대한 분석 연구 (Study and Analysis of the Outpatients of the Dental Imfirmary, Yonsei Medical Center)

  • To analyze the outpatient of the Dental Infirmary Yonsei Medical Center, anthor ruled out the charts of 1,622 patients which were sampled from 18.921 patients among the selected years 1973.1977 and 1981. The results are as follows : 1. By the age-group the twenties and thirties were about 50% of the total patients. 2. According to the analysis by the district, the distance from residence was most influencial to select the hospital. 3. The frequency of the patient by the service departments, many patients wanted oral sugery tretment, and operative & periodontic tretments followed, And there were some necessities of dental health education for the early care and the protective tretment. 4. Of the season, there were more patients in Summer and Winter, especially in July and August than Spring and Autumn. It might be related with the vacations. 5. The ratio of professor's and general patients, were decreasing compared with the data of three selected years, and the fact might be studied to improve for the better quality of service, even the application of the medical and dental insurance is getting wider.
