Ornithine transcarbamylase (OTC) deficiency is the most common inborn error of urea cycle metabolism; it is inherited in an X-linked manner. The OTC catalyzes the third step of the urea cycle, the conversion of ornithine and carbamyl phosphate to citrulline. Deficiency of OTC leads to the accumulation of ammonia, causing neurological deficits. In most affected hemizygote males, OTC deficiency manifests as hyperammonemic coma that often leads to death in the newborn period, and those who recover from the coma may be neurologically impaired due to the sequelae of the hyperammonemic encephalopathy. In some, late-onset manifestations develop. We report a male neonate with early onset OT deficiency that had apnea and was comatous. On mutation analysis using DNA sequencing after polymerase chain reaction (PCR) amplification of the 10 exons, deletions of 10 bases in codon 285, causing a frame shift was detected in exon 8. The mother and a sister were diagnosed as female carriers. Therefore, genetic counseling and the risk assessment could be provided to the family.
Iron Deficiency Anemia is defined as deficiency Fe in blood. The causal relationships between this and gastrointestinal bleeding, impaired absorption, excessive menstrual flow and other maladies are not well understood. In oriental medicine Iron Deficiency Anemia is generally characterized as deficiency of ki(氣)and blood(血) defined in terms of hypofunction of the spleen and stomach, and insufficiency of the spleen and kindneys. Two Iron Deficiency Anemia patients were treated with Ickibohyoul-tang, Samchulgunbi-tang, Ickibohyoul-tang ga antler, and Sachulgunbi-tang ga antler. Symptoms improved and Blood exams (RBC, Hb, Hct) yielded higher counts. Further research concerning this is necessary.
Objective : To expend the oriental medicine-based strategies or therapeutics for anemia, including iron deficiency anemia. Methods : A 23 year-old man suffering from severe and chronic iron deficiency anemia was believed to have disorder of iron absorption. He had neither specific medical cause nor positive response to western treatments. Blood and biochemical parameters such as levels of hemoglobin, ferritin, transferrin and serum iron were serially chased during treatments. Result : Bojungicki-tang was given to the patient based on diagnosis as a deficiency of spleen qi. The hemoglobin level was normalized along with administration of Bojungicki-tang. Also, the distortions of biochemical indicators (ferritin, transferrin and serum iron) reached a normal range within three months. Conclusion : Bojungicki-tang could be a curing remedy for iron deficiency anemia caused by problems in iron absorption if symptom-differentiation has deficiency of spleen qi.
In order to develop experimental research in oriental medicine, it is necessary to make experimental model of diagnostic pattern(證), On model of the condition of a disease maked in china, there are cold-pattern(寒證), heat-pattern(熱證), deficiency of vital energy-pattern(氣虛證), blood-deficiency-pattern(血虛證), yin-deficiency-pattern(陰虛證), yang-deficiency-pattern(陽虛證), deficiency of both yin and yang-pattern(陰陽俱虛證), yang-exhaustion-pattern(亡陽證), blood stasis-pattern(血瘀證), pattern of defferential diagnosis according to states of viscera(臟腑辨證).
The diagnosis of iron deficiency rests upon the correct evaluation of body iron stores. Morphological interpretation of blood film and the red cell indices are not reliable and often absent in mild iron deficiency. Serum iron levels and iron-binding capacity are more sensitive indices of iron deficiency, but they are often normal in iron depletion and mild iron deficiency anemia. They are also subject to many variables which may introduce substantial errors and influenced by many pathologic and physiologic states. Examination of the bone marrow aspirate for stainable iron has been regarded as one of the most sensitive and reliable diagnostic method for detecting iron deficiency, but this also has limitations. Thus, there is still need for a more practical, but sensitive and reliable substitute as a screening test of iron deficiency. Pollack et al. (1965) observed that the intestinal absorption of cobalt was raised in iron-deficient rats and Valberg et al. (1969) found that cobalt absorption was elevated in patients with iron deficiency. A direct correlation was demonstrated between the amounts of radioiron and radiocobalt absorbed. Unlike iron, excess cobalt was excreted by the kidney, the percentage of radioactivity in the urine being directly related to the percentage absorbed from the gastrointestinal tract. Recently a test based on the urinary excretion of an oral dose of $^{57}Co$ has been proposed as a method for detecting iron deficiency. To assess the diagnostic value of urinary cobalt excretion test cobaltous chloride labelled with $1{\mu}Ci\;of\;^{58}Co$ was given by mouth and the percentage of the test dose excreted in the urine was measured by a gamma counter. The mean 24 hour urinary cobalt excretion in control subjects with normal iron stores was 6.1% ($1.9{\sim}15.2%$). Cobalt excretion was markedly increased in patients with iron deficiency and excreted more than 29% of the dose. In contrast, patients with anemia due to causes other than iron deficiency excreted less than 27%. Hence, 24 hour urinary cobalt excretion of 27% or less in a patient with anemia suggets that the primary cause of the anemia is not iron deficiency. A value greater than 27% in an anemic subject suggests that the anemia is caused by iron deficiency. The cobalt excretion test is a simple, sensitive and accurate method for the assessment of body iron stores. It may be particularly valuable in the epidemiological studies of iron deficiency and repeated evaluations of the body iron stores.
Objective : The interpretation of 'yang-deficiency and yin-prosperity, yang-prosperity and yin-deficiency in cold diseases' from Nangyeong has been various until now. For further understanding Nangyeong, the exact interpretation of this phrase has been required. Methods : Collect the contents that are related this phrase from the commentary of Nangyeong, Sanghannon(傷寒論), and other traditional Chinse and Korean medical books. Based on analyzing them, compare Nangyeong with Sanghannon. Results : The meaning of Yin-Yang in the phrase have 5 viewpoints, except one that this phrase is not correct. If the phrase is interpreted according to Sanghanseorye(傷寒序例), the valid interpretation is that yin-yang has two different meaning in the one phrase. Conclusion : 'Yang-deficiency and yin-prosperity' from Nangyeong goes for the outer symptoms of Taeyangbyeong(太陽病) in cold diseases, yang-prosperity and yin-deficiency from it goes for the inner symptoms of Yangmyeongbyeong(陽明病) in cold diseases.
Structures may need retrofitting as a result of design and calculation errors, lack of proper implementation, post-construction change in use, damages due to accidental loads, corrosion and changes introduced in new editions of construction codes. Retrofitting helps to compensate weakness and increase the service life. Fiber Reinforced Polymer (FRP) is a modern material for retrofitting steel elements. This study aims to investigate the effect of deficiency location on the axial behavior of compressive elements of Circular Hollow Section (CHS) steel short columns. The deficiencies located vertically or horizontally at the middle or bottom of the element. A total of 43 control column and those with deficiencies were investigated in the ABAQUS software. Only 9 of them tested in the laboratory. The results indicated that the deficiencies had a significant effect on the increase in axial deformation, rupture in deficiency zone (local buckling), and decrease in ductility and bearing capacity. The damages of steel columns were responsible for resistance and stiffness drop at deficiency zone. Horizontal deficiency at the middle and vertical deficiency at the bottom of the steel columns were found to be the most critical. These symptoms are due to deficiency of the kidney. The kidney stores vital essence and has a function to keep activities as motive power. If the kidney is abnormal in storing the essence and holding Gi, various symptoms like dizziness, tinnitus, amnesia are caused by deficiency of the kidney. In deficiency of the kidney-nourishing the Eum music therapy, Water-Gi music which can store the essence is mainly used. In case of patients with deficiency of the kidney, dizziness, tinnitus, and amnesia, we can make use of NO.3, BWV 1068, Air of J.S.Bach and Jinyangjo of Geomungo. If there are flaring-up and fidgetiness due to deficiency of vital essence, Earth-Gi music that can help the vigor of spleen and stomach should be used prior to Water-Gi music. This study examined whether or not the iron that is accumulated in the recombinant microbes that produce ferritin is bioavailable to rats with iron deficiency. Rats induced with iron deficiency were treated with iron preparations of $Fe(NH_4)_2(SO_4)_2$, horse spleen ferritin, control yeast, and ferritin-producing recombinant yeast for 14 days. The bioavailability of iron was examined by measuring hemoglobin concentration, hematocrit value, and tissue iron stores. Differences between dietary groups were determined by one-way ANOVA, at the level of significance p<0.05. Based on hemoglobin concentration and hematocrit value, iron in $Fe(NH_4)_2(SO_4)_2$, horse spleen ferritin, and ferritin-producing yeast were bioavailable in rats and cured iron deficiency. The efficacy of ferritin and ferritin-producing yeast was confirmed in establishing tissue iron stores after the induction of iron deficiency. The iron sources of ferritin and the ferritin-producing yeast seemed to be as effective for the recovery from iron deficiency as the iron compounds of ferric citrate and ferrous ammonium sulfate. The results suggest that the iron stored in ferritin of the recombinant yeast is bioavailable, and that the recombinant yeast may contribute widely as a source of iron to resolve the global problem of iron deficiency.
Prognosis in oriental medicine gathers information by four examination methods. It provides important information to understand the degree of deficiency - excessiveness of a patient to treat properly. To generalize the degree of deficiency - excessiveness can be found by seeing the patient's muscle response and pain perception to the palpitations.The theoretical basis to generalize deficiency - excessiveness, oriental and western medical understanding of pain perception and the elasticity of muscle were discussed.The usual symptoms for the excessiveness could include Pain (dislikeness to the palpitation), Stiffness of nape and limbs, Contracture of the limbs, Clonic convulsion and Fast pain. The symptoms for the deficiency could include Pain (likeness to the palpitation, Gastrocnemius muscle spasm, Flaccid paralysis of limbs and Slow pain. More theoretical bases for generalization of deficiency - excessiveness are needed along with the simplifying the complex clinical symptoms. In this way, we can discuss about deficiency - excessiveness with the regard to western medicine to help its generalization.
