AVAILABILITY OF RADIAL FOREARM FREE FLAP IN RECONSTRUCTION OF INTRAORAL SOFT TISSUE DEFECTS : REVIEW OF 50 CASES (구강내 연조직 결손부 재건시 유리 전완 피판(Radial Forearm Free Flap)의 유용성)

  • Purpose: The purpose of this clinical retrospective study was to evaluate our experience of radial forearm free flap for reconstruction of the oral cavity. Material and methods: From 1997 to 2006, 50 radial forearm free flaps were employed for head and neck reconstruction in 50 patients at department of oral and maxillofacial surgery, Yonsei University, Korea. Data were obtained from chart review, and clinical follow-up. Results: Patients' age ranged from 26 to 82 years (mean 53.2). There were 39 men and 11 women. 43 of the 50(86%) patients had squamous cell carcinoma. The total flap survival were 47(94%), complication rate were revealed for 15(30%). Conclusion: In the reconstruction of soft tissue within the oral cavity, several free flaps have been used. Because of its constant anatomy, long pedicle allows a hypothetic vascular anastomosis in the contralateral neck, contourability for various type of oral defects, pliability and can be used simultaneous reconstruction in intraoral and extra oral defects, the radial forearm free flap constitutes one of the best choice of intraoral soft tissue reconstruction.

  • To understand the current status of neonatal surgery in Korea, a survey was made among the 27 members of Korean Association of Pediatric Surgeons. Response rate among surgeons was 78 percent, eighteen hospitals participated in this study. Five hundred and three cases of neonatal surgical patients were analyzed. In Korea, about 50% of cases were treated at the hospital in the capital city area. Regional number of patients were closely related to the regional population. Imperforate anus(19%), atresia/stenosis of gut(12%), and Hirschsprung's diseases(12%) were sitting at the top in the list. Majority of operation was done within the first week of life, especially during the first 24 hour period. Eighty per cent was major or so called index cases. Mortality in so-called index cases was 17%. High mortality was observed in patients with diaphragmatic hernia(47%), gastrointestinal perforation(65%) and esophageal atresia(28%). Low birth weight babies showed higher mortality in gastrointestinal perforation, esophageal atresia and abdominal wall defect. These were compared to 1993 survey of Japanese Society of Pediatric Surgeons.

Artificial Dermis Composed of Gelatin, Hyaluronic Acid and (1\longrightarrow3),(1\longrightarrow6)-$\beta$-Glucan

  • Porous scaffolds composed of gelatin and polysaccharides such as hyaluronic acid and $\beta$-glucan were prepared by using the freeze-drying method after cross-linking with l-ethyl-(3-3-dimethylaminopropyl) carbodiimide hydrochloride (EDC). The scaffold had an inter-connected pore structure with the sufficient pore size for use as a support for the growth of fibroblasts. Results for the contact angle and cell attachment confirmed that high gelatin content in a mixture was suitable for cellular attachment and distribution in two- or three-dimensional fibroblast cultures. However, the addition of polysaccharides aroused the synergistic effects of morphologic and mechanical property of gelatin-based scaffolds. To prepare the artificial dermis for the wound dressing to mimic the normal human dermal skin, fibroblasts were isolated from a child's foreskin, and cultured in gelatin-based scaffolds. An in vivo study showed that the artificial dermis containing the fibroblasts enhanced the wound healing rate and re-epithelialization of a full-thickness skin defect rather than the acellular scaffold after one week.

  • The vehicle navigation system is developed for helping driver to retrieve driving information more easily and fastly. Navigation System informs driver many pieces of driving information - roadway structure and system, on-line traffic condition, the position of vehicle, route guidance, destination and other information service. As the style of information is diverse and the amount of information is large, driver may have mental and visual overload. The display of information can disturb the driver's attention and this can cause accidents. This state is caused by the defect of human-machine interactions. When the navigation system is designed, human factors - cognitive, judgment, operating - must be considered. The display style must be designed simply and easily, not to be obstacle of human -machine interface. In this study, outside-in view display style and inside-out view display style are compared each other. Two factors are measured. One is cognitive factor-time of cognition on information that is displayed by screen display, cognition error rate. The other is image of screen display - subject's feeling about several styles of display, degree of subject's preference. The prototype of roadway is four kinds - Cross, T-cross, Y-cross and O-cross. Roadway display for test is taken from paper maps. Traffic condition display style, vehicle position display style and route guidance display style are taken from current display style. Traffic condition display style is symbol. Vehicle position display style and route guidance display style are described as color and symbol. The test on screen display is implemented doing given tasks. Then the test is analyzed statistically, The result of test analysis gives the guideline to the designer for the map display of the vehicle navigation system.

Effect of Alloy Elements on Microstructure of Modified Area via Friction Stir Process in Steel Materials (마찰교반공정을 통한 강재의 개질 영역에서의 미세조직에 미치는 합금원소의 영향)

  • In this study, to confirm the effect of alloying elements on the phase transformation and conditions of the friction stir process, we processed two materials, SS400 and SM45C steels, by a friction stir process (FSP) under various conditions. We analyzed the mechanical properties and microstructure of the friction stir processed zone of SS400 and SM45C steels processed under 400RPM - 100mm/min conditions. We detected no macro (tunnel defect) or micro (void, micro crack) defects in the specimens. The grain refinement in the specimens occurred by dynamic recrystallization and stirring. The microstructure at the friction stir processed zone of the SS400 specimen consisted of an ${\alpha}$-phase. On the other hand, the microstructure at the friction stir processed zone of the SM45 specimen consisted of an ${\alpha}$-phase, $Fe_3C$ and martensite due to a high cooling rate and high carbon content. Furthermore, the hardness and impact absorption energy of the friction stir processed zone were higher than those of base metals. The hardness and impact absorption energy of FSPed SM45C were higher than that of FSPed SS400. Our results confirmed the effect of alloying elements on the phase transformation and mechanical properties of the friction stir processed zone.

  • As the society is being developed, the desire about life quality is also growing. Even though the process of waterproof work has very close relation with these life quality, we always focus on management after problem than the precaution of problem. So this process has the highest rate of mengel. We develop LCC analysis model considering the precaution and try to show the necessity of the precaution by comparing and analyzing every alternative proposal during LCC. By studying of the previous work and the result of questionnaire, we deduce the problem of waterproof work in construction area. By discussion with an expert in this process and research, we make solutions about precaution and set the cost items in every alternative proposal about these solutions. We understand the effect of each elements through sensitive analysis of the cost items, and find the economically best suited solution.

Shear capacity of stud shear connectors with initial damage: Experiment, FEM model and theoretical formulation

  • Initial damage to a stud due to corrosion, fatigue, unexpected overloading, a weld defect or other factors could degrade the shear capacity of the stud. Based on typical push-out tests, a FEM model and theoretical formulations were proposed in this study. Six specimens with the same geometric dimensions were tested to investigate the effect of the damage degree and location on the static behavior and shear capacity of stud shear connectors. The test results indicated that a reduction of up to 36.6% and 62.9% of the section area of the shank could result in a dropping rate of 7.9% and 57.2%, respectively, compared to the standard specimen shear capacity. Numerical analysis was performed to simulate the push-out test and validated against test results. A parametrical study was performed to further investigate the damage degree and location on the shear capacity of studs based on the proposed numerical model. It was demonstrated that the shear capacity was not sensitive to the damage degree when the damage section was located at 0.5d, where d is the shank diameter, from the stud root, even if the stud had a significant reduction in area. Finally, a theoretical formula with a reduction factor K was proposed to consider the reduction of the shear capacity due to the presence of initial damage. Calculating K was accomplished in two ways: a linear relationship and a square relationship with the damage degree corresponding to the shear capacity dominated by the section area and the nominal diameter of the damaged stud. This coefficient was applied using Eurocode 4, AASHTO LRFD (2014) and GB50017-2003 (2003) and compared with the test results found in the literature. It was found that the proposed method produced good predictions of the shear capacity of stud shear connectors with initial damage.

The Effects of Levelers on Electrodeposition of Copper in TSV Filling (TSV 필링 공정에서 평활제가 구리 비아필링에 미치는 영향 연구)

  • Defects such as voids or seams are frequently found in TSV via filling process. To achieve defect-free copper via filling, organic additives such as suppressor, accelerator and leveler were necessary in a copper plating bath. However, by-products stemming from the breakdown of these organic additives reduce the lifetime of the devices and plating solutions. In this research, the effects of levelers on copper electrodeposition were investigated without suppressor and accelerator to lower the concentration of additives. Threelevelers(janus green B, methylene violet, diazine black) were investigated to study the effects of levelers on copper deposition. Electrochemical behaviors of these levelers were different in terms of deposition rate. Filling performances were analyzed by cross sectional images and its characteristics were different with variations of levelers.

Survey of Unmet Dental Treatment Needs among Adult Disabled (장애인의 미충족 치과치료 실태조사)

  • This study analyzed the rate of unmet dental care service for the disabled who were judged to require the dentistry due to oral health-related problems and those reasons, and also examined the oral health status according to the unmet dentistry. 375 subjects, who were 19 years or older and completed oral health examination and a survey, among 444 responded positive of disability registration after participation in National Health and Nutrition Examination Survey(NHANES), were analyzed. Disability type was classified into 6 kinds(physical disability, brain lesions, sensory defect, developmental disability, mental disorder, endocrine disorder). Participants had 128 of their dental care needs unmet when it was required. 'Financial burden' was the main reason, with the subject of 49.2%, followed by 'lower on the priority scale' were 18.0%. The high frequency and progression permanent teeth caries experiencs and periodontal status of their dental disease were confirmed. In availability aspect of dentistry resources, it is required for using dentistry service smoothly through providing sufficient proper dental care service comparing with the number of the disabled.

Clavicle Nonunion: Matchstick Bone-grafting and Osteosynthesis

  • Background: This study was conducted to evaluate the surgical outcomes of plate fixation using autologous iliac bone graft shaped in the form of a matchstick in clavicle nonunion resulting from prior surgical or non-surgical management. Methods: From May 2005 to February 2013, 17 patients underwent surgery for clavicle nonunion. The mean age at the time of surgery was 48.8 years. The iliac bone was harvested and shaped into slivers approximately 3 cm long, which resembled matchsticks. After opening of the medullary canal, the plate and screws were applied and cancellous bone was placed at the nonunion site. Matchstick bone grafts which were longitudinally placed and encircled on the nonunion site were tied with periosteum using 3-4 stitches. Union was determined via postoperative plane X-ray. Clinical status was evaluated using the visual analogue scale score for pain, and Constant and American Shoulder Elbow Surgeon score. Results: All patients had a stable radiological union at the follow-up. The mean duration from index operation to nonunion operation was 13.2 months (7-32 months). The mean follow-up period was 20.1 months (8-56 months), and the mean duration until union was 11.2 weeks (8-16 weeks). All clinical scores were improved at the final follow-up (p<0.001). The mean segmental defect was $3.3{\pm}2.6mm$ (1-18 mm); and the difference in clavicular length between operative and non-operative site was $5.9{\pm}6.9mm$. Conclusions: Matchstick shaped autologous iliac bone grafting technique in clavicle nonunion is acceptable with a high union rate.
