외상 및 외상진료체계의 인식도 조사 (Survey of Recognition of Trauma and Trauma Care System)

  • Purpose: Trauma is one of the most common and lethal causes of death in Korea, especially in people under the age of 40. However, a considerable percentage of trauma patients are lost each year due to the scarce resources of the trauma system. The purpose of this study was to determine the recognition of trauma and trauma system. Methods: From April 8th to 22nd, 2014, visitors and in-patients in our medical center were interviewed and surveyed with a questionnaire, which included 28 questions regarding the trauma system, such as the most common cause of death, the locations of trauma centers, the importance of trauma centers, and consent for supporting trauma centers financially. Results: The majority of the respondents recognized trauma as a common cause of death; this was particularly true for people younger than 40. Most respondents' expectancy for the optimal time for trauma patient transport was high, recognizing that major trauma patients should receive urgent care. The respondents felt that trauma centers are important and needed, just as much as police stations and libraries are. Among 178 respondents, 140 (80.5%) were willing to financially support the trauma system. Conclusion: The respondents were aware of the seriousness of trauma and generally agreed on the need for trauma centers. In order to meet the needs and the demands of the people, and to reduce preventable death rate, the trauma system should be improved not only in quality but also in quantity with better and more facilities and manpower, with the aid of publicity from trauma organizations and funding from the government.

우리나라 폭염 인명피해 발생특징 (Characteristics of Heat wave Mortality in Korea)

  • Analysis of the cause of death (Statistics Korea) showed that heat wave caused 442 deaths (21 per a year) from 1991 to 2011 in Korea. The number of summer heat-related deaths (heat disorders) increased exponentially as heat wave prolonged. In 1994 when the extreme heat wave prevailed, there were 92 heat-related deaths. Seasonally, heat-related deaths occurred most frequently in early August when air temperature is highest for a year, but it is frequent as well in late July when air temperature increases rapidly after withdrawal of Changma. The frequency of deaths by age has begun to increase from 40s and more than half of total deaths were occurred in the elderly (${\geq}60$). Except retired elderly, the most vulnerable group (job and age) was the elderly agricultural workers and the next was the jobless people in 40 s~50 s, assumed as homeless people. The most vulnerable regions were Hapcheon-gun, Uiryong-gun, and Sanchen-gun, which are inland rural area in Gyeongsang-do where the heat wave occurs most frequently in Korea. The heat-related death rate increased rapidly when air temperature exceeded $33^{\circ}C$ in both of the urban and rural area. Interestingly, the heat-related death were observed in the relatively lower temperature in the cities ($29^{\circ}C$), as compare to the rural area ($31^{\circ}C$).

CHO 세포에서 누에 혈림프 유래 Storage-protein 2의 세포응집 및 세포사멸 억제 효과 (Effects of Storage-protein 2 Derived from Silkworm Hemolymph on Reduction of Aggregation and Cell Death in CHO Cells)

  • Chinese hamster ovary (CHO) cells have been widely used for production of various recombinant proteins such as cytokines and monoclonal antibodies. The cell aggregation and cell death in CHO cell culture directly affect cell viability, and productivity and quality of products. In this study, we investigated preventing effects of storage-protein 2 (SP2) derived from silkworm hemolymph on cell aggregation and cell death in CHO cell culture producing albuminerythropoietin (Alb-EPO). The viable cell density in the culture supplemented with 2 mg/mL SP2 was 1.71-fold higher than that in control culture. Increased titer of Alb-EPO was also found in the culture with SP2. Morphology of CHO cells in SP2 supplemented cultures did not differ from that of control. In addition, the cell aggregation rate of the SP2 cultures was reduced 20% compared to the control. Finally, we confirmed that the apoptosis was strongly suppressed by addition of SP2 in the cultures. These results clearly demonstrate that SP2 can be served as an effective supplement for enhancing titer of Alb-EPO via reducing cell aggregation and cell death.

고혈압 환자에서 한방의료기관 이용과 심혈관 위험 요소와의 관계: 국민건강보험공단 표본코호트 DB (Association between Korean Medicine Hospital Utilization and Cardiovascular Risks in Patients with Hypertension: a National Korean Cohort Study)

  • Objectives: This study aims to investigate the effects of Korean Medicine Hospital Utilization (KMHU) on major adverse cardiovascular events (MACE), myocardial infarction (MI), stroke, and death in hypertensive patients taking antihypertensives. Methods: Using the Korean National Health Insurance Service-National Sample Cohort database, this study identified and diagnosed 68,457 hypertensive patients taking antihypertensives between 2003 and 2006. They were divided into KMHU and non-KMHU groups. The follow-up period ended with the diagnosis of myocardial infarction, stroke, or death. After propensity score matching (PSM), there were 18,242 patients each in the non-KMHU and KMHU groups. We calculated the incidence rate, hazard ratio (HR), and 95% confidence interval (CI) for MACE, myocardial infarction, stroke, and death in patients with hypertension using a stratified Cox proportional hazard model. In addition, secondary outcome analyses for stroke and cardiovascular mortality were performed. Results: After PSM, the HRs for MACE (HR: 0.84, 95% CI: 0.81-0.87), all-cause mortality (HR: 0.75, 95% CI: 0.72-0.79), and myocardial infarction (HR: 0.90, 95% CI: 0.83-0.97) were significantly lower in the KMHU group than in the non-KMHU group. Moreover, the HRs for stroke-related mortality, haemorrhage and ischaemic stroke-related mortality, and ischaemic heart disease-related and circulatory system disease-related mortality were significantly lower in the KMHU group than in the non-KMHU group. Conclusions: On long-term follow-up observation, this study supported the effect of KMHU for managing hypertension and reducing the burden of cardiovascular diseases.

과산화수소로 유도된 SH-SY5Y 신경세포 사멸에 대한 오미자·칠해목 추출혼합물의 보호효과 (Neuroprotective Effects of Schisandra chinensis and Ribes fasciculatum Extract on Hydrogen Peroxide-Mediated Oxidative Stress in Neuroblastic SH-SY5Y Cell Line)

  • In neuronal cell deaths, oxidative stress is normally implicated with a most of these deaths occurring in neurodegenerative disorders such as the Alzheimer's and Parkinson's diseases. In this study, the neuroprotective effects of Schisandra chinensis (SC) and Ribes fasciculatum (RF) extracts on hydrogen peroxide ($H_2O_2$)-induced oxidative stress in neuroblastic cell line were investigated. For an hour, hydrogen peroxide of $100{\mu}M$ concentration, was induced on neuroblastic cells, causing apoptic cell death. For the neuroprotection, a sample of neuroblastic cells had been pre-treated with SC and RF extracts for 24 hours before application of the hydrogen peroxide. No neurotoxic effects were observed in the cells that had been treated by SC and RF. This prove that the treatment of SC and RF extract prevented apoptotic cell death of neuroblastic cell line exposed to oxidative injury. In addition, applying both SC and RF extracts at a 7:3 ratio increased the neuronal cell survival rate, compared to individual treatments of SC and RF extract. This study suggests that SC and RF extracts may be potential therapeutic agents for the prevention of neuronal cell death.

Inequalities in External-Cause Mortality in 2018 across Industries in Republic of Korea

  • Background: External-cause mortality is an important public health issue worldwide. Considering its significance to workers' health and inequalities across industries, we aimed to describe the state of external-cause mortality and investigate its difference by industry in Republic of Korea based on data for 2018. Methods: Data obtained from the Statistics Korea and Korean Employment Information System were used. External causes of death were divided into three categories (suicide, transport accident, and others), and death occurred during employment period or within 90 days after unemployment was regarded as workers' death. We calculated age- and sex-standardized mortalities per 100,000, standardized mortality ratios (SMRs) compared to the general population and total workers, and mortality rate ratios (RRs) across industries using information and communication as a reference. Correlation analyses between income, education, and mortality were conducted. Results: Age- and sex-standardized external-cause mortality per 100,000 in all workers was 29.4 (suicide: 16.2, transport accident: 6.6, others: 6.6). Compared to the general population, all external-cause and suicide SMRs were significantly lower; however, there was no significant difference in transport accidents. When compared to total workers, wholesale, transportation, and business facilities management showed higher SMR for suicide, and agriculture, forestry, and fishing, mining and quarrying, construction, transportation and storage, and public administration and defense showed higher SMR for transport accidents. A moderate to strong negative correlation was observed between education level and mortality (both age- and sex-standardized mortality rates and SMR compared to the general population). Conclusion: Inequalities in external-cause mortalities from suicide, transport accidents, and other causes were found. For reducing the differences, improved policies are needed for industries with higher mortalities.

NEWTON'S METHOD FOR SOLVING A QUADRATIC MATRIX EQUATION WITH SPECIAL COEFFICIENT MATRICES

  • We consider the iterative solution of a quadratic matrix equation with special coefficient matrices which arises in the quasibirth and death problem. In this paper, we show that the elementwise minimal positive solvent of the quadratic matrix equations can be obtained using Newton's method if there exists a positive solvent and the convergence rate of the Newton iteration is quadratic if the Fr$\acute{e}$chet derivative at the elementwise minimal positive solvent is nonsingular. Although the Fr$\acute{e}$chet derivative is singular, the convergence rate is at least linear. Numerical experiments of the convergence rate are given.

구매종속성이 존재하는 상황에서 주문충족율을 계산하는 근사법에 관한 연구 (Approximate Approach to Calculating the Order Fill Rate under Purchase Dependence)

  • This paper proposes a new approximate approach to calculate the order fill rate and the probability of filling an entire customer order immediately from the shelf in a business environment under purchase dependence characterized by customer purchase patterns observed in such areas as marketing, manufacturing systems, and distribution systems. The new approximate approach divides customer orders into item orders and calculates fill rates of all order types to approximate the order fill rate. We develop a greed iterative search algorithm (GISA) based on the Gauss-Seidel method to avoid dimensionality and prevent the solution divergence for larger instances. Through the computational analysis that compares the GISA with the simulation, we demonstrate that the GISA is a dependable algorithm for deriving the stationary joint distribution of on-hand inventories in the type-K pure system. We also present some managerial insights.

개발도상국의 인구변천 유형과 특징 (The Pattern and Characteristics of Demographic Transition in Developing Countries)

  • 본 연구는 개발도상국의 지역별 인구변천 유형과 특징을 검토하는데 그 목적이 있다. 이를 위해 본 연구는 개발도상국의 인구변천 유형을 인구변천 단계와 관련시키고 있다. 그 다음으로 출산력과 사망력이 지난 40년간 어떻게 변화되어 왔는지를 다양한 지표를 활용하여 검토하고 있다. 분석결과 출산력의 경우 아프리카 지역은 최근까지도 비교적 높은 출산율을 보여 인구변천의 제2단계에 돌입한 것으로 보인다. 이 지역은 또한 알제리, 리비아, 모로코 등 아프리카 북부지역의 국가와 나머지 국가들이 인구변천 과정에서 뚜렷한 차이를 보인다. 서남아시아의 국가들 역시 인구변천의 제2단계에 있다고 볼 수 있다. 서남아시아 국가 증 아프카니스탄과 예맨은 지난 40년 간 출산력 수준의 변화는 거의 없고 사망력 수준만 약간 감소하는 전형적인 아프리카 형태를 보이고 있다. 이에 반해 동아시아 지역은 출산력의 감소 속도가 사망률의 감소 속도보다 훨씬 빠르게 나타나서 인구의 증가폭이 둔화되는 양상을 보이는 인구변천의 제3단계에 속한다고 볼 수 있다. 중남미 지역 국가들도 동남아시아 국가들의 경우와 비슷하게 출산력의 감소 속도가 사망력의 감소 속도보다 빠르게 나타나 인구의 증가 폭이 둔화되는 인구변천의 제3단계에 속한다. 특히 동남아시아의 한국과 싱가포르는 매우 빠른 속도로 출산력 감소를 보였으며 최근에는 오히려 저출산이 문제가 될 정도로 낮은 출산력 수준을 보이고 있다. 사망력의 경우 특히 평균수명은 지역별로 큰 차이를 보인다. 평균수명이 가장 높은 국가 중의 하나인 싱가포르가 78.0세인데 반해 르완다는 39.9세에 머무르고 있다. 아프리카의 평균수명이 낮은 것은 여러 가지 설명이 가능하나 최근에 크게 확산되고 있는 에이즈의 영향이 가장 클 것으로 판단된다.

한산만에서의 굴(Crassostrea gigas) 부유유생의 출현과 생존율 (Occurrence and Survival Rate of the Larvae of Pacific Oyster Crassostrea gigas in Hansan Bay)

  • 우리나라 굴 양식의 중심해역인 한산만에서 효율적인 채묘예보를 기하기 위한 방안으로써, 이 수역에 발생한 굴 부유유생을 대상으로 하여 각 유생의 발육단계별 출현상황과 생존율을 조사하였다. 1984년 6월 13일 부터 10월 16일까지의 조사기간 중 크기가 약 $72{\times}61{\mu}m$의 D 형 유생은 7월 16일과 9월 20일에 각각 출현 Peak가 있었으며, $189{\times}217{\mu}m$의 umbo 형 유생은 7월 21일과 9월 25일에, $303{\times}308{\mu}m$의 성숙유생은 7월 28일과 9월 30일에 각각 Peak가 있었다. D형유생에서 umbo 형 유생으로 되는데는 약 5일이 걸렸으며, 순간사망률은 0.24, 일간생존율은 0.78, 일간사망률은 0.22, 평균생존율은 약 $29.38\%$였다. umbo형유생에서 성숙유생으로 되는데는 $5{\sim}7$일 소요되었으며, 순간사망률은 0.15, 일간생존율은 0.86, 일간사망률은 0.14, 평균생존율은 약 $38.54\%$였다. D형유생의 발생에서 채묘의 대상이 되는 성숙유생으로 되는데는 $10{\sim}12$일 걸렀으며, 이 기간중의 평균생존율은 약 $11.32\%$였다.
