동위원소분석을 이용한 질산염의 오염원 추적에 대한 고찰 (A Review on Nitrate Source Identification using Isotope Analysis)

  • Nitrate ($NO_3^-$), a common surface water and groundwater pollutant, poses a serious environmental problem in regions with intensive agricultural activities and dense population. It is thus important to identify the source of nitrate contamination to better manage water quality. Due to the distinct isotope compositions of nitrate among different origins, the dual isotope analysis (${\delta}^{15}N$ and ${\delta}^{18}O$) of nitrate has been widely applied to track contamination sources. This paper provided the underlying backgrounds in the isotope analysis of nitrate, which included typical ranges of ${\delta}^{15}N$ and ${\delta}^{18}O$ from various nitrate sources, isotope fractionation, the analytical methods used to concentrate nitrate from samples, and the potential limitations of the dual isotope analysis along with the resolutions. To enhance the applicability of the dual isotope analysis as well as increase the ability to interpret field data, this paper also introduced several case studies. Furthermore, other environmental tracers including ${\delta}^{11}B$ and $Cl^-/Br^-$ ratios were discussed to accompany the dual isotope analysis for better assignment of contamination sources even when microbial transformation of nitrate and/or mixing between contaminant plumes occur.

중환자실의 간호조직문화와 팀효과성에 관한 연구 (A Study on the Relationship Between Nursing Organizational Culture and ICUs Team Effectiveness)

  • Purpose: The purpose of this research is, by investigating organizational characteristics, types of nursing organizational culture and team effectiveness in ICU, to ascertain the type of nursing organizational culture and the organizational characteristic that can improve the team effectiveness. Method: The research targeted 427 nurses from 33 ICUs of 14 general hospitals which have more than 250 beds and the data were gathered by using self-report questionnaires from April 10, 2003 to April 24, 2003. For this research, the following tools were used; the tool for measuring organizational characteristics and organizational cultures and the tool for measuring team effectiveness. Result: The most significant nursing organizational characteristic in ICU is the centralization. The organizational culture in ICU is generally rank-oriented culture. There was a significant difference (p<.01) in four types of organizational cultures; relation-oriented, innovation-oriented, rank-oriented and task-oriented. Verifying influence power of organizational cultures upon team effectiveness of ICU, relation-oriented culture had 49.2% of an influence upon team effectiveness, innovation- oriented and relation-oriented culture had 60.4% of an influence, and rank-oriented, innovation-oriented and relation-oriented culture had 61.2% of an influence. The organizational culture profiles according to the types of nursing organizational cultures in 33 ICUs were found by a cluster analysis. They were classified into five culture profiles; strong balance culture profile, weak balance culture profile, innovation-oriented and task-oriened culture profile, strong relation culture profile and strong rank culture profile(p<0.5). According to me organizational culture profiles, a significant difference of team effectivenesses(coworker satisfaction, team performance perception, team satisfaction and team commitment) was found(p<.01). The strong balance culture profile had the best team effectivenesses. Conclusion: For nursing culture management, a nursing administrator should identify the relevant nursing organizational culture at first by utilizing an innovative team-leader. After identifying the organizational culture, the administrator should make strategic plans and practices that can distinguish good organizational cultures to be expanded from ones to be sublated so that a strong balance culture can be developed.

  • Protected vegetable production area is greater than 26% of the total vegetable production area in Korea, and portion of protected production area is increasing for flowers and fruits. To secure stable productivity and profitability, continuous and intensive monitoring and control of protected crop production environment is critical, which is labor- and time-consuming. Failure to maintain proper environmental conditions (e.g., light, temperature, humidity) leads to significant damage to crop growth and quality, therefore farmers should visit or be present close to the production area. To overcome these problems, application of remote monitoring and control of crop production environment has been increasing. Wireless monitoring and control systems have used CDMA, internet, and smart phone communications. Levels of technology adoption are different for farmers' needs for their cropping systems. In this paper, potential of wireless remote monitoring of protected agricultural environment using CDMA SMS text messages was reported. Monitoring variables were outside weather (precipitation, wind direction and velocity, temperature, and humidity), inside ambient condition (temperature, humidity, $CO_2$ level, and light intensity), irrigation status (irrigation flow rate and pressure), and soil condition (volumetric water content and matric potential). Scenarios and data formats for environment monitoring were devised, tested, and compared. Results of this study would provide useful information for adoption of wireless remote monitoring techniques by farmers.

Prediction of pharmacokinetics and drug-drug interaction potential using physiologically based pharmacokinetic (PBPK) modeling approach: A case study of caffeine and ciprofloxacin

  • Over the last decade, physiologically based pharmacokinetics (PBPK) application has been extended significantly not only to predicting preclinical/human PK but also to evaluating the drug-drug interaction (DDI) liability at the drug discovery or development stage. Herein, we describe a case study to illustrate the use of PBPK approach in predicting human PK as well as DDI using in silico, in vivo and in vitro derived parameters. This case was composed of five steps such as: simulation, verification, understanding of parameter sensitivity, optimization of the parameter and final evaluation. Caffeine and ciprofloxacin were used as tool compounds to demonstrate the "fit for purpose" application of PBPK modeling and simulation for this study. Compared to caffeine, the PBPK modeling for ciprofloxacin was challenging due to several factors including solubility, permeability, clearance and tissue distribution etc. Therefore, intensive parameter sensitivity analysis (PSA) was conducted to optimize the PBPK model for ciprofloxacin. Overall, the increase in $C_{max}$ of caffeine by ciprofloxacin was not significant. However, the increase in AUC was observed and was proportional to the administered dose of ciprofloxacin. The predicted DDI and PK results were comparable to observed clinical data published in the literatures. This approach would be helpful in identifying potential key factors that could lead to significant impact on PBPK modeling and simulation for challenging compounds.

결혼이주여성의 인적자본투자 경험에 관한 연구 (Married Female Immigrants' Experience of Investing in Human Capital)

  • The purpose of this study was to figure out married female immigrants' experience and process of investing in human capital in Korea. The participants were 11 women from multicultural families who had experience of investing in human capital in Korea, could communicate in the Korean language, and were living in Daegu. The data were collected using intensive interviews. The major findings were as follows: The participants' level of education and work experience in native country were not accepted as human capital and offered no advantages or obtaining employment in Korea, while English language skills were useful for those pursuing an English lecturer position. The motives for investing in human capital were 'adjusting to early stage of marriage and a new environments', 'for their childrens' education', 'to improve female immigrants' level of education', 'seeking a better job', and 'advocating more education'. The types of investing in human capital were 'learning Korean language and culture', 'applying to Open University or taking a middle or high school qualification examination', 'obtaining certification', and 'entering regular university'. The participants' plans for investing in the future were 'finding their own job', 'entering regular university', and 'participating in useful educational programs including Korean language skills for daily life' The results showed that the participants' motives for investing in human capital were related in the stage of their marriage. The phases and changes in their motives during a marriage proved that the participants continued to invest in human capital according to their needs and situation.

도뇨환자의 유치기간별 요로감염상태에 관한 연구 (Urinary Tract Infection Related to The Indwelling Cathete with Closed Drainage)

  • This study attempted to identify important problems of the factors related to the indwelling catheter with closed drainage and to enhance recognization of Urinary Tract Infection management to protect patients with indwelling catheter from UTI. We selected the 58 patients with indwelling catheter with closed drainage from md-surg. intensive care units, and general neurological and neurosurgical ward, in one hospital in Pusan, Korea from May 30 to October 15, 1993. Patients were examined by urinary analysis before inserting catheter and then by urine cultures three times. The obtained data was analized using an SPSS /PC$^{+}$ The statistical analyses employed here was the frequency, percentage and x$^2$-tests. 1. Analyses for Indwelling Catheter induced UTI and infection rates were found to be presented in patients related to the factors of being on antibiotic drugs (x$^2$=14.105, p=.000) and the duration of the indwelling catheter(x$^2$=58.0, p=.000), both of which were found to affect the UTI. 2. UTI rates according to the duration of the in-dwelling catheter were high 8.6% for the 2nd day, 27.6 for the 4th day, and 36.2% for the 7th day, and as the duration for the indwelling catheter was longer, UTI rates were higher. 3. The UTI rates according to the duration of the indwelling catheter related re factors, on the 7th day, were high-52.0% for women, 37.0% for more than 50 years in ages, 30.0% for patients under sur-gery, 33.3% for altered mental states outsides the alert, 76.7% for patients not given antibiotic drugs, 36.4% for less than 3500cc of fluids intakes, it was found that these affected to increase the UTI. 4. Causative organisms of UTI were found ; E.Coli appears the most than all other isolated organisms in women and Klebsiella and Pseudomonas in men. For antibiotic susceptibility of causative organisms, all causative organisms were found to be susceptible to Sul -cefoperazone, Trim-prim, and unasyn and Pseudomonas mostly net susceptible.e.

간호단위의 팀 효과성에 대한 고찰 (A Literature Review of Team Effectiveness on Hospital Nursing Units)

  • Purpose: The purpose of this study was to analyze literature related to nursing team effectiveness and to summarize the definition variables included, measurement tools, and findings. Basic data on operation and research for team effectiveness in nursing units was sought. Methods: A literature search was performed to identify all studies published between 2003 and 2016 from periodicals indexed in PUBMED, CINAHL, SCIENCE DIRECT, RISS, KISS, and NAL databases. The following keywords were used in the search: 'team effectiveness', 'nurse', 'nursing', and 'hospital'. Ten studies were analyzed. Results: The analysis included domestic and international literature on nursing team effectiveness. The foreign literature included studies of various organizations including nurses, use of various research tools, leadership programs and/or new nursing delivery systems. In the Korean studies, most of the research on team effectiveness surveyed nurses on team satisfaction, team commitment, and team performance in general nursing units, operating rooms, and intensive care units. Conclusion: The findings show the necessity to develop a definition of team effectiveness that can be accepted comprehensively in nursing organizations in Korea. The definition should reflect team effectiveness that includes all cooperating units not only nurses of the nursing unit but also all other related health care teams.

개두술을 받은 환자의 수술부위감염 관련요인 분석 (Risk Factors for Surgical Site Infections in Patients Undergoing Craniotomy)

  • Purpose: The objectives of this study were to determine the prevalence, incidence, and risk factors for postoperative surgical site infections (SSIs) after craniotomy. Methods: This study was a retrospective case-control study of 103 patients who had craniotomies between March 2007 and December 2008. A retrospective review of prospectively collected databases of consecutive patients who underwent craniotomy was done. SSIs were defined by using the Centers for Disease Control criteria. Twenty-six cases (infection) and 77 controls (no infection) were matched for age, gender and time of surgery. Descriptive analysis, t-test, $\chi^2$-test and logistic regression analyses were used for data analysis. Results: The statistical difference between cases and controls was significant for hospital length of stay (>14 days), intensive care unit stay more than 15 days, Glasgrow Coma Scale (GCS) score (${\leq}7$ days), extra-ventricular drainage and coexistent infection. Risk factors were identified by logistic regression and included hospital length of stay of more than 14 days (odds ratio [OR]=23.39, 95% confidence interval [CI]=2.53-216.11) and GCS score (${\leq}7$ scores) (OR=4.71, 95% CI=1.64-13.50). Conclusion: The results of this study show that patients are at high risk for infection when they have a low level of consciousness or their length hospital stay is long term. Nurses have to take an active and continuous approach to infection control to help with patients having these risk factors.

자가보고 능력이 없는 중환자의 통증사정을 위한 한국어판 Critical Care Pain Observation Tool의 신뢰도와 타당도 검증 (Reliability and Validity of a Korean Version of the Critical Care Pain Observation Tool (CPOT) for ICU Patients Unable to Self Report)

  • Purpose: The purpose of this study was to examine the reliability and validity of a Korean version of the Critical Care Pain Observation Tool (CPOT) developed for assessment of pain in critically ill nonverbal patients. Methods: Data were collected from a convenience sample of 30 critically ill patients admitted to a medical ICU in a hospital. The CPOT was tested at before, during and 20 minutes after changing a position and suction. Upon establishment of content and translation equivalence between the English and Korean version of CPOT. Results: The interrater reliability was found to be acceptable with the kappa coefficients of .76-1. The construct validity of the pain scores were increased from 0.43 to 2.5 in changing a position (t=-8.60, p<.001)and 0.1 to 3.23 (t=-9.36, p<.001) in suctioning. The pain scores were decreased from 6.06 to 4.01 in changing a position (t=-10.19, p<.001) and 6.45 to 4.13 (t=-10.39, p<.001) in suctioning. The concurrent validity the correlations between pain scores and physiological indicators, and a increased in Heart rate before and after changing a position (r=.65, p<.001). Conclusion: The CPOT can be used as a reliable and valid measure to assess pain in critically ill nonverbal a medical ICU patients.

아로마 흡입이 밤번근무 간호사의 수면과 피로에 미치는 효과 (The Effects of Aroma Inhalation on Sleep and Fatigue in Night Shift Nurses)

  • Purpose: This study was conducted to describe the effects of aroma inhalation on sleep and fatigue in night shift nurses. Methods: The research design was one group pre-posttest design. The subjects were 17 night shift nurses who work at neurosurgery intensive care unit of K hospital of C university in Seoul. During three consequent night shifts, the participants were worn the Actigraph to measure the sleep pattern on their wrist since their first night shift work. The quality of sleep and fatigue was measured by survey when they awoke at 6 pm after second night shift work. And these measures were repeated during the night shifts in second and third week. Aroma inhalation was applied before going to bed after finishing each night shift work during second and third week. SPSS 12.0 program was used for data analysis. Results: There were no statistically significant differences in sleep latency and total sleeping time. However, significant differences were found in the sleep efficiency, awakening number, sleep quality, subjective symptom and intensity of fatigue. Conclusion: These results showed that aroma inhalation had positive effects on the pattern and quality of sleep and reducing fatigue in night shift nurses.
