Changes of Characteristics of Patients in Daily Regional Emergency Room after Execution of Five-day Workweek System (주 5일제 시행 후 일 지역 응급실 환자 특성 변화)

  • Purpose: Ths study aims to examine characteristics of patients using emergency room after execution of five-day workweek system by government and provide basic materials for operation of efficient emergency room. Methods: Data were collected tbrough medical records of patients visiting emergency room from July of 2004 to October of 2006 and they were analyzed with SPSSlPC 10.0. Conclusion : 1. The number of patients visiting emergency room was average 16.7 persons a day in 2004, 17.5 in 2005 and 18.6 in 2006 and it was found that it was increasing every year since the execution of five-day workweek system 2. Gender distribution of subjects using emergency room was higher in male than in female every year. 3. Means of transport to emergency room were mostly private car and others(public transport or on foot), but use of ambulance was increasing. 4. Residential areas of subjects were mostly 'Myeon area' in 2004~2005, but it was changed to residents at 'Eup area' in 2006. 5. Distribution of patients by medical departments was highest in internal medicine and surgery in 2004~2006 and rate of visiting pediatrics was increasing every year. 6. Time to stay at emergency room was most at 'below 30 min'. in 2004~2006, but cases of stay for 'more than 2 hours' were increasing every year. 7. On presence or absence of trauma in patients visiting emergency room, rate of visit to emergency room with 'no trauma' was higher and this result was increasing every year. 8. As a result of classifying severity of patients visiting emergency room, use rate of emergency room by 'non-emergency' patients was over 90% in 2004~2006 and such a phenomenon was deepened in 2006 compared to that in 2004. 9. Measures after emergency care of patients were most in case of 'discharge' in 2004~2006, but cases of admission to hospital after emergency care were increased every year. 10. According to use of emergency room by a day of the week, use on Sunday was most frequent in 2004~2006, but use on Friday the day before holiday was increasing. 11. According to distribution by age, use by those between '40~49' was most in 2004~2005, but use by those 'below 10' was most in 2006. 12. According to time to visit emergency room, using emergency room at "15:31~23:30 was most in 2004~2006, cases of visiting emergency room at day working hour were decreased every year and those at evening and night working hours were increased. Conclusion: In sum, it was found that characteristics of patients visiting emergency room and their actual status were changed after the execution of five-day workweek system and efforts to rearrange emergency medical system are required.

A Study on the Quality of Life, Family Support and Hope of Hospitalized and Home Care Cancer Patients (입원한 암환자와 재가 암환자의 가족지지, 희망, 삶의질 정도에 관한 연구)

  • With the occurrence of cancer, most cancer patients experience various emotional changes such as anxiety, depression, and emotional shock. Especially in our country, cancer has been recognized as an incurable disease resulting in death. The number of cancer patients increases daily. But as the survival rate of cancer patients is also increasing, there is a need to find the better methods of nursing care for cancer patients. The purposes of this paper are as follows : 1) To understand family support and hope and quality of life for the cancer patient both during hospitalization and at home. 2) To determine the relationship between family support, hope and quality of life. To examine the problems. we used a questionnaire and obtained data form the records of 45 home care and 90 hospitalized (in 3 university hospitals) patients in Taegu area from the period of June 15 to August 15, 1996. SPSS /PC was used for the data analysis and the statistical methods used were the T-test and ANOVA. The results of this paper are as follows : 1) In the aspect of family support, there is no difference between hospitalized and home care cancer patients(t=1.63, P>0.01). 2) In the aspect of hope, hospitalized cancer patients have a higher score than home care cancer patients(t=3.08, P>0.01). 3) In the aspect of quality of life, hospitalized cancer patients have a higher score than home care patients(t=2.96, P<.01). 4) There is a correlation between quality of life and hope with a correlation coefficient r=0.5195 and P=0.000. In addition, the correlation coefficient between quality of life and family support is 4179 with P=0.000. 5) The family support of the cancer patient is influenced by sex(F=9.1863, P<0.01), education(F=4.3641, P<0.01) and the level of life (F=5.5002, P<0.01), 6) The hope of cancer patients is influenced by the number of hospitalizations(F=3.6413, P<.05), education(F=6.0113, P<.01). and the level of life(F=5.0649, P<.01). 7) The quality of life of cancer patients is influenced by the number of hospitalization(F=5.1167, P<0.05), education(F=3.1590, P<0.01) and the level of life(F=5.6942, P<0.01).

Variability of Seawater Temperature in the Coastal Waters off the Dangjin Power Plant, Asan Bay, Korea (서해 아산만 당진화력발전소 인근해역 수온 변동 특성)

  • This study focused on variability of the sea water temperatures observed off the Dangjin Power Plant in the central west coast of Korea for the period of 1998-1999. Spatial averaged temperature shows the annual range of $20.3^{\circ}C$, with minimum of $3.3^{\circ}C$ in February and maximum of $23.6^{\circ}C$ in August. Horizontal distribution patterns are seasonally reversing: The temperatures are increasing toward inshore of the period of April to October, while they are increasing toward of offshore for the rest of year. Spectral analyses of temperature records show significant peaks at M2 and S2 tidal periods, since the water movement in the study area is influenced by strong tide. The responses of temperature variations to tidal phase show different seasonal characteristics: The temperatures are increasing at flood phases in winter and ebb phases in summer. Amplitudes of the components at M2 and S2 periods are $0.8^{\circ}C\;and\;0.5^{\circ}C$, accounting for 70-80% of daily variation. Coherency analyses between non-tidal components of temperature and wind speed show that in summer, northerly wind components significantly coherent with temperature at 2.8 days period, while in winter, southerly wind component is coherent with 2.4 days period, with 0.6 and 0.7 day phase-lags, respectively.

Analysis of Characteristics and Symptoms in Home-Based Hospice-Palliative Care Patients Registered at Local Public Health Centers (일 지역 보건소 등록 호스피스 완화돌봄 대상자의 특성 및 증상 분석)

  • Purpose: This study was aimed at analyzing the characteristics and symptoms in home-based hospice-palliative care (HBHPC) patients registered at local public health centers. Methods: A retrospective study was performed; Data of 144 HBHPC patients registered at six public health centers in Pusan City were analyzed, including their initial visit records (registration cards, initial pain evaluation and symptom evaluation). Results: The average age of the patients was 67.7 years old. Among all, 46.2% of the patient lived alone, and 65% had middle school education or lower. The most popular (36.3%) religion was Buddhism, and 47.5% received medical assistance from the government. The most frequent diagnosis was lung cancer followed by stomach cancer and liver cancer in that order. Of all, 48.9% were functionally too weak to lead a daily life, 39.6% were under cancer treatment when registered at the public health center, and 84.5% were aware of the fact that they have reached the terminal phase. Moreover, 83.6% complained about pain, and the pain level was moderate or severe in 36.5% of them. Besides pain, fatigue was the most complained symptom (84.7%), and 49.3% of them rated their fatigue as moderate or severe. Conclusion: Most of the HBHPC patients were socio-economically underprivileged and complained about moderate or worse pain and symptoms. Therefore, it appears necessary to develop an integrated strategy that is tailored for each patient reflecting their characteristics.

Survey on the Intake and Consumption Patterns of Milk and Dairy Products by Elementary School Students (초등학생의 우유 및 유제품 섭취 양상과 소비 성향에 관한 조사)

  • We conducted a study to investigate milk and dairy products consumption among elementary school students to identify ways to increase their intake of milk and dairy products. We asked 518 students in Seoul to fill out questionnaire. Only 55.6% of the subjects consumed milk daily, and 43.7% of them drank at least 1 cup per day. The frequency and quantity of milk consumption were higher in male students than female students. The majority of the subjects consumed whole milk, and most students(59.5%) preferred flavored milk to whole milk. The reasons given for drinking milk included 'wanting to be tall'(66.1%) and 'for good health'(47.1%), while the main reason given for not drinking milk was stomach pain (68.4%). Records of having an unpleasant experience and upset stomach after drinking milk occurred at a higher rate for those who did not drink milk(84.2%) than for those who did(53.3%). Of the students questioned, 36.9% consumed dairy products other than milk 3~4 times per week, and liquid-type yoghurt(43.8%) and ice cream(35.4%) were the preferred dairy products among students. The reasons given for consuming dairy products were 'good taste'(56.6%), 'good to eat'(46.6%), 'wanting to be tall'(32.4%), and 'for good health' (25.4%). Among those students who did not consumed dairy products, the major reason given was 'family does not eat at home'(33.3%). After consuming the dairy products, 71.4% of participants thought that dairy products did not result in physical improvement. Most students had experienced purchasing milk and dairy products by themselves. When they bought milk or dairy products, the reasons given for making a particular selection were 'shelf-life'(61.6%) for milk, and 'favorite dairy products'(54.1%) for dairy products. According to participants, the factors that need to be considered for milk and dairy product consumption among elementary school students are 'higher nutrition' (60.8%), 'better sanitation'(57.3%), and 'better quality and taste'(55.2%).

Shift Work and Occupational Stress in Police Officers

  • Background: Shift work has been associated with occupational stress in health providers and in those working in some industrial companies. The association is not well established in the law enforcement workforce. Our objective was to examine the association between shift work and police work-related stress. Methods: The number of stressful events that occurred in the previous month and year was obtained using the Spielberger Police Stress Survey among 365 police officers aged 27-66 years. Work hours were derived from daily payroll records. A dominant shift (day, afternoon, or night) was defined for each participant as the shift with the largest percentage of total time a participant worked (starting time from 4:00 AM to 11:59 AM, from 12 PM to 7:59 PM, and from 8:00 PM to 3:59 AM for day, afternoon, and night shift, respectively) in the previous month or year. Analysis of variance and covariance were used to examine the number of total and subscale (administrative/professional pressure, physical/psychological danger, or organizational support) stressful events across the shift. Results: During the previous month and year, officers working the afternoon and night shifts reported more stressful events than day shift officers for total stress, administrative/professional pressure, and physical/psychological danger (p < 0.05). These differences were independent of age, sex, race/ethnicity, and police rank. The frequency of these stressful events did not differ significantly between officers working the afternoon and night shifts. Conclusion: Non-day shift workers may be exposed to more stressful events in this cohort. Interventions to reduce or manage police stress that are tailored by shift may be considered.

LIFE AND ASTRONOMICAL ACTIVITY OF LEE DEOK-SEONG AS AN ASTRONOMER IN THE LATE OF JOSEON DYNASTY (조선후기 천문학자 이덕성의 생애와 천문활동)

  • The life and astronomical activity of Lee Deok-Seong (李德星, 1720-1794) was studied using various historical sources, including the astronomical almanac, Seungjeongwon-Ilgi (Daily records of Royal Secretariat of Joseon dynasty), and the Gwansang-Gam's logbooks during Joseon dynasty (A.D. 1392-1910). We present the results of the study including the following main findings. First, from the investigation of Lee's family tree, we find that a number of his relatives were also astronomers, notably Samryeok-Gwan (三曆官, the post of calendrical calculation). Second, we find that he took part in the compilation of an annual astronomical almanac over a period of at least 16 years. His major achievements in the astronomy of the Joseon dynasty were to establish a new method of calendar-making calculation and to bring astronomical materials to the Joseon court through a visit to China. The Joseon dynasty enforced the Shixianli (時憲曆, a Chinese calendar made by Adam Shall) in 1654 without fully understanding the calendar. So an astronomer and an envoy were dispatched to China in order to master the intricacies of the calendar and to learn as much of Western science as was available in that time and place. Lee Deok-Seong worked at the Gwansang-Gam (觀象監, Royal Astronomical Bureau) during the reigns of King Yeongjo (英祖) and Jeongjo (正祖). As best as we can ascertain in relation with the calculations in the Shixian calendar, Lee visited China four times. During his trips and interactions, he learned a new method for calendar-making calculations, and introduced many Western-Chinese astronomical books to Joseon academia. Lee greatly improved the accuracy of calendrical calculations, even while simplifying the calculation process. With these achievements, he finally was promoted to the title of Sungrok-Daebu (崇祿大夫), the third highest grade of royal official. In conclusion, history demonstrates that Lee Deok-Seong was one of the most outstanding astronomers in the late-Joseon dynasty.

Comparison of the Quality of Life of the Elderly inpatient with that of the normal elderly people (입원노인과 일반노인의 삶의 질의 비교)

  • This research was designed to compare the health condition and the quality of life of the elderly inpatients with those of the normal elderly people. The subjects of study were 482 elderly inpatients and 304 normal elderly people. The research was conducted in July and August using the instrument WHOQOL. The results of the research are as follows: For the level of health, it was recognized that male was healthier than female, having higher the education, living in the city rather than in the rural, keeping the normal life than being in the hospital. There were little difference in the past medical histories of the elderly inpatients and the normal elderly people. As to the diseases currently under the treatment, there were some difference between the elderly inpatients and the normal elderly people but the tendency was similar in the kinds and the frequencies. As to the recognition for the quality of life between the elderly inpatients and the normal elderly people based on the records reflected on each of the domains of WHOQOL, the normal elderly people more positively recognized in the overall quality of life and the following domains: physical, psychological, level of independence, and spirituality/religion/personal beliefs. The normal elderly people more positively recognized especially in the level of independence domain (mobility, activities of daily living, dependence on medication or treatments, working capacity). In the social relationships domain only the sexual activity was significant and the normal elderly people more positively recognized. The elderly inpatients showed the correlation of over 0.5 in the overall quality of life and the following domains: environment, social relationships, physical, psychological, level of independence. It was over 0.6 in the physical domain and the domains of psychological and level of independence. and the psychological domains of level of independence and social relationships. For the points of overall quality of life, it had no correlation with hospitalization but the health condition, residential district, occupation, and taste exerted a significant effect. As a result of separate analyses of the elderly inpatients and the normal elderly people, the health condition and the age only were the common variable which would exercise a significant effect. Besides, the primary factors which would exercise the quality of life were the occupation and taste for the elderly inpatients, and the residential district and source of income for the normal elderly people. In conclusion, it is first and foremost important to improve the standard of health for the overall quality of life for the elderly people, regardless of hospitalization. Therefore, a plan must be urgently drawn up for revitalization of the health promoting projects for the elderly people and the public health projects for the elderly people, and the investment must be increased for settlement of health problems of the elderly people.

A Study of the Therapeutic Effects of Ginseng on Chi-deficiency Syndrome with Sphygmography (맥파계법을 이용한 인삼의 기부족 증상 치료효과에 관한 연구)

  • Since ancient times, records pertaining to chi-reinforecenent of ginseng have been documented in books of Chinese nedicine. We, therefore, assessed the therapeutic effects of ginseng on patients with chi-deficiency syndrome. Eigheen cases of chi-deficiency, screened at the outpatient service in the internal department of Chinse medicine at the China Medical College Hospital. were prescribed two grams of Korean red ginseng powder three times daily for a two week period. For evaluation, a questionnaire and diagnosis by sphygmography were conducted prior to and after administration of the prescription. For control purposes, fourteen volunteers fasted for more than five days (seven had access only to ginseng. and the other seven were given only mineral water) and were subsequently evaluated by the same criteria. Three results were obtained for the ginseng treated group: (1) improvements in symptoms of chi-deficiency such as fatigue and dizziness; (2) changes in pulse waves with time domains showing alternate P-waves, strengthened dicrotic waves, and some minute waves in the end-diastolic period; (3) frequency domains with enhanced amplitudes around 12Hz. These three findings, which are consistent with each other, suggest the reinforcement of the effect of ginseng on 'heart-spleen chi' in Chinese medicine.

A Study on "Beijijiufa" ("비급구법(備急灸法)"에 대한 연구(硏究))

  • "Beijijiufa" is a medical book republished by Sun Ju Qing in 1245. He compiled this book having added "Qizhumajiufa" and "Zhugejingyanbeijiyaofang" to the contents of "Beijijiufa" authored by Wen Ren Qi Nian. In "Beijijiufa" the author described treatment methods making use of moxibustion methods in connection with 22 cases of acute diseases. The author had collected the moxibustion methods used to treat acute diseases, which had been practiced by the medical practitioners of many generations, and quoted total 13 medical men's practices. In the book, the greatest parts of details were quoted especially from the writings of Sun Si Miao and Ge Hong, and this shows that the medical philosophies of both Sun Si Miao and Ge Hong were reflected onto "Beijijiufa". He had differed on his moxibustion practice: the size of moxa wool, the number of moxibustion treatment, and method of moxibustion for male and female were differed from one another according to the disease. As to the area of moxibustion, he chose the body parts around under four limbs and joints, and mostly used extraordinary acupoints rather than twelve main meridians. In his descriptions of finding meridian points, he did not describe it by its specific name of the reaction point, but explained the location of moxibustion points in detail through pictures. "Qizhumajiufa" is related to moxibustion method and prescriptions to treat surgical diseases, like skin boils or furuncle on the back, etc. He easily explained the method to find the meridian points for moxibustion treatment by using particular way through diagrams and pictures. Eight prescriptions he used were the collections among the historical practices of medical practitioners of many generations for skin boils which showed excellent therapeutic actions. In "Zhugejingyanbeijiyaofang", there are prescribed for 36 disease, also is the records of treatment methods for medical emergency which would be encountered easily in everyday life. As to therapeutic remedies, varied treatment methods, including the treatment by means of pasting and attaching medicinal substance to the spot, the treatment by means of mixing medicinal substance with alcoholic beverage, cleansing method, smoke inhalation remedy etc. were introduced. In "Beijijiufa" moxibustion was regarded as a top priority for treatment of acute disease, and the author strived to present remedies to the readers as easily as possible through 19 pictures. Regarding prescriptions, the author introduced diverse treatment methods with respect to various disease symptoms, and described the method to treat disease symptoms making use of medicinal ingredients which can easily be found in daily life. Likewise, "Beijijiufa" compiled by Sun Ju Qing was intended for clinical practice, and was indeed a medical book having been utilized for treatment of acute diseases in those days.
