On the treatment of C.V.A, I obtain the results through the bibliographic investigation in the method of Chong-Yol-Sa-Hwa as follows ; 1. The method of Chong-Yol-Sa-Hwa on the treatment of CVA. is used for the occation of flaming fire in the heart caused by fire emotions in excess (五志過極), difficiency of Yin, flaming up of excessive in liver and plegm-heat. 2. The representative symptoms which can be used by the method of Chong-Yol-Sa-Hwa are firerishness, redness on face, foul breath, fidgets, angry, high fever, constipation slimy and greasy yellow coat of the tongue, and the pulse shaped on full-rapid (洪數), tautsmaooth and rapid (弦滑血數). 3. The general prescription on the method of Chong-Yol-Sa-Hwa are Bang pung tong sung-San, Yangkyuk-San, Backho-Tang, Sosiho-Tang, Samhwa-Tang, Chibo-Tan and Woohwang chongsim-Hwan on excessive symptom-complex, Yookmichihwang-Tang-Kakam, Samool-Tang-Kakam and Youngyangkak-Tang on insufficency symptom complex. 4. The most frequently dosed medicinal plants are Seok-ko, Chi-mo, Dae-Hwang, Mang-cho, Hwang-kewm, Hwang-back, Chi-ja, Si-ho, Han-Su-Seok, Yong-Tam-Cho, Mok-Tan-Pi and Saeng-Chi-Hwang. 5. The method of chong-Yol-Sa-Hwa might be expressed good medicinal effects adopted on the symptoms or flaming evil fire (火旺) after awakend and ‘Yang’ occlusion of Chung-Chang-Pu (中臟脈) which is accompanied with the method of purgation, break through the plegm, resuscitate and nourishing the Yin.
Purpose of this study was to investigate correlation between blood pressure (systolic and diastolic) and dietary sodium, potassium intake pattern in the family members of normal cerebrovascular (CVA) disease, excluding patients themselves. Both mean values of systolic (125.8$\pm$23.7 vs 119.3$\pm$19.2mmHg) and diastolic(76.1$\pm$16.7 vs 71.6$\pm$12.5mmHg) bllood pressure in the family members of cerebrovascular disease patients were significantly higher than those of normal subjects. Systolic blood pressure was positively correlated with age, weibght, sodium in soybean paste, potassium in hotpepepr paste, soybean paste and meats in normal subjects group. In the family members of cerebrovascular patient, systolic blood pressure was possively correlated with age, weight, sodium in soy sauce, drinking water and potassium in soups. Interestingly, table salt intake was positively correlated with systosolic blood pressure in the family members of cerebrovascular disease patients. Diastolic bolld pressure was positively correlated with age, weight, table salt intake potassium in hotpepper paste and soybean paste in normal subjects group. Diastolic blood pressure was positively correlated with age, weight and table salt intake in the family members of cerebrovascular disease patients. Urinary potassium excretion was negatively correlated with both systolic and diastolic blood pressure in the family members of cerebrovascular disease patients.
본 연구의 목적은 거북목을 가진 사람에게 메켄지 폄운동과 어깨안정화운동이 앞쪽머리자세에 미치는 영향에 대해 알아보는 것이다. 연구대상자는 30명의 거북목을 가진 평범하고 건강한 사람들로 이루어져 있다. 대상자는 어깨안정화운동과 메켄지 폄운동으로 나누어(인원=30, 남자 15명, 여자 15명) 무작위로 나누어 6주 동안 실시되었다. 본 연구는 마름근, 아래등세모근의 운동프로그램과 깊은목굽힘근, 위등세모근, 어깨올림근의 신장프로그램을 통한 목뼈의 머리척추각(Cervical Vertebra Angle) 값과 각 근육별 근전도의 차이를 알아보았다. 앞쪽머리자세와 경우에는 목과 등근육의 활성도가 감소하고 피로도는 증가함을 보였다. 이러한 결과는 목과 머리의 불안정한 상태를 안정시키기 위해서는 위등세모근, 깊은목굽힘근, 어깨올림근의 신장과 마름근, 아래등세모근의 운동을 통해 거북목의 개선에 긍정적인 변화를 줄 것으로 보인다. 이러한 근육들의 불균형한 수축과 과도한 긴장으로 근육의 경직과 피로가 누적되며 이러한 상태가 계속 유지되게 되면 만성화상태가 되어 근육의 불균형으로 인해 통증을 유발할 수 있으며, 피로의 누적으로 인하여 자극에 대한 반응과 근력을 약화시켜서 목 디스크가 발생할 확률이 높아질 것으로 사료된다.
본 연구는 뇌졸중 환자의 기능회복에 대한 가상현실 운동프로그램의 효과를 검증하기 위해 수행된 체계적 문헌고찰 및 메타분석 연구이다. 데이터베이스는 학술연구정보서비스(RISS), 한국학술정보(Korean studies Information Service System, KISS), 한국 의학논문데이터베이스(KMbase)를 활용하여 '뇌졸중', '뇌경색', '상지기능', '균형', '일상생활능력', '운동훈련', '운동중재' '재활' '가상현실', '가상현실 교육', '가상현실 프로그램'을 검색어로 최근 10년(2009년~2018년)간 문헌을 검토하였다. 분석은 R을 이용한 임의효과모형을 적용하여 상지기능, 균형과 일상생활능력의 효과 크기를 각각 산출하였다. 최종적으로는 RCT 9편을 분석하였다. 개별문헌은 Cochrane Collaboration의 RoB (Risk of Bias) 도구를 통해 비뚤림 위험을 평가하였으며, 전반적인 비뚤림 위험은 낮았다. 본 연구결과 균형에 대한 효과크기(g=.77), 일상생활 활동에 대한 효과크기(g=.80)로 "큰 효과크기"를 나타내었다. 따라서, 뇌졸중 환자의 균형과 일상생활능력 향상을 위한 가상현실 운동프로그램이 효과적임을 확인하게 되었다. 향후 가상현실 운동프로그램 이용한 보다 효과적인 프로그램 개발과 적용에 대한 연구가 필요하다고 생각된다.
Background: The purpose of this study is to investigate basic data about the effects of muscle energy technique on the shoulder complex range of motion and posture alignment in the round shoulder posture. Methods: The subjects included 15 women that gave consent to participate in the study voluntarily. They performed the muscle energy technique for 30 minutes twice. The round shoulder posture was measured with a straight edge ruler. The shoulder complex range of motion was measured with the apley scratch test. The forward head posture was measured with ImageJ. The pectoralis minor muscle length was measured with a tape measure. Results: There were statistically significant differences in the round shoulder posture both right and left (p<.05). The experiment group showed statistically significant differences in the pectoralis minor muscle length (p<.05). There were significant differences in the shoulder complex range of motion including flexion, left lateral flexion, right lateral flexion, left side bending, and right side bending (p<.05), but no significant differences were found in extension (p>.05). The forward head posture showed significant differences in CVA changes (p<.05) and no significant differences in CRA changes (p>.05). Conclusion: These findings demonstrate that the muscle energy technique relaxed muscles around the shoulders and increased the shoulder complex range of motion. The technique is also expected to prevent pain in the neck and shoulders and lower injury risk. In conclusion, the muscle energy technique can be applied as an effective intervention for round shoulder posture.
Background: Forward head posture (FHP) causes various posture imbalances associated with the head and neck. Myofascial release is an effective treatment method used for relaxing muscles and reducing muscle hyperactivity, but no studies have been conducted on suboccipital and neck muscles related to FHP. Objects: The purpose of this study was to investigate the immediate effect of roller massages on the cranio-cervical flexion (CCF) range of motion (ROM) and CCF strength applied to suboccipital and neck muscles in subject with forward head posture. Methods: Twenty-four FHP subjects (male: 13, female: 11) were recruited for this study. All subjects were recruited with a craniovertebral angle (CVA) of 53 degrees or less and a head tilt angle (HTA) of 20.66 degrees or higher. CCF strength was measured using Pressure biofeedback unit (PBU) in the supine posture and CCF ROM was measured using smartphone-based inclinometer. Roller massage (RM) was applied to suboccipital and neck muscles for 2 minutes and CCF ROM and strength were remeasured. Results: These results of this study showed that CCF ROM was a significant difference in CCF ROM before and after RM (p < 0.05). CCF strength also showed a significant difference before and after RM (p < 0.05). Conclusion: RM method might be recommended to increase the immediate ROM and strength of CCF in subjects with forward head posture.
Objectives: This study investigated the effect of complex Korean medicine treatment on decreased mental status in a progressive supranuclear palsy patient with a cerebrovascular accident. Case presentation: The patient was treated with Korean medicine composed of herbal medicine (modified Guibi-tang), acupuncture, moxibustion, and cupping combined with Western medicine (a dopamine precursor and an anticholinergic) and physical therapy. The Glasgow Coma Scale, Mini-Mental State Examination, and Hoehn and Yahr Scale were used to evaluate the effects of the treatments. After treatment, Glasgow Coma Scores increased from 9-10 to 12-13, and Mini-Mental State Examination for dementia screen scores increased from 1 to 2. The Hoehn and Yahr score remained at 5. There were no side effects of the treatment. Conclusion: This study suggests that complex Korean medicine treatments may be effective for the decreased mental status of a progressive supranuclear palsy patient with a cerebrovascular accident, but further studies are needed.

Background: The current guidelines for the diagnosis of acute pyelonephritis (APN) recommend that APN be diagnosed based on the clinical features and the presence of pyuria. However, we observed that some of the patients who are diagnosed with APN do not have characteristic clinical features or pyuria at the initial examination. We performed this study to investigate the characteristics of APN without pyuria. Methods: A retrospective, cross-sectional study was conducted on 391 patients diagnosed with APN based on clinical and radiologic findings, between 2015 and 2019. The clinical features, laboratory results, and computed tomography (CT) findings were compared between patients with normal white blood cell (WBC) counts and those with abnormal WBC counts (WBC of 0-5/high power field [HPF] vs. >5/HPF) in urine. Results: More than 50% of patients with APN had no typical urinary tract symptoms and one-third of them had no costovertebral angle (CVA) tenderness. Eighty-eight patients (22.5%) had normal WBC counts (0-5/HPF) on urine microscopy. There was a negative correlation between pyuria (WBC of >5/HPF) and previous antibiotic use (odds ratio, 0.249; 95% confidence interval, 0.140-0.441; p<0.001), and the probability of pyuria was reduced by 75.1% in patients who took antibiotics before visiting the emergency room. Conclusion: The diagnosis of APN should not be overlooked even if there are no typical clinical features, or urine microscopic examination is normal. If a patient has already taken antibiotics at the time of diagnosis, imaging studies such as CT should be performed more actively, regardless of the urinalysis results.
Objective: This study identifies the difference among the heights of a chair's backrest (High, Mid, No), the biomechanical changes chair users undergo over time, and the variables that can measure musculoskeletal disorders, eventually providing information on the appropriate type of backrest. Method: Eleven healthy subjects in their 20s and 30s who had no experience with musculoskeletal disorders or surgical operations within the last 6 months participated in this study. Computer typing tasks were randomly designated and performed according to the type of chair backrest, and evaluation was performed for Flexion-Relaxation Ratio (FRR) analysis after the computer typing tasks. This study used eight infrared cameras (sampling rate: 100 Hz) and nine-channel electromyography (sampling rate: 1,000 Hz). ANOVA with repeated measures was conducted to verify the results, with the statistical significance level being α = .05. Results: Although there was no significant difference in craniovertebral angle (CVA), this study showed time and interaction effects depending on the height of the backrest (p<.05). When working without the backrest, the head-spine angle was lower compared to the chairs with backrest, based on the computer work. As for the head angle, the higher the back of the chair was, the less the head flexion and the body angle became, whereas the body flexion became less when there was a backrest. In addition, the body flexion increased over time in all types of backrests (p<.05). The muscle activity of the upper body tended to be high in the high backrest chair. On the other hand, a lower muscle activity was found with a low backrest. Conclusion: These results show that a chair is more ergonomic when the body angle is correctly set without bending and when it is supported by a low backrest. Accordingly, this study determines that the backrest affects shoulder and neck musculoskeletal disorders during typing and that medium-height backrest chairs can help prevent musculoskeletal disorders, contrary to the expectation that high-backrest chairs are preferable.
Dyslipidemia is a major risk factor for cardiovascular accidents (CVA) and heart disease, especially the ischemic type. Lowering of serum low-density lipoprotein cholesterol (LDL-C) levels is a primary measure for preventing atherosclerosis. Many medications are available for the treatment of dyslipidemia; however, these drugs have some side effects. Therefore, we treated dyslipidemia patients with Chunghyul-plus. Before treatment, patients' levels of total cholesterol, triglyceride, high-density lipoprotein cholesterol (HDL-C), and LDL-C were measured on an empty stomach. Thereafter, patients were administered 1000 mg (2 capsules) of Chunghyul-plus two or three times a day for 2 weeks. After treatment with Chunghyul-plus, patients' serum triglyceride, LDL-C, and total cholesterol levels decreased. The results of this study suggest that Chunghyul-plus might be useful in the treatment of dyslipidemia.
