Risk Factors of Potentially Inappropriate Medications and Cost by Polypharmacy among Elderly Patients of a Community Pharmacy near a Top Tier General Hospital (상급종합병원 근처의 한 지역약국 처방전 분석에 의한 노인 환자의 다약제복용과 약제비용 및 잠재적으로 부적절한 약물사용 관련 위험인자)

  • Objective: Polypharmacy is one of the main causes of inappropriate medication use, adverse drug-related events and cost. It aimed to investigate the status of polypharmacy and potentially inappropriate medication (PIM), the factors affecting polypharmacy and cost in elderly outpatients. Method: A pharmacy claim data were retrospectively analyzed with elderly patients prescriptions at a pharmacy located near a top tier general hospital. The numbers of medications per person, prevalence of polypharmacy and PIM according to the 2012 Beers criteria and Korea PIM list, medication cost and the factors affecting polypharmacy were investigated. Results: Forty-six percentages of the elderly outpatients received polypharmacy and over 21% of them had medications listed in Beers or Korean PIM. In multiregressional analysis, we found that age, gender and insurance types were affective factors of polypharmacy. (p < 0.001, 0.047, 0.009, respectively). The cost of polypharmacy with PIM in elderly outpatients was increased with age. Various approaches of interventions would be further required.

Health Economics Evaluation of a Gastric Cancer Early Detection and Treatment Program in China

  • Objective: To use health economics methodology to assess the screening program on gastric cancer in Zhuanghe, China, so as to provide the basis for health decision on expanding the program of early detection and treatment. Materials and Methods: The expense of an early detection and treatment program for gastric cancer in patients found by screening, and also costs of traditional treatment in a hospital of Zhuanghe were assessed. Three major techniques of medical economics, namely cost-effective analysis (CEA), cost-benefit analysis (CBA) and cost-utility analysis (CUA), were used to assess the screening program. Results: Results from CEA showed that investing every 25, 235 Yuan on screening program in Zhuanghe area, one gastric cancer patient could be saved. Data from CUA showed that it was cost 1, 370 Yuan per QALY saved. Results from CBA showed that: the total cost was 1,945,206 Yuan with a benefit as 8,669,709 Yuan and an CBR of 4.46. Conclusions: The early detection and treatment program of gastric cancer appears economic and society-beneficial. We suggest that it should be carry out in more high risk areas for gastric cancer.

Optimum Model for Analyzing Lifetime Profitability of Holstein Cows

  • This study was on the relative net income (RNI) for 18,286 Iranian Holstein cows from 799 herds, with first freshening between 1991 and 2000. Two kinds of production system, which differed mainly in milk pricing system and feed cost, were considered. Four different models adopted from the literature were examined to find the optimum model. They differed by the cost of rearing and growth after first calving and they needed different amounts of economic data at the farm level. Results showed that four measures of RNI were highly correlated (>0.96) and could be used equally to measure lifetime profitability of cows. Therefore, in herds without a regular system for recording economic and management data, use of the simplest model is recommended. Multiple regression analysis revealed that RNI was affected by age at first freshening, milk yield and days of productive life (DPL), regardless of production system, and a similar breeding goal could be defined for the two systems. Multiple regression analysis of RNI showed that in order to obtain an unbiased estimate of economic value for DPL, the per day milk yield, not total lifetime milk yield, should be included in the regression model along with DPL. Regression analysis suggested that it is possible to predict RNI using information on age at first freshening along with the length of first lactation and per day milk yield with a coefficient of determination ranging from 0.44 to 0.47.

  • This paper describes an optimal design method of VSI output filter for CVCF system. The cost function is used for design, and all parameters are described in p.u(per unit) so that can be adapted to the change of CVCF rating. The capacitor current feedback scheme is proposed to control the damping ratio of the filter plant to meet the system more stable. This means that the LC resonance can be suppressed well in transient condition and can use the same control gains under the change of system capacity. The validity of the proposed method is well verified with the theoretical analysis and simulation results.

  • Environmental pressures from such sources an economic condition, the government and inter-institutional competition create managerial challenges. Economic pressures may be forcing dietetic dept, in hospital to utilize cost∙benefit analysis to assist them in their problem solving. Cost∙benefit analysis have been widely used in business, industry and many other fields with only limited application to foodservice. Due to the lack or this information the purposes of this study were to identify use of cost∙benefit analysis in hospital foodservice system to evaluate the economic efficiency of alternatives, and to make recommendation for operation system change. Using the cost∙benefit method, cash flows are separated into cost and benefits. For an alternative to be selected, indicators, such as NPV, benefit-cost ratio (B/C ratio) with 5% discount rate per annum. The sensitivity analysis was also conducted with difference rate 3%, 7% respectively and reduced employee payroll change. The result of this study can be summarized as follows : 1. The total cost of investment for operation system change was 390,570 thousand won and the total benefit through operation system change was 865,808 thousand won. 2. Net present value(NPV) for 5 years was 475,239 thousand won and benefit-cost ratio was 2.22. 3. In sensitivity analysis with different discount rate 3%, 7%, benefit-cost ratio was 2.25, 2.18 respectively, with total reduced employee payroll change, benefit-cost ratio was 2.86. In conclusion, total benefits were exceeded total costs. Therefore, the project of operation system change in hospital foodservice was found to be economically efficient.

  • Because of the rising healthcare costs, there is a growing need for developing efficiency indicators for medical resources use and measuring efficiency of healthcare providers and healthcare systems using them. In this study, we aimed to develop efficiency indicators for medical resources use by means of Delphi technique. We systematically reviewed the existing measures of medical resource use. Thirty nine indicators were selected as a candidates across the six domains: medical personnel, medical equipment, medical facilities, ethical management, resource efficiency, and drug use. To develop efficiency indicators with professional consensus, a 2-round Delphi survey was conducted among 29 professional experts. The following indicators were selected based on the Delphi survey results: adjusted number of the patient per day and level of the nurse number medical personnel in medical personnel domain; the number of the scan a professional physician and the quality of the scan in medical equipment domain; bed utilization rate in medical facility domain; drug price reported pharmaceutical price by medical institutions, medical fee billing transparency, and medical care appropriateness in ethical management domain; costliness index in resource efficiency domain; and utilization of high cost drug and items per prescription in drug use domain. The efficiency indicators could provide valid information about efficiency of healthcare providers and healthcare systems with respect to their resources use and facilitate policies to improve their efficiency.

  • This paper considers a Bayesian approach to replacement policy following the expiration of non-renewing combination warranty. The non-renewing combination warranty is the combination of the non-renewing free replacement warranty and the non-renewing pro-rata replacement warranty. We use the criterion based on the expected cost and the expected downtime to determine the optimal replacement period. To do so, we obtain the expected cost rate per unit time and the expected downtime per unit time, respectively. When the failure times are assumed to follow a Weibull distribution with uncertain parameters, we propose the optimal replacement policy based on the Bayesian approach. The overall value function suggested by Jiang and Ji (2002) is utilized to determine the optimal replacement period. Also, the numerical examples are presented for illustrative purpose.

Costs During the First Five Years Following Cancer Diagnosis in Korea

  • Objective: We estimated the total medical costs incurred during the 5 years following a cancer diagnosis and annual medical use status for the six most prevalent cancers in Korea. Methods: From January 1 to December 31, 2006, new patients registered with the six most prevalent cancers (stomach, liver, lung, breast, colon, and thyroid) were randomly selected from the Korea Central Cancer Registry, with 30% of patients being drawn from each cancer group. For the selected patients, cost data were generated using National Health Insurance claims data from the time of cancer diagnosis in 2006 to December 31, 2010. The total number of patients selected was 28,509. Five-year total medical costs by tumor site and Surveillance, Epidemiology, and End Results (SEER) stage at the time of diagnosis, and annual total medical costs from diagnosis, were estimated. All costs were calculated as per-patient net costs. Results: Mean 5-year net costs per patient varied widely, from $5,647 for thyroid cancer to $20,217 for lung cancer. Advanced stage at diagnosis was associated with a 1.8-2.5-fold higher total cost, and the total medical cost was highest during the first year following diagnosis and decreased by the third or fourth year. Conclusions: The costs of cancer care were substantial and varied by tumor site, annual phase, and stage at diagnosis. This indicates the need for increased prevention, earlier diagnosis, and new therapies that may assist in reducing medical costs.

Effects of HACCP System Implementation on Medicine Use and Productivity of Medium Scale Swine Farms in Korea (HACCP시스템 적용이 중규모 양돈농장의 동물용의약품 사용 및 생산성에 미치는 영향)

  • The aim of this study was to examine the effect of HACCP system implementation on the budget for medicine use, antibiotic utilization, and productivity of swine farms in Korea. Data were collected form thirty swine farms before and after implementation of HACCP system. Parturition rate, total number of piglets, number of initial weaning piglets, number of weaning piglets, survival rate of piglets after weaning, date of weaning piglets, piglet per sow per year, and market piglet per sow per year tended to be higher after HACCP system implementation. In contrast, price for medicine used per month, number of antibiotic used, the farm number of feeds containing antibiotics in each, growing stages added at the feed company, and the farm number of feeds containing antibiotics in each growing stages added at the swine farm were lower after HACCP system implementation. Due to increase in the feed cost recently, production cost for shipment in HACCP system implemented swine farms had increased in this study. In conclusion, our results indicated that the implementation of HACCP system might be effective in reducing medical expenses and improving the productivity.

Developing Continuous Stabilization Process for Textile-Grade PAN Fiber-Based Carbon Fiber Using UV Irradiation (저가형 탄소섬유 개발을 위한 자외선 조사 기반 의류용 PAN 섬유의 연속식 안정화 공정 개발)

  • Carbon fibers (CFs) are considered promising composite materials for various applications. However, the high cost of CFs (as much as $26 per kg) limits their practical use in the automobile and energy industries. In this study, we developed a continuous stabilization process for manufacturing low-cost CFs. We employed a textile-grade polyacrylonitrile (PAN) fiber as a low-cost precursor and UV irradiation technique to shorten the thermal stabilization time. We confirmed that UV irradiation on the textile-grade PAN fibers could lower the initial thermal stabilization temperature and also lead to a higher reaction. These resulted in a shorter overall stabilization time and enhancement of the tensile properties of textile-grade PAN-based CFs. Our study found that only 70 min of stabilization time with UV irradiation was required to prepare textile-grade PAN-based low-cost CFs with a tensile strength of 2.37 ± 0.22 GPa and tensile modulus of 249 ± 5 GPa.
