Cost-benefit Analysis of Massive Screening for Inborn Errors of Metabolism in Korea (선천성대사이상검사 사업의 비용편익 분석)

  • Objectives: Since 1991, nationwide massive neonatal screening program for phenylketonuria (PKU) and congenital hypothyroidism have been performed in Korea. As in many other countries, efficiency of this program has not been definitely concluded. For the purpose of evaluation of this program, from the perspective of efficiency, a cost-benefit analysis was carried out. Methods: Costs of the detection and the treatment program were compared with the projected benefit(avoided costs) that results from the prevention of the mental retardation associated with the disorders due to PKU and hypothyroidism. Costs and benefits were discounted at an annual rate of 5%, and duration of life-long labor was assumed to be 30 years. Cost and benefit were estimated based on the detection rates of one case of PKU per 5,572 and one case of congenital hypothyroidism per 32,554 babies screened during 1991-1997. Results: The benefit-cost ratio was 0.418. The sensitivity analysis for the discount rates and labor durations showed that most cost-benefit ratios were lower than one(1.0) except when discount rate was changed to 3% and detection rate to two- or threefold and/or labor duration to 40 years. Conclusion: The result of this study suggested that present program of mass screening for PKU and congenital hypothyroidism could not be justified in terms of efficiency. It doesn't coincide with the results of previous studies in major developed countries, presumably because of difference in detection rates and welfare cost for the disabled.

  • This paper presents a method to select optimal device for mitigating voltage sags. The method is based on economic evaluation and voltage sag assessment involving sag duration as well as magnitude. The economic evaluation is performed by using the operation cost and economic benefit of the mitigation devices. The optimal device can be determined from the values of NPV (net present value) which is widely accepted in cost-benefit analysis. The proposed method can help sensitive customers to select optimal mitigation device. In this paper, the case study considering two sensitive customers was performed by using the proposed method.

  • This paper presents a reliability evaluation method for considering the voltage quality. The proposed evaluation methods are contained the sustained interruption, momentary interruption and voltage sag. For momentary interruption, evaluation indexes are divided the duration based index and the interruption cost index. For voltage sag, the final result of evaluation method represents the magnitude for customers' risk due to the voltage sag. The proposed method is tested using the RBTS model and a reliability data in KEPCO's system.

Comparison the Diagnostic Value of Dilatation and Curettage Versus Endometrial Biopsy by Pipelle - a Clinical Trial

  • Background: Several methods have been presented for the evaluation of the endometrium in patients with abnormal uterine bleeding, which include minimal invasive and invasive approaches such as diagnostic curettage or endometrial biopsy by Pipelle. Many studies have been performed in order to compare two methods; diagnostic curettage and outpatient endometrial biopsy. This investigation compared sampling adequacy, endometrial histopathology, failure rates, duration and costs between diagnostic curettage in a hospital and endometrial biopsy. Materials and Methods: This single blind clinical trial was performed on 130 patients older than 35 years who was referred to Amir training hospital in 2013 for elective diagnostic curettage because of abnormal uterine bleeding. For all patients eligible for the study, an endometrial sample by Pipelle was taken without anesthesia or dilatation. Then under general anesthesia diagnostic curettage was performed by sharp curette. Sampling duration was calculated and both samples were sent to the same pathologist. The diagnostic values of two methods in the diagnosis of normal endometrium, endometrial hyperplasia and carcinoma were compared. The costs of these two methods were also compared. Data analysis was performed by SPSS (version 16.0) software. Chi-Square, Fisher, and Pearson tests were used and were considered statistically significant at P values less than 0.05. Results: Two methods were agreed upon 88% of sampling adequacy and 94% of pathological results. Specificity of 100% and sensitivity of 90% for detection of proliferative endometrium, secretory endometrium, simple hyperplasia without atypia and 100% for cancer were recorded. Pipelle diagnostic accuracy in comparison with curettage, have been reported over 97%, so the failure rate in this study was below 5%. Sensitivity of Pipelle for detection of atrophic endometrium was reported below 50%. Duration and cost was lower in Pipelle versus curettage. Conclusions: It is concluded that due to high agreement and cohesion coefficient between curettage and Pipelle on the issue of sampling adequacy, histopathology finding (except atrophic endometrium), low failure rate, duration of sampling and cost, Pipelle can be introduced as a suitable alternative of diagnostic curettage.

Benefit-Cost Analysis for Developing Jeongja Port in Ulsan (울산지역 어항개발의 경제성 평가 - 정자항을중심으로 -)

  • The objective of this study is to review the methodology of economic analysis of fishing ports by examining the economical feasibilities of a national fishing port (Jeongja Port) in Ulsan. This study utilized market value evaluation method to measure the benefits and costs related to the development of ports. The benefit variables are income effects resulting from the developments while the cost variables are sum of construction costs and maintenance costs. The income effects are measured in two ways: (1) income from individual project resulting from the developments, (2) the income effects by utilizing investment multipliers. The results shows that the BC ratio (Benefits/Costs) of Jeongja port by using (1) income from individual project resulting from the developments was 1.07 while the BC ratio by using (2) the income effects by utilizing investment multipliers was 1.10 due to a relative short period of useful life for investment multipliers. However, the income variable utilizing investment multipliers is more sensitive to the period of duration than the income variable from individual project.

  • This paper presents summary results of direct interview survey of Korean industrial customers conducted to evaluate the direct and short duration interruption costs resulting from local random electrical supply interruptions. The cost is evaluated using the questionnaires authors designed, which are mainly on the experience or preparatory actions consumers predicted they would take. Economic evaluation of reliability that traditionally takes into account duration and frequency of interruption does not accurately reflect the lost value or economic impacts of industrial customer, especially large size, caused by any kind of interruption. So, to imply the substantial difference in costs incurred by different customer group, the authors apply the international standards, i.e. IEC, IEEE, to the analysis on the interruption costs.

  • This study sought to find importance factors for the quality of Mongolian consumers' evaluation for fresh food through online purchase. To compare the priorities of factors determining the choice of service quality of online purchase for fresh food, this study used a decision model using the appropriate Analytic Hierarchy Process (AHP). Through a prior study, the main factors of quality were classified as delivery quality, product quality, marketing, and system quality, respectively According to the results of AHP the quality of deliver information and deliver duration time under delivery quality are the main factor, followed by hygiene and freshness of product quality were the next highest. When consumers purchase fresh food through an online market. they considered deliver information, delivery duration time, hygiene, freshness, and deliver cost as important factors.

A Survey on the interruption costs for the sensitive industrial customers to the short duration interruption (순간정전에 민감한 산업수용가의 정전비용에 관한 조사연구)

  • This paper addresses the analysis of interruption costs and the studies made through the direct interview with Korean Industrial customers for the period of two years. It is conducted to evaluate the direct and short duration interruption costs resulting from local random electrical supply interruptions. The cost is evaluated by providing a designed questionnaire focused on the experience or the preparative actions which consumers predicted they would take. In the first phase, authors made a survey on all types of industry with 176 industrial customers and evaluated them as per standard industry classification (SIC). In that case, the evaluation data obtained are not so reasonable, because the number of sampled industries was small and the sampled industries were surveyed by being divided into 18 types. So, in the second phase the authors concentrated on only 4 types of 302 industries who have many and large sensitive loads to the short duration interruption.

Comparative evaluation of different offshore wind turbine installation vessels for Korean west-south wind farm

  • The purpose of this study is to evaluate various means of wind power turbines installation in the Korean west-south wind farm (Test bed 100 MW, Demonstrate site 400 MW). We presented the marine environment of the southwest offshore wind farm in order to decide the appropriate installation vessel to be used in this site. The various vessels would be WTIV (Wind turbine installation vessel), jack-up barge, or floating crane ${\cdots}$ etc. We analyzed the installation cost of offshore wind turbine and the transportation duration for each vessel. The analysis results showed the most suitable installation means for offshore wind turbine in the Korean west-south wind farm.
