Does an Adjuvant Chemotherapy Really Help Patients with Glioblastoma? (교모세포종 환자에서 부가적인 항암치료의 효과)

  • Objectives : Chemotherapy remains part of the treatment triad that includes surgery and radiotherapy for the management of glioblastomas, but disappointing results of chemotherapy have raised the suggestion that chemotherapy should perhaps be abandoned. In order to determine the chemotherapy effect given in addition to radiotherapy, we performed a randomized clinical study of irradiation alone and combination of irradiation with chemotherapy in the treatment of glioblastomas. Methods : From 1991 to 1999, 204 consecutive patients suffering from supratentorial glioblastomas were treated in our hospital. We compared the survival rates/times of these patients according to the treatment modalities[group I-67 patients treated by surgery with radiotherapy and adjuvant chemotherapy(ACNU, paclitaxel, tamoxifen, and others) ; group II-106 by surgery with radiotherapy ; and group III-31 by surgery only]. Results : The overall median survival time was 12 months, with overall survival rates at 1 and 2 year of 46.7% and 16.6%, respectively. On univariate analysis, median survival and 1- and 2-year survival rates were statistically improved by the use of chemotherapy ; group I-15 months, 75.7%, and 25.9%, group II-11 months, 39.3%, and 15.4%, and group III-3 months, 9.7%, and 6.5%, respectively(p=0.0001). But, on multivariate analysis considering compounding variables, survival was independently associated only with radiotherapy(p=0.0112). Conclusion : These results suggest that the addition of chemotherapy to radiotherapy does not affect the overall survival in glioblastomas. Mainly long-survivor glioblastoma patients might benefit by adjuvant chemotherapy, which probably means patients with initial favorable prognostic factors(young age, minimal residual tumors, good performance status). It is necessary to continue to search for an effective chemotherapy regimen to prolong survival of patients with glioblastomas.

  • The foaming of poly(butylene succinate) (PBS) using supercritical $CO_2(scCO_2)$ was studied. In order to improve the melt strength, PBS was modified using the reactive compounding technique. Rapid decompression of $scCO_2$-saturated PBS at a temperature above the depressed $T_m$ yielded expanded microcellular foams. The resulting foam structure could be controlled by manipulating process conditions. Experiments varying the foaming temperature while holding other variables constant showed that higher temperatures produced larger cells and reduced cell densities. Higher saturated pressures led to higher nucleation densities and smaller cell sizes. Decreasing the rate of depressurization permitted a longer period of cell growth and therefore larger cells were obtained.

  • We propose a prescription transmission system based on XML in his paper, and it is not to attach a former signature to only a XML document for encoding of XML/EDI, and it is construction, one with the prescription transmission system which is safer with what use a way to attach a digital signature to DTD. I defined sub element to manage information prescription DTD defined prescription information, patient information, medical care organ information, prescription details information, compounding of medicines details information element according to for each a component of a prescription I went along, and to have looked up, and to have obeyed information transmission at he low rank. I read a DTD file for safe prescription transmission, and I do element or property, the entity which I do it, and is extracted here, and Pasing is saved in a table while being a field. If Pasing is finished, I read and lift a hash table and carry out message a digest. I compose it with an early private key and create a digital signature.

  • The cellular rubber products for industrial purpose have been applied in many fields such as auto-motive parts, ship-building, machinery, sports goods, diving suit or interior housings etc. The purpose of this dissertation is to study the physical properties of celluar rubber products particulary for those elastomers such as EPDM, CR and NBR with heat resistance property, weather proofness, and oil resistance characteristics respectively, aiming at improving their quality, and renovating the manufacturing know-how which is beyond our technical power at the present time in Korea. In order to meet this requirement an ideal recipe is being shown for the three elastomers, and also a practical recipe which is easily available in terms of compounding ingredients in domestic market has set up as shown in Table 1. for the investigation of vulcanization characteristics by means of Rheometer. The optimum Mooney viscosity of compounded rubber was found to be approximately $ML_{1+4}(100^{\circ}C)$ $30\sim45$. Excess mustication makes a dispersion of ingredients worse, consequently it causes deformation of shapes and heterogenous cell distribution. In other words the articles are rejected because of its insufficient workmanship. The results of physical properties of the products are indicated in Table 3. It has shown that the quality meet requirement when tested in accordance with ASTM D572, 573 and D 395. The test results o CR/IR blends in terms of hardness, volume change by blowing, tensile strength and elongation have been shown.

  • In this work, the effect of oxygen plasma treatment of nano-scaled silica on the mechanical interfacial properties and thermal stabilities of the silica/rubber composites was investigated. The surface properties of the silica were studied in X-ray photoelectron spectroscopy (XPS) and contact angles. And, their mechanical interfacial properties and thermal stabilities of the composites were characterized by tearing energy ($G_{IIIC}$) and thermogravimetric analysis (TGA), respectively. As a result, it was found that the introduction rate of oxygen-containing polar functional groups onto the silica surfaces was increased by increasing the plasma treatment time, resulting in improving the tearing energy. Also, the thermal stabilities of the composites were increased by increasing the treatment time. These results could be explained that the polar rubber, such as acrylonitrile butadiene rubber (NBR), showed relatively a high degree of interaction with oxygen-containing functional groups of the silica surfaces in a compounding system.

  • As sustainability of fashion brands is becoming more important in the prolonged depression, the fashion market is also seeking ways to create new value through convergence with various fields. In the fashion market, convergence marketing is engaged with diverse technologies combined with advanced technologies and strategies. This study established the conceptual characteristics of convergence and divergence from a market perspective. Convergence can be summarized as Borderless, Compounding, Diversification, Recombination and Affiliation. The conceptual characteristics of the divergence can be summarized as de-fusion, individualization, differentiation, specialization, and segmentation. The characteristics of convergence revealed through the case of fashion market include the expandability of the new category, the expandability of the consumer, the brand experience, and the expandability of time and space. Divergence is the differentiation of brand value, target customer, and product nature. Through this study, we could understand the differentiation of the fashion market and the direction of the marketing strategy by looking at the characteristic of the convergence marketing - oriented research that has the contrary conceptual characteristics from the extended viewpoint. Based on this, it is possible to see the changes and patterns of brand marketing strategies and techniques in recent fashion market. In the future, it is expected that the systematic study of the marketing strategy of the domestic fashion brand based on the convergence and change characteristics of the fashion market proposed in this study will continue.

Usability of Project Finance with Sinking Fund (감채기금을 이용한 프로젝트 파이낸스의 유용성)

  • A pure project finance is exposed to high default risk because it is made solely based on the business value of the project. Project finance with sinking funds can reduce the high default risk, in that it uses sinking funds to ensure the principal without requiring collaterals or guarantees. In view of economic efficiency, this paper analytically compares project finance with sinking funds to usual project finance and derives the condition for its superiority in corporate tax saving. Because sinking funds generate the repayments for the principal with compounding interest rates, in terms of effectiveness, project finance with sinking funds is suitable for long-term projects whose period is longer than 10 years. Pension funds can be good sources for project finance with sinking funds, since they should be managed to ensure stable investment returns for long periods.

  • In this study, an experiment was conducted to evaluate the properties of cement matrix using diatomite and silica gel as adsorbents of radon. The adsorption properties of diatomite of a natural adsorbent and silica gel of an artificial sorbent were examined to confirm the reduction of radon gas concentration of the removal of radon gas in the indoor environment of the human body. We conducted a performance evaluation for the study. The fluidity, air content, density, absorption, flexural failure load, thermal conductivity and radon gas concentration of the specimen using diatomite and silica gel were measured. the fluidity and the air content of the adsorbed matrix with diatomite were decreased as the diatomite replacement ratio increased. Which seems to affect the subsequent matrix by the absorption of the compounding water of diatomite. As the replacement rate of silica gel increased, the fluidity decreased and the air content increased up to constant replacement rate. It is judged that the surface of the silica gel has a critical point at which it can react with moisture.

A Study on Patent Dispute Countermeasures of Chinese Companies (중국 기업의 특허분쟁 대응 방안에 대한 연구)

  • In this research, we would like to understand the measures to be taken in the event of a patent dispute by Chinese companies that are growing rapidly and competing with Korean companies in the global market, at a time when the fusion and compounding technology are accelerating. For this reason, the patent dispute countermeasures shown in the previous research were derived through deep interviews with experts, and a questionnaire survey was conducted with patent practitioners of Chinese companies. As a result of the analysis, the importance of cross licensing, patent invalidation proceedings, proceedings through collaboration with other companies, royalty payments, and patent invalidation proceedings in the case of a patent dispute in the Chinese industry is high. Shown. The results of this study provide practical guidelines to help corporate patent practitioners understand the best course of action in the event of a patent dispute, respond in a timely manner, and save time and money. It seems that it can be done.

Impact of Off-Hour Hospital Presentation on Mortality in Different Subtypes of Acute Stroke in Korea : National Emergency Department Information System Data

  • Objective : Several studies have reported inconsistent findings among countries on whether off-hour hospital presentation is associated with worse outcome in patients with acute stroke. However, its association is yet not clear and has not been thoroughly studied in Korea. We assessed nationwide administrative data to verify off-hour effect in different subtypes of acute stroke in Korea. Methods : We respectively analyzed the nationwide administrative data of National Emergency Department Information System in Korea; 7144 of ischemic stroke (IS), 2424 of intracerebral hemorrhage (ICH), and 1482 of subarachnoid hemorrhage (SAH), respectively. "Off-hour hospital presentation" was defined as weekends, holidays, and any times except 8:00 AM to 6:00 PM on weekdays. The primary outcome measure was in-hospital mortality in different subtypes of acute stroke. We adjusted for covariates to influence the primary outcome using binary logistic regression model and Cox's proportional hazard model. Results : In subjects with IS, off-hour hospital presentation was associated with unfavorable outcome (24.6% off hours vs. 20.9% working hours, p<0.001) and in-hospital mortality (5.3% off hours vs. 3.9% working hours, p=0.004), even after adjustment for compounding variables (hazard ratio [HR], 1.244; 95% confidence interval [CI], 1.106-1.400; HR, 1.402; 95% CI, 1.124-1.747, respectively). Off-hours had significantly more elderly ≥65 years (35.4% off hours vs. 32.1% working hours, p=0.029) and significantly more frequent intensive care unit admission (32.5% off hours vs. 29.9% working hours, p=0.017) than working hours. However, off-hour hospital presentation was not related to poor short-term outcome in subjects with ICH and SAH. Conclusion : This study indicates that off-hour hospital presentation may lead to poor short-term morbidity and mortality in patients with IS, but not in patients with ICH and SAH in Korea. Excessive death seems to be ascribed to old age or the higher severity of medical conditions apart from that of stroke during off hours.
