결핵 환자의 가족 지지와 환자 역할 행위에 관한 연구 (A Study on Support by Families of Patients with pulmonary Tuberculosis and Compliance with Sick Role Behavior)

  • The purposed of this study was to identify compliance with sick role behavoir and support by families of patients with pulmonary tuberculosis and to identify factors affecting these variables md factors that increase compiance with sick role behavoior This study was a survey design done in K. general hospital from 1992. 7 1 to 1992. 7. 20. The data were collected through personal interviews with 70 subjects who had pulmonary tuberculosis. A questionnaire was used to collect the data. 1. Demographic characteristics of the subjects The factor causing the tuberculosis for 28.6% of the subjects was that they lived with another family member who had pulmonary tuberculosis, 11.4% of the subjects had a family member who died of tuberculosis, 24.3% of the subjects had experienced an adverse reaction to medication. Gastroenteric disorders(28.6%) had the highest rate for this kind of adverse reaction to medication. 2. Support by families for patients with pulmonary tuberculosis The items which showed high support for compliance were taking medicine regularly(80.0% ) eat-ing a balanced diet(80.0%), loving them(84.3%), taking care of them(82.8%), getting the support from people for being sick(81.1%) The items which showed low support for compliance were meeting relatives or clergymen(20%), talking with the people suffering from pulmenary tuberculosis to relieve frustrated because they were not recovering. 3. General characteristics and family support The degree of family support showed that 75 was highest score and 30, the lowest score with 55.5(74. 1%)the average score. The degree of family support appeared similar in cases where the person was married and where the person did not have a family member with pulmonary tuberculosis(P〈.01) 4. The degree of compliance with sick role behavior by patients with pulmonary tuberculosis The degree of compliance with sick role behavior showed that 100 was the highest scoure47, the low-est score and 76.4, the average score out of a Possible score of 100. 5. Correlation between compliance with sick role behavior and family support The compliance with sick role behavior and family support showed a positive correlation(r=.2094 p〈.5) So for patients with pulmonary tuberculosis. compliance with sick role behavior is related to the sup-port given by their families.

  • Purpose: This study was conducted to explore the relationship among health belief. health locus of control and patients sick-role behavior compliance of diabetic mellitus patients visiting public health center. Method: The subjects of this study were 193 of the diabetic patients who were visiting 4 Public Health Center in B city. The instrument used for measuring health belief was Park's(1985). for health locus of control was Wallston. et al's(1978) and for sick-role behavior compliance was Park's(1984). The data were collected with structured questionnaires; total 58 items contained about health belief. health locus of control and sick-role behavior compliance from 1st to 31st July. 2001. The data was analyzed by the SPSS/PC programs using t-test. Pearson's correlation coefficient. ANOVA and Scheffe-test. Result: The average score of the health belief was $57.99\pm9.45$ health locus of control was $66.83\pm9.48$ and sick-role behavior compliance was $42.81\pm7.00$. Statistically significant factors influencing the health belief among social demographic characteristics were family number(F=3.818. p=0.024), monthly income(F=5.153, p=0.002), time of diagnosis(F=3.937. p=0.002) and difficult to control disease(F=5.803. p=0.000). The significant factors influencing the health locus of control were marital status(F=4.669. p=0.010). Also significant factors influencing the sick-role behavior compliance were monthly incomes(F=5.245, p=0.000). the time of diagnosis(F=4.424. p=0.001) and admission to hospital with diabetes(F=9.031. p=0.000). There was negative mild correlation comparatively between health belief and sickrole behavior compliance(r=-0.142, p<0.05) but no correlation in sensitiveness/severity, barrier, benefit(p<0.05). There was no correlation between internal. external. chance health locus of control and sick-role behavior compliance (P>0.05). Conclusion: There was a negative weak relationship between health locus of control and patient's sick role behavior compliance. Therefore further study to investigate the relating factor of the sick role behavior compliance among above of middle aged diabetes mellitus patients is necessary.

  • This study examined the effect of patient teaching on Compliance with sick role behavior in diabetic patients. The purpose was to improve diabetic patients Compliance by D. M. patient teaching. The study objectives were to determine the effect of patient teaching on Compliance with sick role behavior, and factors influencing compliance with sick role behavior of diabetic patients. The Subjects, consisting of 52 diabtic patients diagnosed in the C. and Other hospitals in K. city were divided into experimental and Control groups. Data were gathered from July 25th to September 3rd, 1988 through interviews by questionnaires, measurment of blood sugar level by Reflolux. D. M. Patient teaching was defined as informational intervention of social support by the nurse. A booklet representing patient education and questionnairs were developed by the investigator, and were tested for Content validity, and reliability by Item Analysis: Cronbachs $\alpha$ for any instrument to measure variables was patient Compliance .83, perceived health belief .65, diabetic knowledge .70. Analysis of data were done by paired t-test, t-test, Anova, Pearson correlation and Stepwise multiple regression, The results of the study may be summarized as follows : 1. The effect of patient teaching on Compliance with sick role behavior, on diabetic Knowledge and health belief was Confirmed by significant differences between the experimental and the control group before and after the experiments. (P=000 P=006, P=004). 2. Factors influencing compliance with sick role behavior of diabetic patient were diabetic patient teaching (P<.005), perceived health belief(r=.5597, P<.005), blood sugar(r=.3205, P<.01), diabetic knowledge(r=.2876, P<.05).

지지간호가 관상동맥조영술을 받은 관상동맥질환자의 질병관련 지식과 환자역할행위 이행에 미치는 효과 (The Effect of Supportive Nursing Care on the Knowledge Level and Compliance of Sick-Role Behavior in Patients with Coronary Artery Disease after Coronary Angiography)

  • Purpose: The purpose of this study was to determine the effect of supportive nursing care on the level of knowledge and compliance with sick-role behavior in patients with coronary artery disease after coronary angiography (CAG). Methods: A quasi-experimental research was performed with 81 subjects with coronary artery disease who were admitted for CAG to a cardiovascular department. Among the selected subjects, 40 of them were assigned to experimental group and the rest of them were assigned to control group by convenience. Supportive nursing care was implemented twice by a researcher. Results: 1. The mean score of knowledge related to coronary artery disease was higher in the group who received supportive nursing care than that of the control group (t=2.259, p=.027). 2. The mean score of compliance of sick role behavior was higher in the group who received supportive nursing care than that of the control group (t=4.580, p=.001). Conclusion: The supportive nursing care after CAG was effective in increasing the knowledge level and compliance of sick-role behavior in patients with coronary artery disease. Further studies would be recommended to identify long-term effectiveness of supportive care on patients' outcomes.

사회적 지지가 고혈압환자의 역할행위 이행에 미치는 영향과 지지요법 효과의 지속에 관한 연구 I (The Effect of Social Support on Compliance with Sick Role Behavior in Hypertensive Clients and Duration-of the Effect for up to 6 months)

  • This study was done using a Quasi-experimental research design to determine the effects of social support on compliance with sick role behaviors in hypertensive patients and to know if the effect of the social support on compliance lasted for at least 6 months. The subjects consisted of 81 hypertensive patients who were registered in the Cardio-Vascular OPD at Chonnam National University Hospital. They were divided by random sampling into 42 people for the experimental group and 39 for the control group. Data were gathered from June 3, 1996 to June 10, 1997 through individual interviews using a structured questionnaire. The results of the study were summarized as follows : 1. Compliance with sick role behaviors in hypertensive clients was significantly increased in the experimental group who received social support from the nurse as compared to the control group who did not receive social support(t=15.99. p<.001). 2. The effect of social support on compliance with sick role behaviors in hypertensive clients lasted for 6 months(t=7.99, p<.001). 3. Four of six people stopped smoking in experimental group after the intervention of social support, but none of the five in control group were able to stop smoking. Fisher's Exact test showed a significant difference between the experimental and control group(x²=4.385. p< .05). Mantel Haenszel test showed that the effect of social support on stopping smoking in the experimental group lasted for six months because there were no significant differences between one month after the social support and six months after, in the number of subjects who stopped smoking(x²=1.154, P>.05). Finally, social support was effective on compliance with sick role behaviors and stopping smoking in the hypertensive clients, and the effect of social support on compliance lasted for 6 months.

혈액투석 환자를 위한 통합적 관리 프로그램의 개발 및 효과 (Development and Evaluation of Integrated Management Program for Hemodialysis Patients)

  • Purpose: This study aimed to develop and evaluate an integrated management program to enhance self-efficacy, compliance with sick-role behaviors, symptom management, and biomarker indication in hemodialysis patients. Methods: The integrated management program was developed through a systematic review of literature, analysis of relevant online data, and expert validation. It comprised 480 min of video-based education delivered eight times over four weeks, supplemented by weekly phone consultations and text message support from a nurse. To evaluate the program's effectiveness, it was administered to 44 patients with hemodialysis in a single group in a pre-post test experimental study. Changes in self-efficacy, sick-role behavior compliance, dialysis symptom index, and biomarkers were assessed. Results: The program yielded statistically significant improvements in self-efficacy (t=-7.13, p<.001), sick-role behavioral compliance (t=-7.35, p<.001), dialysis symptom index (t=4.32, p<.001), and blood urea nitrogen levels (t=2.55, p=.014) among the participants. Conclusion: The integrated management program is an effective intervention for improving hemodialysis patients' self-efficacy, compliance with sick-role behaviors, and experience of symptoms. Additionally, it is considered an intervention with high clinical applicability and efficiency through video reproducibility.

가정방문을 통한 개별교육이 당뇨병환자 역할행위 이행에 미치는 영향과 교육효과의 지속에 관한 연구 (Effect of Individual Patient Teaching through Home Visiting on Compliance with Sick Role Behavior in Diabetic Patients and Duration of the Effect of the Teaching)

  • In order to determine the effect of individual patient teaching through home visiting on compliance with sick role behavior and the blood sugar level in diabetic patients, to determine if the effectiveness of the education was still present four year later and to inquire as to the effective time for a repeat education program this study was done through two quasi-experimental researches. The subjects consisted of 52 diabetic patients. The results of the study may be summarized as follows ; 1. Hypothesis I, in which the compliance with sick role behavior, the knowledge on diabetes and the health belief of the experimental group who received a diabetic education program will be higher than those of the control group who didn't receive the diabetic education, was supported by both studies in 1984 and 1988, confirming the effect on diabetic patients of the individualized education through home visiting ; In the 1984 study : Compliance(t=-11.7, p<.001) Knowledge(t=-5.41, p<.001) Health belief(t=-4.74, p<.001) In the 1988 study : Compliance(t=-4.85, p<.001) Knowledge(t=-2.85, p<.01) Health Belief(t=-2.99, p<.005) 2. The Hypothesis II, the blood sugar level of the experimental group will be lower than that of the control, was rejected in both studies, 1984 and 1988. 3. The Hypothesis III, the compliance, knowledge and health belief of the expermental group who received the education program in 1984 will not last till 1988, was supported in part, in compliance and health belief, but not in knowledge. In conclusion those who received the education program twice with an interval of 2 weeks, 4 years ago still had knowledge of diabetes but compliance and health belief had disappeared.

사회적지지 요법이 고혈압 환자 역할 행위 이행에 미친 효과의 지속에 관한 연구II (How Long the Effect of Social Support Would Be Continued for the Patients with Hypertension?)

  • The purpose of this study was to identify the effect of social support revealed in the time duration of sick role behavior compliance on the patients with hypertension using Quasiexperimental research design. Data collection was made through the interview survey technique from the hypertensive patients who received social support intervention (experimental group, n=41) and from those who were not exposed to the intervention(control group, n= 34). The subjects were registered in the cardiovascular outpatient clinic at the Chonnam National University Hospital from June 3, 1996 to November 30, 1997. $\chi^2$-test or t- test, Repeated measures ANOVA were utilized in the data analysis. The results were as follows: 1. The effect of social support intervention on sick role behavior compliance was significant in 1 month(F=69.17, p=.000), 6 months (F=11.51, p=.001), and 12 months(F=.07, p=.789) and between two groups(1 month; F=153.70, p=.000, 6 months; F=13.94, p=.000, 12 months; F=6.72, p= .011). 2. The effect of social support intervention on blood pressure was not significant through all the periods of time (F=1.21, p=.274) between the two groups(F=.12, p=.732). In conclusion, it was showed that social support had an effect on sick role behavior compliance and the effect of social support continued for twelve months(F= 10.03, p=.002) However, the score of compliance tends to decrease after 6 months of intervention. Therefore, this study indicated that social support re-intervention would be needed between six and twelve months.

관상동맥 중재술후 추적 관상동맥조영술 실천에 대한 조사연구 (A survey on Patients' Compliance with Follow-up Coronary Angiogram after Coronary Intervention)

  • Coronary intervention is now a well established method for the treatment of coronary artery disease. Coronary restenosis is one of the major limitations after coronary intervention. So medical teams advise the patients to get the follow-up coronary angiogram in 6 months after coronary intervention to know if the coronary artery stenosis recurs or not. This study was done in order to know how many patients complied with the advice, and to identify the relative factors to the compliance with getting the follow-up coronary angiogram. The subjects were 101 patients (male: 58 female: 22, mean age: $61{\pm}15$), who received coronary interventions from Jan. 1st to Mar. 31st 1997, and their data were collected from them by questionnaires one year after intervention. The questionnaires consisted of family support scale, self efficacy scale and compliance with sick role behavior scale. The result may be summarized as follows. 1. The number of patients who complied with getting the follow-up coronary angiogram were 37 people(36.6%) and did not comply with it were 64 people(63.4%). All scores of family support(t=5.56, p<.0001), self efficacy (t=4.13, p<.0001) and compliance with sick role behavior(t=5.66, p<.0001) were significantly higher in the patients who got the follow-up coronary angiogram than in those who did not get it. But there was not any relative factor in demographic variables (p>.05). 2. The major motivations for getting follow-up coronary angiogram were recurrence of subjective symptom(40.5%), the advice of medical team(32.4%), and fear of recurrence (27.1%). The restenosis rate in patients who got the follow-up coronary angiogram was 37.8%. 3. The restenosis rate was higher in the patients who had subjective symptoms than in those who did not have any subjective symptom. So subjective symptom and restenosis rate showed a high positive correlation(r=39.9, p<.001). However, 27.2% of the patients who did not have any subjective symptom showed coronary restenosis. 4. The reasons why they did not get the follow-up coronary angiogram were economic burden(37.5%), improved symptom(34.4%), busy life schedule(10.9%), fear of invasive procedure(9.4%), negative reaction of family member(3.1%), no helper for patient(3.1%) and worry about medical team's mistake (1.6%). The relative fators on compliance with getting the follow-up coronary angiogram after coronary intervention were family support, self-efficacy and Compliance with sick role behavior. And the most important reason why the patients did not get the follow-up coronary angiogram after coronary intervention was an economic burden.

퇴원교육 프로그램이 뇌혈관 수술환자의 역할행위 이행에 미치는 영향 (The Effect of Discharge Education Program on Compliance with Sick-Role Behavior in Cerebrovascular Surgery Patients)

  • Purpose: The purpose of this study was to examine the effect of the discharge education program on compliance with the sick role behavior for patients having undergone cerebrovascular surgery. Method: Research was done using a posttest only design. The subjects were 60 patients who were admitted to the neurosurgery unit at C.N.U. Hospital in G. City and were divided into the experimental and control groups. The discharge education program were intervened two times in the experimental group by the researcher; the first one was at the time of discharge using a booklet about knowledge related to disease and compliance, and the other one was a telephone education session after a week from discharge. Data were collected two times by interview and telephone using questionnaires from January 19, to June 10, 2000. The first one was at hospital before discharge, and the other one was one month later from discharge. Data were analysed by $\chi^{2}$, t-test, ANOVA, and Pearson's correlation. Results: The experimental group showed a higher score of compliance(t=2.772, p=.008) than those of the control group, but knowledge about CVD was not significant between the two groups(p>.05). Conclusion: The discharge education program was effective on the compliance of the patients having undergone cerebrovascular surgery.
