고혈압이 있는 재가노인의 낙상발생의 차이 및 영향요인 (Differences of Prevalence and Associated Factors of Falls in Community-Dwelling Older People with Hypertension)

  • Purpose: A comprehensive analysis of demographical, disease, functional status and fall risk related factors identified factors associated with falls in elderly hypertensive. Method: A descriptive research design was used. The participants were 124 persons aged 65 years or older registered at the community center in Daegu city. The data were collected from October, 2008 to February, 2009. Frequency, Fisher's exact test, $X^2$-test, t-test, and logistic regression were done using the SPSS V17.0. Results: Ninety (72.6%) subjects had experienced falls. The occurrence differed according to number of medications, activities of daily living and competence of vision. Logistic analysis revealed number of medication and impaired vision as independent risk factors for subsequent falls. Conclusion: Supportive nursing for the elderly needs to focus on dizziness and impaired vision to prevent falls in community-dwelling elderly with hypertension.

지역사회 거주 노인의 활동 특이적 균형자신감에 영향을 미치는 요인 (Factors Influencing Activities-specific Balance Confidence in Community-dwelling Old Adults)

  • Purpose: The purpose of this study is to identify factors influencing activities-specific balance confidence in community-dwelling older adults. Methods: This is secondary analysis of data from an intervention study for improving cognitive function. The data were collected from March 2 to September 30, 2017 at a senior center. Data of 131 older adults were included for this secondary analysis, and were analyzed by using t-test, ANOVA, and multiple regression. Results: The mean score of activities-specific balance confidence is 65.08 out of a possible range of 0-100. The significant factors affecting activities-specific balance confidence among old adults include 'more than 85 years old', 'waist circumference', 'depressive symptoms', 'activity restriction due to fear of falling', and 'self-rated health' which explained 52.8% of the variance. Conclusion: The study results indicate that psychologic factors as well as physical condition should be considered for interventions to increase activities-specific balance confidence.

지역사회 노인의 연령군에 따른 낙상예측요인 (Predictors of Accidental Falls in the Community-dwelling Elderly by Age)

  • Purpose: The purpose of this study is to identify risk factors of accidental falls by age in the Korean elderly. Methods: The study subjects included 10,242 community-dwelling elders aged 65 or over. The data were analyzed by logistic regression analysis. Results: The risk factors of accidental falls in the younger old group (aged 65~74) include fear of falling (OR=16.28, 95% CI: 12.03~22.03), depression (OR=1.24, 95% CI: 1.06~1.44), and hearing impairment (OR=1.20, 95% CI: 1.01~1.43). The risk factors of accidental falls in the older old group (aged 75~84) include fear of falling (OR=11.54, 95% CI: 7.04~18.93), muscle strength (OR=1.36, 95% CI: 1.15~1.60), number of chronic disease (OR=1.65, 95% CI: 1.09~2.50), and presence of spouse (OR=1.27, 95% CI: 1.06~1.51). The risk factors of accidental falls in the oldest old group (aged 85 years old or older) include fear of falling (OR=8.05, 95% CI: 1.85~34.98) and exercise (OR=2.91, 95% CI: 1.36~6.24). Fear of falling is defined as a common factor in the all age groups. Conclusion: Different strategies should be developed for age groups to prevent elderly falls with understanding of contributing factors of falls in each group.

경도인지장애 여성노인의 낙상두려움 예측 요인 (Predictors of the Fear of Falling among Elderly Women with Mild Cognitive Impairment)

  • Purpose: The purpose of this study was to identify the predictors influencing fear of falling in community-dwelling elderly women with mild cognitive impairment (MCI). Methods: A secondary data analysis was performed using data of 65 years or older elderly women with MCI participating in the 7th Korea Longitudinal Study of Ageing of the Korea Employment Information Service. The study subjects included 368 elderly women with MCI. For data analysis, descriptive statistics and logistic regression with complex samples were performed using IBM SPSS ver. 23.0. Results: 89.9% of the elderly women with MCI had fear of falling. There were significant factors such as religion (OR=8.85, 95% CI: 3.39~23.15), restriction of activity (OR=6.84, 95% CI: 2.14~21.90), depression (OR=0.75, 95% CI: 0.62~0.90), and MMSE (OR=1.30, 95% CI: 1.03~1.63), predicting fear of falling in community-dwelling elderly women with MCI. Conclusion: Differentiated strategies should be developed for elderly women with MCI to decrease fear of falling and prevent falls with understanding of contributing factors. This study will provide fundamental information on programming and a policy proposal related to fear of falling for elderly women with MCI.

지역사회 거주 노인의 근감소증 유병률, 인지기능과의 관계 및 중재연구 동향: 체계적 문헌고찰 (Current Research Trends on Prevalence, Correlates with Cognitive Function, and Intervention on Sarcopenia in Community-dwelling Older Adults: Systematic Review)

  • 본 연구는 국내 외 지역사회에 거주하는 노인의 근감소증과 인지기능에 관한 연구 동향을 파악하기 위해 수행된 체계적 문헌 고찰이다. 본 연구의 선정 논문은 국내 외 문헌 중 지역사회에 거주하는 노인을 대상으로 근감소증의 유병률 또는 근감소증과 인지기능의 상관관계를 조사하였거나, 근감소증 완화 목적으로 시행된 중재 연구로, 총 43편의 논문이 포함되었다. 국내 외 근감소증의 유병률은 총 25편의 논문에서 2.5-42.4%의 다양한 범위로 보고되었으며, 인지기능과의 관계를 분석한 6편의 논문 중 4편에서 근감소증과 인지기능 저하가 유의한 양의 상관관계로 보고되었다. 근감소증 개선을 위한 중재 연구 12편에서 근육량, 근력 또는 신체 기능 향상에 대한 중재 효과를 확인하였으나, 근감소증 결과지표로 인지기능 평가를 시행한 논문은 없었다. 지역사회거주 노인을 대상으로 근감소증을 예방 및 개선하고 효과를 평가하기 위해서는 표준화된 진단을 적용한 지역사회 내 근감소증 감시체계와 신체기능 뿐만 아니라 인지기능 평가를 포함하는 포괄적이고 지속적인 근감소증 관리체계가 필요하다. 또한 지역사회 기반 근감소증 예방을 위한 근거를 생산하기 위해 지속적인 중재연구의 개발 및 적용이 요구된다.

관절염 노인의 우울위험요인 (Risk Factors Influencing Depression among Elderly with Arthritis in Korean)

  • The purpose of this study was to identify the prevalence of depression and its affecting factors among a community dwelling elderly with Arthritis in Korea. A total of 4,050 subjects from the 2008 Korean National Older Adults Life Survey were selected. The data analysis was performed using the complex sampling function of SPSS version 20.0. Logistic regression analysis shows that depression was more prevalent as elderly who were uneducated, who had ADL limitation, who did smoke currently or formerly than had never smoked. The finding of prevalence of its risk factors is expected to promote the screening or prevention strategy for community dwelling elderly with Arthritis in Korea.

일본 초등학교시설의 발달 동향에 관한 연구 (A Study on the Development Trend of the Elementary School Facilities in Japan)

  • This research investigates the development trend of the elementary school facilities in Japan. To achieve this purpose, it is investigated that policies and the necessary area and the plans of 14 priorities at the elementary schools which have been built since mid 1970's in Japan. And plan compositions are analyzed focused on the classroom & multipurpose space and the concept of the plan. In this study, the elementary schools are analyzed as the following three specific features: 1. the countermeasure for the variety of the teaching methods, 2. the concept of dwelling space, 3. the concept of community center. The result of this study can be the fundamental data for a plan of the elementary schools in Korea in the future.

일개 지역사회 거주 노인의 의약품 오남용 실태 (A Study on the Status of Drug Misuse and Abuse among Community-dwelling Elders)

  • Purpose: This study was done to investigate the status of drug misuse and abuse in community-dwelling elders. Methods: The participants in this study were elders who were 65 years or over, and lived in ChungNam province. Descriptive statistics were used to analyze the data. Results: All of the participants reported taking both prescription and non-prescription drugs, and 78.4% used two or more kinds of drugs. Of the elders, 74.5% reported that they did not receive any education about drug misuse and abuse. The mean score for behaviors related to drug misuse was 8.76. Conclusions: Results indicate that many elders take medicine frequently, but they do not have any knowledge about drugs and possible side effects. This lack of knowledge might mean that they continue to use and misuse prescription and non-prescription drugs. It is important that elders be provided with precise information about medicines.

Age-Related Loss of Skeletal Muscle and Associated Risk Factors in Middle-Aged Men: A Comprehensive Study

  • PURPOSE: This study examined the specific clinical risk factors in middle-aged men with age-related loss of skeletal muscle mass (ALSMM). METHODS: The present research analyzed the data from a cross-sectional study of 1,564 community-dwelling participants aged between 40 to 49 years old. The participants were screened for ALSMM. The study examined various risk factors, including age, height, weight, body mass index, waist circumference, skeletal muscle mass index, smoking and drinking status, systolic and diastolic blood pressure, fasting glucose levels, and triglyceride and cholesterol levels. RESULTS: The risk factors of ALSMM were height, body mass index, waist circumference, skeletal muscle mass index, systolic blood pressure, diastolic blood pressure, drinking status, fasting glucose, and triglyceride levels (p < .05). The weight, triglyceride, and smoking status variables were non-significant (p > .05). CONCLUSION: The risk factors for ALSMM among community-dwelling adults were determined. These results are expected to contribute to the existing literature on ALSMM and provide potential risk factors associated with the development of ALSMM in middle-aged males.
