Comparison of Perceptions of Local Residents and Public Health Center Staffs on the Performance of Medical Service Improvement Plan in Rural Areas (농어촌 의료서비스 개선사업 성과에 대한 지역주민과 사업담당자간의 인식 비교)

  • Objectives: The purpose of this study is to compare the perceptions of the rural healthcare service improvement project' performance and reorganization of public health centers between project staffs and local residents. Methods: Data collection from this study was performed in 141 project areas using structured questionnaires. Data analysis was used in SPSS 22.0 version. Results: The public health center staffs were more positive about the items for improving health facilities than the local residents. Residents in the Si area generally perceived performance as more positive than residents in the Gun area, while public health center staffs in the Gun area perceived performance as more positive than public health center staffs in the Si area. Local residents expressed negative opinions about the reduction in the number of branches of public health clinics and health medical clinics. Conclusions: In conclusion, careful improvement projects for rural health care and the establishment of health care systems will be necessary, reflecting the opinions of local residents, along with a variety of regional characteristics.

The Effects of Customized Home Visiting Health Service in Gangwon-do - Focused on Hypertension and DM Clients - (강원도 맞춤형 방문건강관리 사업 효과 - 고혈압.당뇨 사례관리 중심 -)

  • Purpose: This study was to present the effects of home visiting health service on hypertension and DM clients. Method: The one-group pretest-posttest design was applied to 127 hypertension and 116 DM clients. To evaluate the effects of visiting nursing service, biological indexes and self-care abilities were measured from April to July, 2007. Result: Biological indexes and self-care abilities were improved in the hypertension and DM clients who received visiting nursing service. Especially, blood pressure and fasting blood sugar showed a dramatic decrease. Conclusion: Home visiting health service by public health centers is considered to be an effective nursing service in Gangwon-do. Therefore, case management is demanded not only for hypertension and DM but also for other chronic diseases.

  • According to the 2005 Korean census, the 65 and over population now exceeds 9.5% of the total population and is growing rapidly. Meeting elderly environment and care needs of this rapidly growing segment of the population becomes a major challenge for public policies and planners. Since great deal amount of elderly will reside in their houses, aging in place concept becomes important. For the success of aging in place, the quality of individual house unit, community support systems, and quality of senior center of the community are crucial. Since elderly environments and facilities serve not only medical and/or care programs but also social activity program in aging society, senior centers need to promote social activities and other care programs. The purpose of this study is to analyze characteristics of environmental affordance of a well received senior center in Tokyo, Japan. In order to analyze the characteristics, Murtha & Lee user benefit criteria and Lawton's environmental affordance approaches were used. As results, design characteristics and interior elements which provide environmental affordance were enumerated by type of space. Based on needs and user benefit criteria, those features were analyzed. This study shows design characteristics, elements, and attributes which are well received and utilized by elderly users.

The Effects of Early Childhood Teachers' Civic Consciousness on Actions for Building an Community: The Mediating Effect of Innovative Culture (유아교사의 시민의식이 공동체 구현 실천 행동에 미치는 영향에서 혁신문화의 매개 효과)

  • Objective: The purpose of this study is to examine the mediating effect of innovative culture in early childhood education institutions on the relationship between teachers' civic consciousness and actions for building a community in order to obtain methodological information for implementation at a time when the need to build an education community has been raised. Methods: The data were collected through 275 early childhood teachers working in kindergartens and daycare centers Then, descriptive statistics, correlation analysis, and regression were conducted. Results: All three variables civic consciousness, innovative culture, and actions for building a community showed a significant positive correlation. As a result of analyzing the mediating effect of innovation culture, it was found that the indirect effect of teachers' civic consciousness on the actions for building a community through the innovative culture was found to be statistically significant. Conclusion/Implications: This study was found that it is important to strengthen the teacher's civic consciousness and to create an innovative culture of early childhood education institutions in order to promote actions for building an educational community of teachers. This study is meaningful in that it provided implications for the direction of teacher education and formation of an innovative culture in early childhood education institutions for the purpose building an educational community.

Development and Analysis of Community Based Independent Home Care Nursing Service (지역사회중심의 독립형 가정간호 시범사업소 운영체계 개발 및 운영결과 분석)

  • The purpose of this study was to develop the framework of community-based home care nursing delivery system, and to demonstrate and evaluate the efficiency of it. The study was carned out over a period of 3years from September 1996 to August 1999. The researchers developed Standards for operations, this was all aimed toward a home care recording system, and an assessment intervention algorithm for various diseases quality control and standardization. In the center, 185 patients enrolled, and of the enrollments cerebrovascular disorder and cancer were the most prevailment diseases. Also, a home care nursing activity classification was developed in six domains. Those domains were assessment, medication, treatment, education and consultation, emotional care, and referral or follow-up care. Ten sub-domains were divided according to the systematic needs. Among these nursing activities, treatment, assessment, and education and consultation were frequently performed. In sub-domain classification, skin integrity, respiration, circulation, and immobility related care were provided most frequently. The cost of home care nursing per visit was also suggested. The cost include direct and indirect nursing care, management, and transportation cost. Also, the researchers tried to overcome the limitations of hospital-based home care to provide more accessible, efficient, safe, and stable home care nursing. Therefore, clients were referred from other patients, families, public health care centers, industries, and even hospitals. As a result of this study, several limitations of operation were found. First, it was difficult to manage and communicate with doctor in the emergency situations. Second, there was too much time spent for transportation. This was because they are only five nurses, who cover all of the areas of Seoul and nearby cities. Third, preparation for special care of home care nurses was lacking. Fourth, criteria for the termination of care and the frequency of home visits were ambiguous. Finally, interconnection with home care machinery company was so yely needed. New paragraphs' strategies for solving these problems were suggested. This study will be the basis of community-based home care nursing, and the computerized information delivery system for home care nursing in Korea.

Primary Health Care and Desirable Policy Directions in Korea (1차보건의료와 바람직한 정책방향)

  • The World Health Organization and its member states, in 1978, declared that primary health care is a key to attain the goal of Health for All by the goal of Health for All by the yeas 2000. As a member state of WHO, the Republic of Korea has participated in the declaration of ALMA-ATA and committed to put national efforts for devedoping and implementing primary health care approach with the spirit and content of this Declaration. Since 1978, to translate the spirit of the Declaration into realization, Korean goverment has developed a new category of health manpower such as Community Health Practitioners serving people living in remote rural areas and Village Health Workers serving voluntarily their own village, strengthened the function of Health Centers and Health Subcenters through their reorientation and improved the infrastructure by their new construction or renovation. While primary health care is viewed as an essential health care in Korea, there are some circles who follow a narrow definition in referring to the health care at the periphey of a health system, which is erroneous. Considering the PHC is accepted as the best alternative approach to health care to solve problems that modern health systems are facing, we propose the followings as desirable health policy directions that modern health systems are facing, we propose the followings as desirable health policy directions which might translate the persopective into action at the national level after reviewing past and current PHC approach in Korea : 1. To improve the equity through the reduction of gaps between those who have access to health care and those who have not. 2. To reinforce multisectoral approach and intersectoral coordination through the re- establishment of the National Health Council or establishment of equivalent organization at the central level. 3. To stengthen community participation through lacal people's empowerment by leadership training, changing planning process from the top-down approach to bottom-up and giving the priority to human resources rater than technology, 4. To reinforce the Ministries of Health and Social Affairs through upgrading its role and function to Coordinate Ministries which involve human welfare policies, and creating a Division which is in charge of PHC in the Ministry.

Analysis of the utilization of health promotion program in health care centers based on a socio ecological model (사회생태모형에 기초한 보건소 건강증진 프로그램 이용 분석)

  • Purpose: The research was performed to distinguish the group of users of the health promotion programs in health care center, to identify the characteristics of this group, and to investigate the factors that have influence on the use of the health programs. The specific research purposes are: First, to compare the group which uses the health promotion program of health care center and that which does not. Second, to analyze the factors that have influence on the use of the health promotion program. Method: This study was done from November 15th 2006 and March 30th 2007. The study subjects were Seoul and Provinces. Seoul was divided into 4 areas of eastern western, northern, and southern area. Provinces were Gimchon, Gumi and Sosan. From each area, one health care center was chosen from the ones that wanted to participate. Total number of observations was 994. The survey questionnaire consisted of individual, interpersonal, organizational, community, policy factors based on socio-ecological model. The analysis method was logistic regression. Results: Odds ratios of individual factors is sex(1.39), age(1.05), marriage status(1.71) severity(1.20). Odds ratios of interpersonal factors is use and support of family(1.96), use and support of resource persons like friends and neighbor(4.58). Odds ratios of organizational factors is the comfortness of space(0.74), the satisfaction of health care center facility(1.40), kindness of employ(1.97). Odds ratios of community factors is health care center program recognition(1.70). Odds ratios of policy factors is advertisement(4.69) and expense(1.42). Conclusion: Socio-ecological model of health promotion in health care center is obtained based on five factors and health planners should consider these determinants of health promotion program in health care center and develop intervention methods.

A Study on the Recognition of Client Home Visit Nursing Care Services in Public Health Centers (방문간호사업에 대한 대상자의 인식에 관한 연구)

  • In this, analyzing the type of subjectivity in which people would have about home visit nursing services originating from public health care centers. I tried to research more effective ways to improve home visit nursing care services. and later. for the development of home visit nursing care. to supply basic data. The method for this study was the Q-method. created by William Stephenson. and was adequate for the study of subjectivity. For this study. through the deep interview. literature inquiry, and the discussion course. 206 Q-statement sentences were abstracted. and based on them, after Q-sample-selection. I then collected the Q-categorized-result from 32 subjects from Mar. 10. 2000 to Mar. 25. 2000. Through the statistic a analysis of PC-Qunal program. the subjectivity species were categorized and analyzed. The study results show that there are 3 sorts of recognition types. and they are analyzed in the following; The first type: the positively receiving type shows that they feel thankful and a trusting feeling about home visit nursing. The second type: the negatively mistrusting type shows that they had doubtful attitudes about the specialty of home visit nursing: they wanted medicine or nutrition remedies rather than health education and concerning the their own health care, they prefered the hospitals or clinics. The third type: the conditional receiving type shows that even though they had a positive receiving attitude about home visit nursing wanting to consult with the home visit nurses about the difficult problem which could not easily be settled, hoping that the home visit nurses could visit them more often, in their actual lives. they strongly indicated their attitudes concerning money as more important than home visits. The subjects in these 3 types commonly had a good feeling about the kindness of the home visit nurses: the first and third types also had a positive recognition about home visit nursing; however. in aspects of the evaluation and receiving attitudes, they showed a big difference. When all the above results are integrated. in the case of the first type the home visit nursing service, which satisfied the demand for health care of the medically weak people. should be continuously supplied. Additionally in case of the second type (negatively mistrust). continuous education and support should be supplied with enough interest to lead their concerns about their own health care as well as lead medical spending in a productive and effective direction in order to change their impressions. Through this study. I learned that the recognition of the objectives of home visit nursing services can be categorized in to 3 types and could be analyzed. Thus I wish that this study helps to present basic data which contributes to the development of the home visit nursing field.

An Exploration and Comparison of Infant Feeding Practices in Home and Center Contexts

  • This study compared parents' and teachers' feeding practices with young children. Parents and teachers of children aged 0-3 years were recruited at 24 child care centers to complete surveys regarding their demographic characteristics, parenting styles, and feeding practices with young children. Respondents included 106 parents and 102 teachers. Participants' feeding beliefs and values were found to be related to their parenting style classifications (i.e., Authoritative, Authoritarian, or Permissive), ethnicity, income, and other demographic characteristics. Findings indicate the need for teachers and parents to begin communicating about their longterm goals for a child's development as soon as the child enters care. Understanding the goals and variation of feeding practices used at home and at school can help teachers and parents begin to construct a shared vision for care.
