Purpose: To assess the safety and efficacy of a gemcitabine plus docetaxel regimen as a second line therapy for patients with advanced soft tissue sarcoma (STS) resistant to doxorubicin and ifosfamide-based therapy. Patients and Methods: Medical records of 64 patients with advanced STS who received gemcitabine plus docetaxel regimen as a second line treatment between May 2006 and June 2011 were examined. All patients had been previously treated with doxorubicin plus ifosfamide-based regimen at first line setting. Patients received gemcitabine 900 $mg/m^2$ on days one and eight intravenously over 90 minutes, followed by docetaxel 75 $mg/m^2$ on day eight intravenously over one hour. Cycles were repeated every 3 weeks. Results: The male-to-female ratio was 37/27 and the median age was 44 years (range; 19-67 years). Objective responses were observed in 13 (20.3 %) patients (2 CR, 11 PR) and stable disease in 21 (32.8 %). Total clinical benefit (CR+PR+SD) was observed in 34 (53.1 %). Median overall survival (OS) was 18 months (95% confidence interval (CI):12.1-23.9) and Median time to progression (TTP) was 4.8 months (95% CI: 3.6-6). A total of 243 cycles of chemotherapy were administered. The median number of cycle was 3 (range;1-11). The most common grade 3-4 hematologic toxicity was neutropenia (35.9 %). The most common nonhematologic toxicities consisted of nausea/vomiting (37.5 %), mucositis (32.8 %), peripheral neuropathy (29.7%), and fatigue (26 %). There was no toxicity-related death. Conclusion: The combination of gemcitabine plus docetaxel is an active and tolerable regimen as a second line therapy for patients with advanced soft tissue sarcoma who have failed doxorubicin and ifosfamide-based therapy.
In this paper, a new classification method based on the combination of semi-supervised learning with spatial similarity of adjacent pixels is presented for crop classification in inaccessible areas. Iterative classification based on semi-supervised learning is applied to extract reliable training data from both the initial classification result with a small number of training data, and classification results of adjacent pixels are also considered to extract new training pixels with less uncertainty. To evaluate the applicability of the proposed method, a case study of the classification of field crops was carried out using multi-temporal Landsat-8 OLI acquired in the Daehongdan region, North Korea. From a case study, the misclassification of crops and forests, and isolated pixels in the initial classification result were greatly reduced by applying the proposed semi-supervised learning method. In addition, the combination of classification results of adjacent pixels for the extraction of new training data led to the great reduction of both misclassification results and isolated pixels, compared to the initial classification and traditional semi-supervised learning results. Therefore, it is expected that the proposed method would be effectively applied to classify areas in which it is difficult to collect sufficient training data.

Background: The purpose of this study was to analyze the predictive value of neutrophil/lymphocyte ratio (NLR) to better clarify which patient groups will benefit the most from particular treatments like bevacizumab. Materials and Methods: A total of 245 treatment-naive metastatic colorectal cancern (mCRC) patients were retrospectively enrolled and divided into 2 groups: 145 group A patients were treated with chemotherapy in combination with bevacizumab, and 100 group B patients were treated as above without bevacizumab. Results: Group A patients had better median overall survival (OS) and progression-free survival (PFS) (24.0 and 9.0 months) than group B patients (20 and 6.0 months) (p=0.033; p=0.015). In patients with low NLR, OS and PFS were significantly longer in group A patients (27 vs 18 months, p=0.001; 11 vs 7 months, p=0.017). Conclusions: We conclude that NLR, a basal cancer related inflammation marker, is associated with the resistance to bevacizumab-based treatments in mCRC patients.

This paper presents an algorithm based on a combination of Discrete Wavelet Transforms and neural networks for detection and classification of internal faults in a two-winding three-phase transformer. Fault conditions of the transformer are simulated using ATP/EMTP in order to obtain current signals. The training process for the neural network and fault diagnosis decision are implemented using toolboxes on MATLAB/Simulink. Various cases and fault types based on Thailand electricity transmission and distribution systems are studied to verify the validity of the algorithm. It is found that the proposed method gives a satisfactory accuracy, and will be particularly useful in a development of a modern differential relay for a transformer protection scheme.
An expectation-maximization (EM) based Bayesian adaptation method for the mean of noise is proposed for noise-robust speech recognition. In the algorithm, the on-line testing utterances are used for the unsupervised Bayesian adaptation and the prior distribution of the noise mean is estimated using the off-line training data. For the noisy speech modeling, the parallel model combination (PMC) method is employed. The proposed method has shown to be effective compared with the conventional PMC method for the speech recognition experiments in a car-noise condition.
Some pattern recognition methods have been used to characterize Korean ancient porcelainsherds using their elemental composition as analyzed by instrumental neutron activation analysis. A combination of analytical data by means of statistical linear discriminant analysis(SLDA) has resulted in removal of redundant variables, optimal linear combination of meaningful variables and formulation of classification rules. The plot in the first-to-second discriminant scores has shown that the three distinct territorial regions exist among porcelainsherds of Kyungki, Chunbuk-Chungnam, and Chunnam, with respective efficiencies of 20/30, 22/27 and 14/15. Similar regions have been found to exist among punchong porcelain and ceradonsherds of Kyungki, Chungnam and Chunbuk, with respective efficiencies of 7/9, 15/16 and 6/6. Classification has been further attempted by statistical isolinear multiple component analysis(SIMCA), using the sample set selected appropriately through SLDA as training set. For this purpose, all analytical data have been used. An agreement has generally been found between two methods, i.e., SLDA and SIMCA.
We propose a network traffic prediction model based on linear and nonlinear model combination. Network traffic is modeled by an autoregressive moving average model, and the error between the measured and predicted network traffic values is obtained. Then, an echo state network is used to fit the prediction error with nonlinear components. In addition, an improved slime mold algorithm is proposed for reservoir parameter optimization of the echo state network, further improving the regression performance. The predictions of the linear (autoregressive moving average) and nonlinear (echo state network) models are added to obtain the final prediction. Compared with other prediction models, test results on two network traffic datasets from mobile and fixed networks show that the proposed prediction model has a smaller error and difference measures. In addition, the coefficient of determination and index of agreement is close to 1, indicating a better data fitting performance. Although the proposed prediction model has a slight increase in time complexity for training and prediction compared with some models, it shows practical applicability.
The purpose of this study is to find application of combination of insotonic(CI) in clinical approach. This is a literature study with books and clinical experience. CI was developed after Margaret Knott that is very useful in clinically. CI has two lever system. It has class II lever system for eccentric contraction and class III lever system for concentric contraction. Therefore it make two different contraction that is not only magnitude of motion and speed increase of motion but also biomechanical advantage. Application of CI has much variety by patient position and treatment position. Especially, it is very effectable on mat activities and training for mid stance period. CI apply in supine position on treatment table, mat or high mat activities, bridging activities, all fourth activities and sit on chair activities. CI apply serially with different techniques. These are rhythmic initiation, dynamic reversal, approximation, rhythmic stabilization and replication.
