Telemedicine Protocols for the Management of Patients with Acute Spontaneous Intracerebral Hemorrhage in Rural and Medically Underserved Areas in Gangwon State : Recommendations for Doctors with Less Expertise at Local Emergency Rooms

  • Previously, we reported the concept of a cloud-based telemedicine platform for patients with intracerebral hemorrhage (ICH) at local emergency rooms in rural and medically underserved areas in Gangwon state by combining artificial intelligence and remote consultation with a neurosurgeon. Developing a telemedicine ICH treatment protocol exclusively for doctors with less ICH expertise working in emergency rooms should be part of establishing this system. Difficulties arise in providing appropriate early treatment for ICH in rural and underserved areas before the patient is transferred to a nearby hub hospital with stroke specialists. This has been an unmet medical need for decades. The available reporting ICH guidelines are realistically applicable in university hospitals with a well-equipped infrastructure. However, it is very difficult for doctors inexperienced with ICH treatment to appropriately select and deliver ICH treatment based on the guidelines. To address these issues, we developed an ICH telemedicine protocol. Neurosurgeons from four university hospitals in Gangwon state first wrote the guidelines, and professors with extensive ICH expertise across the country revised them. Guidelines and recommendations for ICH management were described as simply as possible to allow more doctors to use them easily. We hope that our effort in developing the telemedicine protocols will ultimately improve the quality of ICH treatment in local emergency rooms in rural and underserved areas in Gangwon state.

일부 치위생과 재학생의 학과선택 만족도 및 취업의식변화 (Satisfaction with Department Selection and Changes in a Sense of Employment Among Some Students Majoring in Dental Hygiene)

  • The purpose of this study was to examine and analyze motivation of entrance into the college, knowledge prior to the entrance, satisfaction with their major, and a sense of employment after graduation among students majoring in dental hygiene at some colleges and subdivide them by grades for comparative analysis to determine the progress of changes in general characteristics, department satisfaction, and a sense of employment among students majoring in dental hygiene for the recent three years and provide basic data for improving dental hygienic education. Questionnaires were distributed to 520 students majoring in dental hygiene at some college in South Chungcheong, South Jeolla, and North Jeolla Provinces, followed by explanation of the purport of the research and the content of the questionnaire; after completion, 513 copies were used for analysis with exception of 7 copies with insincere responses. The analysis of the data examined resulted in the following conclusions: First, Changes in Department Satisfaction among Students Majoring in Dental Hygiene As for motivation of entrance into a college, less and less students chose the department of dental hygiene through recommendation by parents or seniors while more and more students chose their major due to easy employment. Less and less students obtained information on the department from a college application guidebook while more and more students obtained it through Internet. There was also a gradual increase in prior knowledge about the department and in satisfaction with their major. More and more students had no will to change their course. Second, Changes in a Sense of Employment among Students Majoring in Dental Hygiene There was a gradual decrease in the number of students who would follow friends' or their own decision in selecting workplace but an increase in the number of those who would follow their professors' opinion. There was a gradual increase in the number of students who wanted to be employed in workplace with good social perception; many students wanted to be employed in a personal dental clinic, a general hospital, or a public health center; and more and more students also wanted to be employed in a school dental health center or to become a government official or an educator.

사상체질진단설문지(四象體質診斷設問紙)의 문항(問項) 개정(改定)에 대한 연구(硏究) (Revision of the Questionnaire for the Sasang Constitution Classification)

  • Background In Sasang Constitutional Medicine (SCM), human beings are classified into four constitutions: Taeyangin, Taewnin, Soyangin, and Soeumin. SCM presents various lifestyles and treatments for each constitution. The problem of SCM is that its diagnosis is so subjective that the result of the diagnosis depends on the doctor. In order to solve this problem, multiple approaches have been objectively researched. To use a questionnaire is the most common method among the different approaches. Since there are a few problems in the Questionnaire for the Sasang Constitution Classification II (QSCC II), which in recent days has been used frequently, the necessity of a new questionnaire has arisen. Objective The purpose of this study is to develop a new questionnaire for the objectivity of the Sasang Constitution Classification by complementing the existing questionnaire. Methods In order to search for the existing questionnaire's problems, I first compare the ratio of the amount of questions regarding external appearance to that of questions of mental characteristics and physiologic & pathologic symptoms in QSCCII and QSCCII +. We created the new questionnaires by reinterpreting ${\ulcorner}Dongyi$ Soose $Bowon{\lrcorner}$ through SCM professors' and medical residents' discussion and by adding some items related to the diagnosis about the constitution from other SCM books and clinical experience. Results 1. The new questionnaire for the Sasang Constitution Classification is self-reported questionnaire based on a multiple choice system of 4 questions. 2. The new questionnaire for the Sasang Constitution Classification consists of 28 questions, which include 7 questions about external appearance, 4 questions about mental characteristics, and 17 questions about physiologic & pathologic symptoms. 3. The new questionnaire for the Sasang Constitution Classification consists of 24 questions of Taeyangin, 28 of Taeumin, 28 of Soyongin, 28 of Soeumin. 4. We will have to verify validity and reliability of new questionnaire for rhe Sasang Constitution Classification.

PBL/S-PBL 패키지 평가도구 개발 및 평가 : 보건계열(학)과와 간호(학)과를 중심으로 (Development and Evaluation Frameworks for PBL/S-PBL Packages: in the case of the Dept. in Allied Health Professionals and Nursing)

  • 본 연구의 목적은 PBL/S-PBL에 사용되는 패키지(교재)를 측정할 수 있는 타당하고 신뢰할 수 있는 평가도구를 개발하는 것이다. 이러한 목적을 실현하기 위해 문헌 고찰을 통하여 PBL 및 S-PBL 패키지를 측정하는 도구의 3항목 15문항의 예비문항들을 만들었다. 예비문항을 대학의 교수 9인의 전문가를 통해 내용타당도를 검증하여 PBL은 14문항, S-PBL은 15문항으로 구성하였다. 개발된 평가도구의 구성 타당도 검증을 위하여 요인분석을 하였다. PBL은 동기부여 3항목, 통합성 7항목, 체계성 4항목, 총 14항목으로, S-PBL은 동기부여 4항목, 통합성 6항목, 체계성 5항목, 총 15항목으로 이루어진 평가도구가 제시되었으며 Cronbach's $\alpha$계수가 PBL은 0.9112, S-PBL은 0.9166으로 적절한 신뢰도를 갖는 평가도구로 검증되었다.

한의과대학 본초학 교육과정의 개정 및 보완을 위한 설문조사 연구 (Survey on Revision and Complements for the Current Curriculum of Herbology)

  • Objects: This study was conducted to investigate the current educational environment of herbology and to develop a future-oriented curriculum for oriental medicine. The questionnaire used in this research was drawn up based on the current curriculum referring to the current curriculum of herbology and pharmacognosy. Methods: The survey was carried out presenting the questionnaires to a total 12,754 of the students and doctors of oriental medicine through e-mailing five times; of these, 2,074 replied. Results: 1. Among the respondents, about 97% agreed that it was necessary to revise and complement the current curriculum of herbology. 2. The respondents felt that the assigned lecture time of subject was "sufficient" (19%), "insufficient" (39%) and "average" (39%), respectively, and the level of lecture was "insufficient" (37%) or "average" (43%) respectively. According to priority, it showed that the contents which needed complement in lecture were discrimination of medicinal herbs (24%), practical use of action and indications (23%), and correlation with modern disease (21%). In theoretical lectures, 69% of the respondents agreed on the introduction of natural scientific methods 3. In practice, 51% of the respondents replied that the lecture time for practice was insufficient. The contents which needed to be complemented in practice were as follows: audio-visual materials for discrimination of medicinal herbs (22%), concrete exercise for the processing of medicinal herbs (21%), and attempts for the objective discrimination of medicinal herbs using instruments (microscope, analytical instrument, residual pesticide, heavy metal, genetic analysis) (16%). 70% replied that the discrimination of medicinal herbs of high price and rarity was "none or insufficient". 4. 56% replied that it was necessary to introduce and practice physicochemical analysis, and they showed higher requests according to the increase of their educational level. However, 86% replied that they had never experienced concrete attempts for objective discrimination of medicinal herbs, which seemed to indicate that, excepting some schools, practice exercise was rarely performed. Conclusions: According to results, it seems that an urgent review on the current course of herbology and a workshop on the process of experimental practice for professors is needed.

보건계열 대학생의 자아존중감과 스트레스 대처방안 (Self-Esteem and Stress-Coping Strategies of the College Students in the Department of Public Health)

  • 본 연구는 보건계열 대학에 재학 중인 대학생의 자아존중감, 스트레스 대처방안 간의 관계를 파악하여 학교생활과 진로지도에 관한 프로그램 개발을 위한 기초자료로 활용하고자 시도하였다. 호남지역에 위치한 6개의 보건계열 학과에 재학중이며, 연구 참여에 동의한 대학생 244명을 대상으로 실시하였다. 자료수집은 자아존중감과 스트레스대처 방식을 포함한 구조화된 설문지를 활용하여 자기기입식 응답법으로 2008년 11월 1일부터 12월 21일까지 하였다. 연구결과는 일반적 특성들 중 자아존중감과는 종교가 있는 경우와 교우관계가 매우 원만한 경우에 높은것으로 나왔으며, 적극적 스트레스 대처방안과는 학년과 어머니의 교육정도가 높을수록, 소극적 스트레스 대처방안과는 종교가 있는 경우와 군지역에 거주하는 경우에 적극 활용한 것으로 나왔다. 적극적 스트레스대처(문제중심과 사회적 지지)가 잘 될수록 자아존중감이 높으며, 소극적 대처(정서완화와 소망대처)가 잘 될수록 자아존중감이 높은 것으로 나와, 대학생들의 전반적인 스트레스 대처방안이 높을수록 자아존중감이 높음을 나타냈다. 결론적으로 보건계열 대학생들이 학업과 실습, 취업 등에 대한 스트레스 대처방안을 모색 할 있도록 자아존중감 향상 커리큐럼을 대학 정규 교육과정에 마련하고 학생 상호간의 유대관계 향상과 지도교수와 학생간의 정서적 교류체계를 형성할 수 있는 프로그램이 대학생활 적응에 도움이 되리라 사려된다.

전문대학 안경사 양성제도 및 안경광학과 교육과정 개선방안 (Optician Training System at a Professional School Education and Improvement Plan of Curriculum in the Department of Optical Science)

  • 바람직한 안경사 양성을 위해서 전문대학 안경광학과 교육과정의 문제점을 분석하고 다음과 같은 결론을 얻었다. 안경사 양성기관은 단기방안으로 현행 전문대학 2년제 안경광학과에서 3년제로 연장하고, 장기적으로는 4년제 안경광학과를 설치하여야 한다. 국가시험은 과목별 반영비율을 재조정하고 콘택트렌즈학을 추가해야 하며, 시험형태는 현행 객관식 지필고사에다 필기가공테스트를 추가해서 국가시험제도를 강화해야 한다. 전문대학 안경광학과 교육과정은 실험실습이 강화된 형으로 재구성되어야 한다. 즉 특수안경 조제, 포롭터 검사법 등의 실험실습 과목이 확대되어야 하고 임상실습에는 상품지식 등이 포함되어야 하며 이론과 실습비율도 5:5로 하는 것이 합리적이다. 안경광학과 교수구성은 학생 40명당 2명 이상으로 증원하여야 하며 안경광학 전공교수, 보건의학 전공교수, 안경사 출신교수 등으로 교수진을 구성해서 비전공 교수들에 의한 강의가 이루어지지 않도록 하는 것이 바람직하다. 실험실습은 과목을 확대하고 실습시간과 실습장소, 기자재, 실습조교 등의 확보 또는 채용이 필요하다. 임상실습은 실습내용에 상품지식, 경영실무 등이 포함되는 것이 바람직하며 실습평가자의 자격도 전문대 출신 이상의 안경사가 하는 것이 합리적이다.

전문대학(專門大學)의 방사선과(放射線科) 교육(敎育)에 관(關)한 조사연구(調査硏究) (Investigation on Education of Radiological Technology in Junior College)

  • The result of this study from surveying and analyzing the present situation of educational institutes of radiologic technology, educational situation of radiologic technology, and students' attitude and recognition about the education of radiologic technology, the faculty's thought of it throughout the country from October, 1981 to April, 1982 is as follows: 1. The total number of educational institutes for radiologic technology is 11, but all of them are private colleges classified by foundation and 1,414 freshmen were admitted to enter the colleges by admission quotas of students at present in March, 1982. 2. The number of male(1965-1982) is 3,046(78.0%), that of female is 856(22.0%), and total of them is 3,904 that graduated from the departments of radiologic technology in 9 junior colleges in Korea. 3. The number of the instructional staffs in radiologic technology in Korea is 32, but among them 1 professor, 1 associate professor, 16 assistant professors and 14 full-time instructors. 4. The credits for graduation are 80-96 credits and students need required liberal arts and required major studies to get the qualification for graduation. 5. In the national examination for radio-technologist license, total 7,150 persons took the examination, but only 2,796 persons passed it, therefore, the average passed rate is 39.1% from the year 1965 to 1982. 6. 61.3% of students expressed dissatisfaction about their campus lives, the reason of which is mainly poor educational facilities(26.7%), insufficient number of faculty(21.2%) and negative Recognition on junior college from society, etc. 7. About the education of their major studies of colleges, 70.9% of students answered that they were short of laboratory works and clinical trainings rather than theoretical lessons. 8. The chief problem in practice works is about facilities and equipemtns for practice (students 39.1%, faculty 68.8%), and about the fact that too many students usually work at a time(students 17.1%, faculty 21.9%), etc. 9. 87.4% of students and 93.8% of faculty answered that clinical training in school is necessary. 10. Students' viewpoints on the radiologic technologist and the attitudes on the profession as a radiologic technologist are approximately affirmative. 11. Fields of employment to advance after graduation are mainly the fields related to their major studies(hospital, clinic, health clinic, industrial companies related and military officers of medical technology, etc.), which fields arc predominant to 74.5% rather than non-related fields(enrols into 4 year colleges, etc.). 12. 85.2% of students, 87.5% of the faculty expressed their opinions that the present academic years of 2 should be lengthened more. 13. 90.7% of the faculty expressed their opinions that the present students quotas should be reduced in whole or in part.

간호시뮬레이션에서 나타나는 임상판단과정 분석 (Analysis of Students' Clinical Judgment Process During Nursing Simulation)

  • 본 연구는 간호 학생을 대상으로 아동 간호학 실습 교육의 시뮬레이션 상황에서 일어나는 임상적 판단과정을 파악하고자 한다. 이를 위해 LCJR을 적용한 기술적 서술 연구(descriptive study)로 진행되었다. S시에 소재한 2개의 대학의 간호대학생을 대상으로 2013년 9월부터 2014년 12월 까지 모의 환자를 포함한 발열 시나리오와 무호흡 시나리오를 사용하여 시뮬레이션이 진행되었다. 본 연구 결과 간호 대학생의 임상판단 점수는 남학생 30.50(${\pm}2.77$)점, 여학생29.32(${\pm}2.67$)점이었으며, 무호흡 시나리오 임상판단 점수($31.20{\pm}2.84$)가 발열 시나리오 점수($28.71{\pm}2.63$)보다 높았고, 학습자의 자가평가(.911)와 교수자의 평가(.921)에서 반응(responding)영역이 유의한 상관관계가 있는 것으로 나타났다. 교수자와 학습자의 자가 평가 간 비교에서는 해석(interpreting)영역과 반영(reflecting)영역이 유의하게 나타났다. 본 연구는 간호 대학생의 임상 판단력을 높일 수 있는 실습과정을 수정 및 개발하는데 있어 기초 자료가 될 수 있다. 이 후 시뮬레이션 프로그램 내에 디브리핑 동안에 학생들의 간호 수행과 판단을 토론할 수 있는 경험을 제공하는 것이 필요하다.

국내 표준 임상치위생실무지침의 개발 및 타당성 평가 (Development and evaluation of standards for clinical dental hygiene practice)

  • Objectives: The purpose of this study was to develop applicable standards for clinical dental hygiene practice in Korea and to evaluate their validity. Methods: Based on the standards for clinical dental hygiene practice developed in the United States and Canada, the standards were adapted to be applicable in Korea. The validity of the standards was evaluated by a self-writing questionnaire among 14 professors and 10 clinicians using a developed tool for evaluating the standards. A focus group interview was additionally conducted for clinicians to increase the validity of the standards. Descriptive statistics and Mann-Whitney test were performed using SPSS 25.0. To analyze the content of the focus group interviews, content analysis was conducted. Results: The standards for clinical dental hygiene practice consisted of five elements of professionalism for dental hygienists and a total of 28 items to perform the five stages of dental hygiene process of care (assessment, diagnosis, planning, implementation, and evaluation) and included conceptual meaning, clinical significance, and application methods with examples for each item. Conclusions: The standards for clinical dental hygiene practice developed in this study could contribute to standardizing dental clinical practices provided by dental hygienists. It is necessary to consistently improve the standards that are highly practical, to prevent oral diseases and maintain oral health of the public, based on the results of this validity evaluation.
