Objective: Glypican-3 (GPC3) is reported to be an oncofetal protein that is a useful diagnostic immunomarker for hepatoblastoma. However, the results are not inclusive. This study systemically investigated the association between expression of GPC3 and pediatric hepatoblastoma. Methods: Clinical studies evaluating the association were identified using a predefined search strategy. GPC3 immunohistochemistry was applied in the pathological diagnosis of hepatoblastoma using the monoclonal antibodies with formalin-fixed and paraffin-embedded specimens. Positive predictive rates for the association between expression of GPC3 and pediatric hepatoblastoma were calculated. Results: Specimens from four clinical studies which including 134 patients with pediatric hepatoblastoma tested by GPC3 immunohistochemistry were considered eligible for inclusion. Systemic analysis showed that, in all patients, pooled positive predictive rate of the association between expression of GPC3 and pediatric hepatoblastoma was 95.5% (128/134). Conclusion: This systemic analysis suggests that the expression of glypican-3 is highly associated with the diagnosis of pediatric hepatoblastoma.
1. Purpose The accurate diagnosis and proper treatment of group A streptococcus should be emphasized concerning about possible development of late sequelae, such as acute rheumatic fever and acute glomerulonephritis. We would like to know the recover rate of beta-hemolytic streptococci by throat swab culture at the in-patient 2. Methods The throat swab cultures and filled up flow-sheets were undertaken on 619 children who had admitted to hospital, KyungHee university hospital from may 1994 to april 1995 prospectively. At the same time on admission, throat culture was performed. 3. Results The highest recover rate of BHS(Beta-Hemolytic Streptococci)and GAS(Group A Streptococci) were seen in above 10 years old, as 9.1% and 1.9%. BHS were obtained in 39 cases(6.3%) among 619 children while GAS was obtained in 3 cases (0.4%). Among 39 specimens of BHS, 33 specimens were classified as non-grouping streptococcus. 4. Conclusion The poor recovery rate of GAS inpatient compared with normal carrier rate is likely due to possible antibiotic abuse, errors in processing samples, and epidemiologic factors such as seasons and geographic areas. It is necessary to evaluate the clinical significance of non-A,B,C,G streptococcal infections and carriers.
Squamous cell carcinoma(SCC) associated antigen is a subfraction of TA-4, a tumor-associated antigen first described by Kato and Torigoe in 1977. TA-4, obtained from squamous cell carcinoma cancer tissue of the uterine cervix, has been characterized as a glycoprotein with a molecular weight of approximately 45,000 daltons. SCC antigen has been studied in other squamous cell malignancies including lung, esophagus, head and neck, anal canal, and skin. SCC antigen is shed naturally through sweat, saliva and other body fluids. Contamination of specimens, tray, bead dispenser or other accessories with sweat, saliva or aerosols can cause falsely elevated values. To reduce the possibility of contamination, gloves should be worn in all phases of assay preparation, and when handling specimens, accessories or reagents that will be used in SCC and Thyroid function test(TFT).
Composite resins, commonly used in clinical practice, have been developed to improve aesthetics to obtain smooth surfaces. Although the restored composite resin has a smooth surface, it gradually becomes rough over time. Therefore, this study measured glossiness to evaluate the surface of various composite resins and attempted to evaluate the maintenance of glossiness of composite resins by observing surfaces that change to roughness. Specimens were produced using resin used in clinical practice: Gradia direct anterior (GA), Tetric N-Ceram (TN), Ceram.X Sphere TEC one (CX), Filtek Z350XT (FT), Estelite sigma quick (ES). After creating a smooth surface with slide glass, five locations were randomly selected to measure surface gloss, and the average was the representative value of the specimen. Roughness was applied to the specimen under water pouring at the same speed and pressure using SiC paper #2400, 1200, and 400. The gloss unit of different SiC papers was measured. To evaluate the gloss unit and gloss retention between composite resins, one-way analysis of variance and Tukey multiple comparisons test were used. As a result of the study, there was a difference in gloss unit of specimens produced under the same conditions. Although the degree differed depending on the composite resin, there was also a difference in gloss retention. Based on the findings, composite resins show differences in gloss due to their different characteristics. Ceram.X Sphere TEC one (CX) showing the lowest gloss retention and Estelite sigma quick (ES) showing the highest.
Shigellosis is the most common bacterial gastroenteritis both in developing and developed countries, but bacteremia due to Shigella spp. is very rare. In developed countries recent shigellosis is mostly caused by S. sonnei, but S. flexeri infection is rare. We had rare cases of S. flexeri infections in a family in the Jeonbuk Province: an 8-year-old boy with bacteremic shigellosis and 10- and 12-year-old brothers with diarrhea. The isolates had identical biochemical characteristics, and were resistant to ampicillin, chloramphenicol, and co-trimoxazole. PFGE pattern of Not I-restricted genomic DNA suggested that the isolate from blood was closely related to the two strains isolated from stool which had an identical PFGE pattern.
Species and antimicrobial susceptibility of nonfermenting gram-negative bacilli(NFB) isolated from clinical materials at Yonsei Medical Center during the period of September 1980 to August 1981 were analyzed and the following results were obtained. 1. NFB were isolated from 17.1% of sputum, 14.8% of pus, 5.0% of urine, 3.3% of throat and 1.4% of blood specimens. 2. Among the isolates 57.6% were Pseudomonas aeruginosa and 32.7% were Acinetobacter calcoaceticus. P. maltophilia and P. cepacia were 3.2% and 2.8% respectively. Other bacteria were rarely isolated. 3. The monthly isolation rates were high during the June to November period(8.8-12.0%), and low during the December to May period(4.2-8.4%). 4. Many of the isolates showed resistance to various antimicrobials. Although there were variations depending on the bacterial species, amikacin, colistin, gentamicin, tobramycin and co-trimoxazole showed more antibacterial activities than other antimicrobials.

Auditory tube diverticula, also known as guttural pouches, are naturally occurring dilations of the auditory tube in horses that communicate with the nasopharynx through a small ostium. Infection and select other conditions can result in inflammation and narrowing of the nasopharyngeal ostium, which prevents drainage of fluid or egress of air and can lead to persistent infection or guttural pouch tympany. Auditory tube diverticulotomy allows continuous egress from the auditory tube diverticula and is a feature of disease treatment in horses, in which medical treatment alone is not successful. Transpharyngeal endoscopic auditory tube diverticulotomy was performed using a diode laser either at a single dorsal pharyngeal recess location or bilaterally caudal to the nasopharyngeal ostium in 10 horse head specimens. Both methods resulted in clear communication between the nasopharynx and auditory tube diverticula. Diverticulotomy performed in the dorsal pharyngeal recess required less laser energy and activation time and had a shorter surgical duration than diverticulotomy performed caudal to the nasopharyngeal ostium. Further study related to the clinical application of both techniques is warranted.
This study was performed to investigate the effectiveness of the aspiration trap method for collection of sputum by comparing with the conventional method which was collecting specimens at canular cap swab. In this study, the author tested by two methods to collect specimens from 46 patients who were cared with tracheostomy and intubation at the intensive care unit of an university hospital in Pusan, and investigated the incidence of the lower respiratory tract infection, the consistency between the two methods, the level of specimen contamination. Major results were as follows: Among the patients, 35 were cared with tracheostomy and 11 were cared with intubation. In clinical diagnosis we were classified the subjects in to two group, 17 of pneumonia group and 29 of non-pneumonia group. A total of 247 strains were isolated. Among them, most three strains were Serratia marcescens (62 strains; 25.1%), Pseudomonas aeruginosa (52 strains; 21.1%), and Acinetobacter baumannii (19 strains; 7.8%). Out of total, 188 (76.1%) strains were Gram negative bacilli. The isolated strains by the aspiration trap method were the average 2.1 strains, but by the canular cap swab method were 1.6 strains. In spite of the high contaminated possibility from the incision site and the oral cavity swab, the low isolated rates of the canular cap may be the dried environment of the canular of cap area. But the contamination rates were 57.2% of the canular cap, 51.5% of the oral swab and 50.5% of the incision site swab, respectively. The consistency of predominant microorganisms according to collection method were 86.7% of aspiration, 78.3% of canular, 74.3% of incision, and 63.6% of oral. In conclusion, the aspiration trap method fur the sputum collection from the patients with intubation of tracheostomy showed the lower contamination rate of the specimens and it was helpful for rapid, accurated interpretation of the lower respiratory tract infection and hospital infection.
Adenoviruses(Ad) are considered to be second only to rotaviruses as the most significant cause of gastroenteritis in young children in Korea and thus it is essential to know the full spectrum of Ad serotypes routinely present in stool specimens from symptomatic patients. Sixty-six Ad isolates and three questionable ones collected over a 2-year peiord were typed by standard microneutralization, restriction endonuclease digestion and PCR of viral DNA to be able to evaluate these assays comprehensively for their ability to identify Ad associated with gastroenteritis. A total of sixty-one isolates(88.4%) were typed: the predominant types were Ad type 41(Ad41)(26.2%), Ad2(19.7%), Ad40(14.8%), Ad5(9.8%), and Ad7(9.8%) which together accounted for almost 80% of the isolates. The remaining virus isolates were typed as Ad1, 31, 34, 3, 25 and a mixture of 40/41. The incidence of Ad31(4.9%) or Ad3(1.6%) was relatively insignificant. DNA restriction analysis(77.5%) proved to be better than serum neutralization but not so when compared to a PCR-based assay for identification of the enteric Ad serotypes(90%) in stool specimens. In this work, the PCR-based assay was evaluated as a tool for the rapid, yet highly sensitive identification of adenoviral DNA sequences in fresh clinical stool specimens.
There has been increasing interest in the head and neck squamous cell carcinoma (HNSCC) that is caused by high-risk human papillomavirus (HR-HPV) and has posed a significant challenge to Otolaryngologists. A rapid, sensitive, and reliable method is required for the detection of HR-HPV in clinical specimens to prevent and treat HPV-induced diseases. In this study, a multiple cross-linking spiral amplification (MCLSA) assay was developed for the visual detection of HPV-16. In the MCLSA assay, samples were incubated under optimized conditions at 62℃ for 45 min, and after mixing with the SYBR Green I (SGI) dye, the positive amplicons showed bright green fluorescence while the negative amplicons exhibited no obvious change. The specificity test revealed that the developed MCLSA technique had high specificity and could effectively distinguish all five HPV-16 strains from other pathogenic microorganisms. In terms of analytical sensitivity, the limit of detection (LoD) of MCLSA assay was approximately 5.4 × 101 copies/tube, which was 10-fold more sensitive than loop-mediated isothermal amplification (LAMP) and RT-PCR. The detection results of laryngeal cancer specimens collected from 46 patients with suspected HPV infection in the Liaoning region demonstrated that the positive detection rates of MCLSA and hybridized capture 2 kit were 32.61% (15/46). The true positive rate of the MCLSA assay was higher than that of RT-PCR (100% vs. 93.33%) and LAMP (100% vs. 86.67%). Therefore, the MCLSA assay developed in the present study could be a potentially useful tool for the point-of-care (PoC) diagnosis of HR-HPV, especially in resource-limited countries.
