A Comparison of the Clinical Characteristics of Panic Disorder with and without Generalized Anxiety Disorder (공황장애 환자에서 범불안장애 공존 유무에 따른 임상적 특징 비교)

  • Objective : This study aimed to investigate differences in demographic, clinical characteristics, and quality of life between panic-disorder patients with generalized anxiety disorder (PD+GAD) and without generalized anxiety disorder (PD-GAD). Methods : We examined data from 218 patients diagnosed with PD+GAD (150 patients) and PD-GAD (68patients). The following instruments were applied: Stress coping strategies, Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI), Panic Disorder Severity Scale (PDSS), Anxiety Sensitivity Index-Revised (ASI-R), Albany Panic and Phobia Questionnaire (APPQ), NEO-neuroticism(NEO-N), Short Form health survey-36 (SF-36). Results : Compared to the PD-GAD group, the PD+GAD group had higher scores in emotion-focused coping strategies and clinical severity, such as BDI, BAI, PDSS, ASI, APPQ, and neuroticism. The PD+ GAD group showed lower scores in most scales in SF-36 status than PD-GAD group. Conclusions : This study shows that PD+GAD patients are different from PD-GAD patients in coping strategies, clinical severity and quality of life. It emphasizes the need of personalized therapy in clinical approach among patients with PD+GAD.

  • Purpose: Obesity is already a major clinical problem in Korea, so obesity prevention and treatment practices are gradually interested in Korean Medicine doctors. Moreover subjective body shape image entered as a important medical topic recently. To make a graph system for clinical tool which based on the Korean national standard physical scale. Methods: Physical data of the girth of upper arm, abdomen, hip, buttock, flank, calf, and body weight were statistically analyzed, which collected through the fifth national standard physical scale research. And then reviewed those statistical validation. Results: Relationship of the each body scale data and those graphs were fit and right statistically. Conclusion: These new achieved graph system of Korean standard body scales could be adopt as a useful clinical tool for body image related patients such as obesity, PCOD and postpartum patient.

The Temperament and Character Inventory for Integrative Medicine (기질 및 성격검사의 통합의학적 활용)

  • Objectives: The Temperament and Character Inventory (TCI), initially developed by Dr. Cloninger in 1986, has been studied from various perspectives in many countries; however, its application in Korea has been limited due to a lack of information regarding the clinical use of integrative medicine. Methods and Results: A systematic review of previous studies on the theoretical development, structure, and content of temperament and character scales, as well as their stability and variability, is presented. In addition, the distinctive features of TCI as a biopsychosocial model and its clinical application regarding well-being, coherence therapy, and person-centered medicine were systematically reviewed. Discussion and Conclusion: The promotion of well-being with Cloninger's personality model and its importance for integrative medicine were discussed. This study would contribute to the clinical application of TCI in Korea.

Development of a Phlegm Pattern Questionnaire for Functional Dyspepsia and Analysis of Its Reliability and Validity (기능성소화불량에 대한 담음 변증설문지 개발과 신뢰도 및 타당도 검정)

  • Objectives: The aim of this study was to develop a Phlegm Pattern Questionnaire for Functional Dyspepsia (PPQ-FD) by modifying a previously developed Phlegm Pattern Questionnaire (PPQ) and to verify its reliability and validity. Further objectives were to obtain the optimal cut-off value for the PPQ-FD for standardization and for clinical use. Methods: The PPQ-FD was developed by extracting the major symptoms of the phlegm pattern in functional dyspepsia and by using the Delphi method to administer a requested importance survey to experts. The reliability, validity, and optimal cut-off value of the PPQ-FD were analyzed by enrolling a total of 60 subjects in this study. Thirty patients were diagnosed with both functional dyspepsia and phlegm pattern and thirty patients were diagnosed with only functional dyspepsia. All participants were requested to fill out the PPQ-FD. Results: No statistically significant differences were detected in the two groups for sex distribution, age, or body mass index. Five of the survey questions negatively affected its reliability; therefore, we decided to exclude those five questions on further inspection. The Cronbach's α coefficient of the revised PPQ-FD was 0.853, and clinical validity was verified. Construct validity was analyzed by factor analysis and identified four factors. Statistically significant positive correlations were found between the revised PPQ-FD and other dyspepsia scales, such as the SQDQ, SSQD, VAS, NDI-K, and FD-QoL scales. The VAS had particularly strong positive correlations with the PPQ-FD. Conclusions: The PPQ-FD developed in this study has fundamental reliability and validity for use as a pattern-diagnosis questionnaire. The PPQ-FD can help to diagnose the phlegm pattern in patients with functional dyspepsia.

Differential Response Style on the Personality Assessment Inventory according to Compensation-Seeking Status in Patients with Traumatic Brain Injury (외상성 뇌손상 환자에서 보상추구 여부에 따른 성격평가질문지 반응 양식의 차이)

  • Objectives : This study examined the characteristics and differences of PAI(Personality Assessment Inventory) profile between compensation-seeking(CS) and treatment-seeking(TS) patients with traumatic brain injury(TBI) and assessed the clinical meaning of the characteristics and differences of profiles between the two groups. Methods : 36 TBI patients who visited the Wonkwang University Hospital were selected. The patients were categorized as compensation-seeking TBI patients(n=22) and treatment-seeking TBI patients(n=14). The PAI scales and subscales were used to compare differences between two groups. t-verification for each variable and comparison analysis were performed. Results:In validity scales, CS group showed significantly higher NIM scores and lower PIM scores than TS groups. In full scales, CS group showed significantly higher SOM, ANX, ARD, DEP, and SCZ scores than TS group. In subscales, CS group showed significantly higher SOM-S, ANX-A, ARD-P, DEP(-C, A, P), (MAN-I), PAR-H, SCZ(-T, P), BOR(-A, N), and ANT-S scores than TS groups. In supplementary scales, CS group showed significantly higher SUI, NON and AGG-P, and lower RXR scores than TS group. Conclusions:There were significant differences in PAI scales with validity scales, some full and subscales according to compensation seeking status in TBI patients. The CS patients tended to exaggerate their symptoms on PAI, and showed higher scores representing somatic preoccupation and emotional distress. These results show the usefulness of PAI in reflecting the significant psychological differences between two groups.

Association between Global Cortical Atrophy, Medial Temporal Atrophy, White Matter Hyperintensities and Cognitive Functions in Korean Alzheimer's Disease Patients (알츠하이머병 환자의 전반적 피질 위축, 내측두엽 위축, 백질 고강도 신호와 인지기능의 연관성)

  • Objectives The aim of this study is to investigate the correlation between degenerative changes in brain [i.e., global cortical atrophy (GCA), medial temporal atrophy (MTA), white matter hyperintensities (WMH)] and neurocognitive dysfunction in Korean patients with Alzheimer's disease. Methods A total of 62 elderly subjects diagnosed with Alzheimer's disease were included in this study. The degenerative changes in brain MRI were rated with standardized visual rating scales (GCA or global cortical atrophy, MTA or medial temporal atrophy, and Fazekas scales) and the subjects were divided into two groups according to the degree of degeneration for each scale. Cognitive function was evaluated with Korean version of the Consortium to Establish a Registry for Alzheimer's Disease (CERAD-K) and several clinical features, including apolipoprotein E ${\varepsilon}4$ status, lipid profile and thyroid hormones, were also examined. Chi-square test and Fisher's exact test were performed to analyze the relationship between the degree of cerebral degeneration and neurocognitive functions. Results Demographic and clinical features, except for the age, did not show any significant difference between the two groups divided according to the degree of cerebral degenerative changes. However, higher degree of GCA was shown to be associated with poorer performance in verbal fluency test, word list recall test, and word list recognition test. Higher degree of MTA was shown to be associated with poorer performance in Mini-Mental State Examination in the Korean Version of CERAD Assessment Packet (MMSE-KC), word list recognition test and construction praxis recall test. Higher degree of white matter hyperintensities was shown to be associated with poorer performance in MMSE-KC. Conclusions Our results suggest that severe brain degeneration shown in MRI is associated with significantly poorer performance in neurocognitive tests in patients with Alzheimer's disease. Moreover, the degree of GCA, MTA and white matter hyperintensities, represented by scores from different visual rating scales, seems to affect certain neurocognitive domains each, which would provide useful information in clinical settings.

Analysis of Employment Tendencies According to the Satisfaction of Field (Clinical) Practice among Students of the Department of Optics (안경광학과 학생들의 현장(임상)실습 만족도에 따른 취업 성향 분석)

  • Purpose. This research study is to understand the effects of field (clinical) Practice on employment choice and decision. Method. The research study conducts a questionnaire survey of trainees who received field (clinical) Training during the period of 2016 summer vacation to analyze the data, utilizing Likert 5 rating scales and conduct a statistical analysis with the tool of SPSS 18.0. Results.. As for the satisfaction of the participation of Practice, girl students account for the high rate (75.1%) compared with boy students (61.7%). Regarding the satisfaction of each Practice spot, ophthalmic clinic reaches the high rate (77.8%) compared with optical shop (65.7%). Practice about practice courses (53.2%) of regular subjects have somewhat close correlations compared with theory courses (46.8%). 75.2% of respondents in the group that shows the high satisfaction of the participation of Practice replied that they would decide employment as an academic major after Practice. And the figure is proved to assume a high ratio. Conclusion. The analytic research outcome indicates that those trainees who received Practice in an ophthalmic clinic have higher satisfaction than those who received Practice in an optical shop. There is the high accomplishment of employment in relation to academic major area instead of other area, after Practice.

Development of a Professionalism Measurement Scale for Clinical Nurses (임상간호사의 전문성 측정도구 개발)

  • Purpose: The purpose of this study was to develop a scale to measure professionalism of clinical nurses and evaluate the reliability and validity of the scale. Methods: DeVellis's scale-development eight steps were applied. The initial items were developed through a literature review and discussion with investigators, and the content validity was verified by seven experts. The data were collected from 250 hospital nurses for exploratory factor analysis and 217 hospital nurses for confirmatory factor analysis. Exploratory and confirmatory factor analyses were utilized to assess the construct validity. Cronbach's ⍺ was used to test the internal consistency reliability. Results: The results of the exploratory and confirmatory factor analyses showed that the scale comprised four factors: professional skill(eight items), sense of ethics (five items), knowledge-seeking activities (four items), and autonomy(three items). The four-factor structure was validated (x 2 =600.85 p<.001, GFI=.88, CFI=.84, RMSEA=.07), and Cronbach's ⍺ for the total scale was .84. Conclusion: The study results showed satisfactory reliability and validity of the professionalism measurement scale for clinical nurses. This scale has potential as an appropriate instrument for measuring clinical nurse professionalism.

A Case Report of Late Onset Chorea Hyperglycemia Basal Ganglia Syndrome using Yokukan-san (억간산이 유효했던 지연성 Chorea Hyperglycemia Basal Ganglia Syndrome 환자 1례)

  • Objectives: This report presents a case of late onset chorea hyperglycemia basal ganglia syndrome treated using traditional Korean medicine therapies. Methods: A patient was treated with Korean medicine therapies, including the herbal medication, Yokukan-san. We evaluated the improvements in the patient's symptoms based on the total score of the Abnormal Involuntary Movement Scale, the activities of daily living portion of the Universidade Federal de Minas Gerais (UFMG) Sydenham's Chorea Rating Scale, the numeric rating scale, and an assessment of the patient's writing. Results: After undergoing Korean medicine treatment for 15 days, there were improvements in the patient's chorea and writing as well as a decrease in the scores for all three scales. Conclusions: This clinical case study suggests that Yokukan-san might be effective for treating dyskinesia in patients with chorea hyperglycemia basal ganglia syndrome.

Assessing Suicidality in Adolescents (청소년의 자살 가능성에 대한 임상 평가)

  • Objectives : To consider which questions are most useful clinically and to find a framework for assessment of suicidality or risk of suicide for adolescents. Methods : Authors reviewed and integrated both clinical works and researches about measures used to evaluate suicidal behaviors in adolescents. Results : The 'cascade approach' for assessment of suicidality was developed; the quality and quantity of 'distress' leads to an entry question to do with the 'quality of life' and whether it is worth living. These two questions lead to examinations of 'thoughts' which may lead to 'plans' which leads to 'threats' which then leads to 'deliberate self injury' and then on to a history of 'past suicide attempts'. Authors also suggested to use a summed score of cascade approach in clinical decisions and showed how to modify it up to different evaluation situations. Focusing on the good things helps not only to qualify suicidality but also to begin the search for solutions. Conclusion : In the clinical assessment process, while scales assist the clinician, there is nothing to supplant a sensitive clinical approach. The 'cascade approach' was proposed as a framework for assessing adolescents for suicidality.
