Effects of Abdominal Massage or Lactic Acid Fermented Milk for Constipation in Patients Following Spinal Surgery (복부 마사지와 유산균 발효유가 척추 수술 환자의 변비 정도에 미치는 효과)

  • Purpose: The purpose of this study was to identify effects of abdominal massage or oral lactic acid fermented milk to prevent constipation in patients immobilized after spinal surgery. Methods: Participants were 60 patients who underwent spinal surgery (20 in the abdominal massage group, 20 in the lactic acid group, and 20 in the control group). Preoperative constipation was assessed and after starting to eat following surgery, patients were given abdominal massage or lactic acid fermented milk for three days. Then bowel movements were checked using the constipation assessment scale and enemas. Results: Constipation score in the control group without treatment increased from $4.85{\pm}4.73$ to $10.25{\pm}4.28$ whereas, in the abdominal massage group or lactic acid group, the scores decreased from $6.40{\pm}3.71$ to $5.23{\pm}3.43$ and from $5.25{\pm}3.78$ to $4.50{\pm}3.18$ respectively. Also, enemas were given for 15 of 20 in the control group but only 7 of 20 in the abdominal massage group and 4 of 20 in lactic acid group. Conclusion: Results of this study indicate that abdominal massage or lactic acid fermented milk to alleviate constipation in patients with immobility following spinal surgery is highly effective according to the constipation score and need for enemas.

The Effects of Periodic Reminding Interventions on Medication Adherence, Self-Efficacy, and Pain for Home-Based Lung Cancer Patients (주기적인 상기(Reminding) 중재가 재가 폐암환자의 진통제 복용 이행 정도, 자기효능감, 통증에 미치는 효과)

  • Purpose: The purpose of this study was to evaluate the effects of periodic reminding interventions on medication adherence, self-efficacy, and pain intensity for home-based lung cancer patients. Methods: A quasi-experimental nonequivalent control group, pretest and posttest design was used. The intervention comprised of individual education by tailored image-combined medication instructions, daily reminding text message, and weekly telephone calls for four weeks. The subjects in this study consisted of 62 lung cancer patients (31 in the experimental group, and 31 in the control group). Mann-Whitney U-test was applied to analyze the data. Results: Experimental group who received periodic reminding intervention program better adhered to prescribed medication compared to the control group (z=-6.14, p<.001). Experimental group demonstrated higher level of self-efficacy compared to the control group (z=-6.74, p<.001). Experimental group experienced less intense average pain compared to the control group (z=-6.29, p<.001). Conclusion: The findings suggest that periodic reminding interventions can be applied as an effective nursing intervention to promote medication adherence and self-efficacy to improve and pain management for home-based lung cancer patients.

Validation of Critical Care Non-verbal Pain Scale for Critically Ill Patients (중환자 통증사정 도구의 타당성 평가)

  • Purpose: This study was done to examine predictive validity of Critical Care Non-verbal Pain Scale (CNPS) and develop criteria for pain assessment using CNPS with critically ill patients who have communication problems. Methods: Data were collected from intensive care units at three major general hospitals in Seoul and Kyunggi province. During each observation, a nurse assessed pain severity using CNPS ratings (range 0-9) at four treatment stages: at rest, during central catheter dressing change (nonpainful procedure), position change and suctioning (routine painful procedures). Patients also assessed their pain using a self-report 4-point VRS-4. Results: There were significant differences between the four treatment stages except between "at rest" and "nonpainful procedure". Strong correlations were found between CNPS and VRS-4 for "at rest" (r=.552, p<.001), central catheter dressing change (r=.505, p<.001), position change (r=.709, p<.001), and suctioning (r=.662, p<.001). ROC curve analysis of CNPS based on 3 point on VRS-4 showed the cutoff point was 3 for CNPS, the starting point for pain management with 73% sensitivity, 92.2% specificity, 73% positive predictive value, and 92.8% negative predictive value. Conclusion: Results indicate that CNPS is a valid tool for measuring pain in critically ill patients with communication problems and 3 point should be the standardized pain treatment point.

Relationships between Fatigue, Sleep Disturbance, Stress, Self-efficacy and Depression in Hemodialysis Patients (혈액투석환자의 피로와 수면장애, 스트레스, 자기효능감 및 우울 관계)

  • Purpose: This study was conducted to investigate the differences in sleep disturbance, stress, self-efficacy and depression by fatigue level and to identify the factors influencing fatigue in hemodialysis patients. Methods: This study used a cross-sectional design and the subjects were 195 patients undergoing hemodialysis in a local unit. Data were collected using a structured questionnaire and medical records from June to August 2012. The data were analyzed using descriptive statistics, independent t-test, one way ANOVA, $Scheff{\grave{e}}$ test, and Pearson's correlation coefficients. Also a multiple regression analysis was used to assess the independent association between fatigue status and indices of sleep disturbance, stress, self-efficacy and depression. Results: A total of 99 (50.8%) subjects experienced a high level of fatigue and 96 (49.2%) experienced a low level of fatigue. There were significant differences in the levels of sleep disturbance, stress, self-efficacy and depression between the two groups. Fatigue was significantly correlated to sleep disturbance, stress, self-efficacy and depression. The most important factor related to fatigue was stress, followed by sleep disturbance. These factors explained about 37% of fatigue of the respondents. Conclusion: The results of this study showed that sleep disturbance and stress were significant predictors of fatigue in hemodialysis patients. Possible interventions for minimizing fatigue in patients undergoing long-term hemodialysis should be aimed to manage the known contributing factors.

The Influences of Depression, Anxiety, Social Support and Knowledge of Anticancer Drugs on the Chemotherapy-induced Peripheral Neuropathy among Colorectal Cancer Patients Receiving Oxaliplatin (Oxaliplatin을 투여 받는 대장암 환자의 우울, 불안, 사회적지지, 항암제 지식수준이 말초신경병증에 미치는 영향)

  • Purpose: The purpose of this study was to identify factors influencing chemotherapy-induced peripheral neuropathy among colorectal cancer patients receiving oxaliplatin. Methods: A total of 132 patients hospitalized for chemotherapy were surveyed at K University Hospital in Seoul, Korea. This study was a descriptive causal relationship study using a self-report questionnaire survey method. Correlation and multiple regression analysis between the factors were performed using SPSS 18.0. Results: The regression model was significant (F=31.64, p<.001), which meaned that the experience of chemotherapy-induced peripheral neuropathy among the participants was statistically significant. The factors influencing the chemotherapy-induced peripheral neuropathy were depression (${\beta}=.34$, p<.001), followed by anxiety (${\beta}=.32$, p<.001), medical staff support (${\beta}=-.17$, p=.037) and the level of knowledge of anticancer drugs (${\beta}=-.16$, p=.045). The explanatory power of these factors on the chemotherapy-induced peripheral neuropathy of colorectal cancer patients was 69%. Conclusion: The factors influencing the chemotherapy-induced peripheral neuropathy of colorectal cancer patients receiving oxaliplatin were identified as depression, anxiety, level of knowledge of anticancer drugs and medical staff support.

The Adoptions and Use of Electronic Nursing Records in Korean Hospitals: Findings of a Nationwide Survey (국내 전자간호기록 개발 및 실무적용 현황 조사)

  • Purpose: To provide clear estimates of the adoption and use of electronic nursing records (ENRs) with standard terminology in Korea and identification of the scope and use as well as perceived or potential benefits of ENRs. Methods: A survey was done of 733 hospitals at three levels: tertiary advanced hospitals, general hospitals, and community hospitals. After performing a literature review a modified version of an existing survey tool was used for 2 months in 2012. The collected information related to EHR functionality and coverage of nursing documentation and nursing process, application of standard terminology, and perceived satisfaction and benefits of ENRs. Results: The response rate was 39.4% (289/733), and 202 hospitals (70.1%, 95% CI64.8~75.5%) of the respondents had ENR systems (82.5% of tertiary hospitals, 66.7% of general hospitals, and 70.1% of community hospitals). Out of these hospitals less than 10% had ENRs fully covering nursing documentation. The adoption rate of standard terminology was 55%, and hospital satisfaction with ENRs was 70%. But personalized care was identified as needing improvement in ENRs. Conclusion: The ENR adoption rate was high but there are many potential opportunities for improving ENR systems in terms of the data standardization and personalized care.

Physical, Physiological, and Psychological Factors Determing Sleep Quality in Patients on Maintenance Hemodialysis (혈액투석 환자의 수면의 질에 영향을 주는 신체적, 심리적, 생리적 요인)

  • Purpose: The study aimed to examine quality of sleep in hemodialysis patients and explore physical, physiological, and psychological factors determining sleep quality. Methods: The survey was conducted among 165 patients on maintenance hemodialysis at a university hospital. Data were collected from July to October in 2015. The sleep quality and its related factors were assessed by standardized questionnaires, anthropometric measures, and lab tests after obtaining a written consent from the participant. Results: The mean age of the participants was 62.58 years old, and 67.9% of the participants experienced sleep disorders such as 'not falling asleep within 30 minutes' or 'frequently waking up at night'. Pain (t=-3.29, p=.007) and depression (t=-6.028, p<.001) were significantly different between the group with sleep disorder and the other without. The demographic factors (age, shared room, sleep during the day), physical factors (pain and symptom), and psychological factors (depression and trait and status anxiety) together explained 20.0% of variance in sleep quality. Conclusion: Sleep disorder was frequently experienced among the patients on maintenance hemodialysis. The effective nursing strategies should focus on sleep environment, pain and depression management. Further study is warranted to explore the effect of physiological factors related to the disease on sleep disorder in this population.

Comparison of the Effect of Applying Polyethylene Wrap and Aircap in Maintaining Body Temperature of Preterm Infants (폴리에틸렌 랩과 에어캡의 적용이 미숙아의 체온 유지에 미치는 효과)

  • Purpose: This study was an quasi-experimental study to compare the effect of applying polyethylene wrap and aircap in maintaining body temperature of preterm infants. Methods: The participants were 51 preterm infants. Aircap was applied to the experimental group (n=23) and polyethylene wrap was applied to the control group (n=28) when the preterm infants admitted to neonatal intensive care unit. The data was collected at W hospital in J-province from June 2016 to May 2017. A total of 9 body temperature measurements were taken at 3 hours interval from 5 min to 24 hours after admission. Repeated measure ANOVA, independent t-test and χ2 test were conducted used with SPSS/WIN 24.0 Results: There were no significant difference in the homogeneity tests for general characteristics and dependent variables prior to the experiments (t=0.57, p=.566). There was a significant difference on body temperature of preterm infants over time (F=3.24, p=.020). There was no significant difference on body temperature between polyethylene wrap and aircap application groups (F=1.29, p=.261). The interaction between the group and the time was insignificant (F=1.51, p=.214). Conclusion: The findings demonstrated that both methods of applying polyethylene wrap and aircap on the body in preterm infants had effect in maintaining body temperature.

  • For the study on occurrence of backache related to types of daily activities in adulthood Nine hundred twenty six adults living in Seoul were randomly selected as subjects from the eight kinds of occupation; housewives, teachers, nurses, officers, physicians, labourers, beauticians & barbers, and students. The purpose of this study was to identify various factors related to backache in daily activities and to apply these factors in clinical situations as well as in the community situations. This survey was conducted from September 21 through October 8, 1976. Under the ten hypothesis designed for this study, the. followings were obtained; 1. Significant difference on occurrence of backache was revealed according to sex difference at 0.5% level. (x$^2$= 27.85, df: 1, p<0.005) 2. Significant difference on occurrence of backache was revealed according to age groups at 2.5% level. (x$^2$= 11.13, df= 3, p<0.025) 3. Significant difference on occurrence of backache was revealed according to types of occupation at 2.5% level. (x$^2$= 18.04, df: 7, p<0.025) 4. Significant difference on occurrence of backache was revealed according to types of movement originated by hand during occupational activities at 1% level. Needle Insertion Force of Biological Soft Tissue for Haptic based Intravenous Injection Simulator (햅틱 기반 정맥주사 시뮬레이터를 위한 생체조직 바늘 삽입력)

  • Haptics and virtual reality are rapidly growing technologies in medical fields. Physicians and nurses can benefit from medical simulation via training and acquire surgical and clinical techniques. In this paper, the research on needle insertion force of biological tissue for haptic based intravenous injection simulator was carried out. We built the setup for needle insertion (intravenous injection) experiments and performed the experiments on live pigs. The force responses against needle insertion were measured using the experimental setup. In addition, the modeling of needle insertion force was carried out with the experimental results and numerical models via nonlinear least-squares method. The results presented in this paper indicate that the developed models can be applied not only to estimate the force feedback during intravenous injection procedure but also to improve the overall training quality of the medical simulator.
