A Review of the Pain and Function Assessment Instruments for Shoulder Disease Patients in Korean Medicine Clinical Studies Published in South Korea (국내 한의임상연구에서의 어깨질환 환자의 통증 및 기능측정도구 사용에 대한 현황분석)

  • Objectives The purpose of this study is to analyze the current status of measurement instruments used to evaluate pain and function for shoulder disease patients in Korean Medicine clinical studies published in South Korea. Methods 5 Korean online databases from January 2007 to July 2017 were used to source articles for the review. Data of pain and function assessment instruments, measurement methods, time point and frequency for shoulder assessment were extracted. And it was analyzed by investigating the frequency of use for each item. Results A total of 78 studies met the selection criteria. There were many studies with no description for pain assessment instruments or inconsistent measurement methods. In the function assessment, the frequency of use was low and there were a lot of use of instruments with low validity and reliability. Conclusions To develop the objectivity of clinical research, further clinical studies, active use of measurement instruments, and standardization of measurement methods are needed.

Development of a Nursing Competence Measurement Scale according to Nurse's Clinical Ladder in General Wards (병동 간호사의 임상등급 간호역량 평가도구 개발)

  • Purpose: This methodological study was done to develop a Nursing Competency Measurement Scale based on the clinical ladders of nurses working in wards. Methods: Thirty clinical experts and 501 ward nurses evaluated the content validity of the scale. A survey using the Nursing Competency Measurement Scale was conducted with 114 nurses to evaluate reliability and applicability of the instrument. Data were analyzed using SPSS/WIN 21.0. Results: A review of the literature identified 13 components of nursing competencies and 30 core nursing competencies based on each of the 4 grade clinical ladders. Cronbach's ${\alpha}$ coefficient for the total was .92. Cronbach's ${\alpha}$ reliabilities of each clinical ladder grade were .83 for Grade I, .84 for Grade II, .81 for Grade III, and .84 for Grade IV. The Content Validity Index (CVI) of the scale with 120 individual items was 0.976~1.000 for Grade I scale, 0.986~1.000 for Grade II scale, 0.984~1.000 for Grade III scale, and 0.992~1.000 for Grade IV scale. The expert group nurses' average degree of nursing competence measured using the scale was 3.38~3.75 out of 4.0. Conclusion: Through this process, 120 final questions were confirmed to represent items of the Nursing Competency Measurement Scale based on clinical grade.

A Study on the Contents of Fundamental Nursing Practice (기본간호학 실습교육현황과 실습내용 개선을 위한 조사 연구)

  • The purpose of this study was to provide fundamental data for determining contents of Fundamental Nursing Practice and developing desirable bedside nursing techniques for clinical nursing areas. Subjects for this study were 86 nurses who were employees of two university hospitals located in Seoul and a district area. Data were collected by questionnaires. Items of Fundamental Nursing Practice were classified into 72 items according to the result from content analysis of 9 textbooks of Fundamental Nursing. The results are as follows : 1) Items which above 80% of respondents practiced during the school inside practice or clinical nursing practice course were axillary temperature measurement, radial pulse measurement, respiratory rate measurement, application of oral hygiene, hand-washing technique, application of hot and cold bags, intramuscular injection technique, open bed-making, soap enema method, application of nelaton catheterization and oral and nasal suction methods. 2) Above 90% of respondents replied that all items except temperature measurement and bed-making were requisite contents for Fundamental Nursing Practice. Above 10% of respondents replied oral and rectal temperature measurement and bed-making were unnecessary content. 3) Above 90% of respondents replied that operating methods of all items except isolation technique, admission and discharge procedures, and retention enema in the Fundamental Nursing Practice course and clinical situation were consistent. The main reasons that respondents did not apply methods which they learned in the Fundamental Nursing Practice course to the clinical situation were 'insufficient time', 'colleagues were using different methods', 'insufficient supply of instuments' or 'inappropriate appliances'.

Review on Current Status of Use and Measurement Condition of Heart Rate Variability in Clinical Study of Korean Medicine (한의학 임상연구에서의 심박변이도 검사 활용현황과 측정조건에 대한 고찰)

  • Objectives : The purpose of this study is to review on current status of use and measurement condition of heart rate variability(HRV) by reviewing clinical studies published in journal of Korean medicine registered Korea citraion index(KCI). Methods : One hundred twenty articles were found by searching the representative Korean databases. At first analysis was performed by current status of use(journal, academic field of corresponing author, purpose of study, disease of subject, HRV measurement devices, intervention methods and tests compared to HRV). At second analysis was performed by measurement condition(uncontrolled study, relaxation before measurement, measurement position, measurement place, measurement time period and restriction of smoking, alcohol, caffeine, meal). Results : The analysis showed that department of acupuncture & moxibustion, department of oriental neuropsychiatry are major part in journal & academic field. The purpose of study were classified under studies to verify therapeutic effect on normal subject(39%), to verify therapeutic effect on the patients(8%), to investigate characteristics of the patients(34%) and to be compared with other test(19%). Disease of subject & measurement devices in article were so various. Major intervention methods were acupuncture(61.4%), mind-body intervention(19.2%). Studies that measurement condition were not controlled were relax time(24%), measurement position(19%), measurement place(26%), measurement time period(91%) and restriction of smoking, alcohol, caffeine, meal(49%). Conclusions : This study showed that current status of use of HRV is widespread in Korean medicine but more controlled measurement conditon are needed to get more reliable result from clinical studies and to improve diagnosis and treatment of Korean medicine.

A Study on the MEG Imaging (MEG 영상진단 검사에 관한 연구)

  • Magnetoencephalography (MEG) is the measurement of the magnetic fields produced by electrical activity in the brain, usually conducted externally, using extremely sensitive devices such as Superconducting Quantum Interference Device (SQUID). MEG needs complex and expensive measurement settings. Because the magnetic signals emitted by the brain are on the order of a few femtoteslas (1 fT = 10-15T), shielding from external magnetic signals, including the Earth's magnetic field, is necessary. An appropriate magnetically shielded room is very expensive, and constitutes the bulk of the expense of an MEG system. MEG is a relatively new technique that promises good spatial resolution and extremely high temporal resolution, thus complementing other brain activity measurement techniques such as electroencephalography (EEG), positron emission tomography (PET), single-photon emission computed tomography (SPECT) and functional magnetic resonance imaging (fMRI). MEG combines functional information from magnetic field recordings with structural information from MRI. The clinical uses of MEG are in detecting and localizing epileptic form spiking activity in patients with epilepsy, and in localizing eloquent cortex for surgical planning in patients with brain tumors. Magnetoencephalography may be used alone or together with electroencephalography, for the measurement of spontaneous or evoked activity, and for research or clinical purposes.

Domestic Occupational Therapist Awareness Survey for the Need to Apply Artificial Intelligence Measurement Technology for Clinical Observation Evaluation Based on Sensory Integration (감각통합에 기초한 임상 관찰 평가의 AI 측정 기술 적용 필요성을 위한 국내 작업치료사 인식 조사)

  • Objective : This study is to examine the practical use of clinical observational evaluation of sensory integration therapy and the difficulty and importance of measuring results for each sub-item, and through this, to confirm the usefulness of the application of Artificial Intelligence measurement technology in clinical observational measurement and the need for application. Methods : The questionnaire consisted of the actual use of the sensory integration evaluation tool, the difficulty of measurement for each detailed item of clinical observation, the usefulness of AI measurement technology, the importance of evaluation for each detailed item, and the need for developing AI measurement technology. Results : The detailed items that were difficult to measure during clinical observation were the Finger-to-Nose Test and Postural control (71.0%), followed by Eye movement and Protective Extension Test (67.7%). 83.9% of the study subjects answered that it would be useful to apply AI measurement technology when observing images. Postural control (on the ball) (90.3%) was the highest item that answered that AI measurement technology was needed, followed by Eye movement (83.9%), and Prone Extension and Protective Extension Test (77.4%). Conclusion : The results confirmed the desire of therapists that clinical observation is an important evaluation tool in the field of child occupational therapy in Korea.

Magnetoencephalography and Clinical Application (Magnetoencephalography (MEG)의 임상적 유용성)

  • Magnetoencephalography (MEG), the measurement of magnetic fields produced by neuronal current associated with normal and pathologic brain activities, is a totally noninvasive method for localizing functional regions of the brain. During the past several years, many clinical research centers are working to expand various fundamental functional brain regions, which can be easily localized, as well as to characterize magnetic abnormalities which accompany a wide variety of cerebral disease. At present, MEG is used in a number of clinical centers throughout the world for the presurgical functional localization of eloquent cortex, and for the non-invasive localization of epileptiform activity. And also, non-invasiveness means that it can be used for screening and repetitive follow-up measurement without concern for adverse effects. As procedures for activating various functional brain regions are standardized, and as the effects of specific cerebral diseases on the MEG are carefully documented in controlled studies, the number of routine neurological applications for MEG will increase significantly. In this paper, the basic principles of MEG are reviewed briefly with its clinical application to neurologic disease.

Reliability of Lateral Deviation Measurement in the Hyoid Bone With Center Point and Lateral Motion Tests

  • Background: The hyoid bone is the only non-jointed structure among the skeletal tissues of the head and neck region, and its movement and posture depend on the attached muscle, ligament, and fascia. The location of the hyoid bone is important for airway maintenance, vocalization, chewing, swallowing, breathing, and head and scapular position. In general, the location of the hyoid bone is measured using radiographs and 3D computed tomography, and no studies have reported on clinical measurement methods. Objects: This study was performed to suggest clinical measurement methods for lateral deviation of the hyoid bone and to evaluate their reliability. Methods: In this study, 24 healthy volunteers (12 males, 12 females) in Cheongju-si participated. Two examiners performed the center point test and lateral motion test twice each to measure the lateral displacement of the hyoid bone. The reliability of the center point test was analyzed using intra-class correlation coefficients (ICC), and the reliability of the lateral motion test was analyzed using Cohen's kappa coefficient. Results: The intra-rater reliability of the center point test was good, and the inter-rater reliability was moderate. The intra- and inter-rater reliability of the lateral motion test showed substantial reliability. Conclusion: Based on these results, the center point test and the lateral motion test can be used as an alternative methods of the measurement of lateral deviation of the hyoid bone for people who have musculoskeletal disorders of the head, neck, and scapula.

Analysis of the Research Trends of Pelvic Malposition in Korean Clinical Research (골반부와 관련된 국내임상연구동향 고찰 -골반부의 변위를 중심으로-)

  • Objectives The purpose of this review was to analyze the research trends in clinical research related to pelvic malposition published in Korean medicine journals. Methods We searched articles in Korean databases (OASIS, NDSL, RISS, and KTKP), and sorted the articles by publishing date, pelvic malposition type, field of study, and measurement index. Results 1. 25 original articles and 9 case report articles were reviewed. 2. In recent years, this topic has been studied more frequently, with an increasing number of original articles published. 3. 16 articles analyzed pelvic malposition type. And half of the articles focused on ilium. 4. The original articles have figured out the correlation between pelvic malposition measurement indices and other measurement indices or diseases. 5. Treatment tools, such as chuna therapy, the mckenzie method, devices for correcting malposition, acupuncture were used in the case report articles. 6. 29 measurement indices, such as Ferguson's angle, Ilium shadow measurement, and Iliac height difference were used. Conclusions According to the literature, pelvic malposition is related to other diseases and measurement indices, and manual medicine on pelvic malposition can be effective. However, due to the lack of sufficient empirical evidence from medical articles to support this, further clinical research should be conducted.
