International Cross-Sectional Survey among Healthcare Professionals on the Management of Cow's Milk Protein Allergy and Lactose Intolerance in Infants and Children

  • Purpose: The present international survey among healthcare providers aimed to collect data on theoretical knowledge and clinical practices in the diagnosis and management of cow's milk protein allergy (CMPA) and lactose intolerance (LI) in infants. Methods: A global survey was conducted in several countries with diverse health care settings. The survey consisted of multiple-choice questions in 3 main domains: (1) understanding and clinical practices around CMPA and LI; (2) case scenarios; and (3) disease-specific knowledge and potential educational needs. Results: Responses were available from 1,663 participants. About 62% of respondents were general practitioners or general pediatricians, and the remainder were pediatric allergists/gastroenterologists (18%) or other health practitioners (20%). The survey identified knowledge gaps regarding the types of CMPA (IgE-mediated vs. non-IgE-mediated) and the clinical overlap with LI. The survey suggested diverse clinical practices regarding the use of hypoallergenic formulas, as well as misconceptions about the prebiotic benefits of lactose in extensively hydrolyzed formulas in non-breastfed infants with CMPA. Responses to the two case scenarios highlighted varying levels of awareness of the relevant clinical practice guidelines. While respondents generally felt confident in managing infants with CMPA and LI, about 80% expressed an interest for further training in this area. Conclusion: The current survey identified some knowledge gaps and regional differences in the management of infants with CMPA or LI. Local educational activities among general and pediatric healthcare providers may increase the awareness of clinical practice guidelines for the diagnosis and treatment of both conditions and help improve clinical outcomes.

의과대학 교육과정중 임상영양교육의 필요성 평가 (Evaluation of Necessity of Clinical Nutrition Education in the Medical School Curriculum)

  • This study was conducted to evaluate the necessity of clinical nutrition education in the medical school curriculum. One hundred and eighty five medical school students, residents, nurses, and forty undergraduate students majoring in food science and nutrition were involved in this study. The survey included questions about nutritional knowledge, necessity of nutrition counseling, necessity of clinical nutrition and nutritional attitudes. The nutritional knowledge scores of the subjects in medical school and residents were significantly lower than those of the students majoring in food science and nutrition. Subjects of 91.3% strongly agreed to the necessity of nutrition counseling for patients ; 78.9% agreed that the present nutrition education in medical schools was inadequate. Most subjects (94.1%) agreed that clinical nutrition was an important field in the treatment of diseases. The medical profession is facing the challenges of the 1990s with the shift to outpatient care and preventative services. Clinical nutrition is an essential element in the process of curriculum change. The nutrition educator will be expected to take a leading role in integrating clinical nutrition into medical school curriculum.

일부 치위생과 학생들의 AIDS에 대한 지식 및 태도 (Knowledge and Attitudes about AIDS among Dental Hygiene Students)

  • Objective: The purpose of this study was to obtain the basic data for the establishment of the curriculum from the analysis of knowledge and attitudes about AIDS among dental hygiene students. Methods: Data was collected from the students of two colleges majoring in dental hygiene in Daegu, using questionnaire. They were the students of total 390. The analysis was made using t-test, (one-way ANOVA), and multiple linear regression analysis. These were conducted using SPSS 14.0 version. Results: The horizontal and vertical transmission in percent correct knowledge of HIV infection, most of the questions asked of respondents knew the correct answer, AIDS attitudes in the community education hours to receive AIDS education is important then the response was highest at 88.2 percent. AIDS knowledge is based on clinical practice, and AIDS attitudes in accordance with the educational experience of the average difference between groups investigated showed statistically significantly. HIV knowledge and attitudes, no oil, depending on the educational experience of the average difference between groups investigated showed statistically significantly. The grade, the higher knowledge of HIV was found to be lower in a statistically significant negative relationship showed (p<0.001), clinical experience, the more experience, knowledge of HIV was higher statistically significant positive showed a relationship (p<0.001), a higher attitude about HIV AIDS knowledge was higher showed a statistically significant positive relationship (p<0.001). Conclusion: Dental hygiene to prevent infection associated with AIDS in the curriculum and hands-on curriculum to enhance patient care through the AIDS patients in clinical management after graduating as a dental hygienist at the ethical responsibility is considered to be very strong.

간호대학생의 강점인식과 임상실습 감정노동이 진로정체감에 미치는 영향 (The Effects of Strengths Knowledge and Emotional Labor of Clinical Practice on Career Identity in Nursing Students)

  • 본 연구는 간호대학생의 강점인식과 임상실습 감정노동이 진로정체감에 미치는 영향을 파악하고자 시행된 서술적 조사연구로 G시의 간호학과에 재학 중인 3, 4학년 학생을 대상으로 하였다. 자료 분석을 위해 기술적 통계, t-test, ANOVA, Pearson 상관계수, 통제회귀분석을 실시하였다. 연구결과, 강점인식은 진로정체감(𝛽=.384, p<.001)에 정적인 영향을 나타냈고 임상실습 감정노동은 진로정체감(𝛽=-.370, p<.001)에 부정적인 영향을 나타냈다. 연구모형은 진로정체감을 45.1% 설명하는 것으로 확인되었다. 이러한 연구 결과는 간호대학생의 진로정체감 증진을 위해 감정인식을 증가시키고 임상실습 감정노동을 감소시키기 위한 방안들이 필요함을 시사한다.

임상간호사의 의료보험수가 지식정도 (A Study of Knowledge of Medical Insurance Costs by Clinical Nurses)

  • Purpose: This study was done to help provide patients with information on medical insurance cost through medical insurance education for nurses, to increase effective management, check on omissions in treatment and appropriateness and accuracy of fees, and to contribute to the economic growth of hospital by providing nurses with necessary knowledge about medical insurance cost. Method: The participants in this study were clinical nurses in general hospitals. The study instrument was a questionnaire developed by the researcher through reference to data for medical insurance education. The data were analyzed with percentages, means, ANOVA, and Duncan method using SPSS PC+10. Result: The results on knowledge of medical insurance according to general characteristics of the nurses showed that there were significant differences according to age: (p=.0036) highest level of education (p=.0007), position (p=.0010) and place where education on medical insurance was received (p=.0093). Conclusion: Continuous in-service education for clinical nurses is reflected in increased knowledge about medical insurance costs but special attention needs to be given to younger nurses and nurses with less education, as well as staff nurses, and those nurses who only received education on medical insurance during their schooling. Accordingly, in-service education is necessary for nurses at the time of orientation so that they have knowledge on standards for recuperation allowance, guidelines to calculate material costs, and guidelines to calculate drug rates. In addition, as medical insurance cost frequently change, all nurses need continuous in-service education.

응급구조학과 학생들의 호흡보조요법 경험, 지식이 수행자신감에 미치는 영향요인 (Factors influencing performance confidence in paramedic students by knowledge and experience of respiratory-assistant therapy)

  • Purpose: This study was conducted to identify the factors influencing performance confidence by knowledge and experience of respiratory-assistant therapy in paramedic students. Methods: A descriptive survey study used a convenience sample comprising 387 students from September 15 to October 31, 2016. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient, and regression analysis. Results: Knowledge was statistically significant by college(3-year), acquired certification, and observation experience in respiratory-assistant therapy. Performance confidence was statistically significant by sex, academic grade($3.0{\leq}$), acquired certification, and observation and performance experience in respiratory-assistant therapy. There was a significant correlation between knowledge and acquired certification, clinical practice, and major satisfaction. There was a significant correlation between performance confidence and acquired certification, clinical practice, major satisfaction, and knowledge. Regression analysis revealed that performance confidence accounted for 22.9% of the variance by academic grade(3.0~3.99), acquired certification, and major satisfaction. Conclusion: For paramedic students in clinical practice and in-school classes, it is important to improve performance confidence by combining training instruction with experience and education of respiratory-assistant therapy.

객관화된 구조화 임상시험의 학생평가와 교수자 평가 간의 차이 비교 및 영향 요인 (Comparison of Teacher's and Students' Evaluations on OSCE Performance among College Nursing Students)

  • Purpose: This study was aimed to compare and contrast the evaluation scores on students OSCE performance between the instructors and students according to the Bloom's taxonomy. Methods: A total of sixty six students in a nursing college in Seoul, Korea and four instructors were recruited in the study. The 28 sophomore students were assigned to an OSCE on intravenous injection, whereas 38 junior students on tracheostomy care. Knowledge levels were measured by the students using the researcher developed knowledge questionnaires, and the attitudes and skills by the students and instructors by the modified clinical performance checklists. The data were analyzed by SPSS 22.0 program. Results: For knowledge, the higher GPA of the previous semester was only significant factor of higher knowledge scores. There were statistically significant differences in scores of total (t=-2.740, p=.008) and skill (t=-2.528, p=0.014), but not in attitude between the instructors and students' evaluations. The students evaluated their skill scores significantly higher than those of the instructors. For knowledge, the higher GPA of the previous semester was the only significant factor of higher knowledge scores. In attitude scores, the students in higher grade significantly overestimated themselves (t=3.076, p=.003). Conclusion: Clinical performance assessment requires methodological validity and reliability for student to attain necessary clinical skills properly and efficiently. The findings of this study mandate various assessment methods for nursing simulation evaluations to be valid.

한의학 전공 대학생과 식품영양학 전공 대학생의 임상 영양지식과 영양 실천도 비교 (A Comparison of Clinical Nutrition Knowledge and Nutritional Behavior of College Students Majoring in Korean Oriental Medicine vs. College Students Majoring in Food and Nutrition)

  • This study examined two groups of students Korean Oriental Medicine majors and Food and Nutrition majors and compared their knowledge of clinical nutrition as well as their tendency to follow healthy lifestyles. In all, 204 college juniors and seniors majoring in Food & Nutrition, and 324 sophomores majoring in General Korean Oriental Medicine were given a questionnaire. Overall, knowledge of clinical nutrition(out of twenty questions) was significantly higher among the students majoring in food and nutrition (p<0.01). Specifically, these students obtained a higher score in the questions pertaining to the following topics: nutritional support(p<0.05), the relationship between menopause and blood cholesterol(p<0.01), the relationship between weight control and carbohydrates(p<0.01), the relationship between gout and protein(p<0.10), the relationship between hyperlipidemia and carbohydrates(p<0.01), the relationship between trans fatty acids and atherosclerosis(p<0.01), the relationship between blood potassium levels and kidney disease(p<0.01), and the relationship between weight control and diet and water(p<0.05). Conversely, students studying General Korean Oriental Medicine had a better knowledge of the relationship between malnutrition and the mortality rate and health complications(p<0.01). The questions relating to a healthy diet and lifestyle indicated that students majoring in Food and Nutrition had higher incidences of diet irregularity and a higher missing rate (p<0.01); they were more interested in calorie content and nutrients in their diet(p<0.05); and they were more likely to study about nutritional knowledge during their leisure time (p<0.01). The results also indicated, however, Korean Oriental Medicine students consumed proportionally more vegetables(p<0.05); and they were more likely to take the stairs than the elevator(p<0.01).

HPV백신접종 유무에 따른 임상간호사의 HPV 관련 지식도와 암원인 지각 (Clinical Nurses' HPV-related Knowledge and Perception of Cancer Causes: HPV Vaccinated vs. Not Vaccinated)

  • Purpose: The purpose of this study was to compare knowledge level of those clinical nurses who received HPV vaccine and those who did not and their perception of the relatedness of HPV vaccine to causes of cervical cancer. Methods: A total of 249 clinical nurses were surveyed from June to July, 2009. The questionnaire originally developed by Kim & Ahn (2007) examined HPV-related knowledge originally and the tool for perception of the causes of cervical cancer was originally developed by Kim (1993). The total number of subjects equaled to: vaccination group of 52 (20.9%) and non-vaccination group of 197 (79.1%). Results: Vaccination group showed significantly higher score of both knowledge of HPV vaccination and the perception of the cause of cervical cancer in comparison to the nonvaccination group at (p<.05). Among 4 subscales of the perception of causes of cervical cancer, destiny and constitution subscale scores were significantly different between the two groups at (p<.05). Conclusion: Clinical nurses need to constantly update with current knowledge of HPV and be prepared with currently changing cancer prevention strategies, especially in cervical cancer.

병원 간호사의 혈액매개감염 예방지식과 이행: 직무경력을 기준으로 (Knowledge and Compliance with Blood-Borne Pathogen Prevention of Hospital Nurses: Based on Clinical Experience)

  • Purpose: Exposure to blood and body fluids represents a significant occupational risk for nurses. This study was done to identify the level of knowledge of and compliance with blood-borne pathogen prevention of hospital nurses according to clinical experience, and to identify factors affecting compliance with blood-borne pathogen prevention. Methods: A descriptive correlational study was conducted in which self-reported knowledge of and compliance with blood-borne pathogen prevention was assessed. The relationships between variables were examined. Registered nurses who were employed (n=345) were surveyed. Data were analyzed using, t-test, ANOVA, Pearson correlation, and multiple linear regression. Results: Nurses with more than 5 years experience had greater knowledge of blood-borne pathogen prevention than nurses with less than 5 years experience. However, there was no significant difference in compliance with blood-borne pathogen prevention between the two groups. No significant correlation was found between knowledge and compliance with blood-borne pathogen prevention according to experience standards. In nurses with less than 5 years experience, gender, age, reported exposure to a blood-borne pathogen, and compliance with principles of prevention had a significant impact on compliance with blood-borne pathogen prevention. For nurses with more than 5 years experience, reporting after exposure to blood-borne pathogen was a contributing factor to compliance with blood-borne pathogen prevention. Conclusion: The study results show that to improve the implementation of preventive measures against blood-borne pathogens different strategies are needed, depending on clinical experience of the nurses.
